Wraz z wiekiem wiele procesów fizjologicznych i metabolicznych zmienia się. Efektem tych zmian mogą być powstające zaburzenia w funkcjonowaniu tkanek i narządów, co w konsekwencji może prowadzić do zapoczątkowania procesów starzenia. Zaobserwowano, że w podczas starzenia się rozmieszczenie i właściwości tkanki tłuszczowej ulegają zmianom. Zmniejsza się ilość podskórnej tkanki tłuszczowej oraz obserwuje się wzrost tendencji do odkładania tłuszczu trzewnego, którego nadmiar może stanowić czynnik ryzyka rozwoju chorób metabolicznych. W starzejących się adipocytach dochodzi do zmian w rozmieszczeniu receptorów, co może mieć również związek z obniżeniem wrażliwości na działanie hormonów, ponadto obserwuje się zmiany w ekspresji genów, długości telomerów i aktywności enzymów uczestniczących w metabolizmie lipidów. Aktywność wydzielnicza tkanki tłuszczowej zmienia się z wiekiem, ponadto podczas starzenia dochodzi do rozwoju wielu zaburzeń w metabolizmie komórek tłuszczowych. Całokształt zmian prowadzi do obniżenia wydajności metabolicznej komórek i całej tkanki, co może mieć ogromny wpływ na stan zdrowia i pozostawać w związku z rozwojem chorób metabolicznych oraz chorób związanych z wiekiem.

With the increasing age of organisms there are observed numerous changes in their metabolic and physiological processes. These changes may affect functioning of various tissues and organs, leading in consequence to initiation of aging process. Aging process is also taking place in adipose tissue and is characterized by changes in fat distribution. There is observed a decrease in subcutaneous adipose tissue mass while that of visceral fat increases and may contribute to development of metabolic syndrome. In aging adipocytes there were observed alteration in receptor distribution, changes in telomere length, gene expression profile and enzyme's activity. The secretory activity of adipose tissue alters during aging. All these changes lead to reduction of the metabolic capacity of adipocytes and of the whole tissue, which may exert a significant impact on the health and may be associated with development of the age-related and metabolic diseases.
Melatonin is something more than merely a hormone and many of its actions exceed the frames, normally assigned to hormones, including, for example, involvement in numerous oxidoreductive reactions and the function of a potential scavenger of “free radicals”. At present, there are several approved indications for melatonin use in the therapy in humans: 1) sleep disorders, especially in advanced age, 2) regulation of sleep-wake circadian rhythms in the blind persons, and in case of shift-work disorders, 3) alleviation of jet-lag, 4) certain psychiatric diseases, in particular depressions. Dose of melatonin and duration of treatment should always be selected individually, and should depend on the cause of administration. The average therapeutic doses vary from l to 5 mg. There are also more and more frequent data on other therapeutic possibilities of this hormone. Strong trends are now observed toward melatonin recommendation in many diseases and clinical conditions, where, however, the mode and character of melatonin action are still far from the level of thorough understanding. Nevertheless, the authors usually report positive outcomes of melatonin administration, explaining the mechanism of observations mainly on the basis of the free-radical theory and on the participation of melatonin in oxidoreductive reactions. Melatonin should not be used by pregnant and lactating women, by healthy children of all ages, and by persons suffering from immune-system neoplasms. Melatonin should be used with caution by people with autoimmune and allergic diseases.

Melatonina jest czymś więcej niŜ jedynie hormonem i wiele jej działań wykracza poza ramy przydane hormonom, np. udział w szeregu reakcji oksydoredukcyjnych w charakterze potencjalnego zmiatacza wolnych rodników. W chwili obecnej istnieje kilka wskazań do stosowania terapeutycznego melatoniny u ludzi. Są to: 1) zaburzenia snu, zwłaszcza u osób w wieku podeszłym, 2) regulacja rytmu sen-czuwanie u osób niewidomych oraz u osób wykonujących pracę zmianową, 3) niwelowanie zaburzeń wynikających z szybkiej zmiany stref czasowych w czasie podróŜy międzykontynentalnych (jet-lag), 4) niektóre choroby psychiczne, w szczególności depresje. Dawka stosowanej melatoniny oraz długość kuracji powinny być zawsze dobierane indywidualnie i zaleŜeć od przyczyny jej stosowania. Przeciętne dawki stosowane w terapii wahają się od 1 do 5 mg. Występują obecnie silne tendencje, aŜeby melatoninę stosować w bardzo wielu chorobach i stanach klinicznych, w których działania melatoniny naukowo do końca nie wyjaśniono. Tym niemniej, autorzy donoszą zazwyczaj o pozytywnych wynikach terapii melatoniną, opierając tłumaczenie pozytywnego działania tego leku głównie na teorii wolnorodnikowej i udziale melatoniny w reakcjach oksydoredukcyjnych. Nie powinno się stosować melatoniny w okresie ciąŜy i karmienia i u zdrowych dzieci w kaŜdym wieku z powodu niepełnych badań na temat bezpieczeństwa stosowania. W oparciu o przesłanki teoretyczne zaleca się ostroŜność w stosowaniu melatoniny u osób z chorobami autoimmunologicznymi i alergicznymi.
