Poprawa bezpieczeństwa operacji zaćmy – system aktywnej kontroli IOP oraz dystrybucji płynowej

Since the introduction of pioneer phacoemulsification by Kelman, it is observed constant improvement of phaco systems with regard to their safety and efficiency. The present paper is to present contemporary advances in systems of phacoemulsification of cataract, showing in details new technological achievements of Centurion Vision System (Alcon Laboratories, Fort Worth, TX, USA). Compared to other previous systems main progress is as follows: improved cassettes provide much more stable fluidics during surgery, new system providing stable IOP in the anterior chamber and newly designed balanced tip that limits energy production in the wound. Preliminary own experience is encouraging, however, prospective studies are needed to establish and compare the influence of the new system on some outcome measures (e.g. BCVA, corneal thickness, endothelial density, tyndalometry).

Od czasu wprowadzenia pionierskiej fakoemulsyfikacji przez Kelmana stale doskonali się urządzenia pod względem bezpieczeństwa i skuteczności. Niniejsza praca przedstawia aktualne postępy w udoskonalaniu systemu do fakoemulsyfikacji zaćmy na przykładzie urządzenia Centurion Vision System (Alcon Laboratories, Fort Worth, TX, USA). Główny postęp w stosunku do innych urządzeń polega na wprowadzeniu: unowocześnionych kaset, które eliminują wahania ciśnienia w gałce ocznej, systemu zapewniającego stałe, uprzednio zadane ciśnienie wewnątrzgałkowe w czasie operacji oraz nowo zaprojektowanego tipa zmniejszającego generowanie energii w ranie operacyjnej. Wstępne doświadczenia własne są bardzo zachęcające, jednak potrzebne są badania prospektywne określające oraz porównujące wpływ aparatu na określone parametry końcowe (np. ostrość wzroku, grubość rogówki, gęstość śródbłonka, tyndalometria).
