Metatarsalgia – painful forefoot – is a term that refers to all metatarsal changes that have anatomical or biomechanical causes. The forefoot (the area extending between the metatarsophalangeal joint line and the tarsometatarsal line) is the most frequent place of pain symptoms within the foot. This is an area where there may be several symptoms related to each other and occurring independently of each other – from more superficial skin lesions through inflammation of soft tissues to pain caused by bone damage of the metatarsus or changes in the joints of the area and restrictions in the function of the foot. This is one of the most difficult diagnostic areas due to the multitude of conditions that can occur here – from trivial problems that do not require complicated treatment, changes requiring orthopedic provision, oncological treatment, and in some cases amputations as well. Effective treatment of a patient with metatarsalgia requires a proper diagnosis and thus cooperation of the entire therapeutic team.

Metatarsalgia – bolesność przodostopia – to określenie, które odnosi się do wszystkich zmian w obrębie śródstopia mających przyczyny anatomiczne lub/i biomechaniczne. Przodostopie (obszar rozciągający się między linią stawów śródstopno-paliczkowych a linią stawów stępowo-śródstopnych) to najczęstsze miejsce występowania objawów bólowych w obrębie stopy. Jest to obszar, w którym może występować kilka objawów powiązanych ze sobą, jak i występujących niezależnie od siebie – od bardziej powierzchownych zmian skórnych, przez stany zapalne tkanek miękkich, po bóle wywołane uszkodzeniami kości śródstopia lub zmianami w obrębie stawów tej okolicy i ograniczenia funkcji stopy. To jeden z trudniejszych obszarów diagnostycznych z powodu mnogości mogących tu wystąpić schorzeń – od problemów błahych, niewymagających skomplikowanego leczenia, po zmiany wymagające zaopatrzenia ortopedycznego, leczenia onkologicznego, a w niektórych przypadkach nawet amputacji stopy. Skuteczne leczenie pacjenta z metatarsalgią wymaga postawienia trafnej diagnozy, a co za tym idzie często kooperacji całego zespołu terapeutycznego.
The human foot is an important static and dynamic part of the motion organ. On the one hand it is a supporting element and in static conditions enables one to balance the body in a spatial position. On the other hand it is a driving mechanism of the body which gives it propulsion during movement. Aims: to evaluate the correlations between the longitudinal and transverse arch of the foot and the frontal support area of the foot in academic students. A cross- sectional study was carried out in 2008-2009 in a group of 280 academic students aged 20-28 randomly selected from The University of Rzeszów and Beskidy Higher School of Skills in Żywiec. The method used in the study was plantographic evaluation of the feet in static conditions. The prints of the feet were obtained by means of the non- stain technique invented by Ślężyński. In order to evaluate the correlations between the longitudinal and transverse arch of the foot and the frontal support area the Pearson's linear correlation was employed. Results: slight correlations between the longitudinal depth index (W w ) and the heel angle were observed in the examined group of women. There were no correlations between the longitudinal and transverse arch and the frontal support area observed in men. Conclusions: the longitudinal arch shows no correlations with the transverse arch and the the frontal support in the examined students aged 20-28.
Dermatological lesions caused by deformation of the lesser toes are some of the reasons why patients seek specialist help. The most common lesions include calluses under the toe tips, thickened and cracking nails, as well as lesions in the interdigital spaces. In addition to the local treatment of dermatological lesions, it is also worth treating the cause of the lesions with the help of a physiotherapist, who using appropriate methods (kinesiology taping, individual orthoses) and techniques (manual therapy) will relieve the foot. In the case of lesions resistant to conservative treatment, orthopaedic consultation is necessary in order to consider surgical correction of the defect and, as a result, eliminate the cause of the problem.

Zmiany dermatologiczne spowodowane deformacjami palców mniejszych są jednym z powodów, dla których pacjenci szukają pomocy specjalisty. Do najczęstszych zmian należą odciski na opuszkach palców, pogrubiałe i pękające paznokcie oraz uszkodzenia w przestrzeniach międzypalcowych. Poza miejscowym leczeniem zmian dermatologicznych warto też zająć się leczeniem przyczyny zmian z pomocą fizjoterapeuty, który stosując odpowiednie metody (kinesiology taping ortozy indywidualne) i techniki (terapia manualna) odciąży stopę. W przypadku zmian niepoddających się leczeniu zachowawczemu konieczna jest konsultacja ortopedyczna w celu rozważenia chirurgicznej korekcji wady, a co za tym idzie zlikwidowania przyczyny problemu.
Wstęp. Celem badania było zebranie aktualnych kierunków badań w futbolu amerykańskim dotyczących stawów skokowych i stopy. Celem szczegółowym było określenie w jaki sposób futbol amerykańskiego wpływa na stawy skokowe i stopę, tj. częstość, przyczyny i rodzaje urazów oraz powiązań z obuwiem na niewydolność tych struktur. Materiał i metody. Do tego badania włączono 26 publikacji, głównie z bazy PubMed i powiązanych, spełniających kryteria wyszukiwania. Wyniki. W wyniku przeglądu publikacji wykazano częstość występowania urazów stawów skokowych i stopy na tle kończyn dolnych oraz ich rodzaje i warunki predysponujące do urazów. Analizowane prace skupiają się przede wszystkim na bazach nadzoru/ przeciwdziałaniu urazu oraz badaniach wpływu obuwia na stopę i staw skokowy. Pojawiają się prace wykazujące związek urazów stawów skokowych i stopy z równowagą. Dodatkowo niektóre prace dotyczą przebytych operacji, czasu wyłączenia z gry, stosowania usztywnień w celu zabezpieczenia zawodników przed przeciążeniami i uszkodzeniami wymienionych struktur. Wnioski. Badania zwracają uwagę na problem urazowości stóp i stawów skokowych oraz wykazują, iż nadal powinny być ulepszane programy profilaktyczne, treningowe i fizjoterapeutyczne w związku z dużą ich urazowością. Istnieje potrzeba większej ilości badań klinicznych, aby znaleźć szczegółowe przyczyny urazów i skutecznie im zapobiegać.

Background. The purpose of this study was to collect current research lines in American football regarding the ankles and the foot. The specific goal was to determine how American football affects the ankle joints and the foot, i.e. the frequency, causes and types of injuries as well as various links between the footwear and failure of these structures. Material and methods. The study included 26 publications, drawn mainly from PubMed and related databases, meeting the search criteria. Results. As a result of the review of the publications, the frequency and types of ankle and foot injuries with regard to lower limbs were discussed along with the conditions predisposing for such injuries. Current works focus primarily on injury surveillance databases and on the research on the impact of footwear on the foot and ankle joint. There are works showing the relationship between ankle and foot injuries on the one hand, and the balance on the other. Additionally, some of the articles deal with the past treatments, the time of exclusion from the game and the use of stiffeners to protect the players against overloads and damage to the above-mentioned structures. Conclusions. Our research draws attention to the problem of foot and ankle injuries and shows that preventive, training and physiotherapeutic programs should still be improved due to their high level of trauma. More clinical study is needed to find the specific causes of injuries and prevent them effectively.
The tibialis posterior muscle plays an important role in maintaining the correct arching of the foot in the standing position and while walking, allowing proper arching, rolling and shock absorption of the foot. It works with the peroneus longus muscle, forming the so-called tendon stirrup of the foot. Changes in the distal attachment of the tibialis posterior muscle o the navicular bone can cause pain symptoms on the inner edge of the foot called enthesopathy. Enthesopathy is a painful lesion at the attachment of the muscle tendon to the bone. It is most often the result of excessive loads and disturbances in the biomechanics of a muscle, which leads to the formation of inflammation accompanied by pain located at the attachment site and deterioration of the muscle function. Treatment of the above diseases is not described in the literature; hence there are no clear guidelines for conducting medical treatment. The aim of the study is to assess the effect of applying Kinesio Taping in combination with the transverse massage of the distal tendon attachment on the static and dynamic parameters of the foot.

Mięsień piszczelowy tylny odgrywa ważną rolę w utrzymaniu prawidłowego wysklepienia stopy w pozycji stojącej oraz podczas chodu, dodatkowo umożliwiając prawidłowe przetoczenie oraz amortyzowanie stopy. Współpracuje z mięśniem strzałkowym długim, współtworząc tzw. strzemię ścięgniste stopy. Zmiany w obrębie przyczepu dystalnego mięśnia piszczelowego tylnego (do kości łódkowatej) mogą być przyczyną objawów bólowych wewnętrznej krawędzi stopy, zwanych entezopatią. Entezopatia jest bolesną zmianą chorobową w obrębie przyczepu ścięgna mięśniowego do kości. Najczęściej jest wynikiem działania nadmiernych obciążeń i zaburzeń działania mięśnia, co prowadzi do stanów zapalnych, którym towarzyszy ból zlokalizowany w miejscu przyczepu, oraz pogorszenia funkcji mięśnia. Leczenie schorzenia nie zostało jeszcze opisane, stąd brak jasnych wytycznych co do leczenia zachowawczego. Celem pracy była ocena wpływu kinesiotapingu (Kinesio Taping) w połączeniu z masażem poprzecznym przyczepu dystalnego ścięgna na pa-rametry statyczne oraz dynamiczne stopy.
Wstęp: Ocena prawidłowego ukształtowania stóp ma bardzo istotne znaczenie diagnostyczne. Wszelkie odchylenia w ich budowie mają odzwierciedlenie w patogenezie schorzeń w obrębie innych części ciała. Istnieje wiele narzędzi do oceny stóp, jednakże wyniki uzyskiwane różnymi metodami nie zawsze są porównywalne. Foot Posture Index (FPI–6) został opracowany jako szybkie, łatwe w użyciu i rzetelne narzędzie służące do oceny stóp w różnych zastosowaniach klinicznych.Cel pracy: Oznaczenie współczynnika powtarzalności pomiarowej dla kwestionariusza FPI-6 zastosowanego do badania stóp u dzieci i młodzieży.Materiał i Metoda: W badaniach wzięło udział 60 osób w trzech grupach wiekowych 9, 12 i 14 lat. W celu określenia powtarzalności pomiarowej FPI-6 wszystkie pomiary przeprowadzono dwukrotnie w 2 tygodniowym odstępie czasowym. Oceniano następujące parametry: wysokość ciała, masa ciała, BMI (Body Mass Index), oraz wykonano ocenę stóp za pomocą FPI-6. Wyniki: Wartość współczynnika powtarzalności R pomiędzy pierwszym i drugim badaniem, dla wszystkich parametrów wynosił 0.89-0.96, co oznacza bardzo dobrą powtarzalność FPI -6 w ocenie stopy. Wysokie wartości współczynnika powtarzalności występują niezależnie od wieku dziecka, wysokości i masy ciała. Zarówno ocena koślawości i szpotawości kości piętowej jak i ocena przywiedzenia /odwiedzenia przodostopia względem kości piętowej w grupach dzieci dziewięcioletnich i dwunastoletnich uzyskały wartości mówiące o lekkiej pronacji stopy, a w grupie dzieci czternastoletnich parametry te ulegały statystycznie istotnej zmianie uzyskując wartości charakterystyczne dla neutralnego ustawienia stopy.Wnioski: FPI-6 jest prostym i rzetelnym narzędziem oceny stopy u dzieci i młodzieży, charakteryzującym się wysokim współczynnikiem zgodności pomiarów powtarzanych przez tego samego badającego.

Background: The evaluation of proper foot posture is of importance in diagnosis. All abnormalities in the foot structure may reflect the pathogenesis of the dysfunctions presented at other parts of the body. There are many diagnostic devices for foot assessment, but the results obtained using different methods are often not comparable. Foot Posture Index (FPI-6) was created as a quick, simple and reliable tool for quantifying foot posture in a variety of clinical settings.Aim: The evaluation of foot posture in 3 age groups of children using FPI-6 and the assessment of the reliability of this methodMaterial and Methods: 60 subjects in 3 age groups 9, 12 and 14 years participated in the study. For the purpose of FPI-6 reliability level determination all measurements were taken twice, with a 2-week gap. The following variables were evaluated: body height, body mass, BMI (Body Mass Index), the foot assessment using FPI-6.Results: The correlation coefficient R between the first and second measurement for all criterion was 0.89-0.96, what indicates a very good FPI-6 reliability in the foot assessment. Both the inversion/eversion of the calcaneus and abduction/adduction of the forefoot on the rear foot in the groups of 9- and 12-year-old children indicated mild foot pronation, and those parameters changed significantly in 14-year-old children where values were reached typical for a neutral foot.Conclusion: FPI-6 it is a simple and reliable tool for comprehensive foot evaluation, which allows one to use it both in clinical practice as in preventive population based studies.
The main idea of this article is the understanding of the anatomical work of foot rocker mechanisms during gait phases together with the muscle work of the hip, knee, ankle and foot being collectively responsible for the proper positioning of the joints in the lower limb. There are three rockers in a human’s foot: the heel rocker, the ankle and forefoot rocker. The names given to each of them are in accordance with their location in the foot. The term rocker is used to describe the process of the rolling of one element in a foot over another. The existing evidence suggests that analyzing foot work and its disorders in gait phases is crucial in planning the treatment of patients with walking problems, as it is an element of the cyclic opening and closing of the biomechanical chain. This analysis should go together with the awareness of the fact that proper gait is the result of combined work of the hip, knee and foot and demands a knowledge of the correlation between the tibia, calcaneus, metatarsum and forefoot. The information given helps one to evaluate trials of the ankle and foot and to assess the methodological quality of treatment in close chains in opposition to those in open chains, which brings fewer effects - hence stressing the need to widen the terminology of therapists working on gait problems. A knowledge of the ‘rocker mechanism’ can be an indicator in treatment progress, rehabilitation documentation as well as in the choice of exercises and the introduction of abilities acquired during therapy to daily functioning.
