Introduction: Inverted papilloma is benign epidermal neoplasm of not recognized etiology. The lesion is estimated to represent 0.5-4% of sinonasal tumors. Generally inverted papilloma is unilateral, arises from the lateral nasal wall and characterizes with local destruction and rapid growth. The tumor can possibly undergo malignant transformation. The treatment of choice is surgery, recently endoscopic approach is considered to be superior to the open approach. The aim of study: was evaluation of recurrent inverted papilloma treatment results in the experience of Otolaryngology Department, Medical University of Warsaw, from 1982 to 2009. Material: The group of 124 patient (66 men and 58 women) aged from 26 to 79 with inverted papilloma operated between 1982 and 2009 were enrolled in the study. Follow up was from 6 months to 27 years. Results: Most common primary localization of the tumor was combined lateral nasal wall and maxillary sinus (42%). In 37% the lesion involved unilaterally the lateral nasal wall, maxillary sinus and ethmoid sinuses, rarely it was localized only in the lateral nasal wall (16.1%) and exclusively in the nasal septum (4.8%). All the patients were treated surgically, 46% of patients underwent endoscopic procedure, 37.1% lateral rhinotomy, 15% Denker rhinotomy and 1.6% midfacial degloving. Recurrence was observed in 15.3% including 57.9% after endoscopic surgery, 26% after Denker operation and 15.8% after lateral rhinotomy. Malignant transformation was observed in 5 patient (4%). All the patients with recurrent inverted papilloma were treated surgically, 5.3% of patient underwent midfacial degloving, 5.3% Caldwell- Luc operation, 10.5% endoscopic procedure, 26.3% Denker operation and 52.6% lateral rhinotomy. Conclusions: 1. Endonasal and open approach should be considered in the surgical treatment of recurrent inverted papilloma. 2. Endoscopic approach is preferred in the recent years but qualifi cation for each method should depend on lesion localization, extent and volume. 3. Regular follow-up enables early recurrence diagnosis and treatment.

Introduction. Cases of isolated lesions of the sphenoid sinus are rare. For descriptive purposes, clinicians divide them into inflammatory and non-inflammatory with prevalence of the former. Symptoms of the sphenoid sinus disease are difficult to characterise, the most common of them being vague headache and visual disturbances. Thorough preoperative evaluation of the lesion is essential – nasal endoscopy must be performed and computerised tomography or magnetic resonance imaging results analysed. The purpose of the study is to present the assessment of endoscopic surgery outcome in the own group of patients with isolated sphenoid disease. Material and methods. Clinical data of 22 subjects were analysed retrospectively. There were 5 patients with bacterial sinusitis, 6 with fungal sinusitis, 4 with allergic thickening of the mucous membrane with no evidence of bacterial or fungal infection, 2 with mucocele, 1 with sphenoid osteoma, 1 with inverted papilloma, 1 with a foreign body and 2 with cerebral fluid fistula. Each patient had preoperative nasal endoscopy and CT/MRI imaging performed. Then they underwent endoscopic transnasal sphenoethmoidectomy with removal of the lesion or closure of the fistula. The sphenoid sinus was approached through its front wall. Results. The patients’ postoperative course was uneventful. They noted improvement in all preoperative symptoms, except for bilateral oculomotor nerve paralysis in one individual, and were asymptomatic to ophthalmological examination. Conclusions. Isolated lesions of the sphenoid sinus, even very rare tumours like those presented here, can be in most cases safely approached and removed endoscopically. All risks of the method must be considered prior to the operation. High frequency of fungal sinusitis should be noted.

Fungal infections of the ear and paranasal sinus, are recognised quite rarely, but they make sometimes, for many reasons, very important diagnostic-therapy issue. Cantagion may developes near every paranasal sinus. Facilities to invasion give every states, which weaken general and punctual immunity (like immunosupression, chemo-sterydotherapy, blood disease, pregnancy and HIV). After penetrated organism, course of infections, in case of type of patogens, trim of the patient and localization, can have without symptoms, sharp, chronic or fulminant shape. The hardest course with the highest mortality occur in the cases of mucormycosis and aspergillosis. Actually curiosity occur cases of fungal infections, since this times consider to be unpathogenic for humans. For treatment of sinus mycosis in majority chirurgical treatment is required. In addition or the sake of morphological differentiation fungal, in their developing cycle, treatment mycosis recommends serious diffi culty, extra factor, which impeds therapy, is a must of prolongely antifungal treatment and repeated (to total elimination) remove hyphae from sinus, which stay sometimes even for week. Progress of the mycotic infections in paranasal sinus, remain bacterial infections and is often reason of bad diagnosis and incorrect treatment.

Introduction. The aim of this study was to analyse the occurrence of inverted papillomas of the nose and paranasal sinuses in patients that underwent endoscopic sinus surgery in our department. Material and methods. Between 2006 and 2016, 3,574 patients underwent surgery due to paranasal sinus diseases. Patients were qualified for surgery based on medical history, computed tomography, and laboratory tests. Data were gathered from medical files, and they included age, sex, and histopathological diagnosis. Results. Among 3,574 patients that underwent surgery due to chronic inflammatory changes, on histopathology, inverted papillomas were diagnosed in 80 patients, including 31 women (38.75%) and 49 men (61.25%). Most patients were aged 60-70 years (women, 12.5%; men, 15%) or 50-60 years (women, 5%; men, 21.25%). Between 2006 and 2016, the number of surgeries ranged from 264 (7.38%) in 2013 to 355 (9.93%) in 2016, and the number of inverted papillomas ranged from 4 in 2007 and 2015 (1.23%) to 12 in 2014 (3.87%). Over the last 4 years of the study period, the incidence of inverted papillomas increased. Conclusions. Among 3,574 patients operated on due to chronic inflammatory changes, on histopathology, inverted papillomas were diagnosed in 80 cases (2.23%); thus, all patients qualified for endoscopic surgery due to inflammatory or hypertrophic changes should undergo rhino-fiberoscopy. Recurrence of inverted papillomas was observed in 17.50%, typically in patients with nasal polyps that co-occurred with inverted papillomas. We regard rhino-fiberoscopy as the most valuable method for detecting tumour recurrence in patients after surgery for inverted papillomas ranged from 4 in 2007 and 2015 (1.23%) to 12 in 2014 (3.87%). Over the last 4 years of the study period, the incidence of inverted papillomas increased. Conclusions. Among 3,574 patients operated on due to chronic inflammatory changes, on histopathology, inverted papillomas were diagnosed in 80 cases (2.23%); thus, all patients qualified for endoscopic surgery due to inflammatory or hypertrophic changes should undergo rhino-fiberoscopy. Pleomorphic adenoma is benign tumor of the salivary glands. It is the most common neoplazm both major and minor salivary glands. Only in 1% of cases pleomorphic adenoma occur in the sinonasal cavities. The histopalogical wiew of pleomorphic adenomas originating from small salivary glands is different in higher cellularity and lower stromal component than those from major salivary glands. It is a benign tumor but it should be treated as a clinically uncertain. Malignant transformation of pleomorphic adenoma can occur in 2-10% of cases, The aim of this study was to present he case of 37-year-old patient with giant pleomorphic adenoma of the sinonasal cavities causing extensive destruction of sinuses and orbits walls.

Malignant melanoma is highly aggressive type of malignant neoplasm. The skin of the head and neck region is common localisation for appearance of this tumour due to exposition to the sun light. However, some number of cases concern development of the malignant melanoma on the mucous membranes. Most common signs are epistaxis and nasal obstruction. Late diagnosis effects with poor clinical prognosis. In our case, despite complete surgical resection and adjuvant therapy, outcome of the treatment was unsatisfying, because of metastasis and local recurrence.

Inverted papilloma is a locally aggressive lesion most commonly originating from the maxillary sinus. Currently transnasal endoscopic excision is a treatment of choice in the majority of cases. The article presents the evolution of endoscopic approaches and possibilities of minimally invasive treatment in regard to tumor location.

Osteomas are relatively common, benign, slow-growing, often asymptomatic neoplasms of the paranasal sinuses, occurring mainly in frontal and ethmoid sinuses. Surgical removal is done if they extend beyond the boundaries of the sinus, keep enlarging, are localised in the region adjacent to the nasofrontal duct, or if signs of chronic sinusitis are present and, irrespective of their size, in symptomatic tumours. Progressive headaches and chronic inflammation of the adjacent mucous membrane are most common symptoms. Endoscopic surgery plays an important role in management of ethmoid, sphenoid and frontal osteomas. Aim. The aim of the paper was to report own experience in endoscopic treatment of patients with osteomas of the paranasal sinuses. Material and methods. 6 patients with osteomas of paranasal sinuses were included in the group, mean age 36 years (range 15–52). Most common involvement was ethmoid cells (3). There were also patients with frontal, maxillary and sphenoid osteoma. All tumours were removed under endoscopic giudance. Frontoethmoidectomy was performed to remove ethmoid and frontal osteomas. Antrotomy was used in case of maxillary involvement and sphenoethmoidectomy in the patient with sphenoid sinus osteoma. Sphenoid sinus was approached through its anterior wall with a Stammberger punch. All the tumours were removed using fine forceps. Results. No post-operative complications were observed. No recurrences were noted. All patients remain asymptomatic. Conclusions. Resection of small and medium size osteomas of the paranasal sinuses can be safely and radically performed using endoscopic techniques. It allows their radical resection and very good cosmetic effects.

Preface: angiomatous nasal type polyps (SAP ‒ sinonasal angiomatous polyp) are benign lesions. The clinical and radiological picture of SAP may suggest other diseases of the nose and paranasal sinuses. Diagnostic imaging is based on computed tomography and magnetic resonance. SAP treatment involves their surgical removal. A case report: he aim of the study is to present diagnostic and therapeutic difficulties in a 17-year-old female patient with an inflammatory polyposis of the angiomatous of the paranasal sinuses. The course of disease, diagnostic difficulties and its treatment are presented. No complications were observed during hospitalization and in the postoperative period. Summary: SAP is one of the types of nasal polyps characterized in the histological picture of vascular proliferation with the presence of blood clots. Accurate diagnostic and histological diagnostics allow to determine the final diagnosis and differentiation of SAP with nasal and sinus hyperplasia.

Introduction: Osteomas are the most common benign nonepithelial neoplasms of the head. The tumors are slow growing and cause headaches. Osteomas are often diagnosed accidentally, during the head imagining procedures. Objective: Clinical and epidemiological assessments of patients with head and neck osteomas. Material and methods: We carried out the analysis of medical documentation of patients treated in Otolaryngology Department of the Medical University in Gdańsk in the years 2012–2019 with osteoma diagnosis and analyzed the therapeutic approach and treatment results. Results: 38 patients with osteoma have been operated on during the investigated period of time (18 men, 20 women). Most of the osteomas were localized in the frontal sinuses (55.3%). External frontoethmoidectomy was the most commonly performed surgery (73.7%). 10 patients (26.3%) underwent endoscopic ethmoidectomy. It has been proven that there is no link between this neoplasm and age, sex or tobacco smoking. Conclusions: Osteomas are rare bone neoplasms. The course of the illness depends on the primary localization of the osteoma. Asymptomatic osteomas do not require surgical excision. They require only regular otolaryngological control. The most common symptoms of osteomas are intensive headaches, which are difficult to treat. Diagnostics of such tumors is based on Computed Tomography and surgery is in this case a treatment of choice.

The authors present a rare case of endovascular papillary angioendothelioma also known as “Dąbska tumor”, which occurs mainly in children and young adults. This neoplasm was found in the right maxillary sinus and nasal cavity in a 22-year-old woman that remained undiagnosed for one year. After the initial diagnostic imaging, the tumor was removed through the endoscopic approach. After a year, the patient developed a mucopurulent cyst in the postoperative cavity, which was removed without any recurrence of the neoplastic disease in the follow-up histopathological examination. The authors present the clinical picture of this potentially malignant neoplasm and review the current literature.

Ethmoid sinuses are one of the first formed of all the paranasal sinuses, therefore they are most frequently affected by inflammation when it comes to children. The diagnosis of acute ethmoiditis is based mainly on clinical presentation. Due to the specific anatomical conditions of the ethmoid sinuses, inflammation is spreading fast onto the orbital structures and the surrounding tissues. It is estimated that in approximately 91% of cases, orbital cellulitis is related to sinusitis. This work aims at drawing attention to the importance of early diagnosis and appropriate treatment of a common illness that is acute rhinosinusitis. The presentedcase report discusses the course of the disease in a 6-year-old boy who additionally developed orbital cellulitis – a rare complication that concerns approximately 0.5–3.9% of all cases. We can distinguish two types of orbital complications: preseptal cellulitis (anterior to the orbital septum) and posterior to the orbital septum ex. orbital cellulitis (inside the septum). The first group contains inflammatory eyelid oedema, relatively mild and most frequently observed, characterized by favourable prognosis. The complications belonging to the second group are decidedly less frequent and more serious. These are: orbital cellulitis, subperiosteal abscess, orbital abscess and cavernous sinus thrombosis. What helps to differentiate between the two groups are proptosis, eye movement disorder and decreased visual acuity. The systemic treatment consists in the parenteral administration of broad spectrum antibiotics. Despite available modern treatment options one should not forget the fact that late intervention may lead to septic complications such as meningitis, cavernous sinus thrombosis or osteomyelitis. Moreover, discontinuation of treatment may cause permanent, but very rare sequelae including loss of vision and glaucoma. Therefore, acute ethmoiditis is to be treated as a paediatric and rhinological emergency condition.

Spośród wszystkich zatok obocznych nosa najwcześniej rozwijają się zatoki sitowe i to one są najczęściej objęte procesem zapalnym u dzieci. Rozpoznanie ethmoiditis acuta opiera się na obrazie klinicznym i wywiadzie. Ze względu na szczególne warunki anatomiczne komórek sitowia proces zapalny szybko szerzy się na struktury oczodołów oraz tkanki otaczające. Szacunkowo za około 91% przypadków zapaleń tkanek miękkich oczodołu odpowiada zapalenie zatok. W pracy pragniemy zwrócić uwagę na konieczność wczesnego diagnozowania i odpowiedniego leczenia ostrego zapalenia jam nosa i zatok przynosowych. Przedstawiamy opis przebiegu choroby u 6-letniego chłopca, u którego rozwinęło się rzadkie powikłanie w postaci zajęcia procesem zapalnym oczodołów, dotyczące około 0,5–3,9% przypadków. Wyróżniamy dwa typy powikłań oczodołowych: przedprzegrodowe i zaprzegrodowe. Do pierwszej grupy należy stosunkowo łagodny i najczęściej obserwowany zapalny obrzęk powiek, cechujący się pomyślnym rokowaniem. Powikłania zaklasyfikowane do drugiej grupy, czyli obejmujące przegrodę oczodołową, są zdecydowanie rzadziej spotykane i poważniejsze. Należą do nich: zapalenie tkanek miękkich, ropień podokostnowy i ropień oczodołu oraz zakrzepowe zapalenie zatoki jamistej mózgu. Wytrzeszcz, zaburzenia ruchomości gałki ocznej oraz osłabienie ostrości wzroku pozwalają na zróżnicowanie obu grup. Leczenie ogólne polega na parenteralnym podawaniu antybiotyków o szerokim spektrum działania. Nawet przy współczesnych możliwościach leczniczych nie należy zapominać, że opóźnienie wdrożenia leczenia może doprowadzić do powikłań septycznych, takich jak zapalenie opon mózgowo-rdzeniowych oraz zakrzepowe zapalenie zatoki jamistej mózgu, a także zapaleń szpiku i kości. Obserwowano również trwałe następstwa zaniechania leczenia pod postacią utraty wzroku lub jaskry. Z tego względu ostre zapalenie zatok sitowych musi być traktowane jako naglący stan pediatryczny i rynologiczny.
