Introduction Identification of factors determining nurse’s performance of a comprehensive history taking and physical examination. Material and methodsThe material was collected in a group of 828 nurses and nursing students. Inclusion criteria were: practicing as a nurse or studying in the field of Nursing, passing training in Physical examination and voluntary compliance for participation in the study. The authors used the diagnostic survey method, with the application of their own questionnaire. Obtained results were statistically analysed. Results In practice, only 42.8% of nurses declared that a comprehensive interview and physical examination of a patient had been carried out. Respondents pointed out difficulties in the implementation of history taking and physical examination in nursing practice, i.e. low support from doctors, nurses, management and lack of time/excess of responsibilities, lack of knowledge and skills, lack of acceptance by patients, lack of appropriate equipment/rooms. Training of nurses, doctors, management staff, increasing the nursing staff, reducing and their duties, the possibility of cooperation/consultation with a doctor, adaptation of premises along with their appropriate equipment, and patient education may contribute to the liquidation of the above mentioned difficulties. Conclusions Little support from doctors, nurses and lack of time due to an excess of responsibilities are a major factors in the failure of nurses comprehensive history taking and physical examination. Training of nurses, doctors, and management staff in the scope of nurse’s competence, increasing the number of nurses, adapting the rooms and their appropriate equipment are elements that can motivate nurses to conduct independently history taking and physical examination of patients.
Background: Obstacles are a common cause of falls. Mobility tests for healthy elderly that involve maneuvering over an obstacle have not been fully developed. We have shown that a small box placed on the walkway of a representative mobility test, the Timed Up & Go (TUG) for the frail elderly, influences the test performances of the healthy female elderly. However, the validity of this obstructed TUG (OTUG) as a fall risk assessment tool has not yet been clarified. This study examined the relationship between the OTUG and the fall risk for the healthy elderly. Material/Methods: 66 healthy community-dwelling elderly and 19 healthy elderly persons living in a nursing home participated in the study. In the TUG, participants stood up from a chair, walked 3 m, turned around, walked back to the chair and sat down. In the OTUG, a box (height 5 cm, depth 10 cm) was placed at the midpoint of the walkway. Participants were instructed to step over it safely. Times required to perform the respective test were recorded. Fall risk scores, the required times (s) for the TUG and OTUG and difference ratios (OTUG/TUG x 100) (%) were analyzed. Results: Correlations between fall risk and other mobility performances were all significant. However, the correlation between fall risk and the OTUG (r = 0.60) was significantly higher than that between fall risk and the TUG (r=0.49) (t=3.733, p<0.001). Objective: High resolution ultrasonography is a non-painful and non-invasive imaging technique which is useful for the assessment of shoulder pain causes, as clinical examination often does not allow an exact diagnosis. The aim of this study was to compare the fi ndings of clinical examination and high resolution ultrasonography in patients presenting with painful shoulder. Methods: Non-interventional observational study of 100 adult patients suffering from unilateral shoulder pain. Exclusion criteria were shoulder fractures, prior shoulder joint surgery and shoulder injections in the past month. The physicians performing the most common clinical shoulder examinations were blinded to the results of the high resolution ultrasonography and vice versa. Results: In order to detect pathology of the m. supraspinatus tendon, the Hawkins and Kennedy impingement test showed the highest sensitivity (0.86) whereas the Jobe supraspinatus test showed the highest specifi city (0.55). To identify m. subscapularis tendon pathology the Gerber lift off test showed a sensitivity of 1, whereas the belly press test showed the higher specifi city (0.72). The infraspinatus test showed a high sensitivity (0.90) and specifi city (0.74). All AC tests (painful arc IIa, AC joint tendernessb, cross body adduction stress testc) showed high specifi cities (a0.96, b0.99, c0.96). Evaluating the long biceps tendon, the palm up test showed the highest sensitivity (0.47) and the Yergason test the highest specifi city (0.88). Conclusion: Knowledge of sensitivity and specifi city of various clinical tests is important for the interpretation of clinical examination test results. High resolution ultrasonography is needed in most cases to establish a clear diagnosis.

Cel badania: Ultrasonografia wysokiej rozdzielczości jest metodą niebolesną i nieinwazyjną. Jej duża przydatność w diagnostyce bólu stawu łopatkowo-ramiennego wynika z niedoskonałości badania przedmiotowego w ocenie tej patologii. Celem pracy było porównanie wyników badania przedmiotowego i ultrasonograficznego o wysokiej rozdzielczości, wykonywanych u pacjentów z bólem stawu łopatkowo-ramiennego. Materiał i metoda: Badanie obserwacyjne 100 dorosłych pacjentów z jednostronnym bólem stawu łopatkowo-ramiennego. Nie wykonywano procedur inwazyjnych. Kryteria wyłączenia: złamania w obrębie stawu łopatkowo-ramiennego, przebyte operacje stawu łopatkowo-ramiennego oraz iniekcje dostawowe w tym stawie w ciągu ostatniego miesiąca. Wykonujący standardowe badanie fizykalne stawu łopatkowo-ramiennego klinicyści nie mieli dostępu do wyników badań ultrasonograficznych a ultrasonografiści – do wyników badań fizykalnych. Wyniki: Test ciasnoty Hawkinsa i Kennedy’ego stosowany w diagnostyce patologii ścięgna mięśnia nadgrzebieniowego wykazał największą czułość (0,86), a test mięśnia nadgrzebieniowego Jobe’a największą swoistość (0,55). W ocenie zmian ścięgna mięśnia podłopatkowego test uniesienia kończyny od pośladka Gerbera wykazał czułość 1, podczas gdy test ucisku brzucha „belly press” odznaczył się wyższą swoistością (0,72). Oporowy test mięśnia podgrzebieniowego wykazał wysoką czułość (0,90) i swoistość (0,74). Wszystkie testy stosowane w patologiach stawu barkowo-obojczykowego (test bolesnego łukua, test uciskowy stawu barkowo-obojczykowegob, test skrzyżowanego przywodzeniac) cechowały się wysoką wrażliwością (a0,96, b0,99, c0,96). W przypadku patologii ścięgna głowy długiej mięśnia dwugłowego ramienia, test dłoni zwróconej ku górze wykazał najwyższą czułość (0,47), a test Yergasona najwyższą wrażliwość (0,88). Wnioski: Znajomość czułości i swoistości poszczególnych testów jest niezbędna do właściwej interpretacji badania fizykalnego stawu łopatkowo-ramiennego. Ultrasonografia o wysokiej rozdzielczości jest potrzebna do postawienia jednoznacznej diagnozy w większości przypadków.
The meaning of physical activity for maintenance of a healthy life, normal growth and development of children and adolescents is well known. A regular sport training causes favourable functional changes including improvement in cardiopulmonary or metabolic status. Despite these benefits, vigorous physical exertion may trigger mechanisms leading to sudden death in athletes with unrecognized cardiac disorders. That’s why the regular cardiovascular screening in all young competitive athletes in needed. Complete medical examination of young athletes (including a medical personal and family history and physical examination) should give information about functional status of cardiovascular system. Physical examination should include auscultation in supine, seated and upright position, palpation of peripheral pulses, accurate measurement of blood pressure and looking for findings of Marfan syndrome. Physical examination may reveal signs of cardiovascular effects of adaptation to long‑term sport activity, i.e., enlargement of the heart (increase of dimensions, wall thickness and mass of left ventricle) and a slow pulse rate at rest as a result of vagal tone increas Another target of cardiovascular examination of young athletes is to identify underlying heart disease (especially in aspect of prevention of sudden cardiac death) requiring the extension of diagnostic workup. The main aim of this paper is to give for paediatricians and family doctors some practical information helpful in proper interpretation of physiological, adaptive circulatory changes in relation to long‑term physical activity and identification of cardiac pathologies threatening of sudden death in association with sporting activity.

Aktywność fizyczna stanowi jeden z podstawowych warunków dobrego stanu zdrowia, a w przypadku dzieci i młodzieży także niezbędny element prawidłowego rozwoju somatycznego, psychomotorycznego i funkcjonalnego. Regularny trening sportowy korzystnie wpływa na stan czynnościowy organizmu, a w szczególności układu krążenia. Z drugiej strony intensywny wysiłek fizyczny może być także mechanizmem spustowym dla zagrażających życiu arytmii u sportowców z nierozpoznaną wcześniej chorobą serca. Fundamentalną rolę w sporcie wyczynowym odgrywają badania sportowo‑lekarskie, zarówno wstępne – przed rozpoczęciem aktywności sportowej, jak i okresowe – w trakcie intensywnego wysiłku fizycznego. W ocenie stanu zdrowia osoby aktywnej fizycznie szczególne miejsce zajmuje badanie układu krążenia, uwzględniające, poza ukierunkowanym wywiadem, m.in. osłuchiwanie serca w różnych pozycjach ciała, badanie tętna obwodowego, pomiar ciśnienia tętniczego, a także poszukiwanie objawów fenotypowych zespołu Marfana. Badanie przedmiotowe pozwala na ocenę zmian adaptacyjnych układu krążenia zachodzących pod wpływem długotrwałego treningu sportowego, z których najbardziej charakterystyczne są powiększenie serca (efekt zwiększenia grubości mięśnia sercowego z poszerzeniem światła lewej komory) oraz wolna spoczynkowa czynność serca. Ważna jest także umiejętność identyfikacji objawów wskazujących na organiczną chorobę układu krążenia, która może doprowadzić do nagłej śmierci sercowej. Celem niniejszej pracy jest dostarczenie pediatrom i lekarzom rodzinnym informacji pozwalających na właściwą ocenę fizjologicznych, adaptacyjnych zmian zachodzących w układzie krążenia pod wpływem długotrwałego treningu sportowego, a także ułatwiających identyfikację stanów chorobowych, będących przeciwwskazaniem do sportu wyczynowego.
