Ciężka postać rabdomiolizy wtórna do ostrego zatrucia alkoholem

The article presents the case of a 66-year-old man with rhabdomyolysis due to acute alcohol intoxication, manifested clinically by acute kidney injury and a sharp increase in creatine phosphokinase. During hospitalization, the patient suffered from cardiac and respiratory arrest with subsequent death. The post-mortem histopathological examination showed pathologies of numerous organs: putrefactive necrosis, pulmonary edema, pulmonary emphysema, megalocardia, acute myocardial infarction, hemorrhagic erosions of the gastric mucous membrane, atrophy of renal glomerules, colliquative necrosis of the liver, Balser necrosis of the pancreas, ascites, hydrothorax, lacunae in the occipitotemporal region and hydrocephalus. The described case presents a range of clinical problems appearing in the management of patients with severe form of alcohol-induced rhabdomyolysis.

W pracy zaprezentowano przypadek 66-letniego mężczyzny, u którego w wyniku ostrego zatrucia alkoholem doszło do rabdomiolizy manifestującej się klinicznie ostrym uszkodzeniem nerek oraz gwałtownym wzrostem aktywności fosfokinazy kreatynowej. W trakcie hospitalizacji u pacjenta doszło do zatrzymania akcji serca i oddechu, a następnie do zgonu. Posekcyjne badanie histopatologiczne wykazało patologię licznych narządów: proces gnilny, obrzęk płuc, rozedmę płuc, powiększenie serca, świeży zawał serca, nadżerki krwotoczne błony śluzowej żołądka, zanik kłębuszków nerkowych, martwicę rozpływną wątroby, martwicę Balserowską trzustki, wodobrzusze, obecność płynu w jamie opłucnowej, jamy poudarowe okolicy skroniowo-potylicznej oraz wodogłowie. Opisany przypadek prezentuje zakres problemów klinicznych występujących w postępowaniu z pacjentem z ciężką postacią rabdomiolizy indukowanej spożyciem alkoholu.
