Irritable bowel syndrome (IBS) is a functional gastrointestinal disorder of a very complex etiopathogenesis. There are three clinical types of this syndrome: with constipation, with diarrhea and mixed, with alternating constipation and diarrhea. Etiopathogenetic factors include, among others, genetic, environmental and inflammatory predispositions, chronic stress and depression. Therefore, it is contemplated whether the disease affects the digestive or the nervous system. Both, in the central nervous system and in the gastrointestinal tract, there are many same receptors and neurotransmitters involved in the regulation of their activities. Among them, serotonin plays an important role. The main function of serotonin is the regulation of gastrointestinal motility and secretion. L-tryptophan, exogenous amino acid, is its precursor. The deficiency or the excess of both the compounds and disorders in their metabolism affect the abnormal bowel function. Thus, taking this into account, a proper diet should not be forgotten in the treatment of irritable bowel syndrome. It has been demonstrated that patients with diarrhea-predominant IBS have a higher blood serotonin level compared to healthy subjects. No such difference was observed in constipation-predominant IBS. Furthermore, patients with constipationpredominant disease demonstrated lower levels of serotonin metabolite – 5-hydroxy-3-indoleacetic acid (5-HIAA) in urine. Patients with higher postprandial blood levels of serotonin feel discomfort after a meal. This confirms the validity of the use of selective serotonin reuptake inhibitors in the treatment of patients with constipation-predominant IBS, as the availability of this neurotransmitter is increased in the body, which stimulates peristalsis. Thus, it may be reasonable to recommend patients with constipationpredominant IBS a diet rich in tryptophan-containing foods, whereas patients with diarrhea-predominant IBS should avoid their consumption.

Zespół jelita nadwrażliwego (IBS) jest czynnościową chorobą przewodu pokarmowego o bardzo złożonej etiopatogenezie. Wyróżnia się trzy postacie kliniczne tego zespołu: zaparciową, biegunkową i mieszaną. Wśród czynników etiopatogenetycznych wymienia się m. in. predyspozycje genetyczne, środowiskowe, zapalne, przewlekły stres i depresję. Dlatego rozważa się, czy choroba dotyczy układu pokarmowego czy nerwowego. Zarówno w ośrodkowym układzie nerwowym, jak i w układzie pokarmowym występuje wiele takich samych receptorów i neuroprzekaźników, uczestniczących w regulacji czynności tych układów. Wśród nich ważną rolę odgrywa serotonina. Do głównych funkcji serotoniny należy udział w regulacji czynności motorycznej i wydzielniczej przewodu pokarmowego. Jej prekursorem jest egzogenny aminokwas L-tryptofan. Niedobór lub nadmiar obu związków oraz zaburzenia ich metabolizmu mają wpływ na nieprawidłowe funkcjonowanie jelit. Biorąc to pod uwagę, w leczeniu zespołu jelita nadwrażliwego nie powinno się zatem zapominać o odpowiedniej diecie. Wykazano, że pacjenci z biegunkową postacią IBS mają, w porównaniu ze zdrowymi osobami, wyższe stężenie serotoniny we krwi. Różnicy takiej nie zaobserwowano w postaci zaparciowej jelita nadwrażliwego. Dodatkowo, u pacjentów z typem zaparciowym choroby obserwowano niższe stężenie metabolitu serotoniny – kwasu 5-hydroksy-3- indolooctowego w moczu. Pacjenci z wyższym poposiłkowym stężeniem serotoniny we krwi odczuwają dolegliwości po spożyciu posiłku. Potwierdza to zasadność stosowania selektywnych inhibitorów zwrotnego wychwytu serotoniny w leczeniu pacjentów z zaparciową postacią zespołu jelita nadwrażliwego. Zwiększa się wówczas dostępność serotoniny w organizmie, co pobudza perystaltykę jelit. Zasadne może być zatem zalecenie chorym z postacią zaparciową jelita nadwrażliwego spożywania produktów bogatych w tryptofan, podczas gdy pacjenci z postacią biegunkową powinni ograniczyć ich spożycie.
Sleep disorders occur in people who suffer from liver cirrhosis, this usually involves a change in the rhythm of melatonin secretion and its metabolism. Delayed sleep phase syndrome does not always correlate with the degree of liver damage, indicating the involvement of other factors in its pathogenesis. The aim of the study was to estimate the correlation between the night secretion of the serotonin and melatonin and the degree of sleep disorders. There were 60 patients with liver cirrhosis and 30 healthy subjects (control group) included in the study. Compared to the control group, in the first stage of hepatic encephalopathy (according to West Haven Scale) at 2 o’clock a.m. a low serum melatonin level was observed (57.5±10.2 pg/mL and 41.2±9.4 pg/mL, p<0.05) and even lower concentration of serotonin (171.2±45.0 and 108.4±29.3 μg/mL, p<0.01). These results negatively correlated with the degree of sleep disorders. The obtained results indicate that in patients with liver cirrhosis the changes in the homeostasis of both serotonin and melatonin occur, which can cause sleep disorders.

U osób z marskością wątroby występują zaburzenia snu, co zwykle wiąże się ze zmianą rytmu wydzielania melatoniny i jej metabolizmu. Zespół opóźnionej fazy snu nie zawsze koreluje ze stopniem uszkodzenia wątroby, co wskazuje na udział innych czynników w jego patogenezie. Celem badania było określenie zależności między nocnym wydzielaniem serotoniny i melatoniny a stopniem zaburzeń snu. Do badań włączono 60 osób z marskością wątroby (grupa badana) i 30 osób zdrowych (grupa kontrolna). W porównaniu do grupy kontrolnej, u chorych z pierwszym stopniem encefalopatii wątrobowej (wg skali West Haven) o godzinie 2:00 stwierdzono niższe stężenie melatoniny w surowicy (odpowiednio 57,5±10,2 pg/mL i 41,2±9,4 pg/mL, p<0,05), zaś u osób z drugim stopniem encefalopatii - niższe stężenie serotoniny (odpowiednio 171,2±45,0 i 108,4±29,3
Venlafaxine is a drug with at least two significant antidepressant mechanisms of action. Selective serotonin reuptake inhibition (5-HT) is observed even when low doses are used and noradrenalin reuptake inhibition can be seen when doses are higher. The cut-off point is usually a daily dose of 225 mg although a change from one mechanism of action into dual action is usually of spectral nature. It is possible that in daily doses higher than 375 mg, the drug exhibits dopaminergic effects, but such large doses are not recommended by the manufacturer. Using higher doses is associated with greater antidepressant efficacy. One should remember, however, that a change of dosage can be related to a different profile of adverse effects (which also depends on patient’s individual sensitivity). The serotoninergic effects are also perceived as sedating, causing excessive drowsiness or even apathy. The noradrenergic effects can, however, evoke the feeling of excessive agitation and sleep disorders. Moreover, the perception of a given action, as indifferent or troublesome to functioning, depends on the time of a day at which it occurs. The sleep-inducing action is perceived as positive by patients in the evening and agitation may be considered positive in the morning. It seems that the time of using venlafaxine may affect the subjective drug tolerance. The article presents two cases that represent this viewpoint. It is not entirely clear why the action of the drug is experienced as the strongest by patients soon after administration (a specific placebo effect cannot be excluded). Despite this, subjective venlafaxine tolerance is undoubtedly associated with the manner of administration. Physicians should take this into consideration.

Wenlafaksyna jest lekiem o co najmniej dwóch istotnych mechanizmach działania przeciwdepresyjnego. Hamowanie wychwytu zwrotnego serotoniny (5-HT) ujawnia się już podczas stosowania niskich dawek, a hamowanie wychwytu zwrotnego noradrenaliny – dopiero przy dawkach istotnie wyższych. Za punkt odcięcia przyjmuje się na ogół dawkę 225 mg na dobę, choć zmiana z jednego mechanizmu działania na dwa ma najprawdopodobniej charakter spektralny. Możliwe, że w dawkach powyżej 375 mg na dobę lek wykazuje również działanie dopaminergiczne, ale tak duże dawki wenlafaksyny nie są zalecane przez producenta. Stosowanie wyższych dawek przekłada się na większą skuteczność przeciwdepresyjną leku. Należy jednak pamiętać, że zmiana dawki może być związana z ujawnieniem się odmiennego profilu działań niepożądanych (choć zależy to także od indywidualnej wrażliwości pacjenta). Działanie serotoninergiczne bywa odbierane jako uspokajające, sedujące, powodujące nadmierną senność, a nawet zobojętnienie i apatię. Działanie noradrenergiczne może z kolei wywoływać uczucie nadmiernego pobudzenia i zaburzenia snu. To, czy dane działanie jest odczuwane jako niewpływające na funkcjonowanie, czy też jako trudne do zniesienia, zależy ponadto od pory dnia, w której występuje. Działanie nasenne pacjent uzna za korzystne wieczorem, natomiast działanie rozbudzające – rano. Wydaje się, że pora podawania wenlafaksyny może wpływać na subiektywną tolerancję leku. W artykule opisano dwie sytuacje ilustrujące ten punkt widzenia. Nie jest do końca jasne, dlaczego pacjenci odczuwają działanie leku najsilniej wkrótce po jego przyjęciu (nie da się wykluczyć swoistego efektu placebo). Mimo to w przypadku wielu chorych subiektywna tolerancja wenlafaksyny bez wątpienia ma związek ze sposobem podawania leku; lekarze powinni brać tę okoliczność pod uwagę.
BACKGROUND The central serotoninergic system is involved in numerous functions in the human body including mood, emotional and cognitive processes, the control of appetite, temperature regulation and nociception. In the latter case, we have to consider the interaction between serotonin and the opioid system which consists of endogenic peptides, i.e. endorphins, enkephalin and dynorphins together with specific receptors synthetized in various brain parts. The main goal of the present work was to examine the effect of neonatal central serotoninergic system lesions on the antinociceptive effect of morphine assessed in adult rats. METHODS On the 3rd day of postnatal life, male rats were administered 5.7-dihydroxytryptamine (5.7-DHT) (70 μg/10 μl). The rats continued to be housed until 8–10 weeks for further experimentation. The antinociceptive effects of morphine (2.5 mg/kg) were assessed by hot plate, tail immersion, paw withdrawal, formalin and writhing tests. Furthermore, the cerebral frontal cortex serotonin level and its metabolism was assayed. RESULTS It was demonstrated that the central serotoninergic lesion attenuated antinociceptive effects evoked by morphine injection assessed in hot plate, writhing and paw withdrawal tests, whereas it has no effect in tail immersion and formalin tests. Morphine accelerated serotonin metabolism in the cerebral frontal cortex in the control and lesioned rats. CONCLUSION Dysfunction of the central serotoninergic system may lead to a modified biological reaction to analgesic drugs (morphine).

WPROWADZENIE Ośrodkowy układ serotoninergiczny jest odpowiedzialny za regulację nastroju, procesów emocjonalnych, poznawczych, kontrolę apetytu, temperatury ciała oraz modulację doznań bólowych. W tym ostatnim przypadku jego rolę należy rozpatrywać w kontekście interakcji z układem opioidergicznym, na który składają się endogenne peptydy opioidowe, tj. endorfiny, enkefaliny i dynorfiny, wraz ze specyficznymi receptorami syntetyzowanymi w różnych częściach mózgu. Głównym celem pracy było zbadanie, jak zniszczenie ośrodkowego układu serotoninergicznego we wczesnym okresie życia pozapłodowego u szczurów wpływa na przeciwbólowe działanie morfiny oceniane u dorosłych zwierząt. METODY Lezję ośrodkowego układu serotoninergicznego wykonano u 3-dniowych noworodków szczurzych, stosując dokomorowo 5,7-dihydroksytryptaminę (5,7-DHT) w dawce 70 μg/10 μl. Właściwe testy behawioralne i biochemiczne wykonano po osiągnięciu przez zwierzęta 8–10 tygodnia życia. W ocenie działania przeciwbólowego morfiny (2,5 mg/kg) posłużono się testami gorącej płytki, immersji ogona, wycofania łapy, formalinowym oraz wicia. Ponadto zbadano wpływ lezji układu serotoninergicznego na zawartość i metabolizm serotoniny w korze mózgowej czołowej. WYNIKI Stwierdzono, że lezja ośrodkowego układu serotoninergicznego osłabia przeciwbólowe działanie morfiny w testach gorącej płytki, wicia oraz wycofania łapy, natomiast pozostaje bez wpływu w testach immersji ogona oraz formalinowym. Morfina nasilała metabolizm serotoniny w korze czołowej u szczurów kontrolnych oraz z lezją. WNIOSEK Dysfunkcja ośrodkowego układu serotoninergicznego może prowadzić do zmienionej reakcji biologicznej na leki przeciwbólowe (morfinę).
In postmenopausal women various psychosomatic disorders concerning mood and appetite occur. The reason is not only estrogen deficiency, but also other hormones and neurotransmitters. The aim of the study was to estimate serotonin and melatonin levels and myoelectrical activity and gastric motor in postmenopausal women in relation to their nutritional status. The study was conducted in three 30-person groups of women: premenopausal (group I), postmenopausal with a normal body mass (group II), postmenopausal overweight (group III). Compared with group I, in group II there were no significant differences, while in group III serotonin level was lower respectively 156.5±40.2 ng/ml and 83.4±32.5 ng/ml (p<0.01), as well as the percentage of normogastria – 82.9±5.6% and 66.9±8.2 (p<0.05) and gastric emptying half-time 43.6±14.7 min and 27.4±12.2 min (p<0.01). Moreover, a negative correlation between body mass index and serotonin (r = -0.4744) and melatonin (r = -0.7146) levels was observed. The study results indicate the involvement of serotonin and melatonin in the pathogenesis of eating disorders in postmenopausal women.

U kobiet po menopauzie występują różnorodne zaburzenia psychosomatyczne, w tym dotyczące nastroju i łaknienia. Przyczyną tego jest niedobór estrogenów, ale także innych hormonów i neurotransmiterów. Celem badania było określenie stężenia serotoniny i melatoniny oraz czynności mioelektrycznej i motorycznej żołądka u kobiet po menopauzie w odniesieniu do ich stanu odżywienia. Badania przeprowadzono w trzech 30-sto osobowych grupach kobiet: przed menopauzą (grupa I), po menopauzie z prawidłową masą ciała (grupa II), po menopauzie ze współistniejącą nadwagą (grupa III). W porównaniu z grupą I, w grupie II nie stwierdzono istotnych różnic, natomiast w grupie III niższe było stężenie serotoniny, odpowiednio 156,5±40,2 ng/mL i 83,4±32,5 ng/mL (p<0,01), a także niższy był odsetek prawidłowej czynności mioelektrycznej żołądka (normogastrii) – 82,9±5,6% i 66,9±8,2 (p<0,05) oraz krótszy był czas połowicznego opróżniania żołądka (43,6±14,7 min. i 27,4±12,2 min.; p<0,01. Ponadto stwierdzono odwrotną zależność między wskaźnikiem masy ciała a stężeniem serotoniny (r = -0,4744) i melatoniny (r = -0,7146). Wyniki badań wskazują na udział serotoniny i melatoniny w patogenezie zaburzeń odżywiania u kobiet po menopauzie.
