Primary Sjögren syndrome (pSS) is a systemic autoimmune disease and can be a serious disease with excess mortality due to severe organ-specific involvements and the development of B-cell lymphoma. PSS is characterized by lymphocytic infiltration of exocrine glands, resulting in glandular dysfunction. Similarly to many other autoimmune diseases, the inciting factors that precipitate pSS are poorly understood. Patients with pSS have specific lymphocytic infiltration of salivary glands. While this infiltration is well characterized, the pathologic events that precede and cause this inflammatory cell recruitment are unknown. This review summarizes the potential cytokine families involved in the etiopathogenesis of primary pSS.

Pierwotny zespół Sjögrena (PZS) należy do grupy chorób autoimmunologicznych. W swoim poważnym przebiegu wiąże się ze zwiększoną śmiertelnością poprzez zajęcie narządów wewnętrznych oraz rozwój chłoniaków B komórkowych. Dla PZS charakterystyczne jest tworzenie się nacieków limfocytarnych powodujących dysfunkcję gruczołów wydzielania wewnętrznego. Podobnie jak w wielu innych chorobach autoimmunologicznych nie są znane czynniki zapoczątkowujące proces chorobowy. U chorych na PZS stwierdza się typowe nacieki limfocytarne w gruczołach ślinowych. Pomimo, że nacieki te są dobrze scharakteryzowane, nie znamy dotychczas patogenezy ich powstawania. W pracy przedstawiono podsumowanie potencjalnych grup cytokin zaangażowanych w etiopatogenezę PZS.
The pathogenesis of subepidermal autoimmune blistering diseases, such as pemphigoid (BP, ang. bullous pemphigoid) and dermatitis herpetiformis (DH, ang. dermatitis herpetiformis, morbus Duhring) is connected with the destruction of basement membrane components and blister formation. Ultrastructural studies confirmed the basement membrane zone as a structure responsible for the integrity of dermo-epidermal junction. The binding of autoantibodies in BMZ zone leads to the activation of immunological and enzymatic mechanisms leading to the destruction of hemidesmosomes and anchoring fibres. Despite a similar initial stage: the connection of autoantibodies with autoantigens ,which are the proteins of the basement membrane structures, the further development of the disease mechanisms is different. The Differences of antigens and the activation of other cytokines, chemokines and enzymes imply the characteristic histological immunological picture, and require a different therapy. In BP and DH the impact of the disorders of certain cytokines expression , especially proinflammatory and complement components, on the development of inflammatory infiltrates and the formation of subepidermal blisters was shown. These phenomena are the cause of the activation of adhesion molecules and subsequent inflammatory mediators such as chemokines, neuropeptides and proteolitic enzymes. Recent experimental studies have also shown that these processes can activate procoagulant factors causing the development of blood thrombus, which is the most common cause of complications in patients with BP. The goal of the study was: 1. The assessment of the role of cytokine network components, such as adhesion molecules, chemokines, neuropeptides, and markers of activation of eosinophils and mast cells in the formation of skin lesions in pemphigoid and dermatitis herpetiformis. 2. The assessment of the role of selected proteolytic enzymes: metalloproteinases, adamalysins and their tissue inhibitors in the development of subepidermal blisters. 3. The assessment of procoagulation factors activity and basement membrane destruction caused by them in pemphigoid and dermatitis herpetiformis. 4. The indication of coagulation parameters useful in the prevention of complications in therapy of BP patients. The study included 61 persons: 27 untreated patients with BP (range: 58 to 84 years, average - 68,5) and 14 with DH (range: 18 to 70 years, average – 49,8) in an active stage of the disease. A Control group consisted of 20 healthy individuals (range:50 to 80 years, average – 71,6 for BP patients and ranged:19 to 49, average – 42 for DH patients). All the study subjects gave their consent before entering the study and the study protocol was approved by The Local Ethical Committee of Medical University of Lodz. Paraffin-embedded sections were used for routine staining and for immunohistochemistry to assess the expression of adhesion molecules, chemokines and their receptors, neuropeptides, metalloproteinases and their inhibitors and adamalysines. The following primary monoclonal antibodies were used: 1. adhesive molecules: anty- CD29 (β1 family), CD61 (GPIII) (β3 family), CD104 (β4 family) CD 62E (E-selectin), CD62L (L-selectin) from Novocastra 2. chemokines and their receptors: anty- eotaksyna, TARC, MCP-1, CCR- 1, CXCR-1, CXCR-2 from R&D. 3. neuropeptides: anty- Neurokinin B antibody, Endothelin B Receptor, CGRP, Corticotropin Releasing Factor and Substance P, from Abcam. 4. metalloproteinases: anty- MMP1, MMP2, MMP9, MMP1, TIMP1, TIMP2, TIMP 3 from Novocastra. 5. adamalisyns: anty- ADAM8, ADAM10, ADAM15, ADAM17 from R&D. 6. anty- TNF α from R&D. 7. tissue factors: anty- TF from American Diagnostica Inc. The Significantly increased expression of adamalysines: ADAM8, ADAM10, ADAM15, ADAM17 in the tissues (immunohistochemistry technique assessed) was confirmed by in situ hybridization and immunoblotting. In order to determine the concentrations of the examined protein in the serum the enzyme-linked immunoassay method was used using ELISA kits: TARC, MCP-1, eotaxin from R&D ECP – ELISA Kit for Human Ribonuclease A3 from Uscn Life Science; PAF ELISA Kit for Human Platelet Activating Factor (PAF from Uscn Life Science, Tryptaza, ELISA Kit for Human Tryptase) from Uscn Life Science, Chymaza ELISA Kit for Human Chymase 1 Mast Cell from Uscn Life Science, TNFα Human TNFα -ELISA Kit from Gen-Probe Diaclone, IL-4 Human IL-4 ELISA Kit from Gen-Probe Diaclone, Neurokinin B, Endotelin B, CGRP, Substance P ELISA kit from R&D, MMP1, MMP2, MMP9 i TIMP2 ELISA kit from Quantikine, R&D Systems, IMUBIND Tissue Factor ELISA Kit from American Diagnostica Inc. The Indications of d-dimers were performed using the method of optical test apparatus Turbidity STA Compact (Roche Diagnostics). The Determination of the concentration of fibrinogen was performed using chronometric Clauss and Turbidity method (during the determination coagulation parameters). The results were statistically analyzed. The Results were considered statistically significant with p < 0.05. The localization and expression of adhesive molecules: E- and L-selectins and β1, β3, β4 integrins was studied by immunohistochemistry.The Expression of selectins was detected in the epithelium and skin leukocytes in all DH and BP samples. The expression of β1, β3 integrins was detected mainly in basal keratinocytes as well as focally in the other layers of the epidermis. The expression of β4 integrin was irregular and detected mainly in the blister. In the control group only weak expression of the examined molecules was detected. The expression of one of the chemokines: eotaxin was found in the epidermis and in the cells of inflammatory infiltrate in the epidermis. The expression of eotaxin was significantly higher in the patients with BP compared to the healthy subjects, as well as in the patients with DH compared to the healthy subjects (infiltration - p < 0.02, keratinocytes - p < 0.03). In the patients with BP TARC (thymus and activation regulated chemokine) expression was present in keratinocytes of the lower layers of the epidermis around lesions in individual cells and the inflammatory infiltrate located in the epidermis. TARC expression was significantly higher in the patients with BP compared to the healthy subjects (infiltration - p < 0.001, keratinocytes – p < 0.006) and skin lesions in relation to the perilesional skin in BP (infiltration p < 0.009 and keratinocytes, p < 0.001). In the biopsies from the skin lesions in DH the average intensity of expression TARC was observed in most cases. In keratinocytes within the lesions it differed significantly between the patients with expression of BP and DH (p < 0.05), as well as in keratinocytes of the perilesional skin (p < 0.05). In the control group there was very weak, focal expression of TARC of single keratinocytes located in the lower layer of the epidermis. The expression of chemokin MCP-1 (monocyte chemoattractant protein-1) was present in skin lesions and the perilesional skin in the patients with BP. There was no expression of MCP-1 in DH patients and the control group. The expression of chemokine receptor: CCR-1 in skin lesions and the perilesional skin of the patients with BP was similarly weak. There was no statistically significant difference between the expression of CCR-1 in skin lesions and the perilesional skin in BP and DH. The expression of receptor for chemokines CXCR-1 was present in the biopsies of skin lesions in the patients with BP. CXCR-1 expression was significantly higher in the skin lesions in relation to the perilesional skin in BP (p < 0.03). In all the cases of patients with DH cells expressing CXCR-1were found in lesions. CXCR-1 expression was significantly higher in the patients with DH compared to the healthy subjects (p < 0.001). The Patients with DH showed higher expression of CXCR-1 in the biopsies of lesions compared to the patients with BP (0.67±0.18 vs. 0.15±0.24, p < 0.001). In the skin lesions in BP patients, receptor for chemokines CXCR-2 expression was demonstrated in all specimens and was significantly higher compared to the perilesional skin in BP (p < 0.02) as well as the expression in the healthy subjects (p < 0.04). In the biopsies taken from the blisters in DH patients in all cases the expression of CXCR-2 was present. The Patients with DH showed significantly higher expression of CXCR-2 in skin lesions compared to the patients with BP (1.1±0.87 vs. 0.31±0.24, p < 0.001). There was no expression of CCR-1, CXCR-1, CXCR-2 in the control group. The concentrations of MCP-1 levels were statistically higher in the patients with BP (374.8±30.8 pg/mL) compared to the patients with DH (282.2±21 pg/mL, p < 0.05), although there was no statistically significant difference in comparison to the control group. The concentration of ECP (283.9±66.92 pg/mL) and PAF (136.9±29.87 pg/mL) levels was significantly higher in BP than in the healthy subjects and the patients with DH (p < 0.05). The Concentration of chymase (0.21±0.98 pg/mL) was significantly higher in the patients with BP compared to the healthy subjects and the patients with DH. There was no difference in the concentrations of tryptase, IL-4 and TNF in the treatment groups. The expression of TNFα was demonstrated in inflammatory infiltrates and keratinocytes and in blister liquid of BP and DH patients. The Expression of TNFα was also confirmed in the perilesional skin. Morphometric analysis showed a higher expression of TNFα in the skin lesions as amended in relation to the perilesional skin (1.59±0.58 BP vs. 1.01±0.43, DH 1.39±0.84 vs. 0.69±0.59) as well as in the control groups vs. the control group (p < 0.01). The expression of neither the examined chemokines nor their receptors was observed in the biopsies from the healthy people. The expression of the neuropeptide: corticoliberin CRF (corticotrophin releasing factor) was present in keratinocytes, cells of the sebaceous glands and sweat glands, and the infiltration of inflammatory cells. The results had significantly higher expression of CRF in the biopsies derived from the skin lesions than in the skin biopsies taken from the perilesional skin, and this applied both to the individuals with BP (1.58±0.64 vs. 1.04±0.48, p < 0.001) and with the DH (1.36±0.89 vs. 0.71±0.61, p < 0.05). The lowest expression of CRF was demonstrated in the healthy subjects (0.36±0.15). The expression of endothelin receptor B was found in the endothelial cells, vascular muscle and keratinocytes. This phenomenon was observed for the increased expression of endothelin receptor B in the individuals with DH. This value was significantly higher in the skin lesions than in the perilesional skin (1.24±0.81 vs. 0.66±0.43, p < 0.04). There was no difference in the expression of endothelin B receptor in the perilesional skin in DH patients and the control group (0.66±0.43 vs. 0.58±0.32). The Expression of Substance P, CGRP and neurokinin B was demonstrated in the nerve endings in the epidermis and the skin. The expression of these neuropeptides in the groups was not statistically significant. The expression of metalloproteinases - MMP1, 2, 9 and 10 (MMP, matrix metalloproteinase) in patients with BP was found both in the basal layer and the whole epidermis and in the fluid blister and inflammatory infiltrates. In biopsies from the skin lesions with the blister the linear expression of MMPs has been shown in the bottom of the blister. In patients with DH MMPs expression was found in the basal layer of keratinocytes in all the biopsies from the lesion, in most biopsies it was very intense. The Expression of MMPs is not limited to the basal layer of keratinocytes, but was also present in medium intensity in the other layers of epidermal keratinocytes. The expression of these MMPs was also found in neutrophils, blister fluid and microabscesses. In contrast to the MMPs, the expression of their inhibitors - TIMPs (tissue inhibitor of martix metelloproteinases) was moderately or poorly pronounced. The expression of TIMP1, 2, 3 in the patients with BP was found throughout the epidermis. In the presence of the blister it was ascertained at its bottom. Moderate activity of these enzymes was demonstrated in inflammatory infiltrates and blister fluid. The expression of TIMP1, 2, 3 of medium intensity in all specimens was demonstrated throughout the epidermis of the patients with DH, as well as in neutrophilic microabscesses. In the studied serum of the patients with DH and BP the concentrations of MMP1, MMP2 and TIMP2 are within normal limits. Individual values of MMP9 concentrations were elevated in the patients with BP, in the remaining members of the studied groups they were normal. In the comparison group they were not elevated in the serum concentrations of these enzymes. The expression of adamalysins: ADAM8, 10, 15 and 17 (ADAM, a disintegrin and metalloproteinases domain) was found in the cells of the basal layer and other layers of the epidermis and dermis in BP and DH. Eosinophilic and neutrophilic infiltration in the skin lesions of patients with BP showed a mean intensity expression. The Expression of ADAMs was also shown in perivascular influx and microabscesses in DH. The Expression of ADAM8, 10, 15 and 17 in the control group in all the biopsies was detected only in single keratinocytes. Morphometric analysis showed a higher expression of ADAM8, 10, 15 and 17 in the skin lesions compared to the perilesional skin in keratinocytes, inflammatory infiltration in both the group of patients with BP and DH. Morphometric analysis showed a significantly higher expression of ADAM15 in the skin lesions in BP and in the perilesional skin compared to the patients with DH (p < 0.001). In a study by immunoblotting a significantly higher expression of ADAM17 was detected in the patients with BP compared to the patients with DH (p = 0.007 and 0.007, respectively) and the control group (p = 0.007 and p = 0.006 respectively). The Immunohistochemical study was confirmed by in situ hybridization. The expression of tissue factor (TF, tissue factor) was demonstrated in the neutrophils in inflammatory infiltrates and keratinocytes and blisters fluid of the patients with BP. TF expression was also shown in the perilesional skin. Morphometric analysis showed a higher expression of TF in the skin lesions in comparsion to the perilesional skin in the patients with BP (p < 0.02). The studies of the patients with DH showed no significant expression of the tissue factor in pathologically changed, and normal appearing skin. TF was also detected in the specimens from the healthy individuals. The Expression, however, in this group was significantly lower than in the lesions in BP and comparable to the lesions in DH. In the group of the patients with BP serum TF concentration was 847.2±402.7 values, in the patients with DH 285.7±187.7 pg/mL, and the comparison group for BP - 252.6±169.3 pg/mL, and for DH 221.9±153.3 pg/mL. These results indicate a statistically significant difference in the TF values between the group of patients with BP and DH and BP and the control group (p <0.001). The values of d-dimers in the group of patients with BP before the treatment, most patients were in the upper limits of normal and averaged: 0.49±0.40 mg/mL (0.33-2.9 mg/mL). In 7 patients the values were elevated and had a range between 1.2 and 2.9 μg/mL. These values were higher than in the group of patients with DH and the comparison group. In all patients with DH values of d-dimers correspond to the standard dimers and their average value was 0.31±0.19 mg/mL (0.26-0.36 g/mL). In the control group for patients with BP in an individual d-dimer values exceeded the normal in the other corresponded to the standard 0.29±0.3 mg/mL (0.24-0.67 g/mL), the comparison group for all patients with DH values were normal - mean: 0.34±0.6 mg/mL. There is a statistically significant difference between the group of patients with BP and DH and BP and a group comparison of the value of d-dimers (p < 0.05). In the patients with BP the value of fibrinogen is an average 4.50±0.18 g/L at normal 2.0-5.0 g/L. The Patients with DH had a mean fibrinogen value 2.80±0.14 g/L in the control group for BP - 2.60±0.3 g/L and for DH - 2.45±0.26 g/L. There is a statistically significant difference between the group of patients with BP and DH and BP and the comparison group in the value of fibrinogen (p < 0.05). The obtained results allowed to formulate the following conclusions: 1. Differences in the expression of adhesion molecules and chemokines and their receptors are the cause of a different composition of the inflammatory infiltrate in dermatitis herpetiformis and pemphigoid, in which a special role is played by tissue factor activating eosinophiles. 2. Increase in expression of neuropeptides is one of the factors of development neuroimmunological inflammation and itching in the DH and BP. 3. The increased activity of metalloproteinases and adamalysines and their tissue inhibitors and their balance disorder is the cause of the activation of cytokines and tissue destruction in the course of BP and DH. These data offer new prospects for therapeutic BP and DH. 4. The increase in tissue factor expression in skin lesions and the serum of patients with BP indicates the activation of the coagulation cascade and the coexistence of elevated levels of d-dimer and fibrinogen, points to the particular predisposition to thromboembolic complications of glicocorticosteroids in therapy. 5. The determination of coagulation parameters should be the standard before the planned treatment with glicocorticosteroids in patients with pemphigoid.
Large vessel vasculitis that include giant cell arteritis (GCA) and Takayasu arteritis (TAK), combine autoimmune and autoinflammatory components to a degree that depends on acquired or innate immune response involvement. In GCA, both components may occur together, independently or sequentially. Polymyalgia rheumatica is a part of GCA-disease spectrum mainly associated with innate immune response hyperactivity. It is sometimes considered to be a form of “vasculitis sine vasculitis”. In TAK there are three-phases of the disease traditionally described: from non-specific inflammatory reaction to lymphocytic infiltration of arterial walls leading to their destruction. This article outlines the basics of pathogenesis of large vessel vasculitis, highlighting the links between acquired and innate immunity responsible for the heterogeneous clinical picture of these diseases. It also presents practical clinical implications of understanding pathogenesis.

Zapalenia dużych naczyń, czyli olbrzymiokomórkowe zapalenie tętnic (GCA) i choroba Takayasu (TAK), łączą komponenty autoimmunologiczne i autozapalne w stopniu zależnym od przewagi nabytej lub wrodzonej odpowiedzi immunologicznej. W GCA obie składowe mogą występować łącznie, samodzielnie lub też sekwencyjnie. Polimialgia reumatyczna jest chorobą ze spektrum GCA związaną z nadmierną aktywnością głównie odpowiedzi wrodzonej. Bywa ona uważana za formę „vasculitis sine vasculitis”. W TAK tradycyjnie wymienia się składowe trójfazowego przebiegu choroby: od niespecyficznej reakcji zapalnej do limfocytowych nacieków ściany tętnic prowadzących do ich uszkodzenia. Artykuł przybliża podstawy patogenezy zapaleń du- żych naczyń, podkreślając połączenia między komponentami odporności nabytej i wrodzonej, odpowiedzialne za niejednorodny obraz kliniczny tych chorób. Hypercalcaemia is a serious metabolic complication of neoplastic diseases. Among hematological malignancies, hypercalcemia is particularly associated with multiple myeloma, it has rarely been reported in patients with non-Hodgkin's and Hodgkin's lymphomas, or in cases of acute and chronic myeloproliferative diseases. The pathogenetic mechanism of hypercalcaemia is complex. A key process is an increase in osteoclast activity as a result of disturbances in the NF-kB ligand (RANKL) receptor activator system, its receptor (RANK) and osteoprotegerin. In addition, many other humoral factors were identified – hormones (for example the parathyroid hormone related peptide), vitamin D active metabolites, cytokines/chemokines and their receptors, and also transcription factors which are responsible for bone resorption and the development of hypercalcaemia in cases of particular neoplasms. This article presents the current opinions on the pathogenesis, symptomatology and current treatment methods of hypercalcaemia. Besides that, new experimental methods of therapy are described.

Hiperkalcemia jest poważnym powikłaniem metabolicznym rozwijającym się w przebiegu wielu chorób nowotworowych – w hematologii najczęściej spotykana jest w szpiczaku plazmocytowym, rzadziej w chłoniakach niehodgkinowskich i chorobie Hodgkina oraz w ostrych i przewlekłych schorzeniach mieloproliferacyjnych. Mechanizm patogenetyczny hiperkalcemii jest złożony – za kluczowy proces uważa się wzrost aktywności osteoklastów, wynikający z zaburzeń w układzie: ligand receptora aktywującego czynnik transkrypcyjny NF-kappaB (RANKL), jego receptor (RANK) i osteoprotegeryna (OPG). Poza tym zidentyfikowano wiele innych czynników, w różnym stopniu zangażowanych w rozwój hiperkalcemii, w zależności od typu choroby – hormony (m.in. białko podobne do parathormonu), aktywne metabolity witaminy D, cytokiny/chemokiny i ich receptory oraz czynniki transkrypcyjne. W pracy przedstawiono współczesne poglądy na patogenezę hiperkalcemii, symptomatologię oraz aktualnie stosowane i pozostające w fazie badań metody jej leczenia.
Vitiligo is a chronic depigmentation disease. It is characterized by damage or destruction of melanocytes (melanin-producing cells). The desease affects about 2% of population. The pathogenesis is complex and not fully understood. Recent insights into vitiligo pathognenesis highlight impact of various factors like environmental factors, genetic factors, cellular stress and autoimmune factors. In vitiligo patients abnormal response of innate immune system is observed. The latest research shows that the IFN-γ/CXCL10 axis plays a special role in the pathogenesis of vitiligo. Interferon γ (IFN-γ) can be produced by autoreactive CD8 + T cells. Receptors for interferon gamma (IFN-γR) are located on the surface of keratinocytes. After binding IFN-γ to its receptor (IFN-γR) there is an activation of the JAK-STAT pathway. That causes the release of the chemokines like CXCL9 and CXCL10. Patients with vitiligo have a higher serum concentration of CXCL10 chemokine compared to healthy people. CXCR3 receptors for the CXCL10 chemokine are located on autoreactive CD8+ lymphocytes. The CXCL10 chemokine binds to its receptor CXCR3. That leads to the migration of autoreactive T cells to the epidermis and destruction of melanocytes. Furthermore, changes in cytokine secretion by T helper cells Th1, Th2, Th17 and regulatory T cells (Treg) may contribute to disease progression. In vitiligo is observed elevated levels of IL-2, IL-6, IL-17 and decreased level of IL-4. Current treatment methods are not satisfying and mainly based on the use of off-label drugs. Better understandig etiology of vitiligo enable searching for new effective treatment methods. To minimize systemic side effects, topical drug forms are searching. The effectiveness of new combined therapies is also being studied, including topical treatment with phototherapy, surgical methods with phototherapy, general therapy with surgical methods, etc. New potential therapies, especially with JAK/STAT inhibitors, may provide new perspectives for the treatment of vitiligo.

Bielactwo nabyte to przewlekła choroba charakteryzująca się odbarwieniem skóry, będąca następstwem niszczenia lub utraty melanocytów (komórek pigmentowych). Występuje u ok. 2% populacji. Mechanizm powstawania choroby jest złożony i nie został w pełni wyjaśniony. Podkreśla się wpływ czynników środowiskowych, genetycznych, związanych ze stresem oksydacyjnym oraz nieprawidłową odpowiedzią immunologiczną. Obecne metody leczenia bielactwa nabytego głównie wykorzystują zastosowanie leków off-label i cechują się umiarkowaną skutecznością. W związku z postępami dotyczącymi zrozumienia patomechanizmu bielactwa nabytego poszukiwane są nowe metody leczenia, umożliwiające efektywną terapię.
Perlakiem określa się torbielowaty twór zbudowany z nabłonka wielowarstwowego płaskiego rogowaciejącego, wypełniony masami keratyny, otoczony zmienioną zapalnie tkanką łączną, rozrastający się w przestrzeniach ucha środkowego i niszczący okoliczne struktury kostne. W niniejszej publikacji przedstawiamy przegląd istniejących hipotez, wyjaśniających jego powstawanie w świetle nowych badań wykorzystujących wysokoprzepustowe techniki biologii molekularnej. Mimo istnienia licznych teorii, starających się wyjaśnić etiologię perlaka, takich jak: teoria migracji, metapalzji, wgłobienia, wrastania w głąb komórek warstwy podstawnej nabłonka, żadna z nich nie wyjaśnia w pełni wszystkich patologicznych procesów, do jakich dochodzi w tkance perlaka. Dotyczy to również nowych hipotez tłumaczących powstawanie perlaka nabytego mechanizmami, mającymi na celu ograniczenie stanu zapalnego, podobnymi do obserwowanych w obrębie jamy brzusznej, wpływem błony śluzowej ucha na przesuwanie się przylegającej kieszeni bądź mechanizmem gojenia ran, który uruchamiany byłby przez mikrouszkodzenia błony podstawnej nabłonka wyścielającego kieszeń retrakcyjną. Wprowadzenie nowych wysokoprzepustowych technik biologii molekularnej pozwoliło na ocenę zarówno całego genomu, jak i proteomu perlaka. Potwierdzają one w większości znane już wcześniej procesy patologiczne zachodzące w tkance perlaka, takie jak: wysoka aktywność proliferacyjna, zaburzenia sygnalizacji komórkowej, aktywna odpowiedź immunologiczna, zmiany w obrębie macierzy zewnątrzkomórkowej, wzrost ekspresji cytokin prozapalnych, neowaskularyzacja i wiele innych. Pozwalają również na bardzo dokładny i całościowy wgląd w mechanizmy molekularne tych procesów. Nadal jednak nie uzyskujemy odpowiedzi na pytania o przyczynę wywołującą hyperplazję nabłonka i zaburzenia migracji komórek nabłonkowych czy przyczynę utrzymywania się procesu zapalnego w obrębie istniejącej kieszeni retrakcyjnej.
Sporadic inclusion-body myositis (s-IBM), the most common muscle disease of older persons, is of unknown cause, and there is no successful treatment. Interest in sporadic inclusion-body myositis has been enhanced by the recent identification within the s-IBM muscle fibers of several abnormally accumulated proteins, which provides novel and important clues to the pathogenesis of this disorder. Here we summarize the clinical presentation, molecular phenotype, diagnostic criteria, and the newest advances related to seeking the pathogenic mechanism(s) of s-IBM. On the basis of our research, several processes seem to be important in relation to the still speculative pathogenesis: 1) increased transcription and accumulation of amyloid-β precursor protein (AβPP), and accumulation of its proteolytic fragment Aβ; 2) abnormal accumulation of cholesterol and its related protein; 3) oxidative stress; 4) accumulations of intramuscle fiber multiprotein aggregates; 5) increased accumulation of myostatin within the muscle fiber and 6) evidence that unfolded/misfolded proteins participate in s-IBM pathogenesis. Our basic hypothesis is that overexpression of AβPP within the aging muscle fibers is an early upstream event causing a subsequent pathogenic cascade.

Wtrętowe zapalenie mięśni (s-IBM) jest najczęstszą chorobą mięśni szkieletowych występującą u starszych osób. Przyczyna tej choroby pozostaje nieznana i jak dotąd brak jest skutecznego leczenia. Na wzrost zainteresowania s-IBM wpłynęło przede wszystkim odkrycie wewnątrz włókien mięśniowych wielu nadmiernie/nieprawidłowo gromadzonych białek, co pozwoliło na wyciągnięcie nowych wniosków dotyczących patogenezy tej choroby. W poniższej pracy podsumowujemy obraz kliniczny, charakterystyczne zmiany patologiczne oraz kryteria diagnostyczne. Na bazie naszych doświadczeń wskazujemy również na kilka zjawisk, które wydają się szczególnie istotne w patogenezie s-IBM. Zaliczyć do nich należy: 1) zwiększoną transkrypcję i nadmierne gromadzenie białka prekursorowego β-amyloidu (AβPP) oraz gromadzenie jego fragmentu β-amyloidu; 2) nieprawidłowe gromadzenie cholesterolu i związanych z nim białek; 3) stres oksydacyjny; 4) gromadzenie wewnątrz włókien mięśniowych wielobiałkowych agregatów; 5) nadmierne gromadzenie miostatyny wewnątrz włókien mięśniowych oraz 6) dowody na to, że białka, które nie posiadają prawidłowej, natywnej konformacji, odgrywają rolę w patogenezie s-IBM. Zgodnie z naszą główną hipotezą nadmierna ekspresja AβPP wewnątrz starzejących się włókien mięśniowych zapoczątkowuje patogenetyczną kaskadę s-IBM.
Uterine papillary serous carcinoma (UPSC) is an extremely malignant tumor associated with a grim prognosis. It accounts for less than 10% of all cases of endometrial cancer. Mean age at presentation is 68 years. Morphological appearance is similar to poorly differentiated serous cancer of the ovary. A characteristic feature is extensive invasion of blood and lymph vessels and rapid intraperitoneal dissemination. Endometrial glandular dysplasia is currently considered a precursor lesion of UPSC. The basic pathogenetic event is mutation of suppressor gene TP53 and overexpression of HER2/neu (human epithelial growth factor receptor 2). UPSC differs morphologically, genetically and clinically from endometrioid cancer. It is usually diagnosed in slender elderly women and is not corre-lated with hormonal risk factors. The serous type, in contrast to endometrioid cancer (EEC) usually developing within atypical proliferative lesion, develops mainly within atrophic endometrium, is characterized by high grade of malignancy (G3) and aggressive clinical course. Metastases within the abdominal cavity and lymph nodes develop rapidly and recurrences are usually distant and multifocal. Every case of endometrial cancer of any grade, with a component of serous cancer, should be treated like serous cancer of the ovary. Properly performed staging includes: hysterectomy with adnexectomy, pelvic and periaortal lymphadenectomy, omentectomy and inspection of the peritoneal cavity, as in the case of ovarian cancer. Adjuvant treatment includes chemotherapy based on platinum and taxane with or without radiotherapy. High frequency of extrauterine serous foci even at early stages of the disease necessitates an aggressive and multimodal therapeutic approach. Unfavorable treatment outcomes in UPSC reported to date, force us to search for novel therapeutic techniques, based mainly on targeted therapies.

Rak błony śluzowej macicy brodawkowaty typu surowiczego (UPSC) to bardzo złośliwy nowotwór związany z poważnym rokowaniem. Stanowi on mniej niż 10% wszystkich przypadków raka endometrium. Średni wiek zachorowania wynosi 68 lat. Obrazem morfologicznym przypomina niskozróżnicowanego surowiczego raka jajnika. Charakteryzuje się znaczną inwazją naczyń krwionośnych i limfatycznych oraz szybkim rozsiewem śródotrzewnowym. Obecnie za zmianę prekursorową raka błony śluzowej trzonu macicy typu surowiczego uznaje się dysplazję gruczołową endometrium. U podstaw jego patogenezy leży mutacja genu supresorowego TP53 i nadekspresja receptora 2. ludzkiego naskórkowego czynnika wzrostu (HER-2/neu). UPSC różni się morfologicznie, genetycznie i klinicznie od raka endometrioidalnego. Diagnozowany jest częściej u szczupłych, starszych kobiet bez związku z czynnikami hormonalnymi. Typ surowiczy, w przeciwieństwie do raka endometrioidalnego (EEC), powstającego najczęściej w przebiegu rozrostu atypowego, rozwija się głównie w obrębie endometrium atroficznego, cechuje się wysokim stopniem złośliwości (G3) i agresywnym przebiegiem klinicznym. Szybko powstają przerzuty w jamie brzusznej i węzłach chłonnych, a wznowy mają charakter odległy i wieloogniskowy. Każdy stopień raka endometrium z komponentą raka surowiczego powinien być leczony jak rak surowiczy jajnika. Właściwie przeprowadzony staging obejmuje: wycięcie macicy z przydatkami, resekcję węzłów chłonnych miedniczych i okołoaortalnych, usunięcie sieci większej oraz ocenę otrzewnej jak w raku jajnika. W leczeniu uzupełniającym stosuje się chemioterapię opartą na schemacie platyna + taksan ± radioterapia. Wysoki odsetek ognisk o utkaniu surowiczym poza macicą we wczesnych stopniach zaawansowania wymusza również agresywne, skojarzone podejście terapeutyczne. Dotychczasowe niekorzystne wyniki leczenia raka surowiczego endometrium wskazują na konieczność poszukiwania nowych metod terapeutycznych opartych głównie na terapii celowanej.

Difficulties in neuropathic pain treatment emerge from the process of structural rebuilding within damaged fibres and neighbouring healthy ones. Early interrupt of the pain prevent the structural rebuilding. Antiepileptic drugs, in addition to antidepressants and opioids, are fundamental and often applied group of medications in neuropathic pain treatment of various aetiology. Their efficacy has been well known for a very long time. The medications of older generation as well as the newest ones are equally applied. Carbamazepine is generally used in treatment of neuropathic pain and the best efficacy and recommendations of this medicine is for trigeminal neuralgia. The medications of newer generation, such as gabapentine and pregabaline, appeared to be most effective in neuropathic pain treatment and diabetic neuropathy as well as after herpes zoster. Most antiepileptic drugs, particularly of the newest generation, exert significant efficiency in neuropathic pain treatment. There is lamotrigine, oxcarbazepine and topiramate among them, evaluated in a number of medical reports as helpful in its treatment.

Wczesne przerwanie dolegliwości bólowych zapobiega tej przebudowie. Leki przeciwpadaczkowe obok leków przeciwdepresyjnych i opioidów są podstawową i często stosowaną grupą w leczeniu bólu neuropatycznego o różnej etiologii. Ich skuteczność jest znana od dawna. Stosowane są zatem zarówno leki starszej generacji, jak i te najnowsze. Karbamazepina jest lekiem powszechnie stosowanym w leczeniu bólu neuropatycznego. Największą skuteczność osiąga w przypadku neuralgii nerwu trójdzielnego i jest rekomendowana do stosowania w tego typu bólach. Leki nowszej generacji, takie jak gabapentyna i pregabalina, okazały się najskuteczniejsze w leczeniu bólu neuropatycznego w przebiegu neuropatii cukrzycowej, jak również po półpaścu. Idiopathic anaphylaxis is a severe, life-threatening systemic or generalized immediate hypersensitivity reaction with symptoms similar to resembling other forms of anaphylaxis. It concers approximately 30-60% of cases in adults and 20% of cases in children. In the pathogenesis of idiopathic anaphylaxis, alpha-gal syndrome, mastocytosis, diseases associated with mast cell activation, and hidden allergens and cofactors -physical exertion, alcohol, drugs can play a significant role. The clinical manifestations of idiopathic anaphylaxis and therapeutic management are identical to those of anaphylaxis with a known causative agent. The paper discusses epidemiology of anaphylaxis, pathomechanism, diagnostics and therapeutic management according to international guidelines.

W patogenezie anafilaksji idiopatycznej istotną rolę mogą odgrywać zespół alfa-gal, mastocytoza, schorzenia przebiegające z aktywacją komórki tucznej oraz ukryte alergeny i kofaktory – wysiłek fizyczny, alkohol, leki. Objawy kliniczne anafilaksji idiopatycznej oraz postępowanie terapeutyczne są identyczne jak w anafilaksji o znanym czynniku sprawczym. W pracy omówiono epidemiologię anafilaksji, patomechanizm, diagnostykę oraz postępowanie terapeutyczne według wytycznych międzynarodowych
Multiple sclerosis (MS) is a complex disease requiring exposure to an environmental factor and a genetic predisposition. MS continues to be thought of as an organ- specific autoimmune disease mediated by CD4+ T- cells. The pathological hallmarks of MS are lesions, known as plaques, which are due to focal loss of myelin with relative preservation of axons and astrocytic gliosis. However, axonal loss is present in all phases of the disease. The pathological features of individual lesions vary and depend on location, age and whether or not there has been any regeneration. MS lesions are typically divided into three pathological categories: active (acute), chronic active and chronic inactive. More recently active lesions have been further classified into four subtypes based on the type of inflammatory reaction, the involvement of immunoglobulin and complement, the expression of myelin proteins, the morphology of the plaque edge and the patterns of oligodendrocyte injury. Whether or not these pathological subtypes represent different stages of the disease or autoimmune variants is speculative. Recently, particular interest focused on the cortical pathology associated with MS. Post mortem studies revealed that extensive cortical demyelination is typically present in patients with progressive disease phases. However, despite extensive studies, the factor initiating the cascade of the pathological events remains unknown. The aim of that article is to present the most recent advances in the understanding of the pathogenesis of MS.

Stwardnienie rozsiane (łac. sclerosis multiplex, SM) jest schorzeniem o złożonym mechanizmie, w którym dochodzi do interakcji pomiędzy czynnikami środowiskowymi a genetycznie uwarunkowaną predyspozycją. Nadal uważa się, że SM jest narządowo specyficzną chorobą autoimmunologiczną, mediowaną przez limfocyty T CD4+. Patologicznymi wykładnikami choroby są zmiany ogniskowe, określane jako plaki, które powstają na skutek miejscowej demielinizacji ze względnym zaoszczędzeniem aksonów i wtórnym przerostem astrogleju. Uszkodzenie aksonalne obecne jest jednakże we wszystkich fazach choroby. Morfologia poszczególnych plak różni się i zależy od ich lokalizacji, czasu trwania i od tego, czy zachodzi w nich regeneracja. Plaki tradycyjnie dzieli się na trzy patologiczne typy: aktywne (ostre), przewlekłe aktywne i przewlekłe nieaktywne. Ostatnio plaki aktywne podzielono na cztery podtypy, biorąc pod uwagę: charakter procesu zapalnego, zaangażowanie immunoglobulin i układu dopełniacza, ekspresję białek mieliny, morfologię obrzeża zmian oraz wzorzec uszkodzenia oligodendrocytów. Pozostaje przedmiotem spekulacji, czy powyższe podtypy reprezentują kolejne fazy rozwoju zmian patologicznych, czy też są wyznacznikami odmiennych wariantów autoimmunologicznych. Ostatnio wiele uwagi poświęca się zagadnieniu zmian patologicznych obecnych w obszarach korowych mózgu. Badania anatomopatologiczne wykazały, że rozległa demielinizacja korowa szczególnie manifestuje się u pacjentów w przewlekłej fazie choroby. Jednakże, pomimo usilnych badań, czynnik inicjujący całą sekwencję zmian patologicznych nadal pozostaje nieznany. Celem niniejszego artykułu jest omówienie najnowszych koncepcji dotyczących patogenezy stwardnienia rozsianego.

Glejak wielopostaciowy klasyfikowany jest do grupy najbardziej złośliwych glejaków o IV stopniu złośliwości ze szczególnie niekorzystnymi prognozami związanymi z jego leczeniem. Pojawienie się u pacjenta szybko postępujących objawów zarówno psychiatrycznych, jak i neurologicznych powinno być zawsze dokładnie zbadane. Artykuł ma na celu zebranie oraz podsumowanie aktualnej wiedzy na temat glejaka oraz zaprezentowanie możliwych perspektyw rozwoju metod diagnostyczno-leczniczych. Rozwój glejaka ma zazwyczaj związek z mutacjami w genach EGFR, PTEN, IDH1 lub p53. Do użytecznych narzędzi diagnostycznych należą m.in. badanie MRI, analiza metylacji promotora genu MGMT oraz immunohistochemiczne oznacznie GFAP. W leczeniu glejaków wielopostaciowych najczęściej stosowanym schematem jest protokół Stuppa bazujący na połączeniu radioterapii z chemioterapią temozolomidem. Niemniej jednak, dalsze możliwości leczenia są limitowane. Zintegrowane wysiłki naukowców są ukierunkowane na poszukiwanie nowych strategii leczenia przy użyciu m.in. teraapii CAR-T, nanocząteczek, przeciwciał monoklonalnych, miRNA, siRNA oraz inhibitorów proteasomów

Glioblastoma is the most severe IV-class glioma and therefore the prognosis for patients remains poor despite some improvement in the treatment area. The neurological or psychiatric symptoms especially fast-developing ones should be fully investigated. This article aims to summarize actual knowledge of glioblastoma and present future perspectives. The underlying causes are usually associated with mutations of EGFR, PTEN, IDH1, p53 genes. The MRI scan, MGMT promoter methylation status, GFAP immunohistochemical detection and Karnofsky performance status are valuable diagnostic tools and some other potential biomarkers with high specificity are proposed. The standard of care is surgery and Stupp protocol which is the combination of radiotherapy and chemotherapy with temozolomide. Nevertheless, after remission the treatment possibilities are limited. Many efforts have been devoted to elaborate novel therapeutic strategies using e.g. CAR-T cells, nanoparticles, monoclonal antibodies, miRNA, siRNA or proteasome inhibitors.
For many years, it was thought that synovial cells and chondrocytes are the only sources of proinflammatory cytokines and growth factors found in the synovial fluid in patients suffering from osteoarthritis and rheumatoid arthritis. Currently, it is more and more frequently indicated that adipose tissue plays a significant role in the pathogenesis of these diseases as well as that a range of pathological processes that take place in the adipose tissue, synovial membrane and cartilage are interconnected. The adipose tissue is considered a specialized form of the connective tissue containing various types of cells which produce numerous biologically active factors. The latest studies reveal that, similarly to the synovial membrane, articular adipose tissue may take part in the local inflammatory response and affect the metabolism of the cartilage and subchondral osseous tissue. In in vitro conditions, the explants of this tissue obtained from patients suffering from osteoarthritis and rheumatoid arthritis produce similar pro- and anti-inflammatory cytokines to the explants of the synovial membrane. At this stage already, knowledge translates into imaging diagnostics. In radiological images, the shadowing of the periarticular soft tissues may not only reflect synovial membrane pathologies or joint effusion, but may also suggest inflammatory edema of the adipose tissue. On ultrasound examinations, abnormal presentation of the adipose tissue, i.e. increased echogenicity and hyperemia, may indicate its inflammation. Such images have frequently been obtained during ultrasound scanning and have been interpreted as inflammation, edema, hypertrophy or fibrosis of the adipose tissue. At present, when the knowledge concerning pathogenic mechanisms is taken into account, abnormal echogenicity and hyperemia of the adipose tissue may be considered as a proof of its inflammation. In the authors’ own practice, the inflammation of the adipose tissue usually accompanies synovitis. However, we also diagnose cases in which the inflammatory process in the joint is no longer active, but abnormal vascularity still persists in the adipose tissue. There are also cases where abnormal adipose tissue is the only sign of inflammation. Therefore, ultrasound examination confirms the existence of the additional site of inflammation, i.e. the adipose tissue which should be evaluated at the stage of initial diagnosis and during follow-up, also in terms

Przez wiele lat uznawano, że jedynym źródłem prozapalnych cytokin i czynników wzrostu stwierdzanych w płynie stawowym u pacjentów z chorobą zwyrodnieniową stawów i reumatoidalnym zapaleniem stawów są komórki błony maziowej i chondrocyty. Obecnie coraz więcej dowodów wskazuje na istotną rolę tkanki tłuszczowej w patogenezie tych chorób oraz obecność szeregu powiązań między procesami patologicznymi zachodzącymi w tkance tłuszczowej, błonie maziowej oraz w chrząstce. Tkanka tłuszczowa jest uznawana za wyspecjalizowaną formę tkanki łącznej, zawierającą różne typy komórek, które wytwarzają wiele aktywnych biologicznie czynników. Najnowsze badania wskazują, że stawowa tkanka tłuszczowa może, podobnie jak błona maziowa, brać udział w lokalnej odpowiedzi zapalnej i wpływać na metabolizm chrząstki i tkanki kostnej podchrzęstnej. W warunkach in vitro eksplanty tej tkanki od chorych na chorobę zwyrodnieniową stawów i reumatoidalne zapalenie stawów wydzielają podobne cytokiny pro‑ i przeciwzapalne jak eksplanty błony maziowej. Wiedza już na tym etapie przekłada się na diagnostykę obrazową. Na zdjęciach radiologicznych zacienienie tkanek miękkich okołostawowych/przystawowych może nie tylko świadczyć o patologii błony maziowej czy o wysięku w jamie stawu, ale także wskazywać na obrzęk zapalny tkanki tłuszczowej. W badaniach ultrasonograficznych nieprawidłowy obraz tkanki tłuszczowej, tj. podwyższenie echogeniczności i przekrwienie, może wskazywać na jej zapalenie. Takie obrazy często były obserwowane w badaniach ultrasonograficznych, co interpretowano jako zapalenie, obrzęk, przerost czy włóknienie tkanki tłuszczowej. Obecnie, dysponując wiedzą na temat mechanizmów patogenetycznych, nieprawidłową echogeniczność i przekrwienie tkanki tłuszczowej możemy prawdopodobnie uznać za dowód jej zapalenia. W praktyce własnej zapalenie tkanki tłuszczowej najczęściej towarzyszy synovitis. Stwierdzamy jednak także przypadki, w których proces zapalny w stawie nie jest już aktywny, natomiast nieprawidłowe unaczynienie utrzymuje się w tkance tłuszczowej. Badamy wręcz przypadki, w których nieprawidłowa tkanka tłuszczowa jest jedynym wykładnikiem zapalenia. Badanie ultrasonograficzne dostarcza zatem dowodów na obecność dodatkowego miejsca zapalnego, jakim jest tkanka tłuszczowa, która powinna być oceniana na etapie wstępnej diagnostyki i monitorowania pacjentów, w tym także pod kątem remisji.
Allergen-specific immunotherapy (ASI) allows for a treatment and influences on the course of, the allergic, IgE-dependent diseases. ASI induces allergen tolerance resulting in a reduction or disappearance of clinical symptoms of allergy. One hundred years ago, in 1910 exactly, Noon published his landmark study. Since then, the knowledge of its mechanisms, indications for ASI and routes of administration of allergen is systematically being broaden. Currently used forms of allergens allow to minimize serious side effects related to the ASI, which were observed in the initial trials of therapy. This paper presents current views on the pathomechanisms of immunotherapy, rules for an application of immunotherapy and possibility of using ASI in atopic diseases. The main reason for this form of a treatment is inability to eliminate the allergen from patient’s environment with a high probability of severe clinical symptoms after contact with the allergen. The best results are achieved in case of hypersensitivity to hymenoptera venom and treatment of allergic rhinitis, allergic conjunctivitis and asthma associated with allergy to grass pollen, trees, weeds and dust mites. Treatment of monoallergy gives better results compared to the sensitisation to multiple allergens. For some of the cases, due inability of pharmacotherapy or its ineffectiveness, one could consider usage of ASI for patients with allergy tied with other allergens. Currently this method is not approved for the treatment of food allergy. This paper quotes meta-analysis, carried out in recent years, evaluating the efficiency of the most popular current methods of immunotherapy – subcutaneous (SCIT) and sublingual (SLIT) – in the treatment of asthma and allergic rhinitis. The publications cited below clearly show that these methods are effective in reducing and eliminating allergy symptoms. This paper presents the latest promising route of administration allergen – percutaneous immunotherapy, particularly beneficial for younger patients. In the era of great advances in medicine, we can expect further improvements of ASI, elimination of serious side effects and widening group of patients eligible for this form of therapy.

Alergenowo-swoista immunoterapia (ASI) umożliwia leczenie i wpływ na przebieg alergicznych schorzeń IgE-zależnych. W wyniku jej działania następuje indukcja tolerancji alergenu, a w rezultacie – zmniejszenie lub ustąpienie objawów klinicznych alergii. Od daty wydania (1910 r.) publikacji Noona, uważanej za przełomową w rozwoju immunoterapii, mija 101 lat. W tym czasie systematycznie poszerzano wiedzę na temat mechanizmów jej działania, wskazań do stosowania ASI oraz formy podawania alergenu. Obecnie stosowane rodzaje ASI pozwalają na zminimalizowanie poważnych objawów niepożądanych, które obserwowano w początkowych próbach terapii. W pracy przedstawiono współczesne poglądy na temat patomechanizmów immunoterapii, zasad jej podawania oraz możliwości stosowania ASI w przypadku schorzeń atopowych. Podstawowym wskazaniem do tej metody leczenia jest brak możliwości wyeliminowania alergenu ze środowiska pacjenta przy dużym prawdopodobieństwie wystąpienia ciężkich objawów klinicznych uczulenia po kontakcie z alergenem. Najlepsze rezultaty osiąga się w przypadku nadwrażliwości na jady owadów błonkoskrzydłych oraz leczenia alergicznego nieżytu nosa, alergicznego zapalenia spojówek, a także astmy związanej z alergią na pyłki traw, drzew, chwastów i roztoczy kurzu domowego. Leczenie monoalergii daje lepszą skuteczność w porównaniu z uczuleniem na wiele alergenów. W wybranych przypadkach przy braku możliwości stosowania farmakoterapii lub jej nieskuteczności można rozważyć użycie ASI u pacjentów z alergią związaną z innymi alergenami. Aktualnie nie jest to metoda zaakceptowana w leczeniu alergii pokarmowej. W pracy przytoczono metaanalizy przeprowadzone w ostatnich latach, oceniające skuteczność dwóch najpopularniejszych obecnie metod immunoterapii – SCIT (śródskórnej) oraz SLIT (podjęzykowej) – w leczeniu astmy oraz alergicznego nieżytu nosa. Z cytowanych poniżej publikacji wynika, że są to metody skuteczne, zmniejszające lub eliminujące objawy choroby u alergików. Przedstawiono również najnowsze, bardzo obiecujące drogi podania alergenu, w tym przezskórną immunoterapię, szczególnie korzystną u najmłodszych pacjentów. W dobie ogromnego postępu medycyny należy oczekiwać dalszego udoskonalania ASI, eliminacji poważnych objawów ubocznych oraz rozszerzenia grupy pacjentów objętych tą formą terapii.
Fibrodysplasia ossificans progressiva is uncommon genetic disease of connective tissue characterized by congenital defects already in newborns. Clinical symptoms of that disease develop usually in the beginning of human life. The disease was displaying forms of big pain swellings. At first, the focus was to treat it like a benign tumour. Later, those swellings started to change in calcinosis. We report of the case 30-year-old woman, who was born with congenital defects characterized by distort of the fingers and feet. That disease was started in 4-5 year of old. The process of ossification walked progressively. The sharp worsening of health occurred after injury (fall of the bike). Diagnoses of downloading stretch the muscle and biopsy of muscle was performed. The diagnosis of the disease was early in 6 years of old and the treatment with etidronate sodium was unsuccessful on natural course of the disease.

Myositis ossificans albo fibrodysplasia ossificans progressiva (FOP) to bardzo rzadkie genetycznie uwarunkowane schorzenie tkanki łącznej objawiające się wrodzonymi wadami paluchów stóp już u noworodków, a w dalszym życiu postępującym procesem kostnienia tkanek miękkich, stopniowo prowadzącym do unieruchomienia pacjenta. Kliniczne objawy choroby rozwijają się zwykle już w pierwszej dekadzie życia. Przybierają wówczas postać dużych i bolesnych obrzmień w obszarach tkanki łącznej. Początkowo ogniska te przypominają proces nowotworowy. Obrzmienia przechodzą później w formę kostnienia. W pracy opisano przypadek 30-letniej chorej, która obecnie jest całkowicie unieruchomiona, a urodziła się z wrodzoną koślawością i zniekształceniem palców stóp i rąk. Początek choroby miał miejsce w 4.-5. roku życia. Proces kostnienia tkanki łącznej postępował u chorej stopniowo, z gwałtownym nasileniem po urazie tułowia (upadek z roweru) i pobraniu wycinka mięśnia do badania histopatologicznego. Diagnozę choroby postawiono dość wcześnie (w 6. roku życia) i wdrożono leczenie etydronianem sodu (Didronel), jednak wydaje się, że bez wpływu na naturalny przebieg choroby.
Early diagnosis of rheumatoid arthritis followed by early initiation of treatment, prevent the destruction of joints and progression to disability in the majority of patients. A traditional X‑ray fails to capture early inflammatory changes, while late changes (e.g. erosions) appear after a significant delay, once 20–30% of bone mass has been lost. Sonography and magnetic resonance imaging studies have shown that erosions are seen in the first 3 months from the appearance of symptoms in 10–26% of patients, while in 75% they are seen in the first 2 years of the disease. Power Doppler ultrasound and dynamic magnetic resonance studies allow for qualitative, semiquantitative and quantitative monitoring of the vascularization of the synovium. In addition, magnetic resonance enables assessment of the bone marrow. The ultrasonographic examination using a state‑of‑the‑art apparatus with a high‑frequency probe allows for images with great spatial resolution and for the visualization of soft tissues and bone surfaces. However, the changes seen in ultrasonography (synovial pathologies, the presence of exudate, tendons changes, cartilage and bone lesions, pathologies of tendon attachments and ligaments – enthesopathies) are not only specific for rheumatoid arthritis and occur in other rheumatic diseases. Qualitative methods are sufficient for diagnosing the disease through ultrasound or magnetic resonance imaging. Whereas semiquantitative and quantitative scales serve to monitor the disease course – efficacy of conservative treatment and qualification for radioisotope synovectomy or surgical synovectomy – and to assess treatment efficacy.

Wczesne rozpoznanie reumatoidalnego zapalenia stawów i wczesne wdrożenie leczenia zapobiega u większości pacjentów destrukcji stawów i niepełnosprawności. Tradycyjne zdjęcie radiologiczne nie uwidacznia wczesnych zmian zapalnych, zaś zmiany zaawansowane, m.in. nadżerki, są widoczne dopiero wówczas, gdy dojdzie do zniszczenia 20–30% masy kostnej. Jak wykazały badania ultrasonograficzne i rezonans magnetyczny, u 10–26% pacjentów nadżerki pojawiają się już w pierwszych 3 miesiącach od wystąpienia objawów, a u 75% pacjentów są widoczne w pierwszych 2 latach trwania choroby. Oba te badania w sposób jakościowy, półilościowy i ilościowy umożliwiają monitorowanie unaczynienia błony maziowej, a rezonans dodatkowo ocenę szpiku kostnego w trakcie leczenia. Rozwój ultrasonograficznych badań dopplerowskich i dynamicznych badań rezonansu wnosi istotny element w poznawanie patogenezy RZS, między innymi związku między zapaleniem błony maziowej i obrzękiem szpiku a tworzeniem się nadżerek. Badanie ultrasonograficzne wykonane na wysokiej klasy aparacie z głowicą o wysokiej częstotliwości wykazuje doskonałą rozdzielczość przestrzenną, obrazując tkanki miękkie oraz powierzchnie kostne. Zmiany widoczne w ultrasonografii nie są jednak swoiste wyłącznie dla reumatoidalnego zapalenia stawów i występują w innych chorobach reumatycznych. Należą do nich: patologie błony maziowej, wysięk, zmiany w ścięgnach, zmiany chrzęstno‑kostne, patologie przyczepów ścięgien i więzadeł (entezopatie). O ile metody jakościowe w badaniu ultrasonograficznym i rezonansie magnetycznym są wystarczające do rozpoznania choroby, o tyle skale półilościowe i ilościowe służą do monitorowania jej przebiegu, w tym skuteczności leczenia zachowawczego i kwalifikacji do synowektomii radioizotopowej czy chirurgicznej i oceny ich skuteczności.
Rheumatoid arthritis is a chronic inflammatory disease with a multifactorial etiology and varied course, which in the majority of patients leads to partial disability or to permanent handicap. Its characteristic trait is a persistent inflammation of the synovial membrane and the formation of an invasive synovial tissue, called the pannus, which in time leads to destruction of the cartilage, subchondral bone tissue, and the soft tissue of the affected joint(s). The pathogenesis of rheumatoid arthritis is complex and involves cells of both innate and adaptive immunity, a network of various cytokines and an immunoregulatory dysfunction. An important role in the discovery of rheumatoid arthritis pathogenesis was played by magnetic resonance imaging, which showed the disease process to extend beyond the synovium into the bone marrow. Many studies have shown a strict correlation between the vascularity of the synovium (assessed through the power Doppler ultrasound and magnetic resonance examinations), bone marrow edema and the clinical, laboratory and histopathological parameters of rheumatoid arthritis. From the current understanding of rheumatoid arthritis, bone erosions could occur from two directions: from the joint cavity and from the bone marrow. With power Doppler ultrasound, as well as in magnetic resonance imaging, it is possible to visualize the well-vascularized pannus and its destructive effects on joint structures and ligaments. In addition, the magnetic resonance study shows inflammatory and destructive changes within the bone marrow (bone marrow edema, inflammatory cysts, and erosions). Bone marrow edema occurs in 68–75% of patients with early rheumatoid arthritis and is considered to be a predictor of rapid disease progression.

Reumatoidalne zapalenie stawów jest przewlekłą chorobą zapalną o wieloczynnikowej patogenezie i zróżnicowanym przebiegu, która u większości chorych prowadzi do częściowej niepełnosprawności lub trwałego kalectwa. Jej cechą charakterystyczną jest przetrwałe zapalenie błony maziowej (synovitis) i tworzenie inwazyjnej tkanki synowialnej, zwanej łuszczką, która z różną szybkością prowadzi do destrukcji chrząstki i tkanki kostnej podchrzęstnej oraz tkanek miękkich stawów. Patogeneza reumatoidalnego zapalenia stawów jest poznana w niewielkim zakresie. W procesie chorobowym uczestniczą komórki układu odporności wrodzonej i nabytej, zaburzenia immunoregulacji i sieć cytokin. W poznaniu patogenezy choroby duże znaczenie miało wprowadzenie badań rezonansu magnetycznego, które ujawniły kolejną, obok błony maziowej, lokalizację choroby w szpiku kostnym. Wiele badań wykazało ponadto ścisłą korelację między unaczynieniem błony maziowej, ocenianym w badaniu ultrasonograficznym power Doppler i w badaniu rezonansem magnetycznym, oraz obrzękiem szpiku a wskaźnikami klinicznymi, laboratoryjnymi i histopatologicznymi reumatoidalnego zapalenia stawów. Według aktualnej wiedzy erozja tkanki kostnej może zachodzić dwoma drogami: od strony jamy stawowej oraz od strony jamy szpikowej. W badaniu ultrasonograficznym z opcją dopplerowską power Doppler oraz w rezonansie magnetycznym można obserwować unaczynioną łuszczkę i efekty jej destrukcyjnej działalności w stawach i ścięgnach. Zmiany zapalne i destrukcyjne w szpiku kostnym (obrzęk szpiku kostnego, geody – inaczej torbiele zapalne oraz nadżerki) można obserwować w badaniu rezonansem magnetycznym. Obrzęk szpiku występuje u 68–75% chorych we wczesnej fazie reumatoidalnego zapalenia stawów i jest uznawany za predykator szybkiego rozwoju choroby.
The cervical myelopathy (CM) belongs to the civilization diseases connected to age. The symptoms of the disease were well known from many years after yet the neuroradiological examinations (CT or NMR) can confirmed the diagnosis of cervical myelopathy. In 1928 year Stookey presented the theory of CM aetiology, since other authors have formed their opinions or modified Stookey’s theory. Now is established that the cervical myelopathy is the effects from following abnormalities: 1) the direct pressure of the bone, osteochondrosis or its calcifications changes to spinal cord; 2) the ischaemic changes to cause by pressure to the spinal cord vessels; 3) repetitive microtraumas connected whit the flexion or extension movements of the neck. It is considered that CM develops when the diameter of canal vertebralis narrows by 30%. The spinal cord gets flattened in the anterior-posterior size. Few anatomopathological investigations have shown that on the level of the lesions were observed gliosis or demyelination of the spinal cord. The lateral columns of the spinal cord are more prone to the pressure than anterior or posterior one. The osteochondrosis as the osteophytes are present in 90% of people over 60 years. The initial symptoms of CM are headache or pain of the neck. Later the pain is radiated to shoulders or brachium, the upper limbs weaken, the patient is feeling paresthesias mostly in palms. After some time the lower limbs were found less efficiency and spastic rigidity. The gait to turn slowly and clumsy. The disturbances with continence of urine was also appeared. The muscles atrophy, mainly hands, have been observed as well as impaired deep sensation or vibration. Is characterized the hyperreflexia in upper and lower limbs. Sometimes is appeared a patellar clonus as well as the Lhermitte’s sign. In the differential diagnosis should be taken under consideration many diseases, but the results of the neuroradiological examination should be a decisive factor. We should remember also that the examination of the cerebrospinal fluid is normal. The CM is disease of the slowly progressive course. The surgical treatment of the disease might give better results in the initial stage of the disease – 30% of patients reported the improvements in their condition. In 2 to 3 years after the surgery the symptoms reoccur and the neurological status of the patients is similar to those who have been treated in the conservative way.
