The purpose of the present work was the elaboration of research methodology of the exhaustion degree of performance properties of pipes based on unsaturated polyester and glass fibers, which were manufactured by the cross winding method on a poly(vinyl chloride) core. Within the frame of the work we conducted fatigue-ageing tests being the simulation tool of the degradation process of polyester-glass pipes. Diagnosis of the composite material condition was conducted with the use of nondestructive ultrasonic testing with the application of the echo method, in which the transition time of ultrasonic wave was determined as the identifying parameter. The registered transition time of ultrasonic wave allowed the identification of the material condition during the course of pipes exploitation.
Urinary tract malformations are frequently detected by routine prenatal ultrasound examination performed in normal pregnancy. Abnormalities of kidney tissue and structure are diagnosed in prenatal examinations less frequently than urinary tract dilatation. These include: renal agenesis or hypoplasia (uni- or bilateral), hyperechogenicity or renal parenchyma or part of it (medulla), dysplastic kidneys, multiple or single cysts. From the epidemiological point of view, they are less frequent but may be important for single patient. For example they may put the foetus and newborn at high risk (bilateral kidney agenesis). Postnatal evaluation of those problems relies mostly on neonatologist and paediatrician with nephrologist consultant. The initial evaluation is very similar to malformations with urinary tract dilatation presented in the first part of series (first evaluation in first week of life with follow up after 4-6 weeks). Usually these malformation do not put a newborn to a direct danger in the neonatal period or in infancy, provided proper exclusion of processes affecting both kidneys (dysplasia, hypoplasia or polycystic kidney disease). However, sometimes apart of normal picture in initial evaluation, changes in kidney are detected later in the life. In the present, second part of publication we describe guidelines concerning diagnostic approach in case of kidney tissue abnormalities suggested by prenatal ultrasound screening. Additionally, we described guidelines for prophylaxis of progression of kidney disease and factors that force us to modify the scheme based on clinical symptoms.

Wady wrodzone układu moczowego stanowią znaczącą grupę wad, które mogą być wykryte prenatalnie. Oprócz nieprawidłowych obrazów dróg wyprowadzających mocz stwierdza się również nieprawidłowości w zakresie ilości i struktury miąższu nerkowego. Nieprawidłowe obrazy ultrasonograficzne obejmują zaburzenia, takie jak: niewytworzenie lub zmniejszenie ilości miąższu jednej lub obu nerek, hiperechogeniczność miąższu, zmiany torbielowate lub dysplastyczne oraz pojedyncze torbiele. Z punktu widzenia epidemiologicznego występują one wyraźniej rzadziej niż zaburzenia dróg wyprowadzających mocz. Mimo to część z nich może stanowić zagrożenie dla przebiegu ciąży i dla dziecka – na przykład agenezja nerek czy obustronne zmiany torbielowate. Ciężar diagnostyki pourodzeniowej w tych przypadkach spoczywa na pediatrze, lekarzu rodzinnym oraz konsultującym nefrologu. Jej początkowy schemat jest podobny do zastosowanego w przypadku wad z poszerzeniem dróg moczowych (badanie wstępne pourodzeniowe oraz badanie dodatkowe 4-6 tygodni po poprzednim). Wiadomo, że zaburzenia echostruktury i ilości miąższu nerek zwykle nie stanowią zagrożenia dla dziecka w okresie noworodkowym i niemowlęcym, pod warunkiem wykluczenia zmian dotyczących obu nerek jednocześnie, takich jak wielotorbielowatość, dysplazja lub hipoplazja. Zdarza się także, że mimo braku potwierdzenia wady w pierwszych badaniach, zmiany występują w kolejnych. W niniejszym opracowaniu znajdują się wskazówki dotyczące postępowania pourodzeniowego w ww. jednostkach chorobowych. Dodatkowo omówione zostały obecnie obowiązujące zasady profilaktyki postępu choroby nerek oraz odstępstwa od zaproponowanych wskazówek w oparciu o dane kliniczne.
Ultrasonography is the most widespread imaging technique used in the diagnostics of the pathologies concerning the organs in the abdominal cavity. Similarly to other diagnostic tools, errors may occur in ultrasound examinations. They generally result from inappropriate techniques, which do not conform to current standards, or erroneous interpretation of obtained images. A significant portion of mistakes is caused by inappropriate quality of the apparatus, the presence of sonographic imaging artifacts, unfavorable anatomic variants or improper preparation of the patient for the examination. This article focuses on the examiner-related errors. They concern the evaluation of the liver size, echostructure and arterial and venous vascularization as well as inappropriate interpretation of the liver anatomic variants and the vascular and ductal structures localized inside of it. Furthermore, the article presents typical mistakes made during the diagnosis of the most common gallbladder and bile duct diseases. It also includes helpful data concerning differential diagnostics of the described pathologies of the liver, gallbladder and bile ducts. The article indicates the most frequent sources of mistakes as well as false negative and false positive examples which result from these errors. What is more, the norms used in the liver, gallbladder and bile duct evaluations are presented as well as some helpful guidelines referring to the exam techniques and image interpretation, which allows for reducing the error-making risk. The article has been prepared on the basis of the report published in 2005 by the Polish experts in the field of ultrasonography and extended with the latest findings obtained from the pertinent literature.

Ultrasonografia jest najbardziej rozpowszechnioną techniką obrazowania w diagnostyce chorób narządów jamy brzusznej. Podobnie jak w przypadku innych narzędzi diagnostycznych badanie ultrasonograficzne obarczone jest ryzykiem popełniania błędów. Na ogół wiążą się one z niewłaściwą techniką wykonywania badania, niezgodną z obowiązującymi standardami, bądź mylną interpretacją uzyskiwanego obrazu. Znaczna część popełnianych pomyłek wynika także z nieodpowiedniej jakości stosowanego sprzętu, z obecności artefaktów obrazowania ultrasonograficznego, niekorzystnych warunków anatomicznych lub z niewłaściwego przygotowania chorego do badania. W niniejszej pracy skoncentrowano się na błędach zależnych od badającego. Dotyczą one oceny wymiarów wątroby, jej echostruktury oraz unaczynienia tętniczego i żylnego, niewłaściwej interpretacji wariantów anatomicznych wątroby, położonych w niej struktur naczyniowych i przewodowych. Ponadto przedstawiono typowe pomyłki pojawiające się w trakcie prowadzenia diagnostyki najczęstszych chorób pęcherzyka żółciowego i dróg żółciowych. W pracy zawarto przydatne dane dotyczące diagnostyki różnicowej opisywanych patologii wątroby, pęcherzyka żółciowego i dróg żółciowych. Wskazano najczęstsze źródła popełnianych pomyłek i przykłady wynikających z nich wyników fałszywie dodatnich i ujemnych. Poza tym przedstawiono zakresy norm stosowanych w ocenie wątroby, pęcherzyka żółciowego i dróg żółciowych oraz zawarto przydatne wskazówki dotyczące techniki badania, interpretacji uzyskiwanego obrazu, pozwalające zminimalizować ryzyko popełnienia pomyłek. Artykuł został przygotowany na podstawie opracowania opublikowanego przez polskich ekspertów z dziedziny ultrasonografii w 2005 roku i pogłębiony o najnowsze doniesienia z piśmiennictwa.
Increased availability of prenatal ultrasound examination resulted in high number of early detected abnormalities of urinary tract in neonates and infants. In at least 1/100 normal pregnancies abnormal picture of kidneys or urinary tract is suspected. Most frequently the urinary tract dilatation may be detected. One or both of the kidneys could be affected but dilatation of renal pelvis, ureter or enlargement of urinary bladder may be seen. High percent of abnormalities resolve during infancy and do not constitute an indication for postnatal surgery. On the other hand, enlarged urinary bladder with widened urinary tract accompanied by oligohydramnios clearly indicates serious congenital malformation with high perinatal risk of death. Prenatal evaluation enables both, prenatal intervention and more frequently proper and detailed planning of careful clinical postnatal evaluation. According to Polish regulations the follow up relies mainly on neonatologist and paediatrician. Unfortunately, standards of such care are not available. In the present publication (prepared in multidisciplinary cooperation of urologists, nephrologists and radiologists) we describe a part of guidelines concerning diagnostic procedures with neonates or infants with urinary tract dilatation suggested by prenatal screening. This paper is dedicated to these physicians who deal with postnatal diagnosis of urinary tract malformation. They include scheme for evaluation in case of renal pelvis dilatation, megaureter, posterior urethral valves. Suggestions for antibiotic prophylaxis are also presented.

Szeroka dostępność prenatalnej diagnostyki ultrasonograficznej płodu niesie ze sobą korzyści pod postacią zwiększonej wykrywalności wad wrodzonych układu moczowego. Wadę taką w rutynowym badaniu podejrzewa się mniej więcej u 1/100 płodów. Najczęściej wykrywanym obrazem jest poszerzenie dróg moczowych płodu. Zmiany mogą dotyczyć jednej lub obu nerek płodu. Można zauważyć także poszerzenie moczowodów oraz powiększony pęcherz moczowy. W dużej liczbie przypadków obraz poszerzenia miedniczek nerkowych ustępuje lub nie stanowi ostatecznie wskazań do interwencji zabiegowej. Z drugiej strony powiększenie pęcherza moczowego i dróg moczowych wraz ze skąpowodziem wskazuje na poważną wadę wrodzoną z wysokim ryzykiem okołoporodowym. Obraz prenatalny dróg moczowych nie tylko umożliwia zaplanowanie interwencji w okresie ciąży, ale także pozwala ustalić właściwe postępowanie okołoporodowe, co ze względu na wysoką częstość nieprawidłowych obrazów ułatwia racjonalne postępowanie. W obecnej strukturze opieki zdrowotnej pierwszy etap diagnostyki planowany jest dla lekarzy neonatologów i pediatrów, którzy do tej pory nie mieli dostępu do zunifikowanych zaleceń co do takiego postępowania. Prezentowana praca powstała na podstawie zaleceń Polskiego Towarzystwa Nefrologii Dziecięcej, które przygotował wielodyscyplinarny zespół urologów, nefrologów oraz specjalistów radiologów – pisana była z myślą o lekarzach prowadzących pourodzeniową diagnostykę wad. Zaprezentowano w niej zalecenia dotyczące diagnostyki pourodzeniowej w przypadku poszerzenia miedniczki nerkowej, moczowodu, zastawek cewki tylnej. Zawiera także sugestie co do stosowania profilaktyki zakażeń układu moczowego.
Ultrasonography, like any imaging method, entails the risk of errors. From among all means of imaging, it is the most subjective and dependent on the examiner’s knowledge and experience. This paper presents the causes of examiner-dependent errors as well as those which result from technical settings and preparation of the patient for the examination. Moreover, the authors discuss the most frequent errors in the diagnosis of splenic conditions, which result from insufficient knowledge concerning anatomical variants of this organ, wrong measurements and incorrect examination technique. The mistakes made in the differentiation of focal lesions of the spleen and its hilum are also discussed. Additionally, the differentiation of collateral circulation, lymph nodes and accessory spleens is mentioned. The authors also draw attention to erroneous interpretation of the left liver lobe as a fragment of the spleen as well as the prominent tail of the pancreas filled with gastric contents and intestinal loops as abnormal masses or fluid cisterns in the area of the splenic hilum. Furthermore, the pathologies of the hilum are discussed such as tumors of the splenic flexure of the colon, lesions arising from the left kidney or the left adrenal gland. The authors list characteristic imaging features of the most common focal lesions visualized in a standard ultrasound scan as well as enhancement patterns appearing in contrast-enhanced examinations. The article discusses the features and differentiation of, among others, infarction, splenic cysts including hydatid ones, abscesses and angiomas. The ultrasound appearance of lymphoma and secondary involvement of the spleen by other malignant neoplasms is also mentioned. Moreover, the authors provide useful tips connected with imaging techniques and interpretation of the findings. The ultrasound examination carried out in compliance with current standards allows for an optimal assessment of the organ and reduction of the error-making risk. This article is based on the publication of the experts from the Polish Ultrasound Society of 2005 and updated with the latest findings in pertinent literature. Podobnie jak każda metoda obrazowa ultrasonografia niesie ze sobą ryzyko popełniania błędów. Spośród wszystkich badań obrazowych jest ona w największym stopniu subiektywna, zależna od wiedzy i doświadczenia wykonującego badanie. W pracy wskazano przyczyny pomyłek zależne od badającego, ale również od ustawień technicznych i przygotowania pacjenta do badania. Ponadto przedstawiono najczęstsze błędy popełniane w diagnostyce ultrasonograficznej chorób śledziony, wynikające z nieznajomości wariantów anatomicznych jej budowy, nieprawidłowych pomiarów i nieprawidłowej techniki badania. Omówiono pomyłki w różnicowaniu zmian ogniskowych śledziony i jej wnęki. Opisano różnicowanie naczyń krążenia obocznego, węzłów chłonnych oraz śledzion dodatkowych. Zwrócono uwagę na mylną interpretację lewego płata wątroby jako krwiaka śledziony, a także wydatnego ogona trzustki, wypełnionego treścią żołądka lub pętli jelitowych jako nieprawidłowych mas lub zbiorników płynowych w okolicy wnęki śledziony. Ponadto omówiono patologie wnęki, takie jak guz zagięcia śledzionowego jelita grubego, zmiany wychodzące z lewej nerki lub lewego nadnercza. Wymieniono charakterystyczne cechy w obrazowaniu najczęściej spotykanych zmian ogniskowych w standardowym badaniu ultrasonograficznym, jak również wzorce wzmocnienia w badaniach z użyciem środków kontrastowych. Omówiono cechy i różnicowanie m.in. zawału, torbieli śledziony, z uwzględnieniem torbieli bąblowcowych, ropni i naczyniaków. Wspomniano o obrazach ultrasonograficznych w przebiegu chłoniaków oraz wtórnym zajęciu śledziony przez inne nowotwory złośliwe. Artykuł zawiera przydatne wskazówki dotyczące techniki obrazowania oraz interpretacji uzyskanych obrazów. Wykonanie badań zgodnie z obowiązującymi standardami pozwala na optymalną ocenę narządu oraz zminimalizowanie ryzyka popełnienia błędów.
