At present, sentinel lymph node biopsy is a standard procedure to assess the advancement of breast cancer and cutaneous melanoma. The aim of the study was to assess the role of the sentinel lymph node biopsy in the treatment of patients with breast cancer in our own material. Material and methods. Analyzed was medical documentation of 258 patients with initially operable breast cancer, qualified for operation with sentinel lymph node biopsy in 2004-2014 in the Department of Surgery of the 4th Military Teaching Hospital. A few hours prior to the planned surgery, radioisotope (technitium-99 sulfur colloid) was applied in the area of tumor or under the areola. 1-2 hours after administering the tracer, the lymphoscintigraphy with the labelling of the sentinel lymph node on the skin was performed. Results. On the basis of the gathered material, obtained were the following parameters: sensitivity – 100%, and specificity – 94.6%. Four cases were false negative (5.5%). Conclusions. 1. Marking the sentinel lymph node in breast cancer, based on the single visualisation method with the use of radioisotope, is a useful and effective technique. 2. The factor influencing the results of the sentinel lymph node biopsy (true positive and negative results and false negative result) was the number of the excised lymph nodes except for the sentinel lymph node. 3. Patients with estrogen receptor expression had often metastases to sentinel lymph node (145 cases – 56%). 4. The false negative rate, i.e. 5.5% in our material, is within the limits of acceptability given in the literature. 5. The sentinel lymph node biopsy performed by the experienced surgical team is a reliable diagnostic tool with a low complication rate.
Background: Previous studies have demonstrated that mastoscopic sentinel lymph node biopsy (MSLNB) has good identification rate (IR) and low false negative rate (FNR). However, few studies have directly compared the surgical performance and peri- and post-operative factors of MSLNB with conventional sentinel lymph node biopsy (SLNB). Methodology: Sixty patients diagnosed with breast cancer were recruited and randomly assigned to one of the three groups: MSLNB, SLNB and SLNB with lipolysis injection. Peri- and post-operative parameters were compared using general linear models. To examine the effect of age on these parameters, we performed separate analysis stratified by age (≤50 years old vs. >50 years old). Results: Patients in the MSLNB group experienced longer surgery and suffered higher surgical cost than patients who underwent conventional SLNB or SLNB with lipolysis injection (p<0.0001). Despite this, they had significantly less blood loss than those who underwent conventional SLNB (22.0±7.0 ml vs.73.5±39.6 ml; p<0.0001). Analysis by age group indicates a similar pattern of difference among the three groups. MSLNB and conventional SLNB have similar IR and FNR. Conclusion: As a minimally invasive technique, MSLNB can significantly reduce blood loss while providing similar IR and FNR, indicating that it can be a promising alternative to conventional SLNB. Conclusion: Variations in popliteal artery terminal branching pattern occurred in 7.4% to 17.6% of patients. Pre-surgical detection of these variations with MD CTA may help to reduce the risk of iatrogenic arterial injury by enabling a better surgical treatment plan.
Breast cancer is the most common malignancy and the leading cause of death due to cancer in European women. Mammography screening programs aimed to increase the detection of early cancer stages were implemented in numerous European countries. Recent data show a decrease in mortality due to breast cancer in many countries, particularly among young women. At the same time, the number of sentinel node biopsy procedures and breast-conserving surgeries has increased. Intraoperative sentinel lymph node biopsy preceded by lymphoscintigraphy is used in breast cancer patients with no clinical signs of lymph node metastasis. Due to the limited sensitivity and specificity of physical examination in detecting metastatic lesions, developing an appropriate diagnostic algorithm for the preoperative assessment of axillary lymph nodes seems to be a challenge. The importance of ultrasound in patient qualification for sentinel lymph-node biopsy has been discussed in a number of works. Furthermore, different lymphoscintigraphy protocols have been compared in the literature. The usefulness of novel radiopharmaceuticals as well as the methods of image acquisition in sentinel lymph node diagnostics have also been assessed. The aim of this article is to present, basing on current guidelines, literature data as well as our own experience, the diagnostic possibilities of axillary lymph node ultrasound in patient qualification for an appropriate treatment as well as the role of lymphoscintigraphy in sentinel lymph node biopsy.

Rak piersi jest najczęściej występującym nowotworem złośliwym i jednocześnie najczęstszą przyczyną zgonów z powodu chorób nowotworowych u kobiet w Europie. W kilkunastu krajach europejskich wprowadzano programy przesiewowych badań mammograficznych, mające przyczynić się do zwiększenia wykrywalności zmian nowotworowych o małym stopniu zaawansowania. Ostatnie dane wskazują, że w wielu z tych państw występuje spadek śmiertelności z powodu raka piersi, szczególnie w grupie kobiet młodych. Zwiększeniu ulega natomiast liczba procedur biopsji węzła wartowniczego i operacji oszczędzających. Śródoperacyjne pobranie węzła wartowniczego, poprzedzone badaniem limfoscyntygraficznym, stosuje się u chorych na raka piersi bez klinicznych cech przerzutów w węzłach chłonnych. Ze względu na ograniczoną czułość i swoistość badania fizykalnego w wykrywaniu tego typu zmian wyzwaniem wydaje się opracowanie właściwego algorytmu diagnostycznego w przedoperacyjnej ocenie węzłów chłonnych pachowych. Tematem szeregu prac jest znaczenie badania ultrasonograficznego w kwalifikacji pacjentek do biopsji węzła wartowniczego. W piśmiennictwie porównywane są także różne protokoły limfoscyntygrafii. Ocenie podlega też przydatność nowych radiofarmaceutyków oraz metod akwizycji obrazu w diagnostyce wartowniczych węzłów chłonnych. Celem artykułu jest przedstawienie – na podstawie aktualnych wytycznych, danych z piśmiennictwa oraz doświadczeń własnych autorów – możliwości diagnostycznych badania ultrasonograficznego węzłów chłonnych pachowych w kwalifikacji chorych do odpowiedniego leczenia oraz roli limfoscyntygrafii w procedurze biopsji węzła wartowniczego.
Sentinel lymph nodes are the first lymph nodes in the region that receive lymphatic drainage from a primary tumor. The detection or exclusion of sentinel lymph node micrometastases is critical in the staging of cancer, especially breast cancer and melanoma because it directly affects patient’s prognosis and surgical management. Currently, intraoperative sentinel lymph node biopsies using blue dye and radioisotopes are the method of choice for the detection of sentinel lymph node with high identification rate. In contrast, conventional ultrasound is not capable of detecting sentinel lymph nodes in most cases. Contrast enhanced ultrasound with contrast specific imaging modes has been used for the evaluation and diagnostic work-up of peripherally located suspected lymphadenopathy. The method allows for real-time analysis of all vascular phases and the visualization of intranodal focal “avascular” areas that represent necrosis or deposits of neoplastic cells. In recent years, a number of animal and human studies showed that contrast enhanced ultrasound can be also used for the detection of sentinel lymph node, and may become a potential application in clinical routine. Several contrast agents have been used in those studies, including albumin solution, hydroxyethylated starch, SonoVue®, Sonazoid® and Definity®. This review summarizes the current knowledge about the use of ultrasound techniques in detection and evaluation of sentinel lymph node.

Węzły wartownicze są pierwszymi węzłami chłonnymi na drodze spływu chłonki z okolicy guza pierwotnego. Wykluczenie lub potwierdzenie mikroprzerzutów w węźle wartowniczym odgrywa istotną rolę w ustalaniu stanu zaawansowania nowotworu, szczególnie nowotworu piersi i czerniaka, ponieważ bezpośrednio wpływa na rokowanie i wybór metody chirurgicznego leczenia. Obecnie śródoperacyjna biopsja węzłów wartowniczych z użyciem niebieskiego barwnika oraz radioizotopów są metodami z wyboru umożliwiającymi zidentyfikowanie węzłów wartowniczych. W przeciwieństwie do biopsji standardowe badanie ultrasonograficzne z reguły nie pozwala na ich uwidocznienie. Dzięki badaniu ultrasonograficznemu z użyciem środka kontrastującego oraz opcji obrazowania właściwych dla ultrasonografii ze środkami kontrastującymi można zdiagnozować położone obwodowo nieprawidłowe węzły chłonne. Metoda ta umożliwia ocenę węzłów w czasie rzeczywistym w każdej fazie naczyniowej, a także na uwidocznienie wewnątrzwęzłowych ogniskowych obszarów „nieunaczynionych”, które reprezentują obszary martwicy lub skupiska komórek nowotworowych. W ostatnich latach w wielu badaniach przeprowadzanych na zwierzętach i u ludzi wykazano, że badanie ultrasonograficzne z użyciem środka kontrastującego może uwidocznić węzeł wartowniczy i stać się rutynowym badaniem diagnostycznym. W badaniach tych zastosowano kilka środków kontrastujących, takich jak roztwór albuminy, skrobia hydroksyetylowana oraz SonoVue®, Sonazoid® i Definity®. Niniejsza praca stanowi podsumowanie obecnej wiedzy dotyczącej zastosowania ultrasonografii w diagnostyce węzłów chłonnych wartowniczych.
Breast cancer is one of the most common cancers to complicate pregnancy. Up until recently, such diagnosis was almost invariably bound with an abortion with all its possible psychological and ethical consequences for the patient, her family, and treating physicians. Throughout the past few years, the considerable progress in medical knowledge and experience has allowed a radical change in the situation of this very special patient group. These days, women may continue with the pregnancy, yet at the same time be effectively and safely treated, in a very close adherence to standards applied in the treatment of non-pregnant patients. The diagnostics of breast cancer in pregnant patients largely meets the generally accepted standards of detecting and staging the disease, apart from a few exceptions mostly related to fetus safety. The differences concern e.g. the sentinel lymph node evaluation procedure and some imaging methods. Whether the patient qualifies for treatment as well as therapeutic decisions depend on the gestational age and the advancement of the disease, with some treatment procedures imperatively postponed until postpartum. Certain chemotherapy regimens have been proved safe during pregnancy, even though up until recently they were considered toxic and impossible to use (FAC, FEC, taxoids). It has also been shown that, unless obstetric indications for caesarean section exist, in most cases the pregnancy does not need to be terminated prematurely. No oncologic counter-indications for a vaginal delivery exist either. This paper aims to bring together and sum up the body of knowledge arising from the recent publications which have facilitated diagnostic and therapeutic standards to be followed in these infrequent, yet exceedingly complex, clinical situations.

Rak piersi to jeden z najczęstszych nowotworów wikłających ciążę. Jeszcze niedawno takie rozpoznanie wiązało się prawie zawsze z aborcją, ze wszystkimi jej konsekwencjami psychologicznymi i etycznymi dla chorej, jej bliskich i lekarzy. W kilku ostatnich latach, wraz ze znacznym rozwojem wiedzy i doświadczenia, sytuacja tej szczególnej grupy pacjentek diametralnie się zmieniła: dzisiaj kobiety mogą zachować ciążę, a jednocześnie skutecznie i bezpiecznie się leczyć – w sposób bardzo zbliżony do leczenia stosowanego u pacjentek niebędących w ciąży. Diagnostyka raka piersi u ciężarnej pokrywa się w znacznej mierze z ogólnie obowiązującymi standardami wykrywania i oceny zaawansowania tej choroby, poza kilkoma odstępstwami, związanymi przede wszystkim z bezpieczeństwem płodu. Różnice dotyczą m.in. procedury oceny węzła wartowniczego i części badań obrazowych. Kwalifikacja do leczenia i decyzje terapeutyczne zależą od zaawansowania ciąży i zaawansowania choroby, a niektóre procedury lecznicze muszą zostać bezwzględnie odroczone na czas po porodzie. Udowodniono bezpieczeństwo stosowania w czasie ciąży wybranych schematów chemioterapii jeszcze do niedawna uważanych za toksyczne i niemożliwe do wykorzystania u ciężarnej (FAC, FEC, taksoidy). Wykazano również, że – o ile nie istnieją położnicze wskazania do cesarskiego cięcia – w większości przypadków ciąża nie musi być rozwiązywana przed terminem, podobnie jak nie ma onkologicznych przeciwwskazań do porodu siłami natury. Niniejsza praca podsumowuje wiedzę płynącą z publikacji, które ukazały się w ostatnim czasie i pozwoliły na opracowanie standardów postępowania diagnostyczno-terapeutycznego w tych nieczęstych, ale jednocześnie bardzo skomplikowanych sytuacjach klinicznych.
