INTRODUCTION: The leading place in the nosological structure of pathology in the lower respiratory tract is pneumonia. Community-acquired pneumonia (CAP) remains one of the most common causes of childhood morbidity and accounts for about 16% of all deaths in children under 5 years old. The article presents a review of publications of recent years using the databases PubMed, Scopus, Web of Science, Google Scholar regarding the current possibilities of intensive care of complicated community-acquired pneumonia (CCAP), problems of using extracorporeal detoxication methods (ECDM), the choice of starting antibacterial therapy and microbiological monitoring and the problem of determining the severity of endogenous intoxication syndrome in children with CCAP. PURPOSE: To analyze the relevance of intensive therapy in children with complicated community-acquired pneumonia, to consider current therapeutic interventions and the need to use extracorporeal detoxication methods in complex treatment based on the analysis of scientific literature data. CONCLUSIONS: The epidemiology of community-acquired pneumonia is currently characterized by an increasing trend in morbidity and mortality worldwide. Plasmapheresis is not first-line therapy, because antibacterial drug (ABD), conservative detoxication, respiratory support are the mainstay of treatment like for other infections, but in case of CCAP the course of basic therapy is often not enough and then there is a need for ECDM. Identification of causative microorganisms in children with CCAP remains a constant problem. In most cases, therapy is prescribed empirically and knowledge of local epidemiology, the structure of ABD resistance is an important platform for improving antibacterial therapy. The optimal therapeutic approach for CCAP remains open to research.

WSTĘP: Wiodącym miejscem w strukturze nosologicznej patologii dolnych dróg oddechowych jest zapalenie płuc. Pozaszpitalne zapalenie płuc (CAP) pozostaje jedną z najczęstszych przyczyn zachorowań i zachorowań wśród dzieci na około 16% wszystkich zgonów dzieci poniżej 5 lat. Artykuł przedstawia przegląd publikacji z ostatnich lat z korzystaniem baz danych: PubMed, Scopus, Web of Science, Google Scholar w zakresie obecnych możliwości intensywnej opieki nad skomplikowanym pozaszpitalnym zapaleniem płuc (CCAP), problemów ze stosowaniem metod detoksykacji pozaustrojowej (ECDM), rozpoczęcia terapii przeciwbakteryjnej i monitorowania mikrobiologicznego oraz problemu ustalenia nasilenia zespołu zatrucia endogennego u dzieci z CCAP. CEL: Analiza znaczenia intensywnej terapii u dzieci ze skomplikowanym pozaszpitalnym zapaleniem płuc, ocena interwencji terapeutycznych i potrzeb stosowania złożonych metod detoksykacji pozaustrojowej w leczeniu opartym na analizie danych z literatury naukowej. WNIOSKI: Epidemiologia pozaszpitalnego zapalenia płuc charakteryzuje się obecnie rosnącą tendencją w chorobowości i śmiertelności na całym świecie. Plazmafereza nie jest leczeniem pierwszego rzutu, ponieważ lek przeciwbakteryjny (ABD), konserwatywna detoksykacja i wspomaganie oddychania są podstawą leczenia, podobnie jak w przypadku innych infekcji, ale w przypadku CCAP przebieg podstawowej terapii często nie wystarcza, a następnie istnieje potrzeba ECDM. Identyfikacja mikroorganizmów u dzieci z CCAP pozostaje stałym problemem. W większości przypadków terapia jest przepisywana empirycznie na podstawie znajomości lokalnej epidemiologii, a struktura oporności na ABD jest ważną składową poprawy działania terapii przeciwbakteryjnej. Optymalne podejście terapeutyczne do CCAP wymaga dalszych badań.

Introduction. Results presented in this article are part of an international project called Project of physical activity stimulation in a family (PPASF) held jointly with the Klaipėda University and LASE in Riga. We decided to check whether participation of children in preschool age in active classes in nursery school may be an impetus to change behavior in terms of physical activity of a family. For this purpose, a model was developed that included physical activity program conducted in the nursery school, with homework for the children to do together with their parents at home. The aim of the study was to examine the extent to which a child can persuade the parents to a common physical activity and whether sport and recreation for children with homework are a successful proposal to increase motor activity of the family. Material and methods. A diagnostic survey method was applied in the research, using a questionnaire technique. The survey questionnaire covered 38 families including 30 mothers and 8 fathers. Results and conclusions. Research has shown that children can become facilitators of physical activity in the family. The proposed model classes for children proved to be an accurate proposal, which is worth to implement the practice of teaching.

Health care of children and young people is part of a system of care of the so called “developmental age population.” In this system, prevention plays a key role. The aim of the study was, basing on the obtained results, to develop guidelines for the regional pro-family policy in care of a rural child. The study included children and young people of school age, from 14 to 19 years of age. Overall the analysis included 6,971 children and adolescents and 6,971 parents. Organization of health care of children and school adolescents depended on the solutions approved by individual founding bodies of Health Care Units and Regional Patients’ Funds. The withdrawal of nurses from school was observed. The tasks of prevention character performed previously by nurses employed at schools began to be implemented within the framework of primary health care in the child’s place of residence. For proper implementation of the tasks of school nurses and the proper development of school hygiene, it is important to establish a uniform and maximum number of pupils per one nurse, taking into account all circumstances, and to develop standards of care for the student, taking into account the age of the student, school type, and the region.

Introduction. Family-oriented strategies of promoting physical activity are underused. In the last few years there has been a significant decrease in number of research on family as a promoter of physical activity. The research, when conducted, focuses on the influence parents' activity has on that of their children. There is a need for assessment of child's influence on parents' physical activity. Material and methods. The survey utilises an opinion poll in the form of a questionnaire. The sample consisted of 100 parents who were invited together with their children to a recreational fair. Results. The survey shows that child may be the person encouraging their parents to active recreation. Symptoms of this are: encouraging the parents to take part, satisfaction with the decision to take part in the event and the amount of positive sensations during play. The parents were of opinion that their both, children's as well as teacher's encouragement, was the most important factor in their decision to participate. Conclusions. The research indicated that after the fair children encouraged their parents to play together, whereas parents used games from the fair in their family free time. Because of the fact that the offered model of recreational event for families engaged and interested the participants, it is worth promoting in pedagogical sphere.

Unfavorable living conditions of the population, particularly in rural areas, and the relatively low level of health culture, expressed by bad nutrition, insufficient physical activity, alcohol abuse, and smoking, are not conducive to maintaining the health of the population. The health status of the population, including children and young people, is assessed as highly unsatisfactory. The aim of this study was to evaluate the research on health behavior, serving as a basis for formulation of health education programs in rural areas . To develop the action strategy for health education, addressed to a group of children and young people of school age in rural areas, it is important to know the perception of health in this environment. . Literature reports seem to indicate that this sphere of life and education is a bit neglected, probably highly diverse and determined by the influence of various factors, both environmental and individual. Therefore, identification of hazards, understanding their life situation, diagnosing the situation, especially in a group of school children, is particularly desirable and expected. The development of health promotion and prevention programs among adolescents must be based on a fully reliable diagnosis of social situation, documented and monitored. Circulating information from newspaper reports and the school environmentsuggests the need forparticular actions in the field of school health education, conducted by prepared, authoritative staff of educators. Such are the expectations and needs of young people and those who consider the health issues of children significant. The problem of great importance is the issue of research on the health behavior of young people, including rural areas, and the development of compatible research tools. The lack of such tools makes the comparison of the results obtained by different authors difficult. The acquired theoretical knowledge and implementation of various health programs have often failed to produce practical results. What is needed are the actual steps to promote health in schools and homes, and the skills to use the existing knowledge to make the analysis and search for the determinants of health behavior of young people. The skillful linking of thinking, action and knowledge of the determinants of health behavior, will prevent from one-sided trends in education, and will bring more focus on the skills and versatility in the harmonious development of young people.

For quite some time now an increasing number of voices can be heard in the public space regarding the decline of child and adolescent psychiatry: shortage of specialists, shortage of hospital wards and permanently insufficient financial support of this branch of medicine. However, in all the media hum about this problem and in the debates about how to solve it. are we not losing sight of its essence?

Introduction: Computed tomography is an important imaging technique in Emergency Units. Thanks to its popularity, radiological changes are found in healthy children more commonly. The aim of this paper is to evaluate the incidence of maxillary sinus radiological changes in children with head trauma who admitted to the Emergency Unit of the University Children Hospital in Lublin. Material and Methods: A retrospective analysis of computed tomography scans of children suffering from head trauma admitted to the Emergency Unit of the University Children Hospital in Lublin was carried out. A group of 425 patients was analyzed. Results: Maxillary findings were present in 81 cases (19.06%); in 38 patients (8.94%) the changes were unilateral, while in 43 (10.12%) they were bilateral. Maxillary mucosal thickening was the most common radiological abnormality, present in almost 12% of the investigated cases (approximately 62% of all revealed changes). Maxillary total opacification as an isolated finding was found in younger children only. Retention cysts and maxillary polyps were found with a similar low frequency as maxillary opacification but in elder children only. Discussion: Asymptomatic radiological changes in computed tomography scans are common. Maxillary mucosal thickening is the most frequent asymptomatic abnormality. Maxillary polyps and pseudocysts are rare in the paediatric population. Maxillary opacification suggests other more significant pathologies and requires further diagnostics. Physicians should avoid diagnosing patients with sinusitis without proper examination and based on radiological abnormalities only. Paediatric patients with revealed maxillary changes should remain under regular laryngological control.

Objective. A tropism to epithelial cells and lymphocytes, an inhibition of apoptosis in host cells, an ability to occurrence in persistent form resistant to antibiotic treatment are the features of Chlamydia pneumoniae, which can have connection with chronic inflammation of an adenoid tissue and adenoid hypertrophy. This study aimed to (1) detect the C. pneumoniae in an adenoid in children undergoing adenoidectomy, (2) estimate a connection between C. pneumoniae occurrence and the size of adenoid, (3) demonstration in which of adenoid cells C. pneumoniae occurs most often. Material and methods. The examined group consisted of 200 children aged from 2 to 16 years (mean age 6,4) undergoing adenoidectomy. In all children during qualification for adenoidectomy a fiberoscopic examination of the nasopharynx was performed. A part of removed adenoid tissue was analysed by real-time PCR for C. pneumoniae. Adenoids from children with positive PCR examination and from 10 children with negative PCR examination were examined using immunohistochemistry (IHC). Results. C. pneumoniae in the adenoid was present in 5,5% children. Positive results were obtained most frequently (24,14%, 7/29) in the eldest group (10-16 years). A statistical analysis demonstrated the correlation between C. pneumoniae occurrence in an adenoid tissue and the size of adenoid. In immunohistochemistry C. pneumoniae was found the most frequently in lymphocytes and in epithelial cells. Conclusions. A presence of C. pneumoniae in lymphocytes and epithelial cells of the adenoid first of all in older children with adenoid hypertrophy confirms the participation of this bacteria in adenoid pathology.

Introduction The foot plays an important role as a static-dynamic foundation of the skeleton. The process of the most intensive formation of the foot during the posturogenesis falls on the preschool age and early school age. The aim of the study was to characterize the arch of the feet in children at the age of 5 and to look for the dependence of their arching condition on the Cole’s index level. Material and methods Forty-four preschool children took part in the research. The Cole’s index showed an underweight, normal body weight, overweight and obesity. The plantography method was used to assess the arching of the feet Results Over half of the examined children had normal body weight. Underweight was more common in the boys group, while overweight and obesity in girls. Ninety percent of children had abnormal longitudinal and transverse arches in both feet. The varus of the big toe and the valgus of the small toe was observed in both groups. The position of the heel was set correctly in majority of the participants. Conclusions No relationship was found between the Cole’s index and longitudinal arches of the right and left foot in boys, as well as in girls. The results revealed that excessive body weight in girls was correlated with flattening of the transverse arch and incorrect position of the heel in the right foot. No significant correlation was found between the Cole’s index and the assessed parameters of the foot in boys.

The content of this paper is a report on activities in children’s facilities and their ability to influence parenting skills in terms of institutional childcare at an early age. Children’s facilities (infant homes, children’s homes and children’s centres) provide comprehensive care for children and parents in cases where, for various reasons, a child’s all-round development is disrupted or their life is in danger. The main purpose of these facilities is to provide adequate childcare as well as to support families when restoring basic functions. On the basis of a survey conducted in children’s facilities, the most frequent difficulties in exercising parenting skills are identified, and subsequently, information on the extent to which children’s facilities contribute to the development of parenting skills and help in the rehabilitation of a family is outlined.

Niniejsza praca stanowi raport z działań w placówkach dla dzieci oraz ich umiejętności wywierania wpływu na kompetencje rodzicielskie w zakresie opieki instytucjonalnej nad młodszymi dziećmi. Placówki dla dzieci (placówki dla niemowląt, dzieci i ośrodki dla dzieci) zapewniają wszechstronną opiekę dla dzieci i rodziców w przypadkach, w których z różnych względów kompleksowy rozwój dziecka jest zakłócony bądź jego życiu zagraża niebezpieczeństwo. Głównym celem takich placówek jest zapewnienie odpowiedniej opieki nad dziećmi, a także wsparcie rodzin w procesie odbudowywania ich podstawowych funkcji. Na podstawie ankiety przeprowadzonej w placówkach opieki nad dziećmi określono najczęstsze trudności związane z kompetencjami rodzicielskimi, jak również informacje dotyczące zakresu, w jakim placówki dla dzieci wpływają na rozwój kompetencji rodzicielskich i pomagają w przywróceniu rodziny do normalnego stanu.

Dihydropyrimidine dehydrogenase (DPD) deficiency is a rare defect of the first step of the pyrimidine catabolic pathway. Patients with a complete enzyme deficiency may be clinically asymptomatic or suffer from neurological abnormalities of various severity. We report a case of an 8-year-old girl with psychomotor retardation and mild course of the disease. Analysis of urine showed strongly elevated levels of uracil and thymine, and no DPD activity could be detected in peripheral blood mononuclear cells. Sequence analysis of the DPD gene (DPYD) revealed that our patient was homozygous for the common splice-site mutation IVS14+1G > A, which suggest that the carrier status for this mutation may be not rare in the Polish population.

Technological progress forces us to present after several years the updated standards in ultrasound examination of newborns, infants and older children. It should be emphasized that the examination of the youngest patients requires one to use high-class equipment. Lack of cooperation on the part of the child and imaging small structures constitute a huge challenge for the examiner. The work presents equipment requirements, the technology of examining the abdominal cavity in children and the manner of preparing the examination result. Moreover, modern expectations as regards ultrasound surgery equipment, where children are subject to examination have been presented. The minimum set of transducers the ultrasound apparatus should be equipped with has also been pointed out as well as the minimum requirements concerning image recording. Extending the scope of transducers over “minimum” significantly raises the imaging possibilities in pediatrics. Standard preparation of children for ultrasound has been discussed, taking into consideration the age of the patient and indications. It is essential to understand that it differs from standard preparation for examination in the case of adults. The technique of examining the abdominal cavity has been described, which enables the repeatability of examinations and ensuring making all the available structures visible. It proves imperative especially in the case of a large number of examinations. Attention should be paid particularly to the urinary system imaging – owing to the greatest incidence of congenital diseases in that system.

The foot is an essential part of the human body, and a key factor enabling mobility. The proper functioning of the foot depends on its morphological structure and on the proper shape of its arches – transversal and longitudinal. Having in mind that flat feet develop as a succession of different stages, it is important to systematically monitor the development of feet at various phases of a child’s growth. Therefore, this study aims to evaluate longitudinal foot arch curvature in children aged 7 and 8. The study involved 122 children (77 girls and 45 boys) aged 7 and 8 – students of a primary school in Bezrzecze near the city of Szczecin. Body height and weight, and the length and width of the feet were measured. A podoscope was used to determine the curvature of the longitudinal foot arch, which was then expressed on a scale compatible with the Szriter-Gudanow “KY” foot arch curvature index. This non-invasive method enables preventive measures to be taken at critical stages of a child’s life. The results for the tested groups of children revealed statistically significant relationships for body height and weight, and the length of the feet between the 7 and 8 year olds. Furthermore, it was observed that the vast majority of the tested children has normal curvature of the longitudinal arch of both the right and left foot.

Purpose: The normal limits of spleen size must be known in order for pathological changes to be noticed. The aim of this retrospective study is to determine the normal limits of spleen size in healthy children and to reveal their relation to sex, age, body height and weight. Patients and methods: Three hundred and ten children (150 girls and 160 boys) between 0–16 years of age in Eastern Anatolia Region who had normal spleen ultrasound appearances were included in this study. The greatest longitudinal distance of the spleen from the dome to the tip (splenic length) measured at the hilum in the coronal plane was obtained by ultrasonography. Results: There was no significant difference between the sexes in children. Also, no statistically significant differences were found between the two sexes in any age group for splenic length (t-test, p > 0.05). Therefore, all data were rearranged without being separated according to sex. The correlation analysis has shown a positive and significant correlation between splenic length and age, body height, and weight, with high correlation coefficients (r > 0.80). Splenic length showed the strongest correlation with body height. Conclusion: In children clinically suspected of organomegaly, splenic length can be used to assess organ size. The presented data can be applied to routine ultrasonography examinations.

Cel: Znajomość zakresu prawidłowych wymiarów śledziony jest konieczna by móc rozpoznać zmiany patologiczne. Celem niniejszego retrospektywnego badania było ustalenie prawidłowego przedziału wymiarów śledziony u dzieci zdrowych oraz poznanie ich zależności z płcią, wiekiem, wzrostem i wagą. Materiał i metody: Badaniem objęto trzysta dziesięcioro dzieci w wieku 0-16 lat ze Wschodniej Anatolii, u których badanie ultrasonograficzne wykazało prawidłowy obraz śledziony. W badaniu ultrasonograficznym rejestrowano maksymalną odległość pomiędzy biegunem dolnym a górnym śledziony, mierzoną we wnęce w płaszczyźnie czołowej Wyniki: Nie stwierdzono istotnych różnic w wielkości śledziony w zależności od płci. Nie stwierdzono również statystycznie istotnych różnic w długości śledziony pomiędzy płciami w którejkolwiek zbadanych grup wiekowych (test t-Studenta, p > 0,05). Z tego względu wszystkie dane zostały ostatecznie przedstawione bez podziału ze względu na płeć. Analiza korelacji wykazała dodatnią i istotną statystycznie zależność pomiędzy długością śledziony a wiekiem, wzrostem oraz wagą, przy wysokich współczynnikach korelacji (r > 0,80). Długość śledziony wykazała najsilniejszą korelację ze wzrostem/długością ciała. Wnioski: Długość śledziony jest parametrem pomocnym w ocenie wielkości śledziony u pacjentów z podejrzeniem organomegalii. Dane przedstawione w niniejszym artykule mogą być wykorzystywane jako wartości referencyjne w rutynowych badaniach ultrasonograficznych. The Dennison’s method is aimed at stimulating different mental functions through natural movements of the whole body. This method is mostly used in dyslexic disorders therapy among children through restoring and activating natural developmental mechanisms leading to improvement of integration of both hemispheres, which directly affects the child’s learning processes.

Aim: The aim of the study is to assess the satisfaction of parents with nursing care at a pediatric surgery department.Material and method: The research was conducted among parents / caregivers of children discharged from the ward in the period from February to April 2019. The study involved 50 respondents. The test method was a diagnostic poll method in the form of a questionnaire. The research tool was a questionnaire of own authorship.Results: Factors determining satisfaction with child care on a surgical ward are: the age of the child, the mode of the child’s admission to the ward, accessibility, and the nurse’s support. Thirty-one parents were in favour of the multi-purpose nature of nursing care on the ward. The others felt that nursing care was task-oriented. Parents of the youngest children (mean age 4.9 years) expected the nurse to be kind or professional (mean age 5.5 years). Parents of early childhood children (mean age 7.2 years) were more likely to expect understanding, and parents of the oldest children (mean age 9.0 years) expected communicativeness. It was shown that throughout the entire period of hospitalization, caregivers of children under 3 years of age (n = 13; 100.0%) or aged 3-6 years (n = 15; 78.9%) stayed with their children more often. There was a correlation between the mode of admission and the emotions that parents felt during the admission of their child to hospital, as shown in Tab. 5. The level of significance (p < 0.001) in the hypothesis tested, was lower than the typical level of significance of 0.05. In the case of emergency admission (n = 13; 54%) and planned admission for surgery (n = 17; 74%), emotions related to helplessness and anxiety predominated.Conclusion: Parents rated the quality of nursing care on the ward well. Parents’ expectations of the nurse are not dependent on the age of the child. The mode of admission of the child to the ward did not affect the expectations towards the nurse. The age of the child affected the time the caregiver stayed with the child on the ward. The mode of admission of the child affected the emotional state of the parents of the caregivers.

The aim of the paper was to present the basic methodological approaches to studying the quality of life of patients with regard to interdisciplinary character of the concept, its interrelation with standard of living, specificity in children and teenagers. Studying the quality of life of children and teenagers has specific character. According to modern recommendations, it is the basis of complex estimation of state of health of the child. Two basic methodological approaches are used: the first consists in selection and analysis of factors, significant for health of teenagers from adult people point of view; the second provides research of factors, significant from the point of view of teenagers. Techniques unite two types of criteria: 1. the objective: physical activity, social functioning; 2. the subjective: factors of emotional status, self-estimation of state of health, satisfaction by factors related with health, – that is perception by teenagers how their requirements for health are satisfied and how much possibilities necessary for it are given.
