Lymphangiomas constitute a group of very rare diseases and occur with a frequency of 1/250,000 to 1/20,000 of hospitalizations. Even though they are benign lesions, their complications may turn into a life-threatening condition. They usually occur in children (90%), they are either congenital or they appear before the child’s second birthday. Occassionally they are found in adults. Lymphangiomas are usually localized around neck, but also near armpits and in the groin area. Less than 1% of lymphagiomas are detected in the retroperitoneal space and intestinal mesentery. Vascular lesions derived from lymphatic vessels can be divided into ordinary ones, usually known as capillary, cavernous, and cystic. The cystic tumor (lymphangioma mesenteri) is the least common. It occurs mostly in the retroperitoneal space, mesentery of the colon, or extremely rarely in the mesentery of the small intestine. Preoperative diagnosis is difficult due to non-specific clinical symptoms and noncharacteristic image in the diagnostic tests. Lymphangioma complications, such as intestinal obstruction or perforation, and persistent pain, are the cause of exploratory laparotomy. Final diagnosis requires microscopic examination of material collected during an operation. This article presented the case of a 40-year-old female, operated due to the obstruction of the gastrointestinal tract, in whose case the lymphangioma was recognised in postoperative histopathological examination of the tumor from the jejunal mesentery. Since patients with these tumors have good chances of complete recovery - if there are no serious complications - it appears that the optimal therapeutic procedure should be early surgery, which reduces the possibility of complications.
The authors report a case of a 21-year-old patient with incomplete bowel rotation presenting with symptoms of chronic obstruction of the upper gastrointestinal tract and the diagnostic difficulties posed by the presented case. The decisive diagnostic examination consisted in an abdominal CT scan with contrast administration. The patient was successfully treated by means of Ladd's procedure.
Obesity is a disease of civilization, which may result in and aggravate already existing diseases. One method of weight loss is a gastric balloon – BioEnterics Intragastric Balloon (BIB). Treatment with the BIB is safe. Complications are rare. Aim: Consult doctors about the possibility of this rare and serious complication that can be treated conservatively. Description of case: Patient, 38 years old, hospitalized due to continuing strong 3 days intra-abdominal pain, nausea and vomiting. In an interview founded for over 9 months ago slimming balloon into the stomach. Imaging studies, such as abdominal X-ray and computed tomography (CT) of the abdomen and pelvis, in the left lower abdomen visualized an irregular change in the corresponding slimming balloon. In the absence of indications for emergency laparotomy, conservative treatment was used. Finally, there was a spontaneous expulsion of the balloon. An improvement of the general condition, relief of symptoms. Discuss the case: Treatment of obesity by setting the gastric balloon is a minimally invasive method of treatment. However, the effectiveness and the lack of complications are dependent on the patient’s knowledge about the possible complications of obesity and the chosen therapeutic method, a sense of the disease and the patient’s motivation to achieve the therapeutic success. Symptoms of spontaneous expulsion of the balloon are symptoms of a foreign factor in the gastrointestinal tract. In the case described, these symptoms have evolved along with the movement of the balloon for the next segments of the gastrointestinal tract. There were signs of high obstruction and intestinal colic. The exact position of the balloon in the gastrointestinal tract was possible to determine after taking CT scan of the abdomen. In the case described, a conservative method was used. This was possible thanks to the close cooperation between doctors of conservative and surgical specializations, and a careful observation of the dynamics of disease symptoms and movement of the balloon for the gastrointestinal tract. Conclusions: Because of the potential complications of intervention methods for weight reduction should be preceded by conservative methods. Selection of invasive methods of treatment should be preceded by a detailed analysis of the causes of failure of conservative treatment. Maintenance of balloon slimming timeout therapy may result in dangerous to life and health complications. If symptoms of obstruction or intestinal colic, patient with founded into the stomach slimming balloon should be observed in a hospital. In the absence of clinical signs of gastrointestinal perforation, esophagoduodenoscopy should be performed to locate the balloon in the stomach in the first instance, further radiological imaging studies locating the position of the balloon in the gut and documenting progress in its elimination. Surgical treatment should be considered if there is no progress spontaneous expulsion of the balloon.

Otyłość jest chorobą cywilizacyjną, która sprzyja powstawaniu schorzeń i pogarsza przebieg już istniejących. Jeden ze sposobów redukcji masy ciała stanowi balon żołądkowy. Jest to bezpieczna metoda, powikłania zdarzają się bardzo rzadko. Cel pracy: Zapoznanie lekarzy z możliwością wystąpienia niedrożności jako rzadkiego i groźnego powikłania, które może być leczone zachowawczo. Opis przypadku: Trzydziestoośmioletnia pacjentka, hospitalizowana z powodu utrzymujących się od 3 dni silnych dolegliwości bólowych w śródbrzuszu oraz nudności i wymiotów. W wywiadzie założony ponad 9 miesięcy temu balon odchudzający do żołądka. W badaniach obrazowych, m.in. na przeglądowym zdjęciu rentgenowskim i za pomocą tomografii komputerowej jamy brzusznej, uwidoczniono w lewym podbrzuszu nieregularną zmianę odpowiadającą przemieszczonemu balonowi odchudzającemu. Z uwagi na brak wskazań do doraźnej laparotomii zastosowano leczenie zachowawcze. Ostatecznie doszło do samoistnego wydalenia balonu, uzyskano poprawę stanu ogólnego i ustąpienie dolegliwości. Omówienie przypadku: Leczenie otyłości poprzez założenie balonu żołądkowego jest małoinwazyjną metodą terapii. Skuteczność oraz brak powikłań zależą jednak od wiedzy chorego o możliwych powikłaniach, zarówno otyłości, jak i wybranej metody leczniczej, poczucia choroby oraz motywacji pacjenta do osiągnięcia celu terapii. Objawy samoistnego wydalania balonu są objawami ciała obcego w przewodzie pokarmowym. W opisanym przypadku ewoluowały one wraz z przemieszczaniem się balonu przez kolejne odcinki przewodu pokarmowego – były to objawy wysokiej niedrożności i kolki jelitowej. Dokładne zlokalizowanie balonu oraz ustalenie odcinka przewodu pokarmowego, w jakim się znajdował, były możliwe po wykonaniu tomografii komputerowej jamy brzusznej. W tym przypadku zastosowano zachowawczą metodę leczniczą – było to możliwe dzięki ścisłej współpracy pomiędzy lekarzami o specjalnościach zachowawczych i zabiegowych oraz dokładnej obserwacji dynamiki objawów chorobowych i przemieszczania się balonu przez światło przewodu pokarmowego. Wnioski: Ze względu na możliwość powikłań interwencyjne metody redukcji masy ciała powinny zostać poprzedzone metodami zachowawczymi. Przed doborem metody inwazyjnego leczenia należy przeprowadzić szczegółową analizę przyczyn nieskuteczności leczenia zachowawczego. Utrzymywanie balonu odchudzającego po upływie dozwolonego czasu terapii może skutkować niebezpiecznymi dla zdrowia i życia powikłaniami. W przypadku objawów niedrożności lub kolki jelitowej pacjenta z założonym balonem należy poddać obserwacji szpitalnej. Przy braku klinicznych objawów perforacji przewodu pokarmowego w pierwszej kolejności należy wykonać badanie gastroskopowe w celu zlokalizowania balonu w żołądku, w dalszej – badania obrazowe radiologiczne, lokalizujące położenie balonu w jelitach oraz dokumentujące postęp w jego wydalaniu. Leczenie operacyjne należy rozważyć w przypadku braku postępów samoistnego wydalania balonu.
Viruses, including the respiratory syncytial virus, are the most common aetiological factor of respiratory infections in children. The disease most often occurs between November and April in the northern climate zone, with a peak incidence in January and February. A total of 90% of children up to 2 years of age become infected with respiratory syncytial virus. The virus is recognised as the most common pathogen causing bronchiolitis and pneumonia in children up to 2 years old. Respiratory syncytial virus infection is the cause of 17–20% of all infantile hospitalisations. It stimulates both humoral and cellular responses of the immune system, which involves a number of cells (neutrophils, eosinophils) and inflammatory mediators (IL-1, IL-5, IL-6, IL-11, TNF-α, MIF-1a, IL-8, RANTES). It is assumed that apart from its effects on the immune system, respiratory syncytial virus also affects the neurogenic regulation of respiratory smooth muscle tension, resulting in bronchial contraction. Respiratory syncytial virus infection leads to lymphocyte Th1/Th2 imbalance with the dominance of Th2, which can indicate a further relationship with asthma. The correlation between early respiratory syncytial virus infection and the recurrence of post-infection wheezes and asthma development later in life constitutes a subject of numerous discussions. Long-term research demonstrated a relationship between respiratory syncytial virus infections and obstructive ventilatory impairment in the first decade of life. However, no clear causal relationship was found. It is assumed that respiratory syncytial virus infection can only manifest in children with genetic predisposition to this type of response.

Najczęstszym czynnikiem etiologicznym zakażeń układu oddechowego u dzieci są wirusy, a wśród nich syncytialny wirus oddechowy. Zachorowania w północnoeuropejskiej strefie klimatycznej występują przeważnie od listopada do kwietnia, a szczyt przypada na styczeń i luty. Do 2. roku życia zakażeniom RSV ulega 90% dzieci. Wirus ten jest uznawany za najczęstszy patogen wywołujący zapalenia oskrzelików i zapalenia płuc u dzieci do 2. roku życia. Zakażenie syncytialnym wirusem oddechowym stanowi przyczynę około 17–20% wszystkich hospitalizacji dzieci w wieku niemowlęcym. Stymuluje on zarówno odpowiedź humoralną, jak i komórkową układu odpornościowego, w której bierze udział szereg komórek (neutrofile, eozynofile) oraz mediatorów stanu zapalnego (IL-1, IL-5, IL-6, IL-11, TNF-α, MIF-1a, IL-8, RANTES). Przyjmuje się, że oprócz działania na układ immunologiczny syncytialny wirus oddechowy wpływa też na neurogenną regulację napięcia mięśni gładkich dróg oddechowych, wywołując skurcz oskrzeli. Zakażenie tym patogenem prowadzi do zaburzenia równowagi limfocytów Th1/Th2 na korzyść Th2, co może wskazywać na dalsze związki z astmą oskrzelową. Korelacja między wczesną infekcją syncytialnym wirusem oddechowym a nawrotowością świstów poinfekcyjnych i rozwojem astmy w przyszłości stanowi temat wielu dyskusji. Długoczasowe badania dowodzą związku zakażeń tym wirusem z zaburzeniami wentylacji typu obturacyjnego w pierwszej dekadzie życia. Nie wykazano jednak jednoznacznego związku przyczynowo-skutkowego. Przypuszcza się, że infekcja RSV może jedynie ujawniać dzieci z genetyczną predyspozycją do tego rodzaju odpowiedzi.
Introduction The problem of constipation in Poland concerns 13.4% of the healthy population. In the case of patients treated with opioids, this number increases to 70-90%, which constitutes a serious problem that lowers the patients' quality of life. The aim of the study was to assess the effects of abdominal massage, which successfully reduces constipation in various diseases, on palliative care patients. Material and methods The research included 18 patients of a palliative care facility (mean age 78.3 ± 10 years), 11 of whom completed the study. The study participants were randomly divided into two groups, i.e. a pilot group (abdominal massage and kinesiotherapy) and a control group (kinesiotherapy). The therapy lasted 8 weeks and blind evaluation was carried out once per week. The study involved evaluating constipation intensity (The Bowel Function Index), the frequency of defecation (medical documentation) and abdomen circumference (anthropometric tape). The collected data were analysed statistically with the use of Statistica software. Results The observed differences between the studied groups undergoing different therapies in subsequent weeks were not statistically significant. However, in the group in which abdominal massage was implemented, a decrease in the intensity of constipation, an increase in the frequency of defecation and a reduction in abdomen circumference were noted compared to the control group. The patients also indicated additional positive effects of this form of therapy, i.e. an improvement in breathing, easier urination, release of excessive gas and abdominal pain reduction. Conclusions The collected data led to the conclusion that abdominal massage may result in a decrease in disorders accompanying opioid-induced constipation. Therefore, it is worth considering the implementation of this form of therapy in the case of patients in an advanced stage of cancer. There is also a need for further research in this field which will include a larger number of patients.
