Choroby nowotworowe stanowią jeden z najpoważniejszych problemów współczesnej medycyny. W Polsce częstość zgonów z powodu nowotworów złośliwych jest druga w kolejności, ustępując miejsca jedynie chorobom układu sercowo-naczyniowego. Liczba zachorowań na nowotwory złośliwe w Polsce na przestrzeni ostatnich lat wzrasta w tempie szybszym niż liczba ludności (Narodowy Program...2016-2024). Wyniki analizy przeprowadzonej w 2010 r. w ramach Światowego Badania Obciążenia Chorobami wskazują, że spośród wszystkich czynników wywierających negatywny wpływ na stan zdrowia i dobrostan ludności w Europejskim Regionie WHO, najważniejsze są właśnie czynniki żywieniowe. Stwierdzono, że niewłaściwy stan odżywienia i wynikające z tego choroby są źródłem wysokich kosztów dla poszczególnych osób, rodzin, społeczności i budżetów państw (Europejski Plan... 2014).

Cancer is one of the most serious problems of modern medicine. In Poland, deaths from malignant tumors are the second most common ones, second only to diseases of the cardiovascular system. The number of cancer cases in Poland in recent years is growing at a faster rate than that of population. Results of an analysis conducted in 2010 within the project World Study on Disease Burden indicate that of all the factors that have a negative impact on the health and welfare of the population in the WHO European Region the most important are nutritional factors. In addition, it was found that poor nutritional status and diseases resulting from unhealthy diet are the source of high costs for individuals, families, communities and state budgets.
According to the laws of thermodynamics, a cell, threatened by the end of its metabolism, can extend its life in a neoplastic form by reducing the production of its entropy while at the same time increasing dissipation of matter and energy in its surroundings. The basis for the progression of all irreversible processes including life and of course carcinogenesis are changes (d) in the value of entropy production (diS) in time (dt), called by the names of the sources of generated entropy: di S dt Sgen, for which Sgen 0. For that reason prophylactic and oncological treatment must always take the ageing of the body and the primary illnesses this brings into consideration. The source of entropy production is life itself, meaning a constant spontaneous exchange of matter, information, and the energy of elementary particles. The entropy of living organisms is produced by coupled molecular processes increasing (positive source) and decreasing (negative source) its magnitude. Is such a transformation of cellular forms of life only the initial states of cells and the neoplasms derived from them, while an intermediate state of dysplasia is characterized by an increased fluctuation of opposing metabolic processes. Human health is determined by the material-energy condition of the germ cells which are responsible for the intergenerational passageof life, thus contributing to the creation of individuals with new identities. The processes are programmed as early as at the time of the conception of an individual. The increasingly widespread use of drugs as well as the rising use of contraceptive pills by women, combined with the ever more numerous Cesarean sections, reduced lactation, or delayed first pregnancy help explain the increasingly frequent incidence of cancer diseases.

Zgodnie z prawami termodynamiki zagrożona kresem swego metabolizmu komórka może przedłużyć swoje życie w formie nowotworowej poprzez zmniejszenie produkcji własnej entropii z równoczesnym zwiększeniem rozpraszania (dysypacji) materii i energii w otoczeniu. Podstawę przebiegu wszystkich procesów nieodwracalnych, w tym życia i oczywiście karcinogenezy, stanowią zmiany (d) wartości produkcji entropii (diS) w czasie (dt), noszące nazwę źródeł entropii generowanej: di S dt Sgen, dla których Sgen 0. Dlatego profilaktyka i leczenie onkologiczne musi uwzględniać starzenie się organizmu i zawsze towarzyszące temu wcześniejsze schorzenia. Źródłem produkcji entropii jest życie, czyli ciągła, spontaniczna i wzajemna zamiana materii, informacji i energii cząstek elementarnych. Entropię żywych organizmów produkują sprzężone wzajemnie procesy molekularne zwiększające (dodatnie źródła) i zmniejszające (ujemne źródła) jej wielkości. W takiej przemianie komórkowych form życia stabilne są tylko stany wyjściowe komórek i powstałe z nich nowotwory, a pośredni stan dysplazji charakteryzuje się zwiększoną fluktuacją przeciwstawnych procesów metabolicznych. Ludzkie zdrowie jest zdeterminowane przez materialno-energetyczny stan komórek rozrodczych, które są odpowiedzialne za międzypokoleniowy przekaz życia. Procesy te są już zaprogramowane w momencie poczęcia. Wzrastające zużycie leków, a także rosnąca liczba kobiet stosujących pigułki antykoncepcyjne w połączeniu z coraz większą liczbą cięć cesarskich i skróconych laktacji oraz z opóźnieniem pierwszej ciąży pomagają wyjaśnić wzrost częstości występowania chorób nowotworowych.
Cancer, a popular name of neoplasm and/or neoplastic diseases, is inseparably bound with equally popular definitions of life, health and death, the former and the latter being natural phenomena. The essence of life continuous, spontaneous and mutual exchange of matter and energy, whereby a significant role is played by information about their state as unity of apparent contrasts. The key issue is that life does not arise de novo, but is passed on. Neoplasm is a final cellular form of life of multicellular organisms, able to prolong life processes by dissipative self-organization of cells threatened by death. Its exceptionality consists in the fact that self-organization of tumors can prolong life in daughter cells only, and only as long as affected host lives, becoming increasingly sick due to increasing tumor-dependent dissipation of matter and energy. Clearly increased dissipation of matter and energy by tumor cell in a hitherto normal tissue is the first and non-specific, but still a universally recognized information about local ineffectiveness of host organism. A sufficiently efficient host organism as a whole is able to use its defense mechanisms not only to destroy such a cell featuring an alien neoplastic identity, but also to eliminate dissipative states in cells threatened by death in own tissue.This process, occurring in multicellular organisms, apart of proliferation and apoptosis, determines the size of cellular populations in tissues and organs. From this point of view, carcinogenesis is also a potent quantitative and qualitative regulator of tissues.

Rak jako częste potoczne określenie nowotworów i/lub chorób nowotworowych nierozerwalnie łączy się z równie powszechnymi definicjami życia, zdrowia i śmierci, jako także naturalnych zdarzeń. Istotą życia jest stała spontaniczna, wzajemna zamiana materii i energii z udziałem informacji o ich stanie jako jedności pozornych przeciwieństw. Problem polega na tym, że życie nie powstaje de novo, ale jest tylko przekazywane. Nowotwór stanowi końcową komórkową formę życia wielokomórkowych organizmów, przedłużającą trwanie procesów życiowych przez dysypatywną samoorganizację zagrożonych śmiercią komórek. Jego wyjątkowość polega na tym, że samoorganizacja nowotworów może przedłużać tylko spontaniczne życie w komórkach potomnych, i to dopóki żyje człowiek, chory z powodu coraz większego nowotworowego rozpraszania w nim materii i energii, zwanego dysypacją. Wyraźnie zwiększona dysypacja materii i energii przez komórkę nowotworową w prawidłowej dotychczas tkance jest pierwszą i niespecyficzną, ale za to uniwersalnie rozpoznawalną informacją o lokalnej niewydolności organizmu. Dostatecznie sprawny ustrój jako całość może środkami obronnymi nie tylko zniszczyć taką komórkę o odmiennej od niego biologicznej nowotworowej tożsamości, ale także zlikwidować stany dysypatogenne w zagrożonych śmiercią komórkach własnych tkanek. Tak dzieje się w wielokomórkowych organizmach, co obok proliferacji i apoptozy decyduje o wielkości populacji komórek w tkankach i narządach. W tym ujęciu nowotworzenie okazuje się też istotnym regulatorem wielkości i jakości tkanek.
Cancer is currently among the most prevalent diseases worldwide. Not only does it affect the somatic health of the patients, but also their emotional, mental, and social wellbeing. Impaired functioning on those levels, in turn, has adverse effect on the effects of treatment of the underlying disease as such. Psychiatric disorders of varying severity are quite common in cancer patients, potentially manifesting at any given stage of disease advancement. They may occur in response to the patient’s changed social situation or to the stress it entails, but may also be triggered by physiological or pathological factors influencing the central nervous system in the course of the disease. The list of the most common mental conditions encountered in cancer patients includes adjustment disorders, depressive syndromes, anxiety, disorders of consciousness, psychoorganic syndrome, psychotic disorders, sleep disturbances, chronic pain and stress, chronic fatigue syndrome, and chemobrain. Chemobrain is a peculiar phenomenon, comprising potential disorders of various kinds impeding short-term memory, psychomotor speed, response time, visual-spatial skills and speech fluency, the ability to perform complex tasks, concentration as well as visual and verbal memory. The disorders most frequently afflict patients who undergo systemic therapy, they are subtle and temporary. Cancer is a disease whose occurrence and course are influenced by the functioning of the immune, endocrine, and central nervous system, with the science that explores the interaction between them known as psychoneuroimmunology. Research has demonstrated the systems to communicate through messengers such as neurotransmitters, hormones, growth factors and cytokines. As cancer has effect on every sphere of the patient’s life, so many patients suffer from psychiatric comorbidities. A holistic approach and joint effort of specialists in various fields of medicine, such as oncology, psychiatry or psychology, are hence crucial for cancer patients.

Choroby nowotworowe należą obecnie do najczęściej występujących schorzeń. Dotyczą nie tylko somatycznej, lecz także emocjonalnej, psychicznej i społecznej sfery funkcjonowania pacjenta. Pogorszenie funkcjonowania we wszystkich tych obszarach niekorzystnie wpływa na proces leczenia choroby podstawowej. W grupie pacjentów onkologicznych dość powszechne są zaburzenia psychiczne, które mogą się pojawić na każdym etapie choroby i mieć różne nasilenie. Bywają wynikiem zmiany sytuacji społecznej z powodu choroby nowotworowej, reakcją na stres z nią związany, jak również skutkiem oddziaływania czynników fizjologicznych lub patologicznych na ośrodkowy układ nerwowy. Do najczęstszych zaburzeń psychicznych w przebiegu choroby nowotworowej należą: zespoły dezadaptacyjne, zespoły depresyjne, zespoły lękowe, zaburzenia świadomości, zaburzenia psychoorganiczne, zaburzenia psychotyczne, zaburzenia snu, przewlekły ból i stres, zespół przewlekłego zmęczenia, chemobrain (zaburzenia poznawcze po chemioterapii). Chemobrain jest bardzo ciekawym zjawiskiem, na które składają się zaburzenia pamięci operacyjnej, szybkości psychomotorycznej, czasu reakcji, umiejętności wzrokowo-przestrzennych i płynności mowy, problemy z wykonywaniem czynności złożonych, koncentracją oraz pamięcią wzrokową i werbalną. Zaburzenia te najczęściej występują u osób poddawanych leczeniu systemowemu oraz mają charakter łagodny i przemijający. Na przebieg choroby nowotworowej i jej pojawienie się wpływa działanie układu immunologicznego, układu endokrynologicznego, a także ośrodkowego układu nerwowego. Nauką, która zajmuje się wspólnym oddziaływaniem tych układów, jest psychoneuroimmunologia. Badania pokazują, że podczas stresu wymienione układy komunikują się poprzez działanie neurotransmiterów, hormonów, czynników wzrostu czy też cytokin. Choroba nowotworowa rzutuje na każdą sferę życia, stąd wielu pacjentów cierpi na zaburzenia psychiczne. Dlatego tak ważne są holistyczne podejście do chorego i współpraca różnych specjalistów, takich jak onkolog, psychiatra i psycholog.
