Lymphomas located in the head and neck region are the most commonly recognized non-epithelial carcinomas. Primary lymphoma of the frontal sinus is a very rare disease. Clinical and radiological findings are uncharacteristic, they may be similar to other diseases of the nose and paranasal sinuses, which usually delays diagnosis, resulting in worse prognosis. Treatment of choice for this type of tumor is based on taking diagnostic samples, multidrug chemotherapy and complementary radiotherapy. A case of a 63-year-old woman who was diagnosed with B-cell lymphoma spread from large frontal sinus cells was reported. The patient has undergone surgical treatment, 3 out of 4 cycles of chemotherapy and is expecting radiotherapy for the primary tumor. The course of the disease and treatment are presented. No complications were observed in the postoperative period.

Chłoniaki w obrębie narządów głowy i szyi są najczęściej rozpoznawanymi nowotworami złośliwymi pochodzenia nienabłonkowego. Pierwotny chłoniak zatok czołowych jest bardzo rzadką chorobą. Obraz kliniczny i radiologiczny jest niecharakterystyczny, może być zbliżony do innych schorzeń nosa i zatok przynosowych. Opóźnia to zwykle diagnozę, powodując gorsze rokowanie. Leczenie z wyboru tego typu guzów polega na pobraniu materiału diagnostycznego, chemioterapii wielolekowej oraz uzupełniającej radioterapii. Opisano przypadek 63-letniej kobiety, u której rozpoznano chłoniaka rozlanego z dużych komórek B zatoki czołowej. Pacjentka przebyła leczenie operacyjne oraz trzy z czterech cykli chemioterapii; oczekuje na radioterapię ogniska pierwotnego nowotworu. Przedstawiono przebieg choroby i leczenia. Okres pooperacyjny był niepowikłany
Wprowadzenie: Śluzowiaki (mucocele) zatok przynosowych z różnych przyczyn częściej niż w pozostałych zatokach występują w zatokach czołowych. Ich lokalizacja w bliskim sąsiedztwie oczodołów może prowadzić do powikłań, takich jak: diplopia, ciężka proptoza, a nawet osłabienie wzroku. Tomografia komputerowa, jako preferowana metoda obrazowania, pozwala na uwidocznienie kostnych ścian zatok przynosowych i zasięgu śluzowiaka w obrębie otaczających go struktur. Śluzowiaki można wycinać endoskopowo lub z podejścia zewnętrznego. Opis przypadku: W artykule opisujemy nasze doświadczenie obejmujące trzy przypadki śluzowiaków charakteryzujących się zaawansowanym stopniem zajęcia oczodołu i poddanych skutecznemu leczeniu bez następstw w postaci powikłań lub nawrotów. We wszystkich doszło do znacznej poprawy w zakresie wszystkich objawów, w tym diplopii i proptozy. W świetle powyższego, ponownie podkreślamy preferencyjny charakter endoskopowego podejścia do leczenia śluzowiaków zatok przynosowych jako modalności charakteryzującej się: krótszym czasem rekonwalescencji, łatwością dostępu, niższą śmiertelnością i niższym odsetkiem potencjalnych powikłań.
During last decades Draf III procedure gained popularity in treatment of different pathologies of the frontal sinus such as chronic sinusitis or benign tumors. We present a series of 10 patients treated with this procedure from a 2 year-period. Indications included: chronic rhinosinusitis – four patients, frontal sinus mucocele – four patients (one with destruction of the posterior table), osteoma – one patient (stage 3 according to Kennedy’s grading system), and inverted papilloma – one patient (Krouse T3 lesion). Observation period ranged from 4 months to 2 years. Results: There were no early complications. Gradual narrowing of the created ostium was observed in all of the patients. This led to total obstruction in two, and stenosis (not allowing for passing with 4 mm endoscope) in another two patients. The patient after inverted papilloma removal showed no recurrence in 11 months follow-up. Conclusion: Draf III procedure is alternative for external approach that can be used for treatment of chronic rhinosinusitis and benign frontal sinus tumors of different size.
