The side effects following thoracic sympathectomy for primary hyperhidrosis include pain and compensatory/ reflex sweating. The aim of the study was the evaluation of the results of the endoscopic sympathicotomy with clips with emphasis on the frequency of side effects following the operation. Material and methods. Two-hundred-eighty-three patients were qualified to thoracic T3-T4 sympathicotomy with clips. In all cases bilateral procedure in prone position with CO2 insufflation was performed. The subjective intensity of disease was evaluated by VAS scale (0 - no sweating; 10 - maximal possible sweating) while the recurrence of the sweating in primary localization, intensity and dynamics of compensatory and plantar sweating were evaluated post-operatively. Follow-up data were obtained during office visits 3, 12 and 36 months after surgery. The overall follow-up response was 74.6%. Results. There was no mortality. Perioperative morbidity included 6 cases of pneumothorax. The mean duration of surgery was 57 minutes bilaterally. The postoperative intercostal pain was present in all patients (100%) with mean duration of 21.88 days but in 72.6% of cases it did not demand any medication as early as 48 hours after surgery. Strong or very strong compensatory sweating was observed in 17.5% of cases 3 months after ETS, in 14.1% after 12 months and in 23.6% after 36 months. Conclusions. Thoracic sympathicotomy with clips is a safe treatment that provides satisfactory longterm results. The incidence of side-effects (intercostal pain, compensatory sweating) is high and does not change with time in most of the cases.
Past years have been marked with an increased interest in invasive treatment of acute and chronic pain. Anaesthesiologists when treating intra and postoperative pain use these methods most often. In chronic pain their use is less frequent. Depending on method used to perform the blockade it can have short or long lasting analgetic effect. Local anaesthetics are used with or without adjuvants such as clonidine, opioids, and adrenaline. In case of long-term blockade chemical substances, thermal and surgical methods are used to disrupt nervous structures. Unfortunately the scientific research has not been extensive in this field and that is the reason for them not to be popular. Invasive pain treatment is used in cases of failure of conservative methods or where side effects of the treatment were unacceptable. Presently these methods are more often required to support the pharmacological treatment in every stage of illness so they should be recognized as alternative or adjuvant methods in analgetic ladder. Besides analgetic effect short-lasting blockades are used for diagnostic purposes allowing localizing the place responsible for triggering pain experience as well as prognostic before planed long-lasting blockade. In chronic pain short lasting blocks should be avoided except for diagnostic purposes. Before application of any blockade we should consider if the benefit of the method would be greater than potential side effects.

W ostatnich latach zainteresowanie zabiegowymi metodami leczenia bólu ostrego i przewlekłego znacząco wzrosło. Stosowane są one najczęściej przez anestezjologów w leczeniu bólu związanego z zabiegami operacyjnymi. W przypadku bólu przewlekłego metody zabiegowe wykorzystywane są znacznie rzadziej. W zależności od użytego środka blokady mogą mieć działanie krótko- lub długotrwałe. Do wykonania blokad krótkotrwałych używa się leków znieczulenia miejscowego, z ewentualnym dodatkiem leków wspomagających, takich jak adrenalina, opioidy, klonidyna. W przypadku blokad długotrwałych wykorzystywane są metody chemiczne, termiczne lub chirurgiczne mające na celu uszkodzenie struktur nerwowych. Niestety, ze względu na niewielką liczbę dobrze opracowanych badań naukowych nie cieszą się one popularnością. Inwazyjne leczenie bólu, poza wybranymi przypadkami, było wprowadzane, gdy metody zachowawcze nie przyniosły pożądanego złagodzenia bólu lub gdy skutki uboczne nie były akceptowane. Obecnie przeważa pogląd, że powinny one wspomagać leczenie farmakologiczne na każdym etapie choroby. Uznaje się je za alternatywne lub adiuwantowe metody leczenia w drabinie analgetycznej. Oprócz efektu przeciwbólowego blokady krótkotrwałe wykorzystywane są w celach diagnostycznych (pozwalają zlokalizować miejsce odpowiedzialne za wywołanie doznań bólowych) oraz prognostycznych - przed planowaną blokadą długotrwałą. W bólach przewlekłych należy unikać blokad przynoszących krótkotrwałe złagodzenie dolegliwości, z wyjątkiem blokad diagnostycznych. Przed wykonaniem jakiejkolwiek blokady należy rozważyć, czy wynikające z niej korzyści będą większe od potencjalnych niepożądanych skutków ubocznych.
