Introduction: The incidence of tick-borne diseases in Poland and Europe is increasing which is considered to be due to climate warming which promotes the growth of tick populations. Lyme borreliosis is the most prevalent leptospirosis in the US and Europe. Due to the potential for serious complications and the lack of specific prevention, diagnostic vigilance is essential so as to ensure early diagnosis and implementation of treatment. The main pathogen-transmitting vector is the tick species Ixodex ricinus. The suborder of ticks (Ixodida) belongs to the Acari subclass of arachnids. The natural reservoir of B. burgdorferi consists of small and medium-sized mammals and birds. Around ¾ of patients with Lyme disease report complaints and present disease symptoms within in the head and neck. The symptoms are diverse and may occur at all stages of the disease. These include headaches, neck pain and stiffness, sore throat, dizziness, cervical lymphadenopathy, otalgia, tinnitus, sudden hearing loss, and facial muscle paresis. Despite numerous papers being published on neurological manifestations of Lyme disease, the issues regarding the location of damage within the course of the auditory pathway and the balance system remain unanswered. In some cases hearing disorders may be due to central damage while others may be due to peripheral damage. Aim: The article provides an overview of reports on the clinical course and manifestation of Lyme borreliosis.
Objectives: The aim of the study was the epidemiological analysis and evaluation of selected clinical and sociodemographic factors in Polish patients with primary progressive multiple sclerosis. Methods: The study included patients from 7 provinces in central and eastern Poland registered in the Registry of Patients with Multiple Sclerosis on 31 December 2016. The incidence of various forms of the disease was compared, and clinical, demographic and social disparities between relapsing-remitting and primary progressive multiple sclerosis were analysed. Results: Of 3,199 registered patients, 2,188 persons (66.2%) had the relapsing-remitting form of multiple sclerosis, 774 (24.2%) had the secondary progressive type and 307 (9.6%) suffered from primary progressive disease. The first symptoms of primary progressive multiple sclerosis appeared almost 10 years later than in patients with the relapsing-remitting type (39.2 ± 11.4 vs. 29.8 ± 9.8). The period from the first symptoms to diagnosis was more than twice as long in patients with primary progressive multiple sclerosis (5.8 ± 3.4) as in those with relapsing-remitting disease (2.4 ± 1.6). The average degree of disability in the Expanded Disability Status Scale was similar and amounted to 3.2 ± 2.1 for relapsing-remitting and 3.6 ± 2.4 for primary progressive multiple sclerosis. The relapsing-remitting form was observed more often in women (2.4:1), and the primary progressive form appeared with equal frequency in both sexes (1:1). Disease-modifying treatment was received by 34% of patients with relapsing-remitting and in only 1.9% of patients with primary progressive multiple sclerosis. Conclusions: The primary progressive form affects approximately 10% of Polish patients with multiple sclerosis. The first symptoms appear at about 40 years of age with equal frequency in both sexes, and its diagnosis takes more than twice as much time as in the case of relapsing-remitting multiple sclerosis.

Cel: Celem badania była analiza epidemiologiczna oraz ocena wybranych czynników klinicznych i socjodemograficznych polskich pacjentów z pierwotnie postępującym stwardnieniem rozsianym. Metody: Do badania włączono pacjentów zamieszkujących 7 województw środkowej i wschodniej Polski, objętych Rejestrem Chorych ze Stwardnieniem Rozsianym w dniu 31 grudnia 2016 roku. Porównywano częstość występowania poszczególnych postaci choroby, a także analizowano różnice kliniczne, demograficzne i społeczne między postacią rzutowo-remisyjną i pierwotnie postępującą stwardnienia rozsianego. Wyniki: Spośród 3199 zarejestrowanych pacjentów postać rzutowo-remisyjną miało 2188 osób (66,2%), wtórnie postępującą – 774 (24,2%), a pierwotnie postępującą – 307 (9,6%). Pierwsze objawy postaci pierwotnie postępującej stwardnienia rozsianego pojawiały się prawie 10 lat później niż w przypadku postaci rzutowo-remisyjnej (39,2 ± 11,4 vs 29,8 ± 9,8). U chorych z postacią pierwotnie postępującą okres od pierwszych objawów do rozpoznania był ponad dwukrotnie dłuższy (5,8 ± 3,4) niż u tych z postacią rzutowo-remisyjną (2,4 ± 1,6). Średni stopień niesprawności w Rozszerzonej Skali Niewydolności Ruchowej (Expanded Disability Status Scale, EDSS) był zbliżony i wynosił 3,2 ± 2,1 dla postaci rzutowo- -remisyjnej i 3,6 ± 2,4 dla pierwotnie postępującej. Postać rzutowo-remisyjną częściej stwierdzano u kobiet (2,4:1), natomiast postać pierwotnie postępująca występowała z równą częstością u obu płci (1:1). Leczenie modyfikujące przebieg choroby otrzymywało 34% osób z postacią rzutowo-remisyjną i tylko 1,9% z pierwotnie postępującą. Wnioski: Postać pierwotnie postępująca występuje u około 10% polskich pacjentów ze stwardnieniem rozsianym, pierwsze objawy pojawiają się około 40. roku życia z jednakową częstością u obu płci, a diagnoza postaci pierwotnie postępującej zajmuje ponad dwukrotnie więcej czasu niż rozpoznanie postaci rzutowo-remisyjnej.
Chronic rhinosinusitis with nasal polyps (CRSwNP) is a disease with still not enough known pathogenesis despite the development of genetics, immunological and microbiological research. The number of patients with CRS has been constantly growing. The coexistence of CRS, bronchial asthma and aspirin intolerance (aspirin triad) is an adverse prognostic factor with higher risk of recurrences. The aim of study was to compare the severity of CRSwNP depending of coexistence of bronchial asthma and/or aspirin intolerance. The research was performed in the group of 204 patients operated 2009-2013 with 5 years follow-up. Higher nasal polyps growth in groups of patients with aspirin triad and CRSwNP and bronchial asthma in endoscopic examination (p=0,0005 and p=0,0030 respectively) and CT-scan according to Lund-Mackay point scale (p<0,0001 and p=0,0009) was showed. Also, these patients presented increased severity of nasal symptoms before surgical treatment according to VAS scale (p=0,0126 for CRSwNP with bronchial asthma; p=0,0390 for aspirin triad). Similarly, 6 months after surgery the same groups of patients presented higher severity of the disease symptoms (p<0,0001 for aspirin triad’ patients; p=0,0174 for CRSwNP and bronchial asthma’ patients) . Patients with aspirin triad had also statistically more surgeries in past (p=0,001), what proves that recurrences in this group are very likely to be observed in spite of the use of proper conservative treatment. No such differences have been shown in the group of patients with CRSwNP and isolated aspirin intolerance (without bronchial asthma). Allergy to inhaled allergens, hypersensitivity to aspirin are factors significantly worsening the course of CRSwNP. It would be advisable to consider, despite a lack of history of aspirin intolerance, a hypersensititvity to aspirin test in patients with particularly severe CRSwNP, especially those associated with bronchial asthma. It also seems reasonable to carry out such a test on every patient with newly diagnosed CRSwNP and bronchial asthma in order to be able to plan further treatment in this group of patients accordingly including biological treatment with antimonoclonal therapy against interleukin 4, 5 or13.

Aim of paper: Analysis of clinical course of tumour recurrence in patients undergoing surgical excision and radiotherapy for FIGO stage IB/IIA cervical cancer and retrospective evaluation of selected clinical-pathological tumour features in the group of patients, who developed a recurrence, with particular emphasis on recurrence-associated risk factors based on the GOG (Gynaecologic Oncology Group) scale. Material and method: Retrospective analysis encompassed 107 women treated at the OEwiêtokrzyskie Centrum Onkologii (Holycross Cancer Centre) since 2000 thru 2004. Results: Three-year symptom-free survival was obtained in 92 out of 107 women (85%). Recurrence occurred in 16 out of 107 patients (15%), thereof 9 out of 84 women (10%) originally had no lymph-node metastases and 7 out of 23 women (21%) had confirmed lymph node involvement. Recurrences were associated with lateral metastases (5 cases), distant metastases (7 cases) and lateral and distant metastases (4 cases). Recurrence-associated mortality rate was 81% (13 out of 16 patients). Currently, 3 women are alive, 2 thereof having signs of recurrence. Mean recurrence-free survival was 19.6 months and mean symptom-free survival was 8.5 months. Retrospective analysis based on the GOG scale predicted high risk of recurrence (over GOG score 120) in 14 out of 16 women (88%), who did recur and a moderate risk (GOG score 70-120) in the remaining 2 patients (12%). Conclusions: High risk of recurrence predicted by the GOG score has been confirmed in the group of women with cervical cancer FIGO stage IB/IIA undergoing combined treatment (surgery and radiotherapy) independent of invasion of regional lymph nodes. An essential risk factor for tumour recurrence is presence of tumour cells in pericervical and perivaginal tissues.

Cel pracy: Omówienie przebiegu klinicznego wznów po leczeniu chirurgicznym i uzupełniającym raka szyjki macicy w stopniu IB/IIA według FIGO oraz retrospektywna ocena wybranych cech kliniczno-patologicznych guza w grupie kobiet z nawrotem, ze szczególnym uwzględnieniem czynników ryzyka nawrotu według punktacji GOG (Gynaecologic Oncology Group Study). Materiał i metoda: Retrospektywnej analizie poddano 107 kobiet leczonych w Świętokrzyskim Centrum Onkologii (ŚCO) w latach 2000-2004. Wyniki: Trzyletnie przeżycie bez objawów nawrotu stwierdzono u 92/107 (85%) pacjentek. Wznowa pojawiła się u 16/107 (15%) chorych, w tym u 9/84 (10%) kobiet, u których nie stwierdzono przerzutów w węzłach regionalnych oraz u 7/23 (21%) kobiet z potwierdzonymi przerzutami do węzłów. Odnotowano 5 wznów bocznych, 7 odległych i 4 wznowy boczne z przerzutami odległymi. Zmarło z powodu wznowy 13/16 (81%) pacjentek. Żyją 3 kobiety, w tym dwie z objawami wznowy. Średni czas wolny od wznowy wynosił 19,6 miesiąca, a średni czas przeżycia od wykrycia wznowy - 8,5 miesiąca. U 14/16 (88%) chorych ze wznową stwierdzono retrospektywnie wysokie ryzyko wznowy (>120 punktów według GOG), a u 2/16 (12%) średnie ryzyko wznowy (70-120 punktów według GOG). Wnioski: Wysokie ryzyko wznowy według punktacji GOG potwierdziło się w grupie kobiet z nawrotem raka szyjki macicy w stopniu IB/IIA według FIGO po leczeniu chirurgicznym i skojarzonym, bez względu na zajęcie regionalnych węzłów chłonnych. Istotnym czynnikiem ryzyka wznowy jest obecność komórek nowotworowych w tkankach otaczających szyjkę macicy i pochwę.
Fibrodysplasia ossificans progressiva is uncommon genetic disease of connective tissue characterized by congenital defects already in newborns. Clinical symptoms of that disease develop usually in the beginning of human life. The disease was displaying forms of big pain swellings. At first, the focus was to treat it like a benign tumour. Later, those swellings started to change in calcinosis. We report of the case 30-year-old woman, who was born with congenital defects characterized by distort of the fingers and feet. That disease was started in 4-5 year of old. The process of ossification walked progressively. The sharp worsening of health occurred after injury (fall of the bike). Diagnoses of downloading stretch the muscle and biopsy of muscle was performed. The diagnosis of the disease was early in 6 years of old and the treatment with etidronate sodium was unsuccessful on natural course of the disease.

Myositis ossificans albo fibrodysplasia ossificans progressiva (FOP) to bardzo rzadkie genetycznie uwarunkowane schorzenie tkanki łącznej objawiające się wrodzonymi wadami paluchów stóp już u noworodków, a w dalszym życiu postępującym procesem kostnienia tkanek miękkich, stopniowo prowadzącym do unieruchomienia pacjenta. Kliniczne objawy choroby rozwijają się zwykle już w pierwszej dekadzie życia. Przybierają wówczas postać dużych i bolesnych obrzmień w obszarach tkanki łącznej. Początkowo ogniska te przypominają proces nowotworowy. Obrzmienia przechodzą później w formę kostnienia. W pracy opisano przypadek 30-letniej chorej, która obecnie jest całkowicie unieruchomiona, a urodziła się z wrodzoną koślawością i zniekształceniem palców stóp i rąk. Początek choroby miał miejsce w 4.-5. roku życia. Proces kostnienia tkanki łącznej postępował u chorej stopniowo, z gwałtownym nasileniem po urazie tułowia (upadek z roweru) i pobraniu wycinka mięśnia do badania histopatologicznego. Diagnozę choroby postawiono dość wcześnie (w 6. roku życia) i wdrożono leczenie etydronianem sodu (Didronel), jednak wydaje się, że bez wpływu na naturalny przebieg choroby.
