Perirectal fistulas in the course of Crohn’s disease (CD) constitute an important problem in this group of patients. They are observed in a vast majority of patients with involvement through colorectal inflammation. Perirectal fistulas in CD present a great diagnostic and therapeutic challenge due to the intensified clinical symptoms and worse prognosis than in the case of crypt originating fistulas. The condition for implementation of effective treatment of perirectal fistulas in the course of CD is the correct diagnosis, defining the anatomy of fistulas, presence of potential stenoses and inflammation in the gastrointestinal tract. Treatment of these fistulas is difficult and requires close cooperation between the colorectal surgeon and the gastroenterologist. The combination of surgical and pharmacological treatment has higher efficacy compared to surgical treatment or pharmacotherapy alone. In conservative treatment, aminosalicylates and steroids are of minor importance, while chemotherapeutics, antibiotics, and thiopurines find application in daily clinical practice. TNF-α neutralizing antibodies such as infliximab (IFX), adalimumab (ADA) or certolizumab (CER) prove to be the most effective. Surgical treatment may be provided as ad hoc; in this case, drainage procedures are recommended, usually with leaving a loose seton. Planned procedures consist in the excision of fistulas (simple fistulas) or performing more complex procedures, such as advancement flaps or ligation of the intersphincteric fistula tract Surgical measures can be complemented by the use of video technology (video-assisted anal fistula treatment VAAFT) or vacuum therapy. In extreme cases, it may be necessary to create the stoma. Treatment of perirectal fistulas includes adhesives or so-called plugs. High hopes may be associated with the introduction of stem cells into clinical practice, which is the administration of non-hematopoietic multipotent cells to the fistulas to induce the phenomenon of immunomodulation and tissue healing.
The molecular basis for the formation and growth of vestibular schwannomas (VS) has been elucidated in the recent years. The main genetic and epigenetic aberrations, changes in gene expression and specific signaling pathways involved in pathogenesis of sporadic VS and neurofibromatosis type II (NF2) have been defined. These findings facilitated the search for prognostic markers in VS and potential targets for biological therapy. This publication summarizes the main directions of research in the field of molecular diagnostics and pharmacotherapy of VS based on biological agents.

Uogólniony proces zapalny przebiegający z powstawaniem polipów nosa i nadmiarem eozynofilów w obrębie błony śluzowej zatok przynosowych to zapalenie typu 2., w którym pośredniczą limfocyty Th2 – komórki układu immunologicznego, odpowiadające za przewlekłe procesy zapalne. Dziś znamy też kluczowe mediatory prozapalne, przeciwko którym powstały nowe leki, tzw. leki biologiczne, wytwarzane w liniach komórkowych. W dokumencie przedstawiamy dostępne, w obecnej chwili, leczenie biologiczne zatwierdzone do leczenia pacjentów z T2 – zależnym przewlekłym zapaleniem zatok przynosowych.

Chronic rhinosinusitis with nasal polyps (CRSwNP) has a significant impact on the well-being and social functions of the patient. The generalized inflammatory process with the formation of nasal polyps and excess eosinophils in the mucosa of the paranasal sinuses is called type 2 inflammation, which is mediated by Th2 lymphocytes – cells of the immune system responsible for chronic inflammatory processes. Today, we also know the key pro-inflammatory mediators against which new drugs have been developed, the so-called biological drugs, are produced in cell lines. In this document, we present currently available biologicals approved for the treatment of patients with T2-related chronic rhinosinusitis.
Crohn’s disease is a chronic disease that may occur in any part of digestive tract, causing significant lowering of quality of life and declining of fitness of patients. Most common symptoms are: abdominal pain, diarrhoea, blood in stool, palpable (often tender) mass in abdomen, weight loss, weakness, fever and feverishness. Those symptoms are often accompanied by perianal lesions such as fistulae, fissures and abscesses, and also other enterocutaneous fistulae. Diagnosis is based on endoscopy with biopsies, histopathology and radiological techniques with use of barium contrast. In spite of improving recognition of Crohn’s disease and better accessibility to diagnostic procedures some cases are diagnosed during surgery because of ileus or wrong diagnosis of appendicitis. Typical treatment includes 5-aminosalicilates, glicocorticoids and thiopurines. At the moment there are two monoclonal anti-TNF-α antibodies registered in Poland for biological treatment of Crohn’s disease. Conventional and biological therapy doesn’t always allow us to prevent surgical treatment. In years of observation, most of the patients undergo at least one surgery. Recent publication demonstrate purposefulness of early biologic and immunosuppressive therapy (top-down strategy). The chance to change course of disease is tempting, but possibility of complications (esp. infectious) suggest cautious approach, until further data are known. Those complications also incline us to leave this kind of treatment to experienced sites.

Choroba Leśniowskiego-Crohna jest przewlekłym schorzeniem, które może dotyczyć dowolnego odcinka przewodu pokarmowego (najczęściej końcowego odcinka jelita cienkiego), powodującym znaczne obniżenie jakości życia oraz upośledzenie sprawności chorego. Najczęstsze objawy to ból brzucha, biegunka, krew w stolcu, badalny (często tkliwy) opór w jamie brzusznej, utrata masy ciała, osłabienie, stany podgorączkowe i gorączka. Często towarzyszą im zmiany okolicy odbytowej – przetoki, szczeliny i ropnie, a także przetoki jelitowo-skórne. Rozpoznanie opiera się na endoskopii z pobraniem wycinków, histopatologii oraz metodach radiologicznych z użyciem kontrastu. Pomimo coraz lepszego rozpoznawania i łatwiejszego dostępu do diagnostyki część przypadków jest diagnozowana w trakcie zabiegu z powodu niedrożności lub mylnie rozpoznanego zapalenia wyrostka robaczkowego. W leczeniu najczęściej stosuje się preparaty 5-aminosalicylanów, glikokortykosteroidy oraz tiopuryny. Obecnie w Polsce zarejestrowane są dwa przeciwciała anty-TNF-α, stosowane w terapii biologicznej. Leczenie metodami klasycznymi oraz lekami biologicznymi nie zawsze pozwala uniknąć interwencji chirurgicznej. Z dłuższej obserwacji wynika, że większość chorych przechodzi przynajmniej jeden zabieg operacyjny. Nowe doniesienia coraz śmielej wskazują na celowość wczesnego włączania terapii biologicznej i immunosupresyjnej (strategia top-down). Choć szansa zmiany przebiegu choroby w wyniku agresywnego leczenia od momentu rozpoznania jest kusząca, ze względu na możliwość wystąpienia powikłań takiej terapii, zwłaszcza infekcyjnych, należy zachować ostrożność, dopóki nie będziemy mieli więcej danych. Powikłania te wymuszają również, aby leczeniem tym zajmowały się ośrodki referencyjne, mające duże doświadczenie w tej dziedzinie.
Transcranial magnetic stimulation (TMS) is rapidly developing as a powerful, non-invasive tool for studying the human brain. TMS is currently a target of interest of both basic science investigators and clinicians. In this procedure a pulsed magnetic field generated extracranially induces focal intracranial electrical discharges that can temporarily excite or inhibit specific brain areas. In this way there is possible to alter the functioning of the brain beyond the time of stimulation, offering potential for therapy. TMS has been used in the investigation of neurological and neuropsychiatric disorders, and more recently has emerged as a tool in the study and treatment of depressed mood. TMS is being evaluated as a possible alternative to electroconvulsive therapy for the treatment of refractory depression. This review examines the basic principles underlying transcranial magnetic stimulation. Published studies in this field are summarized and paper reviews TMS as an investigational and treatment tool in schizophrenia, affective disorders and anxiety disorders. It concludes by discussing safety issues, limitations, and directions for future research.

Przezczaszkowa stymulacja magnetyczna (transcranial magnetic stimulation; TMS) stanowi przykład szybko doskonalonej techniki służącej badaniu mózgu. Obecnie TMS jest obiektem zainteresowania zarówno badaczy zajmujących się naukami podstawowymi, jak i klinicystów. Pod wpływem TMS dochodzi do zmian czynnościowych w OUN. W zależności od stosowanej częstotliwości stymulacji jest to pobudzenie bądź hamowanie aktywności kory w stymulowanym obszarze, co znajduje zastosowanie w terapii. Skuteczność TMS w leczeniu była oceniana w badaniach nad chorobami neurologicznymi oraz neuropsychiatrycznymi, a ostatnio zwłaszcza chorobami afektywnymi. Oceniano między innymi TMS jako potencjalnie alternatywną metodę dla terapii elektrowstrząsowej w leczeniu depresji lekoopornej. W publikacji omówiono podstawy teoretyczne i techniczne stymulacji magnetycznej. Zostały także przedstawione wyniki badań nad skutecznością leczenia z wykorzystaniem TMS w schizofrenii, chorobach afektywnych i zaburzeniach lękowych. W końcowej części pracy dyskutowane są kwestie bezpieczeństwa i potencjalnych objawów niepożądanych oraz kierunki przyszłych badań.
