Aim: The aim of the presented study was the preliminary verification of the Jeffrey Young’s theory of early maladaptive schemas and their role in the genesis of personality disorders. According to Young, negative parental attitudes towards the child and the moderating influence of the child’s temperament can develop the schemas. Coping with schemas shapes the traits of a personality disorder. Methods: Four hundred and thirty-five subjects from a non-clinical group were tested. They completed the Young Schema Questionnaire – Short Form (YSQ-S3), Structured Clinical Interview for DSM-IV Axis II Personality Disorders – Personality Questionnaire part (SCID-II), Questionnaire of Retrospective Assessment of Parental Attitudes (KPR-Roc) and Questionnaire of the Formal Characteristics of Behaviour–Temperament Inventory (FCB-TI). The SCID-II was used to determine specific features of behaviour. For the presented study borderline and avoidant personality patterns were chosen. Results: Explanatory models were created using regression analysis. The models were composed of: 1) schemas, 2) schemas, temperament, 3) schemas, parental attitudes, 4) all variables. In the case of borderline features, the models explain 26%, 30%, 35% and 36% of the variance of personality traits, respectively. The most appropriate model 3 includes the following schemas: Abandonment, Defectiveness, Self-Sacrifice, Pessimism and parental attitudes: Overdemandingness, Autonomy, Overprotection of the father and Autonomy and Inconsistency of the mother. In the case of avoidant traits, models explain 40%, 47%, 41% and 49% of the variance, respectively. For avoidant traits temperament is more important than parental attitudes – significant factors are: Social Isolation, Vulnerability to Harm, Subjugation, Self-Sacrifice, Emotional Inhibition, Pessimism and temperamental traits: Emotional Reactivity and Activity. Conclusion: The presented preliminary analysis confirms Young’s theory of the schemas and their influence on the development of personality disorders.

Cel: Celem prezentowanych badań była wstępna weryfikacja teorii wczesnych nieadaptacyjnych schematów Jeffreya Younga i ich roli w genezie zaburzeń osobowości. Według Younga negatywne postawy rodzicielskie w stosunku do dziecka przy moderującej roli jego temperamentu wpływają na tworzenie schematów. Z kolei radzenie sobie ze schematami rozwija cechy nieprawidłowej osobowości. Metoda: Przebadano 435 osób z grupy nieklinicznej. Zastosowano Kwestionariusz Schematów Younga w wersji skróconej (YSQ-S3), Ustrukturalizowany Wywiad Kliniczny do Badania Zaburzeń Osobowości z Osi II DSM-IV – część: Kwestionariusz Osobowości (SCID-II), Kwestionariusz Retrospektywnej Oceny Postaw Rodziców (KPR-Roc), Formalną Charakterystykę Zachowania – Kwestionariusz Temperamentu (FCZ-KT). SCID-II został użyty do oceny wyłącznie specyficznych cech zachowania, nie do ustalania rozpoznań. Do celów prezentowanego badania wybrano cechy charakterystyczne dla osobowości borderline i unikowej. Wyniki: Stworzono modele w oparciu o analizę regresji. Modele tworzono ze: 1) schematów, 2) schematów i temperamentu, 3) schematów i postaw rodzicielskich, 4) wszystkich zmiennych. W przypadku cech borderline modele wyjaśniały odpowiednio 26%, 30%, 35% i 36% zmienności cech osobowości. Najbardziej adekwatny model 3. składał się ze schematów: Porzucenia, Defektu, Samopoświęcenia i Pesymizmu oraz postaw rodzicielskich: Nadmiernych wymagań, Autonomii i Nadmiernej ochrony ojca oraz Autonomii i Niekonsekwencji matki. W przypadku cech osobowości unikowej modele wyjaśniały odpowiednio 40%, 47%, 41% i 49% zmienności. Dla cech unikowych temperament jest istotniejszy niż postawy rodzicielskie – istotne współczynniki to: Izolacja Społeczna, Podatność na Zranienia, Podporządkowanie, Samopoświęcenie, Zahamowanie Emocjonalne i Pesymizm oraz temperament: Reaktywność Emocjonalna oraz Aktywność. Wnioski: Prezentowane badania wstępnie potwierdzają teorię Younga dotyczącą rozwoju schematów i ich wpływu na rozwój cech zaburzonej osobowości.
The ICD-10 classification describes personality disorders as deeply fixed behavioural patterns, which are abnormal when compared to average patterns accepted by a given community, and which are characterized by a low flexibility of reactions to various individual and social situations, as well as difficulties in various scopes of the sociopsychological functioning. Personality is the internal regulating system enabling adaptation and internal integration of thoughts, feelings and behaviour in a given environment within a timeframe (the feeling of stability). Disorder is an outcome of maladjustment of the needs, the way of their fulfilment and the manner of tasks realisation by an individual at a specific stage of his or her life to the social requirements, in other words it is the maladjustment of the individual activity to the social and cultural requirements operating in the environment of the individual. In the case of a possibility of patoloan abnormal personality development in a young person, the diagnostic difficulty consists in a lack of clear diagnostic criteria for children and adolescents, who reveal much more permanent and insusceptible to changes mechanisms of functioning. The aim of this paper is to present a differentiation based on defining the functioning organization of neurotic and borderline personalities, on the basis of two examples of female patients during the period of adolescence who reveal eating disorders. Presented cases reveal similarities within the clinical symptoms of eating disorders, but the role of these symptoms in patients’ lives is different, as is the understanding of them. In the neurotic organization of personality the main defensive mechanisms are focused around denial and other developmentally advanced defensive activities, and in the case of the borderline disorders mainly primitive defensive activities dominate, which is presented by the clinical illustrations.

Klasyfikacja ICD-10 opisuje zaburzenia osobowości jako głęboko utrwalone paradygmaty zachowań, odbiegające od przeciętnych wzorców przyjętych w danej kulturze. Charakteryzują się mało elastycznymi reakcjami na różne sytuacje indywidualne i społeczne oraz trudnościami na wielu płaszczyznach funkcjonowania psychologiczno-społecznego. Osobowość to wewnętrzny system regulacji pozwalający na adaptację i wewnętrzną integrację myśli, uczuć i zachowania w określonym środowisku w wymiarze czasowym (poczucie stabilności). Terminem zaburzenie określa się efekt niedopasowania potrzeb, sposobu ich zaspokajania i sposobu realizacji zadań jednostki na danym etapie życia do wymogów społecznych, czyli aktywności własnej do wymagań społecznych i kulturowych obowiązujących w środowisku jednostki. W przypadku podejrzenia nieprawidłowo kształtującej się osobowości u młodego człowieka problem stanowi wyraźny brak kryteriów diagnostycznych dla dzieci i adolescentów, którzy przejawiają znacznie trwalsze i niepoddające się zmianom mechanizmy funkcjonowania. Celem pracy jest ukazanie problematyki osobowości nieprawidłowo kształtującej się na przykładzie dwóch pacjentek w okresie adolescencji z zaburzeniami odżywiania oraz cechami nieprawidłowo kształtującej się osobowości – w kierunku neurotycznej i z pogranicza. W prezentowanych przypadkach występuje podobieństwo kliniczne objawów z zakresu odżywiania się, jednak rola tych symptomów w życiu chorych oraz ich rozumienie są różne. W neurotycznej organizacji osobowości główne mechanizmy obronne skupiają się wokół wyparcia oraz innych zaawansowanych rozwojowo działań defensywnych, a w przypadku zaburzeń typu borderline dominują przede wszystkim prymitywne obrony, co miały pokazać przedstawione przypadki chorych.
Emotionally labile personality of borderline type (borderline personality) occurs in 1-2% of individuals from general population; 75% of this group are women. Similarly to most of the other mental disorders, the borderline personality results from a combination of biological, social and psychological factors. The subject of this study is a survey of the current knowledge on biological factors of borderline personality. Most researchers are of the opinion that these personality disorders are determined genetically, with such inherited temperamental traits as: dysregulation, impulsivity, and hypersensitivity. Perhaps hereditary is also a defect within the serotonergic system, endogenous opioid system and/or dopaminergic system related to the reward system. Many researchers have recently perceived the dysfunction of endogenous opioid system as an integral component of borderline personality. There is now a lot of evidence showing that this dysfunction as well as that of the reward system may account for most of the borderline personality symptoms which constitute an involuntary attempt of stimulating the inefficient systems. This is how e.g. the presence of reckless sexual behaviours, unstable interpersonal relationships and inability to delay the reward in borderline personality is accounted for. Such observations may in the future constitute an important indication for seeking a more effective pharmacotherapy for patients with borderline personality. It is possible that in some patients the described dysfunctions may be alleviated with time. This is implied by the results of comprehensive prospective studies which show a significant regression of symptoms and improvement in functioning of most patients with borderline personality after at least several years.

Osobowość chwiejna emocjonalnie, typ borderline (osobowość borderline) występuje u 1-2% osób z populacji ogólnej, przy czym 75% tej grupy stanowią kobiety. Podobnie jak większość innych zaburzeń psychicznych, osobowość borderline jest wynikiem działania kombinacji czynników biologicznych, społecznych i psychologicznych. Przedmiotem tej pracy jest jedynie przegląd aktualnej wiedzy na temat biologicznych uwarunkowań osobowości borderline. Większość badaczy uważa, że te zaburzenia osobowości mają uwarunkowania genetyczne, a przedmiotem dziedziczenia są pewne cechy temperamentu: dysregulacja, impulsywność oraz hipersensytywność. Być może dziedziczny jest także defekt w zakresie układu serotoninergicznego, endogennego układu opioidowego i/lub układu dopaminergicznego związanego z układem nagrody. Dysfunkcja endogennego układu opioidowego postrzegana jest ostatnio przez wielu badaczy jako integralna składowa osobowości borderline. Istnieje obecnie wiele dowodów, że dysfunkcja endogennego układu opioidowego oraz układu nagrody może wyjaśniać większość objawów osobowości borderline, które są nieświadomą próbą stymulacji niewydolnych układów. W ten sposób u osób z osobowością borderline tłumaczy się obecność np. ryzykownych zachowań seksualnych, niestabilnych relacji interpersonalnych, niezdolności do odroczenia nagrody. Spostrzeżenia te mogą w przyszłości stanowić istotną wskazówkę do poszukiwania bardziej skutecznej farmakoterapii pacjentów z osobowością borderline. Niewykluczone, że opisane dysfunkcje mogą zmniejszać się u niektórych pacjentów wraz z upływem czasu. Wskazują na to wyniki dużych badań prospektywnych, wykazujących istotne ustąpienie objawów i poprawę funkcjonowania większości pacjentów z osobowością borderline po co najmniej kilku latach.
