The aim of this study was to identify specific aspects of the cognitive and socioemotional functioning of a school-age child with autoimmune-resistant epilepsy, and to provide a detailed explanation of the executive dysfunctions significant in this type of disorder. A battery of neuropsychological tests and clinical trials were used to neuropsychologically diagnose an eight-year-old girl. Data from an interview with the patient’s mother, observational data, and the results of previous psychological and medical tests were also taken account. Neuropsychological diagnosis was performed in line with the clinical–experimental and psychometric -test models. The patient’s current level of intellectual functions is below average. The patient obtained low results in tests examining executive functions, direct auditory memory, phonemic hearing, and visual–spatial organization. In comparison with children of her age group and with similar intelligence quotients, she obtained average results relating to tasks involving direct visual memory. No difficulty was seen in terms of semantic fluency, concentration, or persistence of the attention process. Executive dysfunctions are among the dominant disorders seen in clinical settings; these significantly hinder the systematic and orderly organization of the child’s learning process. Such difficulties in cognitive functioning, but also in emotional and social functioning, can significantly impede the normal development of the child.
Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids (CLIPPERS) is a novel inflammatory central nervous system disorder. The diagnosis is made on the basis of the radiological, neurological, neuro - pathological data and responsiveness to steroids treatment. The aim of this paper is to present a neuropsychological characteristic of cognitive, executive and linguistic symptoms characteristic for patient with CLIPPERS disease. In this paper we present the case of a 42-year-old woman with hyperintense punctate foci in the pons, cerebellum, mid- brain, thalamus, white matter of occipital lobes, with a short- term verbal memory deficits, mild deficiency in verbal and pro cedural learning, executive dysfunctions, hand-eye co ordi - nation impairment, psychomotor speed disabilities and dysarthria, associated with a chronic lymphocytic inflammation. In this challenging diagnosis of CLIPPERS, neuropsychological analysis of cognitive functions can be important to optimize the diagnosis and treatment.
The systemic lupus erythematosus (SLE) is a connective tissue disorder, being developed on the basis of the autoallergy. Clinical symptoms of SLE are connected not only with the occurrence of cutaneous exanthemata, but can also manifest itself with the row of symptoms on the part of internal organs, with haematologic distempers and symptoms about the neuropsychiatric character. The pathogenesis of neuropsychiatric systemic lupus erythematosus (NPSLE) is complicated and one tries to become clear her with the production of autoantibodies and with the postponement himself antigen–antibody complexes, what a succession is the cytotoxic damage of neurons, an irregular expression of inflammatory mediators and an influx of inflammatory cells, vascular distempers and an inclination to thrombuses. The entire frequency of the occurrence NPSLE is estimated on 56.3% patients with SLE. Most often mental disorders in patients with lupus erythematosus are mood disorder, anxiety disorder, cognitive disorders and consciousness clouding. Toczeń rumieniowaty układowy (systemic lupus erythematosus, SLE) jest chorobą tkanki łącznej, rozwijającą się na podłożu autoimmunizacji. Objawy kliniczne SLE są związane z występowaniem nie tylko wykwitów skórnych, ale również dolegliwości ze strony narządów wewnętrznych, a także zaburzeń hematologicznych oraz o charakterze neuropsychiatrycznym. Patogeneza NPSLE (neuropsychiatric systemic lupus erythematosus) jest złożona, próbuje się ją wyjaśniać wytwarzaniem autoprzeciwciał oraz odkładaniem się kompleksów immunologicznych, czego następstwem jest cytotoksyczne uszkodzenie neuronów, jak również nieprawidłową ekspresją mediatorów zapalnych i napływem komórek zapalnych, zaburzeniami naczyniowymi i skłonnością do zakrzepów. Całkowita częstość występowania NPSLE jest szacowana na 56,3% pacjentów z SLE. Najczęściej występującymi zaburzeniami psychicznymi u chorych na toczeń rumieniowaty są zaburzenia nastroju, lękowe, poznawcze oraz świadomości. Istotnym aspektem są także polekowe zaburzenia psychiczne. W podejściu terapeutycznym wobec pacjentów z NPSLE o nieznacznej aktywności choroby i/lub nieznacznym stopniu zmian narządowych stosuje się glikokortykosteroidy w niskich dawkach oraz – w zależności od potrzeb – leki przeciwlękowe, przeciwdepresyjne, niekiedy kwas walproinowy, karbamazepinę, lamotryginę i preparaty przeciwpsychotyczne. W profilaktyce przeciwzakrzepowej u chorych z grupy ryzyka stosuje się kwas acetylosalicylowy.

These studies indicated quite high percentage of children with mental retardation and inborn defects. They are thought to be associated with less effective treatment in pregnancy, application of antiepileptic drugs of marked teratogenic properties (phenobarbital, phenytoin) and also with polytherapy as the principle of antiepileptic management. Then, the groups of pregnant women with epilepsy were not subjected to specialistic care of a neurologist and obstetrician. Current studies evaluating neuropsychological state of offspring of epileptic mothers point to the occurrence of psychomotor retardation, increased risk of manifestation of central nervous system significant and insignificant dysfunction, epilepsy as well as lower than on the average global intellectual ability and insignificant neurological dysfunction. Psychomotor retardation concerns mainly visual and auditory coordination, speech and social contacts. The latest prospective studies of epileptic mothers’ offspring indicate that epilepsy is significantly more frequently manifested among them (7% till the end of the first decade) than it was reported in earlier retrospective studies. In this period, generalized epilepsy (different syndromes) is more often revealed in offspring of mothers with early manifestation of epilepsy with attacks of unconsciousness. Polytherapy and monotherapy with walproine acid is a very important factor of the risk of decreased intelligence quotient and less beneficial results of learning. The importance of other risk factors including environmental has not been recognized sufficiently. Low socioeconomic level and the child’s male gender modify distinctly unbeneficially the effect of antiepileptic drugs administered in pregnancy. However, the so far carried out studies have not comprised sufficiently numerous groups of offspring of epileptic mothers and the methods of assessment applied by particular authors differ significantly. Furthermore, majority of mechanisms of the way different genetic and environmental factors affect the development of cognitive functions in offspring of epileptic patients are not known.

Od lat 80. ubiegłego stulecia prowadzone są badania nad stanem zdrowia potomstwa matek chorych na padaczkę. Badania te wskazywały na dość znaczny odsetek dzieci z upośledzeniem umysłowym i wadami wrodzonymi. Uważa się, że było to związane z mniej skutecznym leczeniem w czasie ciąży, stosowaniem leków przeciwpadaczkowych o większych właściwościach teratogennych (fenobarbital, fenytoina), a także politerapii jako zasady prowadzenia leczenia przeciwpadaczkowego. Kobiet ciężarnych z padaczką nie obejmowano wówczas specjalistyczną opieką neurologa i położnika. Współczesne badania oceniające stan neuropsychologiczny potomstwa matek chorych na padaczkę zwracają uwagę na opóźnienia w rozwoju psychoruchowym, zwiększone ryzyko ujawniania się dużych i małych wad OUN, padaczki, jak również nieco niższej niż przeciętnie globalnej sprawności intelektualnej oraz niewielkich dysfunkcji neurologicznych. Opóźnienia rozwoju psychoruchowego dotyczą głównie koordynacji wzrokowo-ruchowej, mowy i kontaktów społecznych. Ostatnie badania prospektywne dzieci matek chorych na padaczkę wskazują, że znamiennie częściej spotyka się wśród nich padaczkę (7% do końca pierwszej dekady) niż wynikało z dotychczasowych prac retrospektywnych. W tym okresie częściej u potomstwa matek z wczesnym ujawnieniem się padaczki z napadami nieświadomości pojawiają się objawy padaczki uogólnionej (różne zespoły). Politerapia i monoterapia kwasem walproinowym stanowi istotny czynnik ryzyka obniżonego ilorazu inteligencji i mniej korzystnych wyników uczenia się. Znaczenie innych czynników ryzyka, w tym czynników środowiskowych, nie jest wystarczająco poznane. Niski poziom socjoekonomiczny i męska płeć dziecka wyraźnie niekorzystnie modyfikują wpływ leków przeciwpadaczkowych stosowanych w ciąży. Dotychczasowe badania nie obejmują jednak odpowiednio licznych grup potomstwa matek chorych na padaczkę, a metody oceny stosowane przez poszczególnych autorów różnią się istotnie. Recurrent hypersomnia (Kleine-Levin syndrome, KLS) is a very rare disease characterized by recurrent episodes of excessive sleepiness with prolonged sleep time, cognitive and behavioral disturbances such as confusion, derealisation, apathy, compulsive eating and hypersexuality, usually lasting a few or several days, separated by asymptomatic periods. Its incidence is estimated at 1-5 cases per million and men are affected more often than women. Disease onset is usually in the second decade of life. Episodic excessive sleepiness is the main symptom of recurrent hypersomnia that occurs in all patients. During the episode sleep lasting more than 18 hours / day is observed. Cognitive impairment is the second most common symptom and feelings of unreality are common and, possibly, the most specific symptom of the syndrome. Symptoms such as compulsive consumption of high-calorie meals, sexual disinhibition, irritability and aggression may appear in a significant group of patients. There is no standard of care in the treatment of recurrent hypersomnia. Based on the literature data, lithium, valproic acid or carbamazepine preparations are used to reduce the frequency and severity of episodes.

Nawracająca hipersomnia (Zespół Kleine-Levin, KLS) jest bardzo rzadką chorobą charakteryzującą się występowaniem nawracających epizodów nadmiernej senności z nadmiernie długim czasem snu, zaburzeniami funkcji poznawczych i zachowania takimi jak: splątanie, derealizacja, apatia, kompulsywne jedzenie i hiperseksualność, trwających najczęściej kilka, kilkanaście dni, rozdzielonych okresami bezobjawowymi. Jej częstość występowania szacuje się na 1-5 przypadki na milion, a mężczyźni chorują częściej niż kobiety. Początek choroby przypada zazwyczaj na drugą dekadę życia. Pojawiająca się epizodycznie nadmierna senność jest głównym, występującym u wszystkich chorych objawem nawracającej hipersomnii. W trakcie epizodu obserwujemy sen trwający powyżej 18 godzin/dobę. Zaburzenia poznawcze są drugim najczęstszym objawem, a odczucia nierzeczywistości są powszechne i prawdopodobnie są najbardziej specyficznym objawem zespołu. U znacznej grupy chorych mogą pojawiać się objawy takie jak: kompulsywna konsumpcja wysoko kalorycznych posiłków, rozhamowanie seksualne, drażliwość i agresja. W leczeniu nawracającej hipersomnii nie ma standardu postępowania. Na podstawie danych z piśmiennictwa w celu zmniejszenia częstości i ciężkości epizodów stosuje się między innymi preparaty litu, kwasu walproinowego czy karbamazepiny.
