In aging populations osteoporosis is a serious problem. Low adherence to osteoporotic therapies is a well-known fact. There is a crisis in the therapy of osteoporosis: even with a diagnosis of osteoporosis or after an osteoporotic fracture there are patients without adequate therapy. The aim of the study was to analyze the association between the patients’ subjective importance of osteoporosis and medication adherence, quality of life, depression status and self-reported socio-economic factors, and find the risk groups which can be identified in the community pharmacy setting to improve the quality of care of osteoporotic patients. A cross-sectional study was conducted in four public hospitals in Hungary. Medication adherence was measured by the Morisky Medication Adherence Scale, quality of life was measured by the EQ5D-3L, depression status was checked by the Beck Depression Scale. Patients were asked to what extent their life was affected by osteoporosis compared to their chronic co-morbidities. Three patient groups were analyzed: High, Low and No osteoporosis importance groups. Statistical analyzes were carried out by SPSS program. Significant differences were found between the groups of osteoporosis importance in age, T-score, working status, type of the medication, existing fracture, frequency of pain and number of chronic diseases. The main target of pharmaceutical therapy management is to maintain a therapy-length lasting, periodic contact with the osteoporotic patient to improve the patients’ subjective perception of quality of life. Improving the importance of the disease in patients with low osteoporosis importance could help to improve the patients’ quality of life.
Education of community pharmacy workers is a very important part of everyday practice, especially in professional counselling (PC) which brings many health benefits for the patients. The aim of the study was to investigate if the community pharmacy specialization has an influence on the scope and quality of counsels given by pharmacy workers during PC scheme implementation in community pharmacies. The research was carried out between May and November 2017. After PC scheme implementation in community pharmacies, an anonymous questionnaire was administered to 245 pharmacy workers in Poznan and Warsaw (241 women, 4 men): 146 pharmacists (143 women, 3 men) and 99 pharmacy technicians (98 women, 1 man). In the conducted study, 14.7% of pharmacists had community pharmacy specialization (according to Polish law). Pharmacists with a community pharmacy specialization referred patients more often to physicians (p<0.0001), as well as after earlier physician consultation (p<0.0001). Furthermore, they often noticed the interaction between drugs in patients’ self-treatment and in the treatment ordered by a physician (p≤0.001). The indication of medication contraindications after a physician consultation was also significantly higher within pharmacists with specialization (p<0.0001). It can be noted that pharmacists with community pharmacy specialization were asked more often for self-treatment consultation and after physician visits (p<0.0001). Development of PC in Polish community pharmacy, especially among pharmacy owners well-educated in community pharmacy specialization, may help to increase safety of pharmacotherapy.
Background: Pharmaceutical pictograms are useful tools aimed to improve patients’ understanding of medical information. They can be easily implemented into routine settings and can embrace various information, among others, about drug regimen, drug adverse reactions, and potentially harmful drug interactions. Since pharmaceutical pictograms can improve patients’ knowledge, they can also improve adherence to recommended therapy. Thus, it is wise to understand patients’ perspectives on pictograms, particularly due to the cultural discrepancies existing between populations in which pharmaceutical pictograms are used. Aim: The aim of the study was to collect patients’ perspectives particularly in terms of satisfaction on the pharmaceutical pictograms used in routine community pharmacy settings. Materials and methods: The study is immersed in a quantitative approach. The survey, based on an authorial questionnaire, was conducted in a single community pharmacy in the north part of Poland. Labeling was conducted by qualified professional staff according to validated recommendations provided via a computer system. 103 questionnaires were collected and analyzed. Only adult patients participated in the study. Results: Almost all participants (96.11%) acknowledged that such a labeling system could be helpful in following the physicians’ recommendations and can overall simplify the use of medicines. Additionally, 84.47% of patients stated that pictogram labels should be applied to both prescription and over-the-counter drugs. The vast majority (91.26%) also did agree that pharmaceutical pictograms should be available in every community pharmacy. Statistical analysis showed that patients who stated that pharmaceutical pictograms should be available in every community pharmacy and be provided on a patient demand were those for whom drug leaflets were not always understandable. Conclusions: Patients positively evaluated pharmaceutical pictograms. Further studies are warranted, particularly more representative in order to determine the most optimal direction for the development of pharmaceutical services in Poland.

Przedmiot badań: Piktogramy farmaceutyczne są przydatnym narzędziem ułatwiającym zrozumienie informacji medycznej przez pacjentów. Poprzez wzrost wiedzy pacjentów piktogramy farmaceutyczne mogą zwiększać stopień przestrzegania zaleceń terapeutycznych przez pacjentów. Poznanie perspektywy pacjentów jest istotne z uwagi na różnice kulturowe między populacjami, w których stosowane są piktogramy farmaceutyczne. Cel badania: Celem badania było zbadanie perspektywy pacjentów na piktogramy farmaceutyczne w szczególności satysfakcji pacjentów z usługi piktogramów farmaceutycznych dostępnych w aptece ogólnodostępnej. Materiał i metody: Badanie ankietowe zostało przeprowadzone w aptece ogólnodostępnej z udziałem pełnoletnich pacjentów (n=103), którzy otrzymali opakowania produktów leczniczych oznaczone piktogramami farmaceutycznymi. Leki były znakowane przez wykwalifikowany personel apteczny z użyciem oprogramowania komputerowego. W badaniu zastosowano autorski kwestionariusz. Wyniki: Prawie wszyscy badani (96,11%) uznali, że system etykietowania leków może być pomocny w realizacji zaleceń terapeutycznych oraz ułatwić korzystanie z leków, a 84,47% stwierdziło, że piktogramy farmaceutyczne powinny być naklejane na wszystkie leki dyspensowane w aptece. 91,26% respondentów wskazało, że taki system powinien znajdować się w każdej aptece. Zdania, że system etykietowania powinien wchodzić w zakres usługi apteki oraz że powinny być one udostępniane w każdej aptece na żądanie pacjenta były przede wszystkich osoby, dla których ulotki leków były nie zawsze zrozumiałe. Wnioski Piktogramy farmaceutyczne zostały ocenione przez pacjentów pozytywnie. Konieczne jest przeprowadzenie dalszych badań dotyczących piktogramów farmaceutycznych na liczniejszej grupie pacjentów w celu optymalizacji rozwoju opieki farmaceutycznej w Polsce.
A pharmacist is an educated specialist working to provide patients’ with the highest level of care for who business goals should be a secondary aim. Due to an ethical attitude which is in line with the pharmacist code of ethics, a set of most important guidelines, regulating the pharmacist's profession, are them main stream of pharmaceutical care regulation implemented in daily practice. Pharmaceutical care by deploying variety of different pharmacy services, is an important element of the process of optimizing the use of medicines. To start providing pharmaceutical care by implementing a wide catalog of pharmaceutical services, a Polish pharmacist must change their attitude from the typical role of the “seller” or “shop assistant” to the role of a professional adviser to the patient who is using skills and references to provide the best possible advice. The term: "pharmaceutical care" used to refer to the understanding of the responsible action of a pharmacist, whose professional goal is to ensure the safety of pharmacotherapy and improve the patient's quality of life. The process of implementing this type of care is very difficult and this is due to the fact that most Polish pharmacies since the 90s of the twentith century operate mainly as business entities (discounts, loyalty programs, marketing campaigns). Due to the fact that the pharmacy market in Poland is constantly evolving, it seems reasonable to reflect on how to make contemporary Polish pharmacies unique medical facility, in which the patient will receive not only the prescription, but above all - substantive support from pharmacists, being specialist consultancy in the field of health and safety in taking medicines. The pharmacist, which is specialist advice on health and safety in admission drugs could start to build an authority and respect by step by step transferring the pharmacy from price only focused pharmacy to advance advice focused center of patient well being . It is a very simple process which can be start even from tomorrow. It only a matter of will, and eagerness to make a chance.

Farmaceuta to wykształcony specjalista pracujący nad zapewnieniem swojemu pacjentowi opieki na najwyższym poziomie, dla którego cele biznesowe powinny być wtórne. Dzięki etycznej postawie zgodnym z kodeksem etyki zawody aptekarza, realizowane są założenia opieki farmaceutycznej, będący ważnym elementem procesu optymalizacji użycia leków. Polski farmaceuta, aby rozpocząć sprawowanie opieki farmaceutycznej poprzez realizowanie szerokiego katalogu usług, musi zmienić swoje nastawienie z typowej roli sprzedawcy do roli profesjonalnego doradcy dla pacjenta. Pojęcie: “opieka farmaceutyczna” zwykło się odnosić do rozumienia odpowiedzialnego działania farmaceuty, którego zawodowym celem jest zapewnienie bezpieczeństwa farmakoterapii i podniesienie jakości życia pacjenta. Proces wdrażania tego rodzaju opieki jest bardzo trudny i to z uwagi na to, że większość polskich aptek od lat 90-tych XX w funkcjonuje głównie jako podmioty biznesowe (zniżki, programy lojalnościowe, akcje marketingowe). Ze względu na to, że rynek apteczny w Polsce stale się rozwija zasadne wydaje się poczynienie refleksji na temat tego, w jaki sposób współczesne polskie apteki uczynić wyjątkowymi placówkami medycznymi, w których pacjent otrzyma nie tylko realizację recepty, ale nade wszystko - merytoryczne wsparcie ze strony farmaceuty, będące specjalistycznym doradztwem w sprawach zdrowia i bezpieczeństwa w przyjmowaniu leków.
SUBJECT OF THE STUDY: The use of medication is associated with the risk of side effects. In the view of the fact that the consumption of Nonsteroidal anti-inflammatory drugs (NSAIDs) and antibiotics is growing constantly, it is necessary to apply appropriate protection strategies to prevent side-effects of those drugs. AIM OF THE STUDY: The aim of the study is to asses patients' knowledge about protective agents: drugs that reduce the production of hydrochloric acid from the group of proton pump inhibitors and probiotics in the terms of effectiveness and safety of, respectively, inflammations and the bacterial infections pharmacotherapy. MATERIAL AND METHODS: The research was carried out since October 2018 to February 2019 on the random group of people from Poland, using an original, anonymous questionnaire. The data was collected via Internet and direct contact with the respondents participating classes at the University of Third Age of the Medical University of Silesia. The statistical analysis of the collected results was performed with the use of Microsoft Excel 2010. RESULTS: Almost everyone of the respondents (about 95.0%) think that they understand the concept of a protective agents. Many of them only associates them with antibiotic therapy, not being aware of the broader meaning of this concept. 82.0% of respondents believe that they know the concept of a probiotic, but as many as 64.6 % of patients mistakenly think, that probiotics protect the stomach. About 50.0% of people are aware of the need to protect the stomach with chronic Nonsteroidal anti-inflammatory drugs therapy. CONCLUCIONS: The obtained results show that patients have basic information about gastro-enteroprotective agents (including drugs that reduce the production of hydrochloric acid from the group of proton pump inhibitors and probiotics) however their knowledge is still insufficient to guarantee the effectiveness of therapy. The further education is necessary, which could be improved by introducing the Pharmaceutical Care.

PRZEDMIOT BADAŃ: Stosowanie każdego rodzaju leków wiąże się z ryzykiem wystąpienia działań niepożądanych. W świetle rosnącego spożycia leków przeciwbólowych i antybiotyków konieczne staje się stosowanie właściwych strategii ochronnych, zapobiegających ubocznym skutkom działania tych leków. CEL BADAŃ: Celem pracy jest zbadanie wiedzy pacjentów na temat preparatów osłonowych, w szczególności leków zmniejszających produkcję kwasu solnego z grupy inhibitorów pompy protonowej oraz probiotyków. MATERIAŁ I METODY: Badanie przeprowadzono od października 2018 do lutego 2019 na losowej grupie 582 mieszkańców Polski za pomocą autorskiego, anonimowego kwestionariusza ankiety. Analizę statystyczną zebranego materiału, wykonano za pomocą programu Microsoft Excel 2010. WYNIKI: W badaniu 94,5% ankietowanych zadeklarowało, że rozumie pojęcie preparatu osłonowego. Duży odsetek grupy badanej wiąże je jedynie z antybiotykoterapią, nie mając świadomości szerszego znaczenia tego pojęcia. 82,0% respondentów uważa, że zna pojęcie probiotyku, jednak aż 64,6% z nich niewłaściwie zastosowałoby probiotyk jako osłonę żołądka. Niespełna 50,0% osób ma świadomość konieczności ochrony żołądka przy przewlekłej terapii lekami z grupy niesteroidowych leków przeciwzapalnych. WNIOSKI: Przeprowadzone badanie wykazało, że ankietowani posiadają podstawową wiedzę na temat preparatów osłonowych jednak nie jest ona wystarczająca, by w każdym przypadku zapewnić bezpieczeństwo i skuteczność terapii. Konieczne jest zatem edukacja pacjentów poprzez wdrożenie szeroko dyskutowanej obecnie Opieki Farmaceutycznej.
It is still impossible to implement pharmaceutical care and its services in Poland, due to lack of legal regulations in place and sources of financial support. The vision created by Hepler and Strand was successfully implemented in many countries such as United Kingdom (UK), where planning and implementing more and more clinically advanced services is happening very dynamically. The need for support provided by community pharmacy services is increasing every year. National Health Systems (NHS) aim is to provide as wide as possible care for patients within Primary Care System. Therefore NHS funds and supports pharmacists further education which enables them carrying out more and more advanced clinically tasks. Community Pharmacy Services are divided into 3 categories: essential, advanced and local. Every pharmacy can chose the services they want to provide and the decision is based on patients needs in the given area. In July 2019 NHS, Pharmaceutical Services Negotiating Committee and Department of Health and Social Care published ,, The Community Pharmacy Contractual Framework for 2019/20 to 2023/24: supporting delivery for the NHS Long Term Plan’’. This document delivers even more support for development of pharmaceutical care services in UK and acknowledges the potential that community pharmacists have, needed for delivery of clinically advanced pharmaceutical services. The aim of the article is to illustrate what pharmaceutical care services are available in UK, how they are funded and what impact they have on health care system and public health. It has been estimated that in 2015 year alone Community Pharmacy Services saved NHS over 3 bilion pounds and further investments have been predicted. Community Pharmacy plays an important role in prevention and maintaining public health in UK. As proven in UK, pharmaceutical care implemented in systematic way, practiced with passion in competent and responsible manner can create an opportunity for pharmacists to achieve professional fulfilment.

Opieka farmaceutyczna i możliwe w jej ramach świadczenia, wykonywane przez farmaceutów w aptekach ogólnodostępnych, w dalszym ciągu w Polsce nie są realizowane, przede wszystkim ze względu brak regulacji prawnych oraz określenia źródeł finansowania. Wizję i założenia Heplera i Strand udało się natomiast wdrożyć w wielu krajach, m.in. w Wielkiej Brytanii, gdzie planowanie i realizowanie coraz bardziej zaawansowanych klinicznie świadczeń odbywa się bardzo dynamicznie od ponad 20 lat. Niniejszy artykuł ma na celu syntetyczne ukazanie jakie świadczenia w ramach opieki farmaceutycznej są realizowane w Wielkiej Brytanii oraz jakie są ich źródła finansowania i rola w systemie ochrony zdrowia. Doświadczenia z tego kraju wskazują, że opieka farmaceutyczna wprowadzona w sposób systemowy, praktykowana z zaangażowaniem w sposób kompetentny i odpowiedzialny, daje farmaceutom możliwości niezbędne do pełnego wykorzystania ich wiedzy i kompetencji.
