Human prion disease is divided into three broad classes: idiopathic, genetic and acquired, reflecting different causation with resulting variations in clinical and neuropathological features. There are significant differences in clinical presentation both between and within these three groups, but all are progressive, fatal brain diseases with dementia, cerebellar ataxia and involuntary movements being particularly prominent features. Absolutely definite diagnosis requires neuropathological analysis of brain tissue (via cerebral biopsy in life or at autopsy) but there are established clinical diagnostic criteria and a variety of supportive investigations including abnormalities in the EEG, cerebral MRI, PRNP genetic analysis and CSF protein analysis. The precise results of these supportive investigations and their diagnostic utility vary somewhat depending on the type of prion disease. For example, EEG periodic discharges are a characteristic finding in sporadic but not variant CJD, for variant CJD, tonsil biopsy is an additional test and, in genetic prion disease, blood testing is possible for pathogenic PRNP mutations.

Choroby prionowe człowieka dzielą się na trzy duże grupy: idiopatyczne, genetycznie uwarunkowane i nabyte. Wszystkie grupy charakteryzują się obecnością postępującej encefalopatii (zwykle z zaznaczonym otępieniem), zawsze śmiertelnej (nie ma obecnie udowodnionego efektywnego leczenia). Definitywne rozpoznanie opiera się na badaniu neuropatologicznym tkanek mózgu, które zwykle przeprowadza się w wyniku autopsji, niemniej istnieją rzadkie wskazania do wykonania biopsji mózgu. Istnieją znaczące różnice w obrazie klinicznym, zarówno pomiędzy tymi grupami, jak i w ich obrębie, jednak wszystkie są postępującymi, śmiertelnymi chorobami mózgu, przebiegającymi z otępieniem, ataksją móżdżkową i ruchami mimowolnymi jako szczególnie zaznaczonymi cechami. Definitywne rozpoznanie wymaga badania neuropatologicznego tkanek mózgu. Istnieją jednak ustalone kryteria kliniczne oraz badania wspierające, obejmujące EEG, MRI i analizę płynu mózgowo-rdzeniowego, które pomagają w wykluczeniu lub potwierdzeniu przypuszczenia choroby prionowej. Dla wariantu CJD dodatkowym testem diagnostycznym jest biopsja migdałków, a w przypadku chorób genetycznych istnieje możliwość testowania krwi, celem wykrycia mutacji genu PRNP. Najczęstszą chorobą prionową jest sporadyczna postać choroby Creutzfeldta-Jakoba (sCJD) o nieznanej etiologii.
Kuru, the first human transmissible spongiform encephalopathy was transmitted to chimpanzees by D. Carleton Gajdusek (1923–2008). In this review, I briefly summarize the history of this seminal discovery along its epidemiology, clinical picture, neuropathology and molecular genetics. The discovery of kuru opened new windows into the realms of human medicine and was instrumental in the later transmission of Creutzfeldt-Jakob disease and Gerstmann-Sträussler-Scheinker disease as well as the relevance that bovine spongiform encephalopathy had for transmission to humans. The transmission of kuru was one of the greatest contributions to biomedical sciences of the XX century. “Kuru” in the Fore language of Papua New Guinea means to tremble from fever or cold. Kuru was restricted to natives of the Fore linguistic group in Papua New Guinea’s Eastern Highlands and neighbouring linguistic groups. Ritualistic endocannibalism (eating of relatives as part of a mourning ritual in contrast to eating enemies, i.e. exocannibalism) was practiced not only in the kuru area but in many surrounding Eastern Highland groups in which kuru never developed. The first who formally published the hypothesis that kuru spreads through cannibalism was Lindenbaum and Glasse.

Kuru to pierwsza odkryta choroba z grupy pasażowalnych encefalopatii gąbczastych. Została przepasażowana na szympansy przez D. Carletona Gajduska (1923–2008). W niniejszym artykule pokrótce zostanie podsumowana historia tego wyjątkowego odkrycia, a także opisana epidemiologia, obraz kliniczny, neuropatologia i biologia molekularna kuru. Odkrycie kuru otworzyło nowe perspektywy medycyny człowieka i pomogło uzyskać późniejszy pasaż chorób Creutzfeldta-Jakoba i Gerstmanna-Sträusslera-Scheinkera na naczelne, a także przyczyniło się do odkrycia zagrożenia płynącego z pasażu encefalopatii gąbczastej bydła na człowieka. Odkrycie kuru i późniejszy pasaż tej choroby jest jednym z największych odkryć medycyny XX wieku. W języku Fore słowo kuru oznacza drżenie jak z zimna lub gorączki. Kuru występowało w grupie lingwistycznej Fore w Górach Wschodnich Papui-Nowej Gwinei i w przylegających grupach lingwistycznych. Rytualistyczny endokanibalizm (konsumpcja krewnych, a nie wrogów; to ostatnie zjawisko nosi nazwę egzokanibalizmu) praktykowano nie tylko w rejonie występowania kuru, lecz także w okolicach Gór Wschodnich, w których kuru nigdy nie stwierdzono. Hipotezę o kanibalizmie jako środka rozprzestrzeniania się kuru po raz pierwszy opublikowali Lindenbaum i Glasse.
Misfolded prion protein (PrP ) is known as a major agent leading to infectious neurodegenerative diseases, known as transmissible spongiform encephalopathies (TSE). The mechanism of conversion of the physiological form of prion protein (PrP C ) into the pathological PrP TSE as well as the identity of neurotoxic form of this protein is not fully characterized. Under physiological conditions, PrP C one, is predominantly extracellular, tethered to the plasma membrane surface through the GPI anchor. However, cytosolic forms of PrP, termed as cytoPrP have also been found. Interestingly, a significant increase in the concentration of cytoPrP is observed in TSE. Recently, it was shown that mislocalized PrP can be a neurotoxic agent. The mechanism of neurotoxicity might be linked to the direct interaction of this form of PrP with tubulin. This interaction leads to tubulin aggregation, inhibition of microtubules (MT) assembly, disruption of microtubular cytoskeleton and eventually cell death. MT stabilization, by decreasing the level of MAP phosphorylation, can protect neurons from toxic effect of cytosolic forms of PrP.
Creutzfeldt-Jakob disease (CJD) is the most common type of human prion disease. Similarly to other prion diseases, Creutzfeldt-Jakob disease is incurable and invariably fatal disorder. The disease is clinically, molecularly and neuropathologically heterogeneous. Apart from familial, iatrogenic and variant CJD there are at least 6 subtypes of sporadic form of CJD. The typical triad of clinical symptoms of sCJD (rapidly progressive dementia, myoclonus, typical EEG) is not always present. The definite disease still can be diagnosed only by neuropathological or molecular analysis of the brain tissue. Taking into consideration that brain biopsy is rarely performed in prion diseases, most definite diagnoses are done post mortem. Nevertheless, the clinical diagnosis can be improved by using the new diagnostic criteria based on MRI findings, EEG, 14-3-3 protein test and clinical symptoms. All suspected cases should be monitored and the history of surgery, endoscopy, blood transfusion and family cases should be taken.

Choroba Creutzfeldta-Jakoba jest najczęstszą chorobą wywoływaną przez priony. Heterogenna pod względem klinicznym, neuropatologicznym i molekularnym, może sprawiać spore problemy diagnostyczne. Chorobę Creutzfeldta-Jakoba dzieli się na 4 podstawowe postaci: sporadyczną, rodzinną, jatrogenną i wariant choroby CJD. Najczęstsza jest postać sporadyczna, którą z kolei dzielimy na 6 czystych i 3 mieszane podtypy. Typowa triada objawów obejmująca szybko postępujące otępienie, mioklonie i typowy zapis EEG nie jest charakterystyczna dla wszystkich podtypów CJD, a nasilenie i kolejność występowania objawów mogą być różne w poszczególnych podtypach. Podobnie jak inne choroby wywoływane przez priony, CJD jest chorobą nieuleczalną, prowadzącą do śmierci chorego. Rozpoznanie definitywnych postaci chorób wywoływanych przez priony jest nadal możliwe jedynie na podstawie badania histopatologicznego lub molekularnego. Obecnie jednak możliwości przyżyciowego rozpoznania CJD zwiększyły się, przede wszystkim dzięki rozwojowi badań obrazowych. Prawidłowa diagnostyka wymaga jednak wykonania wielu badań dodatkowych, przede wszystkim rezonansu magnetycznego o wysokiej czułości, badania EEG oraz badania płynu mózgowo-rdzeniowego na obecność białka 14-3-3. Nowe kryteria diagnostyczne obejmują obecność zmian w jądrach podstawy i korze mózgu w badaniu rezonansu magnetycznego. Pomimo że infekcyjność postaci sporadycznej jest niska, należy monitorować wszystkie przypadki i dokładnie zbierać wywiad, szczególnie dotyczący zabiegów neurochirurgicznych, okulistycznych i transfuzji krwi.
