COVID-19 manifests itself in a wide spectrum of clinical symptoms, both in terms of their variety and severity. It can be asymptomatic or abortive, mild, moderate, severe and lightning, as septic with multiple organ failure and shock Typical leading symptoms of COVID-19 are: high fever poorly responding to drugs, severe loss of strength, chest pain, dyspnoea, pain headaches, bone and joint pain and muscle pain, until the onset of acute respiratory distress syndrome (ARDS). However, many publications mention among the possible symptoms also others, not related to the involvement of the lower respiratory tract. These are gastrointestinal disorders, damage to the central and peripheral nervous system, catarrh of the upper respiratory tract and dysfunctions of the sensory organs. The aim of this literature review was to determine the frequency of various head and neck dysfunctions that are part of COVID-19. Symptoms of conjunctivitis, nasal mucosa, pharynx and larynx are reported by about of patients, but they do not always occur at the same time, as in infections caused, for example, by rhinoviruses. Anosmi / hyposmia or ageusia / hypogeusia occur with a similar frequency. Symptoms of damage to the equilibrium system, such as dizziness, are reported by approx. 1/3, vertigo and hearing loss approx. 5-6%, tinnitus approx. 10% of patients. Reports of coexistence with COVID-19 of peripheral paresis of the facial nerve are so far relatively few and often included in the neurological disorders, the frequency of which is also about 1/3 of COVID-19 cases. Importantly, both catarrhal symptoms and the others listed here may precede, co-occur or follow the appearance of the leading symptoms of COVID-19. They can also be the only symptoms of this disease. This should prompt otorhinolaryngologists to be particularly vigilant in this regard
Acute appendicitis (AA) still remains the most common acute surgical abdominal emergency. Although 90% of cases occur in children and young adults, the incidence in the elderly amounts up to 10% and is constantly rising. The aim of the study was to assess the differences between clinical presentation in the elderly patients with AA compared to the younger patients. Additional aim was to assess the correlation between in-hospital time delays and patients’ outcomes. Material and methods. We conducted a retrospective analysis of medical data of 274 patients admitted to 3rd Department of General Surgery in Cracow between January 2011 and December 2013 due to AA. The elderly group comprised 23 patients aged 65 and above and the non-elderly group consisted of 251 patients. Results. The groups did not differ in symptoms and their duration, type of surgery and its duration. However, time from admission to ED to the beginning of the procedure was significantly lower in the elderly group (575.56 vs 858.9 min; p=0.03). The elderly had longer hospital stay (6.08 vs 4.69 days; p=0.004). In the elderly group the perforation rate was close to reaching statistical significance (26.1% vs 12.4%; p=0.06). No mortality was noted in both groups and morbidity was slightly higher in elderly group (17.4% vs 10%; p=0.26). Conclusions. There was no difference in the clinical presentation between elderly and non-elderly patients group. However, elderly patients presented with a more progressed inflammation of the appendix. The hospital stay was longer in the elderly group, without any mortality and with higher rate of morbidity in this group. The length of the preoperative phase was significantly shorter, confirming the awareness of importance of time in the elderly patients with acute abdomen.
The authors would like to present a rare case of the middle ear cancer which has been developed in postoperative cave in 67 years old patient operated for cholesteatoma 50 years earlier. The patient was admitted to the ENT Department of Poznań University of Medical Sciences because of the ear suppuration and headache occurring for 3 months. CT and MR images suggested granulation tissue fi lling the postoperative spaces with bone destruction, infi ltration of the dura and temporal lobe abscess formation. Intraoperative fi ndings allowed excluding the preliminary diagnosis of intracranial complication in the course of chronic otitis media, revealing the tissue masses resembling neoplastic infi ltration. The histopathology examination confi rmed the fi nal diagnosis of squamous cell cancer. The patient was directed to radiotherapy. The authors report a case of middle ear squamous cell carcinoma and discuss its diagnostic aspect.
INTRODUCTION: COVID-19 (coronavirus disease 2019), a disease caused by SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) infection, may have a more or less severe course. The aim of this study was to analyse the symptoms which occurred in a group of people who tested positively for the presence of the genetic material of the virus. MATERIAL AND METHODS: The analysis was based on the results of 337 questionnaires collected from Polish patients – 185 (55%) females and 152 (45%) males aged 18–86. The questionnaire included 26 questions, of which 11 were related to systemic symptoms. The frequency of their occurrence and their severity were determined according to Visual Analogue Scale (VAS). RESULTS: The most common symptoms that occurred in over 50% of the respondents included: general weakness (91% of the respondents, severity of 6.25 on VAS), musculoskeletal pain (77%, VAS 5.67), headache (65%, VAS 5.37) and cough (56%, VAS 4.76). Runny nose, fever above 38℃ and sore throat were less common and occurred in 39% of the subjects. The least frequent symptoms were diarrhoea (23%), skin lesions (7%) and vomiting (3%). When determining the correlation coefficient, it was also found that dyspnoea occurred most often with general weakness (r = 1), whereas general weakness was less frequently associated with musculoskeletal pain (r = 0.8). No statistically significant relationships were found between the frequency of the symptoms and age, gender or the presence of comorbidities in the respondents. CONCLUSIONS: The observations showed that in the analysed group of people with SARS-CoV-2 infection, the disease had a mild-to-moderate course. General weakness, musculoskeletal pain, cough, and upper respiratory tract infection symptoms were the most common.

WSTĘP: COVID-19 (coronavirus disease 2019), choroba wywołana infekcją SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2), może charakteryzować się mniej lub bardziej nasilonym przebiegiem. Celem niniejszego opracowania była analiza objawów, jakie wystąpiły w grupie osób z dodatnim wynikiem testu na obecność materiału genetycznego wirusa. MATERIAŁ I METODY: Analizę przeprowadzono na podstawie wyników ankiety przeprowadzonej wśród 337 polskich pacjentów – 185 (55%) kobiet oraz 152 (45%) mężczyzn w wieku 18–86 lat. Kwestionariusz zawierał 26 pytań, z czego 11 dotyczyło objawów ogólnoustrojowych. Określono częstość ich występowania oraz nasilenie w skali VAS (Visual Analogue Scale). WYNIKI: Do najczęstszych objawów, występujących u ponad 50% ankietowanych, należą: ogólne osłabienie (91% ankietowanych, nasilenie 6,25 w skali VAS), bóle układu mięśniowo-szkieletowego (77%, VAS 5,67), bóle głowy (65%, VAS 5,37) oraz kaszel (56%, VAS 4,76). Chorobie rzadziej towarzyszyły katar, gorączka powyżej 38°C oraz bóle gardła, które wystąpiły u 39% pacjentów. Najrzadziej występowały biegunka (23%), zmiany skórne (7%) oraz wymioty (3%). Określając współczynnik korelacji, stwierdzono również, iż duszności występowały najczęściej z ogólnym osła-bieniem (r = 1), natomiast rzadziej ogólnemu osłabieniu towarzyszyły bóle mięśniowo-szkieletowe (r = 0,8). Nie stwierdzono statystycznie istotnych zależności między częstością występowania danych objawów a wiekiem, płcią czy chorobami współistniejącymi u ankietowanych. WNIOSKI: Obserwacje wykazały, iż w analizowanej grupie zakażonych SARS-CoV-2 choroba miała przebieg łagodny lub umiarkowany. Najczęstszymi objawami były: ogólne zmęczenie, bóle mięśniowo-szkieletowe, kaszel oraz objawy nieżytu górnych dróg oddechowych.
Accurate diagnosis of acute appendicitis (AA) is still a problem and is not always easy, even for experienced surgeons. Studies have shown that 20 to 30% of the appendices removedwere normal. Therefore, various scoring systems have been developed to aid in the diagnosis of doubtful cases and reduce the number of unnecessary appendectomies. The aim of this study was to assess the diagnostic value of different scoring systems in acute appendicitis. Material and methods. The study involved 94 patients who underwent laparotomy due to suspected acute appendicitis. Medical examination at hospital admission was performed by a resident and a general surgery specialist. The probability of AA was evaluated using six different scoring systems: Alvarado, Fenyo, Eskelinen, Ohman, Tzankis, and RIPASA. The resident calculated the results in individual systems. The decision to perform the operation was taken by a specialist surgeon who did not know the results. Results. Normal appendix was removed in 26% of cases. Eskelinen, RIPASA and Alvarado systems showed highest sensitivity (99%, 88% and 85%, respectively). Tzankis and Fenyo systems showed highest specificity (62% and 50%, respectively). Conclusions. Our study has shown limited value of scoring systems for the diagnosis of acute appendicitis. The systems may improve diagnostic accuracy as they require obtaining a more detailed medical history, and making a more thorough and organized data analysis. However, the scoring systems should be treated only as an aid to diagnosis
Despite multidirectional activities in the sphere of politics, health care and limiting direct interpersonal contacts to a minimum, the Covid-19 pandemic has covered the whole world and the number of new cases is rising exponentially. This demonstrates the considerable severity of the situation. The doctors and other health professionals are and will be at the forefront of the fight against the pandemic, being at the same time the group most at risk of infection. SARS-CoV-2 virus infection applies to patients of all ages, with a median age of 49–59 years. The most common complaints in patients with Covid-19 include fever, caught, dyspnea, general malaise and muscle aches. In the course of Covid-19 may occur pneumonia, which in about 20% of cases is moderately to severe and associated with respiratory distress. Important for otorhinolaryngologists are reports of smell and taste disorders. Post-infective loss of smell can make up about 30% of all patients with Covid-19. The confirmation of SARS-CoV-2 infection in patients with symptomatic or epidemiological suspicion of Covid-19 are genetic tests RT-PCR. Biological material is usually taken from the nose or nasopharynx. The management of Covid-19 is symptomatic because there is currently no specific cure drug. There is a particularly high risk of transmitting infection from a patient to the physicians, who are treating diseases of the respiratory tract or perform any interventions on the upper and lower respiratory tract. Low-symptomatic first stages of the disease with high viral load mean that during a pandemic, scheduled examinations and procedures should be limited. During the examination of patients or their treatment, adequate full PPE protection is indicated, reducing the risk of accidental infection of the treatment team. For the treatment of patients with confirmed SARS-CoV-2 infection or requiring immediate treatment without the possibility of determining epidemiological factors Covid-19, full protection in PPE is obligatory.
Migraine is a common neurological condition characterized by recurrent episodes of often severe headache accompanied by symptoms such as nausea, vomiting, photophobia, and phonophobia. The prevalence of migraine has been estimated to be approximately 6% in men and 15 to 18% in women. The prevalence of migraine peaks at age 40, and thus most affects individuals in their peak productive years. The diagnosis is based on International Headache Society (IHS) criteria. In addition to classic and common, migraine headache can take several other forms: hemiplegic migraine, ophthalmoplegic migraine, basilar artery migraine, migraine in adolescent and young adult and menstrual migraine and status migrainosus. In child - ren migraine can take picture periodic vomiting, systemic vertigo, abdominal pain. Basilar type migraine should be considered and differentiate with other type of vascular diseases. Chronic migraine is a specific type of disease with pain duration more than 15 days per month and more than three months. It can take picture a whole day’s headache, what concern mainly patients suffering migraine without aura, abusing analgesic drugs. This form should be prophylactic treated especially with antiepileptic and antidepressant drugs. In article have been prescribed those types of disease.

Migrena jest częstą chorobą neurologiczną, charakteryzującą się nawrotowymi epizodami bólów głowy, którym towarzyszą takie objawy, jak nudności, wymioty i światłowstręt. Częstość jej występowania szacowana jest na około 6% wśród mężczyzn i 15-18% u kobiet. Szczyt zachorowania odnotowuje się ok. 40. r.ż., co oznacza, że choroba dotyczy głównie ludzi najbardziej aktywnych zawodowo. Diagnostyka migreny oparta jest na kryteriach opracowanych przez Międzynarodowe Towarzystwo Bólu Głowy. Poza klasyczną postacią choroby występują jej rzadsze postacie, takie jak migrena porażenna, migrena oftalmoplegiczna, migrena typu podstawnego, migrena siatkówkowa, migrena u dzieci i młodzieży, migrena menstruacyjna oraz stan migrenowy. U dzieci i młodzieży napady mogą przybierać inne formy kliniczne, na przykład cykliczne wymioty, napadowe zawroty głowy, bóle brzucha. Migrena typu podstawnego i migrena porażenna wymagają różnicowania z innymi chorobami naczyniowymi mózgu. Migrena przewlekła rozpoznawana jest, gdy napady migrenowego bólu głowy trwają powyżej 15 dni w miesiącu przez okres co najmniej trzech miesięcy. Może ona przybrać postać całodziennego bólu głowy. Dotyczy to zwłaszcza chorych na migrenę bez aury, nadużywających leków przeciwbólowych. Ta postać migreny wymaga leczenia profilaktycznego i częstej kontroli lekarskiej. W terapii skuteczne są leki przeciwpadaczkowe i leki przeciwdepresyjne. W artykule omówiono poszczególne postacie choroby i ich leczenie.
Chronic constipation is one of the most common complaints among children. It is a cause of about 3% visits in paediatric outpatient clinic. The problem, which is often belittled, is an unpleasant and persistent complaint for both child and its family. The causes of constipation may be both organic and non-organic. Among non-organic problems, the most important is probably the chronic retention of faeces, which is made by incorrect hygienic habits, incorrect diet, and insufficient physical activity. The problem often appears when child’s environment changes, for example when child goes to kindergarten or school, which are the places where child for different reasons prevents from defecating. However, to make the final diagnosis, the organic causes should be definitely rejected, because they require causal treatment. Treatment of the most common functional constipation needs complex pharmacological and non-pharmacological methods. The therapy of constipation is usually long. The aim of therapy is to consolidate such model of defecation when child regularly deposits soft faeces without unpleasant pain. Extremely important is also a psychological support for whole family, often absorbed in difficult child’s problem. To achieve the therapeutic success a patient and conscientious cooperation between doctor and patient and his family is required, because positive and permanent effects of treatment are reached after a relatively long time.

Przewlekłe zaparcia są jedną z najczęstszych dolegliwości w populacji dziecięcej. Stanowią przyczynę około 3% wizyt w poradniach pediatrycznych. Problem, niejednokrotnie bagatelizowany, stanowi przykrą, uporczywą dolegliwość, zarówno dla pacjenta, jak i całej jego rodziny. Zaparcia mogą wynikać z przyczyn organicznych lub nieorganicznych. Wśród przyczyn nieorganicznych kluczowa wydaje się przewlekła retencja stolca, wynikająca ze złych nawyków higienicznych, niewłaściwej diety oraz zbyt niskiej aktywności fizycznej. Zaparcia często pojawiają się wraz ze zmianą otoczenia dziecka, np. po rozpoczęciu nauki w przedszkolu czy szkole, w których pacjent z różnych przyczyn powstrzymuje się od oddania stolca. W celu ustalenia ostatecznego rozpoznania należy wykluczyć przyczyny organiczne zaparć, które wymagają leczenia przyczynowego. Terapia najczęściej występujących zaparć o charakterze czynnościowym obejmuje zastosowanie kompleksowych metod farmakologicznych i niefarmakologicznych; zwykle jest długotrwała. Ma ona na celu utrwalenie takiego modelu defekacji, w którym pacjent regularnie oddaje miękki stolec bez przykrych dolegliwości bólowych. Niezwykle istotne jest wsparcie psychologiczne całej rodziny, niejednokrotnie zaangażowanej w trudny problem dziecka. Sukces terapeutyczny wymaga przede wszystkim cierpliwej i sumiennej współpracy pomiędzy lekarzem a pacjentem i jego rodziną, gdyż pozytywne i trwałe efekty leczenia osiągane są po stosunkowo długim czasie.
Trichotillomania, also known as hair pulling disorder, is a chronic psychiatric condition common among women. It is a condition that causes impairment in social, occupational, or other important areas of functioning. The rate of presentation of such cases to medical practitioners is low, as affected individuals conceal it because of the shame associated with hair pulling behaviour. This makes the aetiology and management of the condition to be fairly understood; creating the need for extensive empirical research. This paper discusses the clinical and epidemiological features of trichotillomania and its management.
Marfan syndrome is a disorder of the connective tissue (fibrillins and elastins). It is an autosomal dominant disease associated with a defect of chromosome 15 which encodes fibrillin-1. The syndrome is characterised by a varying extent of connective tissue disorders. A mutation in the gene encoding connective tissue can be manifested in all organs irrespective of the patient’s age. This disorder is characterised by multiple symptoms manifested mainly in the cardiovascular and skeletal systems as well as in the organ of vision. Ghent criteria are commonly recognised in the diagnosis of Marfan syndrome. Moreover, clinical symptoms reported by patients should also be considered. Patients presenting at the office of a general practitioner with symptoms resembling Marfan-like syndromes should undergo a detailed medical interview for the presence of cardiovascular diseases in the family. A general practitioner should examine the patient, drawing particular attention to organs that are frequently affected by disorders of connective tissue encoding (chest, oral cavity and general body stature). The family of a patient with Marfan syndrome should be referred to a genetic clinic to conduct tests for the presence of gene mutation. The diagnosis of Marfan syndrome in neonates is complex since not all symptoms of the syndrome are clinically manifested. In female patients with Marfan syndrome, pregnancy is contraindicated if the size of the aorta exceeds 50 mm. In other cases, women must be informed about possible risks and monitored for a change in the width of the aorta. Sometimes, procedures of prophylactic aortic replacement on its entire course are conducted prior to a planned pregnancy. Regular echocardiographic check-ups (every 4–7 weeks) as well as cooperation of a gynaecologist and cardiologist are necessary. A general practitioner should inform the patient’s family about any sudden conditions that can develop in the course of Marfan syndrome (aortic dissection) and explain to them the algorithms of management (immediate call for an ambulance).

Zespół Marfana wynika z wady tkanki łącznej (fibryliny, elastyny). Choroba najczęściej przenoszona jest w sposób autosomalny dominujący, związany z defektem chromosomu 15 kodującego białko fibrylinę-1. Cechą charakterystyczną zespołu jest różnorodna ekspresja zaburzeń tkanki łącznej. Mutacja w genie kodującym tkankę łączną może objawić się we wszystkich narządach, niezależnie od wieku pacjenta. Zaburzenie to cechuje się mnogością objawów, manifestujących się głównie w układzie krążenia i kostno-stawowym, a także w narządzie wzroku. Powszechnie przyjętymi kryteriami rozpoznawania zespołu Marfana są kryteria Ghenta. Ocenie powinny zostać poddane także objawy kliniczne zgłaszane przez pacjenta. Osoby zgłaszające się do lekarza pierwszego kontaktu z symptomatologią zespołów marfanopodobnych powinny przejść dokładny wywiad medyczny pod kątem chorób sercowo-naczyniowych występujących w rodzinie. Lekarz rodzinny powinien zbadać pacjenta, zwracając szczególną uwagę na narządy najczęściej dotknięte zaburzeniem kodowania tkanki łącznej (klatka piersiowa, jama ustna, ogólna budowa ciała). Rodziny pacjentów z zespołem Marfana należy skierować do poradni genetycznej w celu przeprowadzenia testu na obecność mutacji. Diagnostyka noworodków jest skomplikowana, z uwagi na fakt, że nie wszystkie symptomy zespołu Marfana demonstrują się klinicznie. U pacjentek ze stwierdzonym zespołem Marfana ciąża jest przeciwwskazana, gdy wymiar aorty przekracza 50 mm. W innych przypadkach pacjentkę należy poinformować o istniejącym ryzyku oraz monitorować zmiany w szerokości aorty. Przed planowaną ciążą wykonuje się czasami zabieg profilaktycznej wymiany aorty na całym przebiegu. Nieodzowna jest stała kontrola echokardiograficzna (co 4–7 tygodni) oraz współpraca lekarza ginekologa i kardiologa. Lekarz pierwszego kontaktu powinien wyjaśnić rodzinie chorego stany nagłe mogące wystąpić w przebiegu zespołu Marfana (rozwarstwienie aorty) oraz wytłumaczyć algorytmy postępowania (natychmiastowe wezwanie pogotowia).
The aim of this study is to collect the most important information concerning OA, from the general practitioner's point of view, to guide the patient in the initial diagnosis, cooperate with doctors in other areas, assist in the thera-peutic process, and provide medical support in the case of a lack of access to a consulting specialist or a long period of waiting for the consultation. The paper presents the definition of OA, pathological mechanism of its formation as well as forms depending on the aetiology. The symptoms of the disease with particular emphasis on the location and the principles of diagnostic and therapeutic procedures in the daily practice of the general practitioner are presented. The important role of rehabilitation in order to improve the quality of life of patients is also highlighted. Osteoarthritis is a progressive, irreversible disease, leading to deterioration of the quality of life of the patient. Applying treatment is not able to stop disease progression, but it is able to slow the development and improve the quality of life of patients. The study highlights the important role of the general practitioner as a guide to the health care system for patients with OA.

Celem pracy było zebranie najistotniejszych z punktu widzenia lekarza rodzinnego wiadomości dotyczących ChZS, by w swojej praktyce mógł on, współpracując z lekarzami innych dziedzin, poprowadzić pacjenta we wstępnej diag-nostyce, wspomóc proces terapeutyczny, a także stanowić wsparcie medyczne w razie braku dostępności do konsul-tacji specjalistycznej lub długiego okresu oczekiwania na nią. W pracy przedstawiono definicję ChZS, patomechanizm jej powstawania oraz postaci w zależności od etiologii. Przedstawiono objawy choroby ze szczególnym uwzględnieniem lokalizacji oraz zasady postępowania diagnostycz-nego i terapeutycznego w codziennej praktyce lekarza rodzinnego. Zwrócono również uwagę na istotną rolę rehabilitacji w celu uzyskania poprawy komfortu życia pacjentów. Choroba zwyrodnieniowa stawów jest schorzeniem postępującym, nieodwracalnym, prowadzącym do ograniczenia sprawności i pogorszenia jakości życia chorego. Zastosowane leczenie nie jest w stanie zahamować postępu choroby, może natomiast spowolnić jej rozwój i poprawić jakość życia pacjentów. W pracy podkreślono istotną rolę lekarza rodzinnego jako przewodnika po systemie opieki zdrowotnej w przypadku pacjentów z ChZS.
W artykule przedstawiono podstawowe przyczyny i objawy zaburzeń funkcji poznawczych i zespołów otępiennych oraz proste testy przesiewowe stosowane w celu ich rozpoznania. Jest to niezbędna wiedza, jaką powinien posiadać personel medyczny, w tym fizjoterapeuci pracujący z osobami w podeszłym wieku. Znajomość podstaw z zakresu psychopatologii zaburzeń otępiennych pozwala lepiej zrozumieć i interpretować zachowanie pacjenta, ułatwia komunikowanie się z nim, pozwala dostosować proces usprawniania do możliwości poznawczych seniora oraz zmniejsza natężenie negatywnych emocji ze strony opiekunóow i terapeutów.

The article presents the basic causes and symptoms of cognitive impairment and also simple screening tests used to identify them. This is necessary know-how for medical personnel including physiotherapists working with the elderly. Elementary knowledge of dementia psychopathology allows better understanding and proper interpreting of the patients' behaviour, making communication with the patients easier. Such skills help to adapt the rehabilitation process to the cognitive abilities of the elderly and reduce the severity of negative emotions from caretakers and therapists.
The authors present the classification, symptoms and diagnostic methods of renal injuries. These injuries can be life-threatening, but most of them are mild and can be managed conservatively. No less than 75% patients who were diagnosed with renal injuries are men. The group which is particularly exposed to renal injuries are young men doing sports, the injured of road accidents, victims of accidents in the house or at work, victims of fights and assaults. The results of conducted studies indicate that haematuria, pain and ecchymosis in the lumbar region, fractured ribs as well as abdominal mass can be the symptoms of renal injury. There are blunt and penetrating renal injuries as well as a 5-level classification of kidney injuries according to a degree of injuries – AAST. This classification enables to perform a standardization of different patient groups to choose a proper therapy and to predict the results of treatment. The mechanism of injury, its placement and severity are the standard guidelines within a choice of diagnostic methods and indications for treatment’s strategy. The imaging examinations are necessary in diagnostics of abdomen injuries with a suspicion of renal injury. Among the imaging examinations used in diagnostics of injuries of urogenital system and renal injuries it is to enumerate: ultrasonography, computed tomography, urography, arteriography and angiography. Computed tomography of abdomen with contrast injected into an intravenous line is a gold standard in diagnostics of renal injuries. If computed tomography is not available, it is recommended to perform ultrasonography as an examination of first choice.

Autorzy przedstawiają klasyfikację, objawy i metody diagnostyczne urazów nerek. Urazy te mogą zagrażać życiu, jednak większość z nich ma łagodny przebieg i można je leczyć zachowawczo. Aż 75% pacjentów, u których stwierdzono uszkodzenia nerek, to mężczyźni. Grupę szczególnie narażoną stanowią młodzi mężczyźni uprawiający sport, uczestnicy wypadków drogowych, ofiary wypadków w domu lub w miejscu pracy, ofiary bójek i napadów. Z przeprowadzonych badań wynika, że objawami urazu nerek mogą być: krwiomocz, bóle i zasinienia w okolicach lędźwiowych, złamania żeber, napięty brzuch. Wyróżnia się urazy tępe i penetrujące oraz pięciostopniową klasyfikację urazów nerek według stopnia odniesionych obrażeń – AAST. Klasyfikacja ta pozwala na dokonanie pewnej standaryzacji różnych grup pacjentów i wyboru właściwej terapii oraz stwarza możliwość przewidywania wyników leczenia. Mechanizm urazu, jego umiejscowienie i ciężkość stanowią podstawowe wytyczne w wyborze metod diagnostycznych oraz postępowania leczniczego. Niezbędnym elementem diagnostyki urazu jamy brzusznej z podejrzeniem uszkodzenia nerek jest wykonanie badań obrazowych. Wśród badań obrazowych mających zastosowanie w diagnostyce urazów układu moczowo-płciowego oraz nerek należy wymienić: ultrasonografię, tomografię komputerową, urografię, angiografię i arteriografię. Tomografia komputerowa jamy brzusznej z dożylnym podaniem środka cieniującego stanowi złoty standard w diagnostyce urazów nerek. Przy braku dostępności tomografii komputerowej zalecane jest wykonanie ultrasonografii jako badania pierwszego rzutu.
Bacteriologist’s and clinician’s point of view to infection is variable. What about bacterial infection throat and tonsil? Characteristic: Bacteriologist: Acute pharyngitis and tonsillitis (APT) mainly caused by viruses (rhinovirus, adenovirus, influenza and parainfluenza viruses, RSV). Bacterial etiology is only in 10–30%. The most common bacterial etiologic factor is Streptococcus pyogenes. The gold diagnostic standard is the throat swab culture with antibiogram. Clinician: The infection usually occurs through droplets. The incubation period continues from 12 hours to 4 days. The diagnosis is based on symptoms: sore throat, fever, headache, malaise, slight enlargement of the cervical lymph nodes. On clinical examination: swollen and congested mucous membrane of the throat. Treatment: Bacteriologist: Correct swabbing the throat has a major impact on the entire diagnostic process. We take the swab from the surface or tonsil crypts. Swab is seeded and incubated at 35–37°C. The first assessment is in 18–24 hours. In the GP practice the rapid diagnostic tests (RADT) streptococcal antigen detection clinic may be helpful. Clinician: the first step is limiting the spread of infec‑tion (elimination of contact), second – symptomatic treatment, if there is S. pyogenes etiology, the first-line treatment is fenoksymetylpenicylina (p.o. or .iv.) In case of hypersensitivity, macrolides should be used. Azithromycin: Bacteriologist: Azithromycin is macrolide, and including the spectrum of the most common pathogens causing pharyngitis and tonsillitis. Clinician: Azithromycin is an effective drug in APT, commonly used at a dose: in children (10 mg/kg once daily for three days), and adults (1×500 mg – three days). Recent studies suggest a better antibiotic activity double dose (cumulative dose – 60 mg/kg/treatment). These two approaches (bacteriologist and clinician) complement each other.

Spojrzenie bakteriologa i klinicysty na infekcje bywa różne. W pracy zaprezentowano opinie obu specjalistów na temat bakteryjnego zapalenia gardła i migdałków. Charakterystyka: Bakteriolog: Ostre zapalenie gardła i migdałków (OZGM) głównie wywołują wirusy (rynowirusy, adenowirusy, wirusy grypy i paragrypy, RSV). Tylko 10–30% przypadków ma etiologię bakteryjną. Najczęstszym czynnikiem etiologicznym bakteryjnego zapalenia jest Streptococcus pyogenes. Za złoty standard diagnostyczny uchodzi badanie bakteriologiczne – posiew wymazu z gardła z antybiogramem. Klinicysta: Do zakażenia najczęściej dochodzi drogą kropelkową, okres inkubacji wynosi od 12 godzin do 4 dni. Rozpoznanie ustala się głównie na podstawie objawów: bólu gardła, gorączki, bólów głowy, złego samopoczucia, niewielkiego powiększenia węzłów chłonnych szyi. W badaniu przedmiotowym stwierdza się obrzękniętą i przekrwioną błonę śluzową gardła, czasami z nalotami. Postępowanie: Bakteriolog: Prawidłowe pobranie materiału do badania bakteriologicznego ma zasadniczy wpływ na całość procesu diagnostycznego. Wymaz pobiera się z powierzchni lub krypt migdałków. Pobrany wymaz jest posiewany i inkubowany w temperaturze 35–37°C. Pierwszą ocenę przeprowadza się po 18–24 godzinach. W ambulatorium pomocne mogą być szybkie testy diagnostyczne (RADT) wykrywające antygen paciorkowcowy. Klinicysta: Kluczowe jest ograniczenie szerzenia się zakażenia (wyeliminowanie kontaktu), leczenie objawowe. W OZGM o etiologii S. pyogenes lekiem pierwszego rzutu jest fenoksymetylopenicylina (p.o. lub i.v.). W przypadku nadwrażliwości należy zastosować makrolidy. Azytromycyna: Bakteriolog: To makrolid, obejmujący w spektrum działania najczęstsze patogeny wywołujące OZGM. Klinicysta: Skuteczny lek w OZGM, stosowany w dawce: u dzieci 10 mg/kg mc. raz na dobę przez 3 dni, u dorosłych 1×500 mg przez 3 dni. Ostatnie badania sugerują lepsze działanie podwójnej dawki antybiotyku (dawka kumulacyjna – 20 mg/kg/raz na dobę przez 3 dni). Podsumowując, należy stwierdzić, że te dwa spojrzenia – klinicysty i bakteriologa – uzupełniają się.
Desmoplastic small round cell tumor (DSRCT) is an extremely rare malignancy rated among the sarcomas. It was first described in 1989. As there are only a few hundred cases reported in world literature, to date there are no uniform standards of management of patients presenting with such a diagnosis. This malignancy is characterized by a extremely aggressive clinical course. The disease affects mainly young males in their second and third decade. Most often described primary location of DSRCT is abdominal cavity. Identified cytogenetic marker of DSRCT is translocation t(11;22)(p13q12). Therapy relies in surgery, radiotherapy and chemotherapy. In spite of continuous progress in the treatment of DSRCT, prognosis in this group of patients is still very poor. High propensity to dissemination already at the time of first diagnosis considerably reduces the effectiveness of surgical excision and all patients require systemic treatment. Chemotherapy usually resorts to cyclophosphamide, doxorubicin, vincristine, ifosfamide and etoposide (their combination is known as “protocol P6”). Other cytostatics of confirmed effectiveness in the treatment of DSRCT are cisplatin, carboplatin, topotecan, vinorelbine and irinotecan. In spite of clear clinical activity of chemotherapeutics, eventually all patients develop resistance to implemented treatment and suffer disease progression. Therefore, selection of optimal treatment for this group of patients remains a considerable clinical challenge.

Desmoplastic small round cell tumor (DSRCT) jest bardzo rzadkim nowotworem zaliczanym do rodziny mięsaków. Po raz pierwszy został opisany w 1989 roku. Ponieważ w światowej literaturze przedstawiono dotychczas kilkaset przypadków, do chwili obecnej nie ma jednoznacznych standardów postępowania z pacjentami z tym rozpoznaniem. Nowotwór ten charakteryzuje się bardzo agresywnym przebiegiem. Choroba dotyka głównie młodych mężczyzn. Szczyt zachorowań przypada na drugą i trzecią dekadę życia. Najczęściej opisywaną pierwotną lokalizacją DSRCT jest jama brzuszna. Zidentyfikowanym cytogenetycznym markerem DSRCT jest translokacja t(11;22)(p13q12). W leczeniu ogromną rolę odgrywają chirurgia, radioterapia i chemioterapia. Pomimo ciągłego postępu w leczeniu DSRCT rokowanie w tej grupie chorych nadal pozostaje bardzo złe. Duża skłonność do rozsiewu choroby już w chwili jej rozpoznania ogranicza skuteczność postępowania chirurgicznego, dlatego wszyscy pacjenci wymagają leczenia systemowego. W chemioterapii wykorzystuje się najczęściej cyklofosfamid, doksorubicynę, winkrystynę, ifosfamid i etopozyd (kombinacja tych leków znana jest pod nazwą protokołu P6). Innymi cytostatykami wykazującymi aktywność w leczeniu DSRCT są cisplatyna, karboplatyna, topotekan, winorelbina i irinotekan. Mimo wyraźnej aktywności klinicznej chemioterapeutyków praktycznie u wszystkich chorych po pewnym czasie rozwija się oporność na stosowane leczenie i dochodzi do progresji choroby. Z tego powodu dużym wyzwaniem staje się wybór optymalnego leczenia dla tej grupy pacjentów.
Asperger’s syndrome is a neurodevelopmental disorder from the group of autistic spectrum disorders. Objective: The authors describe the group of children diagnosed with Asperger’s syndrome, treated in the Department of Child Psychiatry or in the Child Psychiatry Outpatient Clinic. The aim of the study was to evaluate the patients’ age when they were diagnosed and the reason for seeking the clinician’s help by parents. Method: A retrospective chart review of children (n = 35) diagnosed with Asperger’s syndrome. Age and reason for the first contact with the specialist, number of health facilities where the patient was treated before diagnosis, age at which the diagnosis was made, the period between the patient’s first contact with the specialist and diagnosis, and the first diagnoses in hospitalized children were analysed. Results: The median age of diagnosis was 11.75 ± 3.29 years, 5 ± 3.44 years after the children’s first contact with the specialist (psychiatrist or psychologist). Before the diagnosis, the patients were treated in different health facilities and often diagnosed with attention deficit/hyperactivity disorder or disruptive behaviour. Conclusions: The results may indicate the major diagnostic difficulties which clinicians are facing or may be the result of insufficient knowledge among various groups of specialists on the Asperger’s syndrome. The results were compared to the results of the research conducted in the USA and Great Britain.

Zespół Aspergera to zaburzenie neurorozwojowe z grupy całościowych zaburzeń rozwoju. Cel badania: Celem pracy było określenie, w jakim wieku w grupie pacjentów z zespołem Aspergera będących pod opieką oddziału psychiatrii dziecięcej lub pozostających pod opieką w poradni zdrowia psychicznego zostało ustalone rozpoznanie oraz jakie objawy były powodem zgłoszenia się do specjalisty. Metoda: W pracy przedstawiono wyniki analiz historii chorób 35 pacjentów z zespołem Aspergera. Analizy dotyczyły wieku oraz powodu pierwszego kontaktu ze specjalistą, liczby placówek, w których chory był diagnozowany, zanim rozpoznano zaburzenie, oraz wieku postawienia diagnozy. Określono, jaki czas upłynął pomiędzy zgłoszeniem się pacjenta po raz pierwszy do specjalisty a diagnozą, a także jakie były rozpoznania wstępne u dzieci hospitalizowanych na oddziale psychiatrii. Wyniki: Na podstawie uzyskanych rezultatów ustalono, że rozpoznanie zespołu Aspergera zostało ustalone w wieku 11,75 ± 3,29 roku, a od pierwszej wizyty u specjalisty (lekarz, psycholog) upłynęło 5 lat ± 3,44 roku. Pacjenci byli wcześniej diagnozowani w różnych ośrodkach i otrzymywali najczęściej rozpoznanie nadpobudliwości psychoruchowej z deficytem uwagi lub zaburzeń zachowania. Wnioski: Wyniki niniejszego badania mogą wskazywać na duże trudności diagnostyczne, które mogą być wynikiem zbyt małej wiedzy specjalistów na temat zespołu Aspergera. Uzyskane rezultaty zostały porównane z wynikami badań przeprowadzonych między innymi w Stanach Zjednoczonych i Wielkiej Brytanii.
Background. Imbalance between training load and body recovery leads to the development of the non-functional overreaching syndrome (NFO) or overtraining syndrome (OTS) and deterioration of athlete’s performance. The aim of the study was to determine the prevalence of these syndromes in a sample of athletes. Material and methods. 111 males aged 18-35 years, training long distance running, cycling, triathlon, powerlifting, crossfit, bodybuilding and weightlifting minimum 2 years. The medical assessment was combined with history taking on athletic career and performance. Aerobic capacity was assessed by means of ergospirometry – cardiopulmonary exercise test (CPET) on a mechanical treadmill and anaerobic capacity was measured using Wingate test on a cycle ergometer. The patients with inflammatory conditions, hematological disorders and metabolic disorders were excluded. Anthropometric measurements were taken. Overreaching level and motivation for training were assessed using Profile of Mood State (POMS) questionnaire. The results were next compared with the reference values. Statistical analysis was performed using Statistica program, v.13.0. Results. Based on POMS results and the changes in physical capacity a group of 26 participants was selected (23.4%) with NFO/OTS syndrome symptoms. These were athletes training long distance running, crossfit and bodybuilding within 73 months and those who started in competitions more often than 20 times a year. Statistically significant differences were noted in athletes with NFO/OTS syndromes in mood assessment using POMS questionnaire. Higher values were noted in the following subscales: tension/anxiety, depression and fatigue while lower values were obtained in vigor/ activity subscale. Lower yet statistically insignificant values corresponding to physical anaerobic and anaerobic activity were observed in this group. The amount of sleep and the number of training sessions had no effect on the results. Conclusions. NFO/OTS syndromes are developed in nearly 25% of athletes involved in various sport disciplines, training within about 73 months and starting in competitions 20 times a year on average. POMS questionnaire turned out a useful tool in diagnosing these syndromes.

Wstęp. Zaburzenie równowagi pomiędzy obciążeniem treningowym a regeneracją organizmu prowadzi do rozwinięcia Zespołu niefunkcjonalnego przemęczenia (NFO) lub Zespołu przetrenowania (OTS) i pogorszenia wyniku sportowego. Celem badania jest określenie częstości występowania tych zespołów w grupie sportowców. Materiał i metody. Przebadano 111 mężczyzn w wieku 18-35 lat trenujących min. 2 lata: biegi długie, kolarstwo, triathlon, trójbój siłowy, crossfit, kulturystykę, podnoszenie ciężarów. Przeprowadzono badanie lekarskie z wywiadem sportowym. Oceniano wydolność fizyczną tlenową – badanie ergospirometryczne (CPET) na bieżni mechanicznej oraz wydolność beztlenową podczas testu Wingate na cykloergometrze. Wykluczono stany zapalne, zaburzenia hematologiczne i metaboliczne. Dokonano pomiarów antropometrycznych. Poziom przemęczenia oraz motywację do treningu oceniano kwestionariuszem Oceny profilu nastrojów- POMS (Profile of Mood State)- wyniki odniesiono do wartości referencyjnych. Materiał poddano analizie w programie Statistica v. 13.0. Wyniki. Na podstawie wyników POMS i zmian w wydolności fizycznej wyodrębniono grupę 26 (23,4%) osób z cechami zespołu NFO/OTS. Byli to sportowcy ze średnim stażem treningowym 73 miesiące trenujący: biegi długie, crossfit i kulturystykę, startujący w zawodach częściej niż 20 razy w roku. Obserwowano istotne statystycznie różnice u sportowców z cechami zespołów NFO/OTS w ocenie nastroju POMS: wartości wyższe w subskalach: napięcie/niepokój, depresja, zmęczenie oraz niższe w subskali krzepkość/aktywność. Obserwowano mniejsze wartości wydolności fizycznej tlenowej i beztlenowej w tej grupie, jednak bez istotności statystycznej. Ilość snu, czas trwania i liczba treningów nie wpływały na wyniki badań. Wnioski. Wystąpienie NFO/OTS dotyczy blisko 25% sportowców różnych dyscyplin sportowych, o średnim stażu treningowym ok. 73 miesiące, startujących w zawodach średnio 20 razy w roku. Kwestionariusz oceny nastrojów POMS okazał się przydatnym narzędziem do diagnostyki tych zespołów.
