Introduction. Tumor necrosis factor alpha, interferon gamma and interleukin-1 beta modulate the interaction between T-cells and macrophages and play the key role in immune defense in Mycobacterium tuberculosis infection. Several studies have demonstrated that corticosteroids may improve clinical conditions of patients with active pulmonary tuberculosis. Aim. The aim of the study was a potential impact of prednisone on the early immune response in HIV-negative young adults with active pulmonary tuberculosis, during the first four weeks of treatment. Material and methods. The study included 38 adults, aged 18-39 years, with active pulmonary tuberculosis. The first group of patients received only anti-tuberculosis chemotherapy whereas the second group of patients was administered antituberculosis chemotherapy and 20 mg prednisone, once daily. Serum levels of tumor necrosis factor-alpha, interferon-gamma and interleukin 1-beta were measured using ELISA kits before treatment initiation as well after two and four weeks of treatment, in both study groups. Results. The highest serum levels of evaluated cytokines were observed before treatment initiation. Serum levels of cytokines were significantly decreased in patients who received anti-tuberculosis drugs and prednisone, compare to those not treated with prednisone (p<0.001), in all study periods. Conclusions. Adjunctive therapy, like 20 mg prednisone daily, remarkably inhibited the secretion of inflammatory cytokines during the early stage of active pulmonary tuberculosis in HIV-negative young adults, which is likely to be beneficial for this group of patients
Bronchiectasis are irreversible, local extension of bronchi, usually accompanied by infection. In about 50% of cases its origin remains unknown. Cystic fibrosis is an inherited chronic disease caused by defective gene and it affects mostly the lungs and digestive system. Chlamydophila pneumoniae is a bacteria transmitted through droplets which can cause atypical pneumonia. Globally, 9.2 million new cases occurred in 2006, of which 7% million cases were in HIV-positive people. The Stop TB Strategy is WHO’s recommended approach to reducing the morbidity and mortality. DOTS goal is to dramatically reduce the global burden of tuberculosis by 2015 by ensuring all TB patients, including those co-infected with HIV and those with drug-resistant TB, benefit from universal access to high-quality diagnosis and patient-centered treatment. We investigated a 27 years old woman with underdiagnosed bronchial asthma who was admitted to hospital due to recurrent episodes of shortness of breath. She was treated with antibiotics and steroids but in spite of this her state worsened and chest radiograms showed progression of the illness. She was diagnosed for cystic fibrosis, chlamydial pneumonia and idiopathic bronchiectasis. Finally, the microbiological detection of acid-fast bacilli (AFB) using The BACTEC radiometric method allowed for TB diagnosis which was later on confirmed by positive cultures for M. tuberculosis. The patient was treated for six months and we noted clinical and radiological improvement of her health.

Rozstrzenie oskrzeli to utrwalone, nieodwracalne, różnokształtne nieprawidłowe rozszerzenia oskrzeli – oskrzela są pogrubione i poszerzone oraz mają zmiany zapalne. U około 50% chorych przyczyna rozstrzeni oskrzeli pozostaje nieznana. Mukowiscydoza jest chorobą uwarunkowaną genetycznie, dziedziczoną autosomalnie recesywnie. W większości jej przypadków dochodzi do zajęcia układów pokarmowego i oddechowego. Chlamydophila pneumoniae to bakteria przenoszona drogą kropelkową, wywołująca głównie ostre infekcje górnych i dolnych dróg oddechowych. Następstwem zakażenia mogą być uszkodzenie nabłonka rzęskowego w oskrzelach i zmiany sprzyjające rozwojowi astmy, nawracających katarów nosa, przewlekłego zapalenia gardła i zapalenia zatok. Według raportu WHO w roku 2006 zanotowano 9,2 miliona nowo rozpoznanych przypadków gruźlicy, z czego 7,7% dotyczyło chorych zakażonych wirusem HIV. Głównym celem światowego programu kontroli gruźlicy jest zmniejszenie zachorowalności i umieralności z powodu tej choroby do 2015 roku. DOTS (Directly Observed Treatment Short-course), czyli strategię krótkich kursów nadzorowanego leczenia, uważa się za najbardziej wydajną ekonomicznie w zwalczaniu gruźlicy. Opis przypadku dotyczy 27-letniej kobiety z astmą oskrzelową, która do końca nigdy nie została zdiagnozowana – pacjentkę przyjęto do szpitala z powodu nawracających napadów duszności. Pomimo wdrożonego leczenia antybiotykami i glikokortykosteroidami stan kliniczny chorej, a także obraz radiologiczny uległy pogorszeniu. Na podstawie wykonywanych badań podejrzewano chlamydiowe zapalenie płuc, idiopatyczne rozstrzenie oskrzeli oraz późną postać mukowiscydozy. W badaniach plwociny początkowo nie stwierdzono żadnych patogenów. Ostatecznie badanie bronchoaspiratu metodą BACTEC wykazało obecność prątków gruźlicy. Stan chorej po 6 miesiącach leczenia przeciwprątkowego według zalecanego przez standardy schematu uległ poprawie.
