Aging population, increases the number of major abdominal surgery (MAS) performed in the elderly. Main goal of physiotherapy after that surgery is prevention postoperative complications and reduction of functional limitation. The aim of the study was to asses functional status elderly people after MAS during early postoperative physiotherapy. Material and methods. In a prospective randomized study involved 34 patients scheduled for elective MAS, aged 65+. Patients were randomly assigned to receive PNF or conventional physiotherapy. The study included forced spirometry (FVC, FEV1, PEF) and functional tests (gait speed, up&go). Measurements were performed before surgery and the fourth day after surgery. Also analyzed age, sex, BMI and the level of postoperative independence (postoperative independence scale SAP). Kolmogorow- Smirnow test was used to check normal distribution, t-Student was used to check whether two sets of data differ significantly, and r-Pearsons for correlations testing. p values <0.05 were considered significant. Results. After surgery the time of gait speed test and up and go test was significant longer in comparison to preoperative value. FVC%, FEV1%, PEF% values was decrease. In the PNF group was found significantly higher postoperative independence(SAP) and shorter length of stay in hospital compared to conventional physiotherapy group. Results of SAP and functional tests were significantly positive correlated. Conclusions. Major abdominal surgery decrees efficiency of walking and lung ventilation after 65 year old in early postoperative period. Some techniques of the PNF concept used in improving older patients after the MAS may favourably affect the postoperative increase independence and reduce the time of hospitalization.
Ischaemic stroke as a complication of acute coronary syndrome occurs in approximately 1.40% of cases. Multimodal rehabilitation is an essential component of treatment following a stroke. Case report: The clinical case refers to a patient after a stroke and acute coronary syndrome preceded by bilateral leg amputation. This paper presents the course of the 25-day psychomotor rehabilitation programme conducted in the department of neurological rehabilitation. Methods: The treatment has been evaluated using functional scales (Activity Daily Living Scale, Modified Rankin Scale, National Institutes of Health Stroke Scale, Rivermead Motor Assessment) and psychological tests (Geriatric Depression Scale, Mini–Mental State Examination). Results: The functional improvement focused mainly on the upper limb and global movements. The intensity of the depressive disorders decreased from severe to mild (21 versus 11). Mini–Mental State Examination results normalized (19 versus 27). Conclusions: Early-applied individual rehabilitation program with intensive psychological therapy is a very important element of treatment of patients with a multifactorial mobility decrease.

Wstęp: Udar niedokrwienny mózgu jako powikłanie ostrego zespołu wieńcowego występuje w około 1,40% przypadków. Kompleksowa rehabilitacja jest niezbędnym elementem leczenia osób po udarze. Opis przypadku: Prezentowany przypadek kliniczny dotyczy pacjenta po udarze mózgu poprzedzonym ostrym zespołem wieńcowym; w przeszłości chory przebył obustronną amputację podudzi. Praca przedstawia przebieg 25-dniowego programu rehabilitacji psychoruchowej, którą prowadzono na oddziale rehabilitacji neurologicznej. Metody: Skuteczność zastosowanego leczenia oceniono za pomocą skal funkcjonalnych (skala czynności życia codziennego, zmodyfikowana skala Rankina, Skala Udaru Narodowego Instytutu Zdrowia, Ocena Motoryczności Rivermead) i psychologicznych (Geriatryczna Skala Oceny Depresji Yesavage’a, Krótka Skala Oceny Stanu Umysłowego). Wyniki: Poprawa funkcjonalna dotyczyła przede wszystkim kończyny górnej i ruchów globalnych. Zmniejszyło się nasilenie zaburzeń depresyjnych (21 versus 11 punktów w Geriatrycznej Skali Oceny Depresji Yesavage’a). Znormalizowały się wyniki uzyskiwane przez pacjenta w Krótkiej Skali Oceny Otępień (19 versus 27 punktów). Wnioski: Wcześnie zastosowany indywidualny program rehabilitacji wraz z intensywną terapią psychologiczną to bardzo ważny element leczenia osób z wieloczynnikowym obniżeniem sprawności ruchowej.
Wstęp Znaczenie fizjoterapii w poprawie jakości życia pacjentów objętych stacjonarną opieką długoterminową nie jest jeszcze do końca poznane. Celem badania była ocena jakości życia i stanu funkcjonalnego pacjentów objętych opieką długoterminową w odniesieniu do prowadzonej rehabilitacji. Projekt badawczy Badanie przekrojowe Materiał i metody Badanie przeprowadzono w grupie 58 osób. Do zebrania danych wykorzystano kwestionariusz dotyczący rehabilitacji, skalę Barthel, skalę VAS oraz kwestionariusz WHOQOL-Bref. Wyniki Stan funkcjonalny oceniono jako średnio ciężki i lekki u 83,9% osób. Ponad 70% pacjentów jakość życia oceniło jako dobrą i bardzo dobrą. Nie wykazano istotnej zależności między uczestnictwem w rehabilitacji a jakością życia badanych (p=0,35). Istnieje monotoniczna zależność pomiędzy zadowoleniem z rehabilitacji a domeną socjalną jakości życia (r=0,34, p=0,01). Wykazano istotną zależność między jakością życia a BMI, wykształceniem, bólem i poziomem sprawności ocenionym w skali Barthel. Wnioski Większość ankietowanych mieszkańców Domów Pomocy Społecznej uczestniczyła w rehabilitacji i była z niej zadowolona, co pozytywnie wpłynęło na ocenę jakości życia w zakresie relacji społecznych.

The importance of physiotherapy in improving quality of life of patients in long-term care is not yet fully understood. The aim of the study was to assess the quality of life and functional status of patients in long-term care with respect to their rehabilitation. Study design: Cross-sectional. The study was conducted among a group of 58 individuals. A questionnaire to collect the data concerning rehabilitation, the Barthel scale, VAS and WHOQOL-Bref were used. The functional condition was assessed as moderately heavy and light in 83.9% of participants. More than 70% of patients rated the quality of life as good and very good. There was no significant relationship between participation in rehabilitation and quality of life of the subjects (p=0.35). There was a monotonic relationship between the social domain of quality of life and satisfaction with rehabilitation (r=0.34, p=0.01). There was a significant correlation between the quality of life and BMI, education, pain and level of functional efficiency assessed using the Barthel scale. Introduction: Brain tumours account for 2% of malignancies in Poland. Comprehensive rehabilitation is essential for regaining psychomotor aptitude following tumour removal. Case report: We present a clinical case of a patient having undergone the removal of a brain tumour (papilloma plexus choroidei) located in the area of the cerebellopontine angle and the 4th ventricle of the brain. The patient went through a 1-month structured rehabilitation programme at the Neurological Rehabilitation Department. Methods: The following tools were used to assess the effectiveness of the treatment: Barthel Index of Activities of Daily Living (Barthel ADL), Rankin, Lovett, Montreal Cognitive Assessment and Beck Depression Inventory scales. Results: First and foremost, the patient’s ability to carry out daily self-care activities improved. She became fully self-reliant within two years (Barthel ADL 12 versus 20). Conclusions: Cases similar to the one discussed here require long-term rehabilitation that may ultimately facilitate significant functional improvement.

Wstęp: Nowotwory mózgu stanowią 2% zachorowań na nowotwory złośliwe w Polsce. Kompleksowa rehabilitacja po usunięciu guza jest niezbędna do odzyskania sprawności psychoruchowej. Opis przypadku: Prezentowany przypadek kliniczny dotyczy pacjentki po usunięciu guza mózgu (papilloma plexus choroidei) umiejscowionego w okolicy kąta mostowo- -móżdżkowego i IV komory mózgu. Chora została poddana miesięcznemu programowi rehabilitacji w oddziale rehabilitacji neurologicznej. Metody: Do oceny skuteczności zastosowanego leczenia użyto skali dla typowych czynności dnia codziennego (Barthel Index of Activities of Daily Living – Barthel ADL) oraz skal Rankina, Lovetta, Montreal Cognitive Assessment i Beck Depression Inventory. Wyniki: Poprawiła się przede wszystkim samoobsługa – w ciągu dwóch lat pacjentka stała się osobą samodzielną (wynik w skali Barthel ADL: 12 versus 20). Wnioski: Przypadki takie jak przedstawiony w pracy wymagają długoterminowej rehabilitacji, która może doprowadzić do znacznej poprawy funkcjonalnej.
