Lightning injuries are one of the most rare type of injuries that are admitted to the ER. They are presenting a wide spectrum of symptoms with less or more dangerous evolutions. The most severe of them are complicated by: rhabdomyolysis, acute kidney injury and danger cardiac arrythmias. Treatment of those patients is challenging for every personnel of emergency ward. In this paper we present a case of 34 years old male which suffered from lightning injury during family fest organized on a public square. Over time, patient status was changing from almost lack of symptoms to severe complications, including: rhabdomyolysis, compartment syndrome, hyperkaliemia and cardiac arrythmias. Treatment process las for 18 days and includes: ER, ICU, Operating Room and Internal Medicine Ward. In the discussion, author analysis retrospectively diagnostic process and is making a questions about improvement methods for dealing with this kind of “unusual” cases that are presented at the ER.

Urazy od wyładowań atmosferycznych to jedne z najrzadszych rodzajów urazów, które trafiają do SOR. Przedstawiają szerokie spektrum objawów z mniej lub bardziej niebezpiecznymi ewolucjami. Najpoważniejsze z nich powodują: rabdomiolizę, ostry uraz nerek i groźną arytmię serca. Leczenie tych pacjentów jest wyzwaniem dla personelu szpitalnego oddziału ratunkowego (SOR). W artykule przedstawiono przypadek 34-letniego mężczyzny, który doznał urazu piorunem podczas festynu rodzinnego na placu miejskim. Z biegiem czasu stan pacjenta zmieniał się od braku objawów do poważnych powikłań, w tym: rabdomiolizy, zespołu ciasnoty, hiperkaliemii i arytmii serca. Leczenie trwało 18 dni i obejmowało interwencje w: SOR, OIT, Sali Operacyjnej oraz Oddziale Chorób Wewnętrznych. W dyskusji autor retrospektywnie analizuje proces diagnostyczny i stawia pytania o doskonalenie metod postępowania z tego typu „nietypowymi” przypadkami, które są prezentowane na SOR.
A high percentage of patients undergoing physiotherapeutic treatment due to motor organ dysfunctions or neurological problems are the ones who additionally suffer from heart rhythm disorder (arrhythmia).This paper presents the most common types of heart rhythm disorders, the health risk factors they involve as well as the treatment methods. Some important aspects of kinesiotherapy and physiotherapy of patients treated pharmacologically or with implanted pacemaker or cardioverter-defibrillator have been considered. The general rules for contraindications against physiotherapy have also been raised. The significance of patient education, the psychical effects and safety during physiotherapy have been emphasized.
