The term “harmful sexual behaviour” refers to young people’s behaviours which are not the result of sexual harassment and/or abuse. These behaviours are not always related to conscious, intentional abnormal behaviour. The NICE 2016 guidelines apply to children, adolescents and young adults (up to the age of 25 years) who are the sole perpetrators of harmful sexual behaviour against themselves or others. The authors of the guidelines point out that these behaviours should be viewed from different perspectives, such as the broadly understood developmental norms, non-harmonious development, coexisting disorders or diagnosed mental disorders. They also point to the need to select a leader for diagnostic and therapeutic processes as well as to establish a local commission. Furthermore, the authors suggest tools for the diagnostic process, critically evaluate some of the therapeutic interventions and point out that the issue of harmful sexual behaviours is underestimated. The NICE recommendations may serve as a guide for developing similar guidelines in other countries.

Termin „szkodliwe zachowania seksualne” odnosi się do tych zachowań młodych ludzi, które nie są wynikiem molestowania/ wykorzystania seksualnego. Ich występowanie nie zawsze związane jest ze świadomym, zamierzonym nieprawidłowym zachowaniem. Wytyczne NICE 2016 dotyczą dzieci, adolescentów i młodych dorosłych (do 25. roku życia) będących wyłącznymi sprawcami szkodliwych czynności seksualnych skierowanych do siebie lub do innych. Autorzy wytycznych zwracają uwagę, iż na zachowania te należy patrzeć z różnych perspektyw: szeroko rozumianej normy rozwojowej, nieharmonijnego rozwoju, zaburzeń współwystępujących lub diagnozy zaburzenia psychicznego. Wspominają też o konieczności wyłonienia lidera w procesie diagnozy i terapii oraz stworzenia komisji lokalnej. Sugerują, jakimi narzędziami posługiwać się w procesie diagnozy, krytycznie oceniają niektóre interwencje terapeutyczne i wskazują na fakt, że problem szkodliwych zachowań seksualnych jest nieoszacowany. Rekomendacje NICE mogą być wskazówką do tworzenia podobnych wytycznych przez inne kraje.
Obstetrical haemorrhage still remains one of the most common causes of death among pregnant women and women in labour. Almost 88% of these women die within the first 4 hours of its onset. Knowing the mechanisms that regulate hemostasis of the woman in labour at the molecular, immunological, hormonal, neurological level and the role and function of the uteroplacental unit has a huge impact on the further therapeutic decisions in case of obstetrical haemorrhage. There are a number of various definitions concerning obstetrical haemorrhage which proves how difficult it is to recognize and diagnose this condition. Taking under consideration the haemodynamic changes, the systemic consequences and the need of making specific therapeutical decisions it seems necessary to include in the definition the dynamics of the blood loss in the specific time unit: cumulated blood loss of 150 ml/min or 1.5 ml/kg of body weight per minute and a 50% loss of circulating blood volume in 3 hours. The prognosis of the haemorrhage depends on many different elements, including: rapid diagnosis, notification and implement of the management therapeutic algorithm and interdisciplinary co‑work of various teams taking part in the diagnostic‑therapeutic process. It is crucial that there is an effective, simultaneous cooperation among the experienced anaesthesiology team, ob‑gyn team, the operating theatre team, diagnostic‑laboratory teams and the regional blood donation agencies. Coordinating work of that many people, that are often spread within the organizational structures of the hospital on different floors or buildings, is excessively challenging. On the other hand, establishing the management algorithm that takes into account the order and the way of notifying as well as implementing certain procedures in individual organizational structures has a huge, direct impact on patient’s survival and the quality of life. Most of the management procedures for the severe obstetrical haemorrhage, such as resuscitation, monitoring, bleeding control, should be performed simultaneously.

Krwotok położniczy wciąż jest jedną z najczęstszych przyczyn zgonów ciężarnych i rodzących. Prawie 88% kobiet umiera w ciągu pierwszych czterech godzin od momentu jego wystąpienia. Znajomość mechanizmów regulujących homeostazę ustroju ciężarnej ma ogromne znaczenie w postępowaniu terapeutycznym w przypadku wystąpienia krwotoku okołoporodowego. O tym, jak trudno rozpoznać i zdefiniować krwotok położniczy, dowodzi obecność kilkunastu różnych jego definicji. Z punktu widzenia zmian hemodynamicznych, następstw ogólnoustrojowych i konieczności podjęcia określonych decyzji terapeutycznych bardzo użyteczne wydają się pojęcia oceniające szybkość utraty w określonym przedziale czasowym: skumulowana utrata krwi wynosząca 150 ml/min lub 1,5 ml/kg m.c./min oraz utrata 50% objętości krwi krążącej w ciągu 3 godzin. Rokowanie w krwotoku położniczym zależy od wielu elementów, do których zaliczamy: szybkie rozpoznanie, powiadomienie i wdrożenie algorytmu postępowania terapeutycznego oraz efektywne działanie interdyscyplinarne różnych zespołów biorących udział w procesie diagnostyczno‑leczniczym. Niezbędna jest jednoczasowa, efektywna współpraca doświadczonego zespołu anestezjologicznego, ginekologiczno‑położniczego, bloku operacyjnego, zespołów pracowni diagnostyczno‑laboratoryjnych oraz regionalnego centrum krwiodawstwa. Skoordynowanie pracy dużej liczby ludzi, często „rozrzuconych” w strukturach organizacyjnych szpitala na różnych piętrach lub w oddzielnych budynkach, jest niezmiernie trudne. Opracowanie algorytmu postępowania uwzględniającego kolejność i sposób powiadamiania oraz wdrażanie określonych procedur w poszczególnych jednostkach organizacyjnych decydują o przeżyciu pacjentki i późniejszej jakości życia. Większość czynności w ciężkim krwotoku położniczym (resuscytacja, monitorowanie, opanowywanie krwawienia) powinna być prowadzona jednoczasowo.
Ovarian cancer continues to be the most lethal malignancy in women. Standard treatment in advanced ovarian cancer is primary optimal cytoreduction with extensive tumour excision combined with platinum- and taxanoid-based chemotherapy. The cornerstone of treatment is surgical reduction of tumour burden. Usually, surgical cytoreduction precedes or follows neoadjuvant therapy, i.e. after several courses of chemotherapy. The key issue in the treatment of ovarian cancer is correct determination of clinical stage of tumour. Nevertheless, still we lack reliable data based on metaanalyses, which might define optimal management of patients presenting with a recurrence of ovarian cancer. Recurrent ovarian cancer is a pathologic process or a chronic disease. We review results of clinical studies aiming at improvement of effectiveness while reducing toxicity of drugs used in systemic treatment of advanced epithelial ovarian cancer. Apart of well-known preparations, e.g. cisplatin, paclitaxel, topotecan and liposomal doxorubicin, great hopes are associated with novel compounds, e.g. gemcitabine, docetaxel, etoposide, irinotecan, vinorelbine and bevacizumab, particularly in platinum-resistant form of recurrent ovarian cancer. Further studies on novel therapeutic protocols focus on different management strategies, e.g. serial administration of drugs, or evaluation of effectiveness of a single drug used as monotherapy. Currently, the most important therapeutic aim is to improve survival of patients with recurrent ovarian cancer while preserving an acceptable quality of life.

Rak jajnika jest nadal najbardziej śmiertelną chorobą nowotworową wśród kobiet. Leczeniem standardowym w przypadku zaawansowanego raka jajnika jest pierwotna optymalna cytoredukcja wraz z rozległym usunięciem guza uzupełniona o następczą chemioterapię składającą się z pochodnych platyny i taksonoidów. Zabieg chirurgiczny jest podstawową formą leczenia. Zwykle optymalny zabieg zostaje wykonany na początku lub po terapii neoadjuwantowej, tj. po kilku cyklach chemioterapii. Kluczowym aspektem chirurgii w raku jajnika jest zapewnienie odpowiedniej klasyfikacji według stadiów zaawansowania klinicznego nowotworu. Nadal brak wiarygodnych danych opartych na metaanalizach, które pomogłyby określić odpowiednie postępowanie kliniczne w przypadkach nawrotów raka jajnika. Nawracający rak jajnika jest procesem chorobowym, a raczej chorobą chroniczną. Przedstawiono w pracy wyniki badań klinicznych mających na celu poprawę skuteczności oraz zmniejszenie toksyczności leków wykorzystywanych w systemowym leczeniu zaawansowanego nabłonkowego raka jajnika. Obok dobrze znanych środków, takich jak cisplatyna, pakli-taksel, topotekan, liposomalna doksorubicyna, coraz większe nadzieje wiąże się z nowymi lekami, takimi jak gemcytabina, docetaksel, etopozyd, irynotekan, winorelbina i bewacizumab, szczególnie w platynoopornej postaci nawrotowego raka jajnika. Dalsze badania nad nowymi środkami skupiają się w innych strategiach postępowania, takich jak na przykład leki podawane seryjnie, bądź też oceniają skuteczność pojedynczej, izolowanej monoterapii. Obecnie najważniejszym celem terapii jest zwiększenie przeżywalności pacjentek z nawracającym rakiem jajnika z zachowaniem odpowiedniej jakości życia.
