We evaluated the effectiveness of three different rehabilitation programs for chronic behavioral disorders in traumatic brain injury (TBI) patients with orbito-frontal damage (OFC), incurred from 13 to 15 years prior to enrollment. From among 547 adult patients with severe TBIs treated neurosurgically from 1999 to 2003, 60 were selected for the study, divided into three groups of 20, matched for age and sex. Each group received one of three different rehabilitation protocols. Group A received a comprehensive program with two components: (1) a 6-month early neuropsychological rehabilitation program targeting cognitive deficits, beginning immediately post-injury, and (2) the “Academy of Life” program, with weekly meetings aimed at reducing behavioral disorders in social transactions. Group B received only cognitive rehabilitation, while Group C received only the “Academy of Life” progam. The Frontal Behavioral Inventory (FBInv) was used to assess behavioral disorders at baseline and again after completion of the study. At baseline, all 60 patients still displayed all or most of the common behavioral symptoms characteristic of frontal syndrome. The improvement achieved by group A (the comprehensive program) was significantly better upon completion of the study than that shown by the patients in groups B (cognitive only) and C (social skills only). A comprehensive program of rehabilitation, which combines early neuropsychological rehabilitation of cognitive deficits with the “Academy of Life” program, aimed at improving social skills, proved to be more effective than either of these two components administered without the other
Tourette syndrome (TS) is a neurodevelopmental disorder characterizedby the co-occurrence of motor and vocal tics. TSusually begins in childhood and its symptoms may also persistinto adulthood. Most patients with TS are diagnosed withcomorbidities that can significantly affect the clinical pictureof the disease. The article presents behavioral and emotionaldisorders that are most often diagnosed in people with TS,namely: attention deficit hyperactivity disorder (ADHD), obsessive-compulsive disorder (OCD), depression, conduct disorder(CD) and self-injurious behaviour (SIB).

Zespół Tourette’a (TS) jest zaburzeniem neurorozwojowym, które charakteryzuje się współwystępowaniem tików motorycznych i wokalnych. Zaburzenie zwykle ma swój początek w dzieciństwie, a jego objawy mogą utrzymywać się również w okresie dorosłości. U większości pacjentów z TS rozpoznaje się zaburzenia współwystępujące, które mogą w znaczącym stopniu wpływać na obraz kliniczny choroby. W artykule przedstawiono zaburzenia zachowania i emocji, które są diagnozowane u osób z TS najczęściej, a mianowicie: zespół nadpobudliwości psychoruchowej z deficytem uwagi (ADHD), zaburzenia obsesyjno-kompulsyjne (OCD), depresję, zaburzenia zachowania (CD) oraz zachowania samookaleczające (SIB).
Cancer and its treatment affect the mental state of patients as manifested by adjustment disorders. The group includes both anxiety and depressive disorders, which are variously classified according to the ICD classification system. Therefore it is suggested to include these disorders in a separate category of so-called “psychological responses to traumatic cancer-related distress.” Implementation of screening for distress (e.g. using distress thermometer) in each Cancer Center is a precondition for good cancer care. Anxiety and depressive syndromes, which require professional treatment and care, are particularly common adjustment disorders. Furthermore, cancer patients may experience so-called organic mental disorders caused by permanent or transient impairment of central nervous system (CNS) function. Disorders of consciousness (delirium) are particularly common, especially in the elderly as well as in individuals with advanced disease stages. It is a known fact that the elderly prevail among cancer patients, thus they often suffer from cognitive impairment (dementia) as well as from personality and behavioral disorders associated with CNS damage. These disorders may coexist with cancer or may be triggered by brain tumors. Cancer patients with mental disorders should be treated with caution due to possible drug interactions and other coexisting somatic disorders. Implementation of psychological support for oncological patients on a larger scale than before is also crucial.

Choroba nowotworowa i jej leczenie wywierają wpływ na stan psychiczny chorych, co przejawia się zaburzeniami przystosowania. W tej grupie mieszczą się zarówno zaburzenia lękowe, jak i zaburzenia depresyjne różnie klasyfikowane w ICD. W związku z tym zaproponowano ujęcie tych zaburzeń w odrębną kategorię tzw. „reakcji psychicznych na traumatyczny dystres zależny od nowotworu”. Warunkiem dobrej opieki onkologicznej jest wprowadzenie do każdej jednostki onkologicznej przesiewowego badania dystresu (np. za pomocą termometru dystresu). Spośród zaburzeń przystosowania szczególnie częste są zespoły lękowe i zespoły depresyjne, które wymagają profesjonalnego leczenia i opieki. Ponadto wśród pacjentów z chorobą nowotworową zdarzają się tzw. organiczne zaburzenia psychiczne spowodowane trwałymi lub okresowymi zaburzeniami funkcjonowania ośrodkowego układu nerwowego. Szczególnie często, zwłaszcza u osób starszych i w zaawansowanym stadium choroby, występują zaburzenia świadomości (delirium). Wiadomo, że wśród chorych na nowotwory przeważają ludzie starsi, stąd często cierpią oni na zaburzenia poznawcze (otępienie), a także na zaburzenia osobowości i zachowania uwarunkowane uszkodzeniem ośrodkowego układu nerwowego. Zaburzenia te mogą współwystępować z chorobą nowotworową lub mogą być spowodowane przez zmiany nowotworowe w mózgu. Leczenie zaburzeń psychicznych pacjentów z chorobą nowotworową należy prowadzić bardzo ostrożnie ze względu na możliwe interakcje lekowe i współistnienie zaburzeń somatycznych. Bardzo ważne jest także wprowadzenie na większą niż dotąd skalę pomocy psychologicznej świadczonej osobom chorym leczonym przez onkologów.
