The objective of this study was to investigate the effect of an aquatic intervention on the gross motor function and aquatic skills of children with cerebral palsy (CP). Twenty-nine children with CP, aged 5 to 14, were recruited. Fourteen children completed an aquatic intervention (EG), and 13 children served as controls (CG). Two participants dropped out due to events (illness) unrelated to the intervention. The aquatic intervention lasted 6 weeks (2 sessions per week at 55 minutes per session) with a follow-up period of 3 weeks. The outcome measures were the Gross Motor Function Measure (GMFM) for motor function and the Water Orientation Test Alyn 2 (WOTA 2) for aquatic skills assessment. A significant improvement was observed in the secondary assessment of GMFM and WOTA 2. In contrast to the aquatic skills improvement, the GMFM change was not maintained at follow-up. Our results indicate that children with CP can improve gross motor function on dry land and aquatic skills with a 6-week water intervention. The intervention period was too short for sustainable improvement in dry-land motor skills after intervention (followup), but time was sufficient to achieve sustainable improvements in aquatic skills.
Bacterial meningitis is a major diagnostic and therapeutic problem among children and neonates, with severe, rapidly progressing course and potentially life-threatening complications. Early antibacterial treatment is essential for the patient’s favorable prognosis. Cerebral imaging plays an important role in the diagnostic process alongside physical examination and laboratory tests. Magnetic resonance imaging is the gold standard for diagnosing bacterial meningitis. Because of limited availability of magnetic resonance imaging, cranial ultrasound is the first imaging procedure to be performed (if the anterior fontanelle is preserved providing an adequate acoustic window). The safety and reliability of ultrasound examination, possibility to perform the examination at bedside without the need for sedation make cranial ultrasound a useful tool both for preliminary diagnostic investigation and for the monitoring of both treatment and long-term complications. Sonographic findings in patients with bacterial meningitis and possible complications are diverse. Changes can be seen on the surface of the brain, in the extra-axial space, in the ventricular system and in brain tissue. In some cases they can also be visible in the lumbosacral segment of the spinal cord. This paper presents ultrasound characteristics of lesions associated with bacterial meningitis in neonates and infants, based on the authors’ own material and data from the available literature.

Bakteryjne zapalenie opon mózgowo-rdzeniowych w grupie noworodków i niemowląt stanowi istotny problem diagnostyczno-terapeutyczny. Przebieg tych zakażeń zwykle jest ciężki, septyczny i szybko prowadzi do groźnych dla życia powikłań. Dla rokowania pacjenta najważniejsze jest jak najszybsze włączenie leczenia antybakteryjnego. W procesie diagnostycznym, oprócz badania fizykalnego i badań laboratoryjnych, istotną rolę odgrywają badania obrazowe. Złoty standard diagnostyki obrazowej w przypadku podejrzenia bakteryjnego zapalenia opon mózgowo-rdzeniowych stanowi rezonans magnetyczny. Jednak ze względu na jego ograniczoną dostępność jednym z pierwszych wykonywanych badań obrazowych jest badanie ultrasonograficzne mózgowia (w przypadku zachowanego i adekwatnego okna akustycznego w postaci ciemienia przedniego). Udokumentowane bezpieczeństwo i wiarygodność diagnostyki ultrasonograficznej, brak konieczności sedacji dziecka oraz możliwość wykonania badania przy łóżku pacjenta sprawiają, że badanie ultrasonograficzne mózgowia stanowi użyteczne narzędzie zarówno we wstępnej diagnostyce, jak i w trakcie monitorowania leczenia oraz powikłań późnych. Obraz ultrasonograficzny bakteryjnego zapalenia opon mózgowo-rdzeniowych i możliwych powikłań jest zróżnicowany. Zmiany mogą być widoczne na powierzchni mózgowia, w przestrzeni przymózgowej, w obrębie układu komorowego oraz w zakresie tkanki mózgowej. W niektórych przypadkach mogą być również widoczne w odcinku lędźwiowo-krzyżowym rdzenia kręgowego. W publikacji przedstawiono obraz ultrasonograficzny zmian możliwych do uwidocznienia w przebiegu bakteryjnego zapalenia opon mózgowo-rdzeniowych w grupie noworodków i niemowląt na podstawie materiału własnego z uwzględnieniem danych z dostępnego piśmiennictwa. Aims: Accurate assessment of inflammatory bowel disease (IBD) activity is the cornerstone of effective therapy. Fecal M2 isoform of pyruvate kinase (M2-PK) and fecal calprotectin (FC) are noninvasive markers of mucosal inflammation in IBD. The aim of this study was to compare performance of M2-PK and FC in assessment of pediatric ulcerative colitis (UC) and Crohn's disease (CD) severity and activity. Materials and methods: 121 patients with IBD, including 75 with UC and 46 with CD were recruited. Control group consisted of 35 healthy children (HS). Patients were assigned to groups depending on disease severity and activity. M2-PK and calprotectin concentration were determined in stool samples using ELISA. Areas under receiver operating characteristic curves (AUC) for FC and M2-PK with cut-off level at which M2-PK specificity was matching FC specificity were calculated and compared. Results: Performance of M2-PK at identifying patients with IBD, UC and CD among HS was inferior to FC. The differences in AUC were respectively: -0.10 (95% confidence interval [CI] [-0.13-(-0.06)], p<0.0001), -0.14 (95% CI [-0.19-(-0.09)], p<0.0001) and -0.03 (95% CI [-0.05-(-0.001)], p<0.02). M2-PK was inferior to FC in discriminating patients with mild UC from those with HS (AUC difference -0.23, 95% CI [-0.31-(-0.15)], p<0.0001). Conclusions: FC reflects pediatric IBD severity and activity better than M2-PK. This difference is particularly pronounced when identifying patients with mild UC and UC in remission.
Three-dimensional ultrasound is an established diagnostic imaging technique in many specialties. However, in neonates, infants and children three-dimensional ultrasound still is underutilized, partially due to time constraints for post-processing and restricted availability, of devices as well as dedicated pediatric transducers. Also reimbursement issues still need to be addressed. This editorial review presents more or less established pediatric three-dimensional ultrasound applications with proven diagnostic benefi t as well as potential future applications of three-dimensional/four-dimensional ultrasound in infants and children, aiming at enhancing research and promoting practical use of three-dimensional ultrasound in relevant pediatric conditions. Particularly, applications in neonatal neurosonography, ultrasound of the urogenital tract as well as some other small part and miscellaneous queries are highlighted. Additional other potential and future indications are discussed briefl y, also mentioning restrictions and potential future developments. In summary, three-dimensional ultrasound holds some potential to widen sonographic diagnostic capabilities throughout childhood and hopefully will be increasingly investigated and introduced into clinical practice provided respective equipment and pediatric three-dimensional/four-dimensional ultrasound transducers become available.

Ultrasonografi a trójwymiarowa znalazła uznanie wśród specjalistów wielu dziedzin medycyny, jednak nadal wykorzystywana jest w niedostatecznym stopniu w badaniu noworodków, niemowląt i dzieci. Powodem tego są przeszkody wynikające z wydłużenia czasu badania oraz ograniczonej dostępności aparatów ultrasonografi cznych i odpowiednich, pediatrycznych głowic; nie bez znaczenia pozostaje również kwestia refundacji badań. W pracy przedstawiono możliwości zastosowania ultrasonografi i trójwymiarowej we wskazaniach o udowodnionej wartości diagnostycznej oraz potencjalne przyszłe obszary praktycznego zastosowania badania z użyciem opcji trójwymiarowej i czterowymiarowej u niemowląt i dzieci. Omówiono użyteczność tych technik w neurosonografi i, badaniach układu moczowo-płciowego i obrazowaniu drobnych struktur anatomicznych, a także przedstawiono ograniczenia oraz prawdopodobne kierunki rozwoju. Autor wnioskuje, że ultrasonografi a trójwymiarowa umożliwia poszerzenie diagnostyki obrazowej w pediatrii, jednak upowszechnienie metody uzależnione jest od zwiększenia dostępności odpowiedniej aparatury.
