The properties of one-component and mixed monolayers of phospholipid 1,2–dipalmitoyl–sn–glycero–3–phosphocholine (DPPC) and polypeptide cyclosporine A (CsA) on the chitosan subphase were studied. DPPC is the main component that builds biological membranes, and CsA is an immunosuppressive drug typically used in medicine to prevent transplant rejections. The stability and reversibility of compression of these insoluble monolayers in the presence of chitosan (Ch) were examined by the Langmuir technique. The stability of the monolayers depended on the monolayer composition as well as the initial pressure (π0) of the relaxation process. The smallest changes in the relative pressure as a function of time were obtained at π0 = 10 mN/m. During compression–decompression cycles, the effect of chitosan was noticeable and caused isotherm shifts.
Cyclosporine A is very effective in treatment of many ophthalmic pathologies like immunological and inflammatory diseases of the cornea, conjunctive or uvea. It is also used to prevent corneal graft rejection and to treat dry eye syndrome. Local ocular administration significantly reduces the risk of systemic side effects. Currently on the market are two products in the form of eye drops: Restasis® (0.05% of cyclosporine) in US and Ikervis® (0.1% of cyclosporine) in the EU. Both are in the form of oil-in-water emulsion. Their availability is still limited and cyclosporine concentration is low. This is a reason that compounded formulations are prepared in hospital and retail pharmacies. The pharmacists do not have an access to the substance and use injectable (50 mg/ml cyclosporine), oral solutions (100 mg/ml cyclosporine) or even oral capsules (25-100 mg/capsule) as the source of cyclosporine. That allows to use of cyclosporine for topical ophthalmic applications in varies formulations, indications, concentration and posology. Due to the fact that cyclosporine is a highly lipophilic and poorly water soluble, it is mostly compounded and administered as an oily solution. Unfortunately, that dosage form is poorly tolerated after administration to the eye, mainly due to the fact of blurring vision commonly observed because of the high viscosity of the oils. However, for many patients use of compounded eye drops with cyclosporine can be the only hope to alleviate the symptoms of the disease, and sometimes even to avoid vision loss. When pharmaceutical compounding of eye drops with cyclosporine, the problem is the presence of auxiliary substances such as, for example, Cremophor, ethanol evaporation, or obtaining the exact dose of the active substance from capsules. This article gives an overview of different approaches for compounding of the cyclosporine eye drops in hospital and retail pharmacies using marketed products off-label. Benefits and the limitations of each method are presented with comments regarding the proper drug compounding.

Cyklosporyna A jest skuteczna w leczeniu wielu chorób oczu, takich jak choroby immunologiczne i zapalne rogówki, spojówki czy naczyniówki. Jest również stosowana w zapobieganiu odrzucenia przeszczepu rogówki i leczeniu zespołu suchego oka. Preferowane jest miejscowe podanie cyklosporyny bezpośrednio do oka, by zmniejszyć ryzyko wystąpienia ogólnoustrojowych działań niepożądanych. Ze względu na ograniczoną dostępność gotowych kropli do oczu z cyklosporyną, zwłaszcza w wyższym stężeniu (0,5-2%), w aptekach szpitalnych i ogólnodostępnych sporządzane są recepturowe krople do oczu z wykorzystaniem roztworu do wstrzykiwań lub roztworu doustnego jako źródła cyklosporyny. Niniejszy artykuł stanowi przegląd różnych metod sporządzania recepturowych kropli do oczu z preparatów handlowych cyklosporyny, z omówieniem ich zalet i ograniczeń, jak również zawiera wskazówki dotyczące właściwego postępowania przy sporządzaniu takich kropli.
Small intestinal bacterial overgrowth syndrome is defined as an increased number of non-pathogenic bacteria over 105 microorganisms in one millilitre of intestinal contents in the initial part of the small intestine. Predisposing disorders include e.g. drug-induced hypochlorydia, congenital and acquired defects of the gastrointestinal tract, immunodeficiency, severe stress situations as well as intestinal microflora imbalance after immunosuppressive therapy. The aim of the study was to assess the incidence of small intestinal bacterial overgrowth in children receiving cyclosporine A due to idiopathic nephritic syndrome. Material and methods: The study included 20 children (11 girls and 9 boys) aged 4–16 years (mean age 8.83 ± 3.75 years), diagnosed with idiopathic nephritic syndrome and treated with cyclosporine for over 3 months. The use of antibiotics or probiotics less than 3 months prior to the study was an exclusion criterion. Serum levels of cyclosporin A were measured in all patients. Hydrogen breath test with lactulose was performed as an additional examination. The exhaled breath was analysed using Gastrolyzer (Bedfont). A minimum increase of 20 hydrogen molecules per million air molecules between the maximum value and the fasting level or values in the first hour of the test was considered as a positive test outcome, indicating small intestinal bacterial overgrowth. Results: Negative result of hydrogen breath test, excluding small intestinal bacterial overgrowth, was observed in all 20 children. Conclusions: The administration of second line immunosuppressive agents in children with idiopathic nephritic syndrome does not induce non-pathogenic bacterial multiplication in the small intestine.

Jako zespół przerostu bakteryjnego jelita cienkiego określa się wzrost liczby niepatogennych bakterii powyżej 105 komórek bakteryjnych w mililitrze treści jelitowej w początkowym odcinku jelita cienkiego. Do zaburzeń predysponujących należą m.in. hipochlorydia polekowa, wrodzone i nabyte wady przewodu pokarmowego, niedobory odporności, silne sytuacje stresowe, a także zaburzenia równowagi flory jelitowej po leczeniu immunosupresyjnym. Celem pracy jest ocena częstości występowania zespołu przerostu bakteryjnego w jelicie cienkim u dzieci leczonych cyklosporyną A z powodu idiopatycznego zespołu nerczycowego. Materiał i metoda: Badaniem objęto 20 dzieci (11 dziewczynek, 9 chłopców) w wieku 4–16 lat (średnia 8,83 ± 3,75 roku) z rozpoznanym idiopatycznym zespołem nerczycowym leczonych cyklosporyną ponad 3 miesiące. Kryterium wykluczającym było przyjmowanie antybiotyku lub probiotyków w czasie krótszym niż 3 miesiące poprzedzające badanie. U wszystkich pacjentów oznaczono stężenie cyklosporyny A w surowicy, a jako badanie dodatkowe wykonywano wodorowy test oddechowy po doustnym obciążeniu laktulozą. Analizy powietrza wydechowego dokonano za pomocą aparatu Gastrolyzer (Bedfont). Za dodatni wynik testu, przemawiający za przerostem bakteryjnym w jelicie cienkim, przyjęto przyrost minimum 20 cząsteczek wodoru na milion cząsteczek powietrza między maksymalnym odczytem a wartością na czczo lub wartościami w pierwszej godzinie testu. Wyniki: U wszystkich 20 dzieci uzyskano ujemny wynik wodorowego testu oddechowego, na podstawie którego wykluczono obecność zespołu przerostu bakteryjnego w jelicie cienkim. Wnioski: U badanych dzieci z idiopatycznym zespołem nerczycowym podawanie leków immunosupresyjnych II rzutu nie powoduje ilościowego namnożenia bakterii niepatogennych w jelicie cienkim.
