Wstęp: Zespół słabości (FS) jest syndromem ograniczeń funkcjonalnych związanych z wiekiem starszym, a jego specyfika, jak i skala występowania, uzasadniają potrzebę jego opisu i uporządkowania wiedzy w tym zakresie Cel: Celem dokonanego przeglądu literatury jest systematyzacja wiedzy w zakresie poszczególnych elementów opisowych FS oraz rehabilitacji dedykowanej FS. Materiał i metody: Publikacja ma charakter przeglądowy i dokonuje systematyzacji materiału badawczego w zakresie frailty syndrome. Przegląd narracyjny oparto na analizie doniesień opublikowanych w bazie Pubmed w okresie 2012-2017 oraz polskich źródłach wydanych w tym okresie. Dokonano selekcji źródeł zgodnie z celem pracy. Wyniki: Zespół słabości jest określany i definiowany w różnorodny sposób, choć główne odniesienie odnosi się do pierwszego jego opisu, sformułowanego przez Fried i wsp. Istnieją trzy główne fenotypy zespołu słabości, klasyfikowane w oparciu o kryteria twórców tego zespołu. W diagnostyce stosowane są różne skale: CHS, FRAIL, EFS, a także wskaźniki GFI, TFI i PLFI. Zespół słabości rozwija się w oparciu o przyczyny i skutki wpisane w tzw. kaskadę słabości, a wśród nich kluczowe znaczenie mają przewlekłe procesy zapalne, sarkopenia i zmiany immunologiczne. Rekomendowane schematy fizjoterapii obejmują różnorodne cykle i rodzaje aktywności fizycznej, przy czym najczęściej podkreślana jest przydatność 12-tygodniowego programu ćwiczeń o różnym charakterze. Wnioski: Frailty jest stanem wyczerpania rezerw i ograniczeń funkcjonalnych wieku starszego, określanym też mianem zespołu słabości, kruchości i wątłości. Najwięcej doniesień potwierdza stosowanie i przydatność skali CHS. Brak jednoznacznych schematów postępowania fizjoterapeutycznego w odniesieniu do FS.

Introduction: The frailty syndrome (FS) is a syndrome of functional limitations related to the older age, and its specificity, as well as the scale of occurrence, justify the need for its description and ordering of knowledge in this area. Aim: The aim of the literature review is to systematize knowledge in the field of individual descriptive elements of FS and rehabilitation dedicated to FS. Material and metods: The publication is a review and systematizes research material in the field of syndrome frailty. The narrative review was based on the analysis of publications contained in the Pubmed database in the period 2012-2017 and Polish sources published in this period. The sources were selected in accordance with the purpose of the work. Results: The frailty syndrome is defined and defined in various ways, although the main reference refers to its first description, formulated by L. Fried et al. There are three main phenotypes of the weakness group, classified based on the criteria of the creators of this team. Different scales are used in diagnostics: CHS, FRAIL, ESF, as well as GFI, TFI and PLFI indicators. The frailty syndrome develops based on the causes and effects of the so-called a cascade of weaknesses, among them chronic inflammatory processes, sarcopenia and immunological changes are of key importance. Recommended physiotherapy regimens cover various cycles and types of physical activity, with the use of a 12-week program of exercises of various types most often emphasized. Conclusions: Frailty is a state of exhaustion of reserves and functional limitations of the older age, also referred to as a team of weakness, frailty and fragility. Advanced age is often burdened with many deficits that are a consequence of the aging process, unfavorable lifestyle and multi-morbidity. They contribute to increased morbidity and disability of the older people. Laryngological problems often concern seniors, particularly progressing with age hearing impairment, dizziness, balance disorders, epistaxis, nasopharyngeal diseases, larynx, sinuses, and salivary glands disorders. They are not only a health threat, such as head and neck cancer, but they contribute to the deterioration of mobility, falls and injuries, depression, cognitive functions impairment and, consequently, functional disability, loss of independence and a sense of isolation. In this context, taking as an example the most common ailment, which is hearing impairment, laryngological procedures in the form of the use of hearing aids or cochlear implant improves cognitive functions, mood, self-esteem, opportunities for social interaction and everyday functioning, and the quality of life of older people. In turn, in view of the typical for older age polypathology and of significant reduction of the reserves of the organism, effective treatment aimed at improving health, preventing complications of diagnostics and therapy as well as disability with the desire to maintain independence and good quality of life requires the cooperation of different specialists. The knowledge and experience of geriatricians and the comprehensive geriatric assessment used as the diagnostic tool, aimed at identifying deficits typical of seniors’ age may significantly contribute to a more adequate risk and benefit assessment, selection of optimal treatment for a given health situation and identification of high-risk individuals requiring during the treatment period special supervision to reduce the risk of complications and the risk of development of geriatric syndromes such as delirium, cognitive disorders, malnutrition, falls and injuries, functional disability.
Zaawansowany wiek często obarczony jest wieloma deficytami będącymi konsekwencją procesu starzenia się, niekorzystnego stylu życia i wielochorobowości. Przyczyniają się one do zwiększenia chorobowości i niepełnosprawności osób starszych. Problemy laryngologiczne często dotyczą seniorów, szczególnie postępujące z wiekiem upośledzenie słuchu, zawroty głowy, zaburzenia równowagi, krwawienia z nosa, choroby nosogardła, krtani, zatok, ślinianek. Stanowią one nie tylko zagrożenie zdrowotne, jak np. choroby nowotworowe w obrębie głowy i szyi, ale przyczyniają się do pogorszenia mobilności, występowania upadków i urazów, depresji, pogorszenia funkcji poznawczych, a w konsekwencji do upośledzenia funkcjonalnego, utraty samodzielności, poczucia izolacji. W tym kontekście, biorąc za przykład najczęstszą dolegliwość, jaką jest upośledzenie słuchu, postępowanie laryngologiczne w postaci zastosowania aparatów słuchowych lub implantacji ślimaka poprawia funkcje poznawcze, nastrój, poczucie własnej wartości, możliwości interakcji społecznych i codziennego funkcjonowania oraz jakość życia osób starszych. Z kolei wobec charakteryzującej okres starości polipatologii i stanu znacznego ograniczenia rezerw organizmu, skuteczne postępowanie mające na celu poprawę stanu zdrowia, zapobieganie powikłaniom diagnostyki i terapii, a także niepełnosprawności z dążeniem do utrzymania samodzielności i dobrej jakości życia wymaga współpracy specjalistów różnych dziedzin. Wiedza i doświadczenie geriatrów oraz stosowana w procesie diagnostycznym całościowa ocena geriatryczna ukierunkowana na identyfikację typowych dla wieku senioralnego deficytów może istotnie przyczynić się do bardziej adekwatnej oceny ryzyka i korzyści, doboru optymalnego dla danej sytuacji zdrowotnej sposobu postępowania oraz identyfikacji osób wysokiego ryzyka wymagających objęcia w okresie leczenia szczególnym nadzorem celem redukcji ryzyka powikłań oraz ryzyka ujawniania się zespołów geriatrycznych, takich jak: majaczenie, zaburzenia poznawcze, niedożywienie, upadki i urazy, niesprawność funkcjonalna.

Introduction. Idea of frailty was elaborated to provide a clinically useful description of age-associated multisystem dysregulations that result in multimorbidity, disability, and finally death. Allostatic load score was proposed as an integrated measure of pathophysiological disorders leading to frailty. We have analysed these items in a group of subjects included to the Geriatric Rehabilitation Program. Material and methods. Twenty four subjects (15 female and 9 male; 72,3 ± 5,6 years; mean ± SD) were submitted to an assessment defined by frailty criteria according to Fried and Gruenewald. Twelve of 13 biomarkers of the allostatic load score according to Gruenewald, Mini-Mental State Examination (MMSE), Geriatric Depression Scale, Tinetti Test, Fullerton Fitness Test, some items of the Takata Test and Functional Reach Test were also performed. Results. Subjects classified as prefrail according to Fried as compared with normal had lower MMSE score (27.9 ± 1.28 vs. 29.9 ± 1.66; p = 0.041), Tinetti Test score (25.5 ± 4.1 vs. 27.4 ± 1.0; p = 0.048), and worse Timed „Up and Go” Test result (13.5 ± 5.1 vs. 9.6 ± 2.2 sec; p = 0.022). Frailty index according to Fried correlated negatively with MMSE score, Tinetti Test score and knees extensors strength moment, while positively – with Timed „Up and Go” Test. Subjects classified as frail according to ruenewald as compared with prefrail or normal had lower Tinetti Test (18.0 ± 8.5 vs. 26.8 ± 1.5 vs. 27.8 ± 0.5; p = 0.034), Reach Test (5.5 ± 3.5 vs. 21.0 ± 5.9 vs. 25.0 ± 4.1 cm; p = 0.030), and 6-Minute Walk Test (6MWT) (201±25 vs. 368 ± 119 vs. 426±27 m; p = 0.047) results. Frailty index according to Gruenewald correlated negatively with Tinetti Test score, distance of 6MWT, knees extensors strength moment, and Reach Test, and positively – with Geriatric Depression Scale, and Timed „Up and Go” Test. Conclusion. Relationship between frailty indices and functional tests used in geriatric rehabilitation suggests a possibility to use these tests as a screening for frailty. Wstęp. Zespół słabości stanowi próbę opisania w sposób klinicznie użyteczny wieloukładowych zmian związanych ze starzeniem, będących podłożem wielochorobowości oraz niepełnosprawności, prowadzących ostatecznie do śmierci. Wskaźnik obciążenia allostatycznego jest propozycją zintegrowanej miary zaburzeń patofizjologicznych prowadzących do zespołu słabości. Przeprowadzono analizę tych zaburzeń w grupie badanych zakwalifikowanych do Programu Rehabilitacji Geriatrycznej. Materiał i metody. Grupę badaną stanowiły 24 osoby (15 kobiet oraz 9 mężczyzn) w wieku 72,3 ± 5,6 lat (średnia ± SD). Przeprowadzono badania zdefiniowane przez kryteria zespołu słabości wg Fried i Gruenewald, pakiet 12 spośród 13 składników wskaźnika obciążenia allostatycznego wg Gruenewald, Krótką Ocenę Stanu Umysłowego (MMSE), Geriatryczną Skalę Oceny Depresji (GDS), test Tinetti, Fullerton Fitness Test, elementy testu Takata oraz test sięgania (Functional Reach Test). Wyniki. Badani zakwalifikowani do grupy prefrailty wg Fried w porównaniu do badanych w stanie prawidłowym mieli mniejszą wartość wyniku MMSE (27,9 ± 1,28 vs 29,9 ± 1,66 punktów, p = 0,041), gorszy wynik testu Tinetti (25,5 ± 4,1 vs 27,4 ± 1,0 punktów, p = 0,048) oraz testu „wstań i idź” („Up and Go Test”) (13,5 ± 5,1 vs 9,6 ± 2,2 s, p = 0,022). Liczba punktów wskaźnika zespołu słabości według kryteriów Fried korelowała ujemnie z wynikiem MMSE, testu Tinetti i tensometrycznym pomiarem momentu siły prostowników stawów kolanowych, a dodatnio – z wynikiem testu „wstań i idź”. Badani spełniający kryteria zespołu słabości wg Gruenewald w porównaniu do badanych spełniających kryteria prefrailty lub normy mieli gorsze wyniki testu Tinetti (18,0 ± 8,5 vs 26,8 ± 1,5 vs 27,8 ± 0,5 punktów, p = 0,034), testu sięgania (5,5 ± 3,5 vs 21,0 ± 5,9 vs 25,0 ± 4,1 cm, p = 0,030) i dystansu 6-minutowego testu marszowego (201 ± 25 vs 368 ± 119 m vs 426 ± 27 m, p = 0,047). Liczba punktów wskaźnika zespołu słabości według kryteriów Gruenewald korelowała ujemnie z wynikiem testu Tinetti, dystansem 6-minutowego testu marszowego, momentem siły prostowników stawów kolanowych i wynikiem testu sięgania, a dodatnio – z wynikiem GDS i testu „wstań i idź”. Wniosek. Zależność między wskaźnikami zespołu słabości a testami czynnościowymi stosowanymi w rehabilitacji geriatrycznej sugeruje możliwość wykorzystania tych testów w badaniach przesiewowych zespołu słabości.
