Multiple sclerosis is a progressive disease of the central nervous system with a complex immunopathogenesis characterised by inflammation, demyelination and progressive loss of axons. In the studies on the complex and multifactorial pathophysiology of multiple sclerosis, an animal model of the experimental autoimmune encephalomyelitis plays an important role. Clinical and experimental data suggest autoimmune background of inflammatory responses in the central nervous system. Unfortunately, the pathology of multiple sclerosis is still not fully understood, and the currently available drugs are only able to inhibit the progression of the neurological deficits. Epidemiological data clearly show that gender significantly affects the incidence and course of multiple sclerosis. In both clinical and experimental studies, pregnancy and hormonal treatment with female sex hormones have been shown to influence the number of relapses and the progression of neurological deficits. Abundant data suggesting the role of both sex and pregnancy in multiple sclerosis gave rise to research on the role of sex hormones in this disease and new forms of treatment. In this paper, we have reviewed the available literature regarding the effects of female sex hormones on the course of multiple sclerosis, with particular emphasis on the role of estrogens and progestagens. Data from both clinical and experimental studies have been included. Estrogens and progestagens are the two most important groups of female steroid hormones. They are necessary for development of sex organs as well as for fertility and during pregnancy. It is known that these hormones are able to modulate the function of the immune system. In this review, we have focused not only on the immunomodulatory, but also on the neuroprotective and neuroregenerative role of estrogens and progestagens. We have also described the new therapeutic possibilities associated with the hormonal therapy targeting sex hormones.

Stwardnienie rozsiane jest postępującą chorobą ośrodkowego układu nerwowego o złożonej immunopatogenezie, w której występują zapalenie, uszkodzenie mieliny i utrata aksonów. W badaniach doświadczalnych nad patofizjologią stwardnienia rozsianego istotną rolę odgrywa model zwierzęcy – eksperymentalne autoimmunologiczne zapalenie mózgu i rdzenia. Badania kliniczne i doświadczalne wskazują na autoimmunologiczne tło zapalenia w tej chorobie. Niestety, przyczyny rozwoju stwardnienia rozsianego nie zostały dotąd w pełni poznane, a dostępne leki tylko hamują progresję objawów neurologicznych. Z badań epidemiologicznych jednoznacznie wynika, że płeć istotnie wpływa na zachorowalność, a także na przebieg stwardnienia rozsianego. Liczne dane wskazujące na ważną rolę zarówno płci, jak i ciąży były bodźcem do rozwoju badań nad rolą hormonów płciowych w tej chorobie oraz do poszukiwania nowych form terapii. W niniejszej publikacji dokonano przeglądu literatury omawiającej wpływ żeńskich hormonów płciowych, ze szczególnym uwzględnieniem estrogenów i progestagenów, na przebieg stwardnienia rozsianego. Pod uwagę wzięto dane z badań klinicznych i eksperymentalnych. Estrogeny i progestageny należą do najważniejszych żeńskich hormonów steroidowych. Są niezbędne do prawidłowego rozwoju i różnicowania narządów rozrodczych, a także do utrzymania płodności i ciąży. Od dawna znany jest wpływ tych hormonów na regulację odpowiedzi immunologicznej. W niniejszym opracowaniu szczególną uwagę poświęcono nie tylko działaniu immunomodulacyjnemu, ale również neuroprotekcyjnemu i neuroregeneracyjnemu żeńskich hormonów płciowych. Omówiono też nowe możliwości terapeutyczne związane z terapią hormonalną.
The main role of melatonin is regulation of circadian and seasonal rhythm. Recently there were carried out a lot of researches on the role and potential application of melatonin in prevention and therapy of various dismelaeases. This work is related to researches about the melatonin infl uence on estrogen-dependent breast cancer and application in treating this neoplasm. Probable oncostatic mechanisms i.e. melatonin acting as SERM, SEEM and its infl uence on the hypothalamic – pituitary – reproductive axis, was analyzed. Moreover the relation between circadian disruptions and increase in breast cancer risk were observed. The aim of this work is to show mechanisms melatonin acting and how we can use this hormone in oncology.

Melatonina pełni przede wszystkim funkcję biochemicznego regulatora rytmów okołodobowych i sezonowych, ale szczególne zainteresowanie naukowców wzbudzają udowodnione już jej właściwości antyoksydacyjne i immunomodulujące oraz jej wpływ na regulację wydzielania innych hormonów. W ostatnich latach zostało przeprowadzonych wiele badań dotyczących roli oraz potencjalnego zastosowania melatoniny w profi laktyce i leczeniu różnych jednostek chorobowych. Praca jest przeglądem piśmiennictwa dotyczącego badań nad wpływem melatoniny na patogenezę raka sutka i jej terapeutycznego zastosowania w tym nowotworze. Badano prawdopodobne mechanizmy onkostatyczne dotyczące wpływu na oś podwzgórzowo-przysadkowo-gonadalną oraz działanie melatoniny jako selektywnego modulatora receptora estrogenowego (SERM) i enzymów biorących udział w biosyntezie estrogenów w tkankach obwodowych (SEEM). Ponadto zaobserwowano związek między zaburzeniami rytmu dobowego u kobiet a wzrostem ryzyka zachorowania na raka sutka. Celem niniejszej pracy jest przedstawienie mechanizmów działania melatoniny i jej zastosowania w onkologii.
Estrogens, without doubt, play a pivotal role in the regulation of both physiological and pathological processes. A plethora of intercellular signaling pathways are regulated by estrogens on both genomic, and non–genomic pathways via canonical ERα and ERβ receptors. Studies published in recent years showed that not all biological effects of estrogens can be attributed to the classical model of estrogen signaling. Aside canonical ERα and ERβ receptors a G–protein coupled estrogen receptor plays its role in estrogen mediated regulation of various tissues and organs. Ubiquitous presence of G–protein coupled estrogen receptor in different tissues, as well as observed de–regulation of its expression in multiple pathologies suggest an important role of this receptor in functioning of cells, tissues and organisms. Activation/deactivation of GPER estrogen receptor takes place during the metabolism of carbohydrates, lipids and immunological response, it is involved in a number of events from reproductive, cardiovascular, neurological and skeletal systems.

Estrogeny, bez wątpienia, odgrywają istotną rolę w regulacji procesów fizjologicznych i patofizjologicznych. Za pośrednictwem kanonicznych recepto– rów estrogenów ERα i ERβ hormony te modulują wiele ścieżek sygnałowych w komórce zarówno na drodze genomowej, jak i niegenomowej. Wyniki badań ostatnich lat ujawniają, że nie wszystkie biologiczne efekty estrogenów wynikają z ich klasycznego modelu działania. Obok kanonicznych receptorów ERα i ERβ w estrogenozależnej regulacji funkcjonowania wielu tkanek i narządów pośred– niczy receptor estrogenów oddziałujący z białkami G. Powszechne występowa– nie receptora estrogenów oddziałującego z białkami G w różnych tkankach, jak i obserwowana deregulacja jego ekspresji w określonych patologiach pozwala domniemywać o istotnej roli tego receptora w funkcjonowaniu komórek, tkanek i organizmów. Aktywacja/dezaktywacja receptora estrogenów GPER ma miejsce podczas metabolizmu węglowodanów i lipidów czy odpowiedzi immunologicznej, zaangażowana jest w wiele zdarzeń ze strony układu rozrodczego, sercowo– naczyniowego, nerwowego oraz kostnego.
