Wstęp: Ocena prawidłowości ukształtowania stóp ma istotne znaczenie z punktu widzenia profilaktyki i korekcji. Pomimo duŜej liczby prac poświęconych diagnostyce stóp konieczne są nowe badania w celu ujednolicenia oceny oraz określenia sposobu klasyfikacji deformacji stóp. Celem pracy jest próba zweryfikowania, które ze wskaźników służących do oceny wysklepienia podłużnego, poprzecznego i proporcji stóp mają największą siłę diagnostyczną, a więc w założeniu są najbardziej trafne. Materiał i metody: Badaniami przekrojowymi w latach 2008-2009 objęto losowo dobraną grupę 150 studentek. Wiek badanych mieścił się w przedziale 20-27 lat. Odbitki stóp wykonano techniką niebrudzącą, przyrządem pomysłu ŚlęŜyńskiego. Z plantogramów uzyskano następujące wskaźniki oceniające wysklepienie podłużne: wskaźnik kątowy Clarke’a (CL), wskaźnik KY, wskaźnik Chippaux-Smirak (CSI), wskaźnik głębokości wysklepienia podłużnego stopy (Wgwp). Wyznaczono wskaźniki wysklepienia poprzecznego: wskaźnik Wwp Wejsfloga, kąt piętowy (γ). Wskaźniki proporcji stopy: wskaźnik tęgości stopy (Ts), wskaźnik długościowo-szerokościowy (Wd-sz). Weryfikacji trafności doboru wskaźników do oceny stóp dokonano metodą analizy czynnikowej. Analizy statystyczne przeprowadzono przy uŜyciu programu Microsoft Excel pakietu Office firmy Microsoft oraz programu STATISTICA 8.0 firmy StatSoft. Wyniki: Wskaźnik głębokości wysklepienia podłużnego stopy (Wgwp) osiągnął najwyższe wartości ładunków w ramach czynnika. Wskaźnik Wwp Wejsfloga osiągnął wyŜsze wartości ładunków czynnikowych niŜ kąt piętowy. Wskaźnik Wd-sz osiągnął wyŜsze wartości ładunków w ramach czynnika niż wskaźnik tęgości stopy (Ts). Wnioski: Do oceny wysklepienia podłużnego stóp najbardziej trafny w zastosowaniu jest wskaźnik głębokości wysklepienia podłużnego stopy (Wgwp). Do oceny wysklepienia poprzecznego najbardziej przydatny jest wskaźnik Wwp Wejsfloga, natomiast w ocenie proporcji stóp najbardziej trafny jest wskaźnik długościowo-szerokościowy (Wd-sz).

Background: The evaluation of correctness in feet shaping is significant from the point of view of the prevention and correction in foot deformation. Besides the ample works devoted to the diagnostic testing of feet, new research is necessary to standardize these evaluations as well as to determine the manner of foot deformation classification. The purpose of the work is an attempt to verify which of the suggested indexes will be more useful in evaluating the longitudinal arch of the foot, the transverse arch and foot proportion.Material and methods: Cross-sectional studies were performed in 2008-2009 and included 150 randomly selected students. The tested women were aged 20-27. A plantographic method non-marking technique was used to evaluate the feet. The longitudinal arch of the foot was evaluated by Clark’s angle (CL), KY index, Chippaux-Smirak Index (CSI) and the author’s index of depth of the longitudinal arch of the foot (IDLA). To evaluate the transverse arch of the foot the Wejsflog index (WI) and heel angle (γ) were used. Foot proportions were tested by using the following indexes: foot corpulence (FCI) and the length and width index (LWI). Verification of choice relevance of the plantographic indexes in relation to foot evaluation was carried out by factors analysis.Results: The index of depth of the longitudinal arch of the foot (IDLA) achieved the maximum load factor values. The Wejsflog index (WI) achieved slightly higher load factor values than did the heel angle. The length and width index (LWI) achieved slightly higher load factor values than did the foot corpulence index (FCI).Conclusions: In evaluating the longitudinal arch the most accurate is the index of the depth of the longitudinal arch (IDLA). To evaluate the transverse arch the Wejsflog index (WI) is more useful. To evaluate foot proportions the length and width index (LWI) is more useful.
W niniejszym artykule opisano zastosowanie Międzynarodowej Klasyfikacji Funkcjonowania, Niepełnosprawności i Zdrowia (ang. International Classification of Functioning, Disability and Health, ICF) w rehabilitacji i fizjoterapii oraz sposób, w jaki można wykorzystać ICF jako narzędzie wspomagające w wyborze właściwych narzędzi oceny stanu pacjentów podczas rehabilitacji. ICF jest klasyfikacją opracowaną przez WHO (Światową Organizację Zdrowia) dla opisania możliwości funkcjonalnych danej osoby. ICF może być uŜywany do klasyfikowania danych dotyczących procesu i wyników rehabilitacji i fizjoterapii, takich jak spostrzeżenia pacjenta, spostrzeżenia terapeuty, diagnoza funkcjonalna (czyli wynik procesu diagnostycznego), cele oraz wyniki postępowania. Narzędzia oceny są narzędziami pomiaru tych charakterystycznych cech danej osoby lub środowiska, które są istotne w codziennej praktyce w rehabilitacji lub fizjoterapii. Narzędziami tymi mogą być testy, sposoby obserwacji oraz kwestionariusze; można je wykorzystywać w roli testów diagnostycznych i prognostycznych oraz jako miary wyniku i narzędzia oceny. Dla wybrania dla danego pacjenta jednego lub więcej narzędzi oceny należy podjąć następujące kroki: (1) znać cel(e) zastosowania narzędzia oceny; (2) wybrać zmienne kliniczne będące w zainteresowaniu; (3) znaleźć narzędzia oceny mierzące wybrane zmienne; (4) określić warunki stosowania, dostępność i możliwość zastosowania danego narzędzia; (5) poznać psychometryczne charakterystyki branych pod uwagę narzędzi oceny; (6) wybrać najbardziej odpowiednie narzędzia oceny; oraz (7) zastosować i ocenić dane narzędzia. W pracy opisane są powyższe działania.

This article describes the use of the ICF, the International Classification of Functioning, Disability and Health, in rehabilitation and physiotherapy and the way the ICF can help to select proper assessment instruments for patients during the rehabilitation process. The ICF is a classification developed by the WHO to describe the functioning of a client. The ICF can be used to classify very relevant data in the process and outcome of rehabilitation care and physical therapy, such as the findings of the patient, the findings of the therapist, the functional diagnosis (the outcome of the diagnostic process), the treatment goals and the treatment results. Assessment instruments are tools to measure characteristics of the person or the environment, which are relevant in the process of rehabilitation or physiotherapy practice. These assessments can be tests, observation instruments and questionnaires and can be used as diagnostic and prognostic test and as outcome and evaluation measurements. In the selection of one or more instruments for an individual patient, there are several steps the professional has to take: 1) know the purpose(s) of using an instrument; 2) select the clinical variables you are interested in; 3) find the instruments that measure the chosen clinical variables; 4) determine the applicability, availability and feasibility of the instrument; 5) know the psychometric characteristics of the chosen instruments; 6) select the instruments that seem adequate; and 7) implement, use and evaluate the instruments. These steps are described.
Objectives: The aim of the study was to evaluate the effectiveness of applied rehabilitation program and compare sensation levels with functional possibilities of the operated hand in a group of patients after surgery due to carpal tunnel syndrome. Material and methods: Fourty patients who underwent rehabilitation were admitted to the hand rehabilitation centre after surgical release of the median nerve within the carpal tunnel. Fifty hands were treated and evaluated. The rehabilitation program was established on an individual basis for each patient and included patient’s education, home exercises and physiotherapy treatment in outpatient centers. Skin sensation of the hands was evaluated by static two-point discrimination test. Based on this examination, sensibility indices for the median and ulnar nerves were calculated. Final results were compared to the Alderson-McGall Hand Function Questionnaire for Patients with Carpal Tunnel Syndrome.Results: After rehabilitation, skin sensation in the area of the median nerve improved, as measured by the sensibility index. Improvement was observed for most of the evaluated functions of the rehabilitated hands, while a different degree of improvement was noted. Sensibility index showed weak correlation with functional status and was observed only for some functions. Conclusion: In patients operated because of carpal tunnel syndrome, improvement in skin sensibility and functional abilities are observed in the operated hands after a rehabilitation program involving education, home exercises and outpatient sessions.
Human rhinoviruses (HRV) are one of the nine genera belonging to a large family of Picornaviridae. They are responsible for the most cases of common cold, as well as one third to one half of upper respiratory tract (URT) infections. However, HRV are also associated with more severe illnesses, like acute otitis media, sinusitis and some lower respiratory tract diseases such as pneumonia, wheezing in children and exacerbations of asthma. Viral infections are associated with the majority of asthma exacerbations both in children (80-85%) and adults (75-80%), and about 60% of these are caused by HRV. However, the exact mechanism of HRV-induced exacerbations of the disease is not well understood, which makes it difficult to establish the effective treatment. There have already been many attempts to develop a sensitive and specific method of HRV detection in clinical samples. Some of them were based on virus cultures followed by acid lability test, whereas others implemented the reverse transcription-polymerase chain reaction (RT-PCR) and amplification of conserved sequences of the rhinoviral genome. As numerous of these sequences are common to both rhinoviruses and enteroviruses (EVs), further analyses were necessary, which made those methods laborious, time-consuming and too difficult to use in routine diagnostics. Steininger et al. established an RT-PCR-based sensitive and specific method of rhinovirus detection in clinical samples, which was tested to amplify 87 different tissue-culturegrown serotypes of HRV. The aim of this study was to evaluate a modified RT-PCR based method of HRV detection in clinical samples obtained from patients with asthma exacerbations. We collected 41 nasal lavages from patients with asthma exacerbations who received hospital treatment either following an admission or in an out-patient clinic. HRV was found in 22 cases (54%), which corresponded well with the published data.
Wstęp: Ocena prawidłowego ukształtowania stóp ma bardzo istotne znaczenie diagnostyczne. Wszelkie odchylenia w ich budowie mają odzwierciedlenie w patogenezie schorzeń w obrębie innych części ciała. Istnieje wiele narzędzi do oceny stóp, jednakże wyniki uzyskiwane różnymi metodami nie zawsze są porównywalne. Foot Posture Index (FPI–6) został opracowany jako szybkie, łatwe w użyciu i rzetelne narzędzie służące do oceny stóp w różnych zastosowaniach klinicznych.Cel pracy: Oznaczenie współczynnika powtarzalności pomiarowej dla kwestionariusza FPI-6 zastosowanego do badania stóp u dzieci i młodzieży.Materiał i Metoda: W badaniach wzięło udział 60 osób w trzech grupach wiekowych 9, 12 i 14 lat. W celu określenia powtarzalności pomiarowej FPI-6 wszystkie pomiary przeprowadzono dwukrotnie w 2 tygodniowym odstępie czasowym. Oceniano następujące parametry: wysokość ciała, masa ciała, BMI (Body Mass Index), oraz wykonano ocenę stóp za pomocą FPI-6. Wyniki: Wartość współczynnika powtarzalności R pomiędzy pierwszym i drugim badaniem, dla wszystkich parametrów wynosił 0.89-0.96, co oznacza bardzo dobrą powtarzalność FPI -6 w ocenie stopy. Wysokie wartości współczynnika powtarzalności występują niezależnie od wieku dziecka, wysokości i masy ciała. Zarówno ocena koślawości i szpotawości kości piętowej jak i ocena przywiedzenia /odwiedzenia przodostopia względem kości piętowej w grupach dzieci dziewięcioletnich i dwunastoletnich uzyskały wartości mówiące o lekkiej pronacji stopy, a w grupie dzieci czternastoletnich parametry te ulegały statystycznie istotnej zmianie uzyskując wartości charakterystyczne dla neutralnego ustawienia stopy.Wnioski: FPI-6 jest prostym i rzetelnym narzędziem oceny stopy u dzieci i młodzieży, charakteryzującym się wysokim współczynnikiem zgodności pomiarów powtarzanych przez tego samego badającego.

Background: The evaluation of proper foot posture is of importance in diagnosis. All abnormalities in the foot structure may reflect the pathogenesis of the dysfunctions presented at other parts of the body. There are many diagnostic devices for foot assessment, but the results obtained using different methods are often not comparable. Foot Posture Index (FPI-6) was created as a quick, simple and reliable tool for quantifying foot posture in a variety of clinical settings.Aim: The evaluation of foot posture in 3 age groups of children using FPI-6 and the assessment of the reliability of this methodMaterial and Methods: 60 subjects in 3 age groups 9, 12 and 14 years participated in the study. For the purpose of FPI-6 reliability level determination all measurements were taken twice, with a 2-week gap. The following variables were evaluated: body height, body mass, BMI (Body Mass Index), the foot assessment using FPI-6.Results: The correlation coefficient R between the first and second measurement for all criterion was 0.89-0.96, what indicates a very good FPI-6 reliability in the foot assessment. Both the inversion/eversion of the calcaneus and abduction/adduction of the forefoot on the rear foot in the groups of 9- and 12-year-old children indicated mild foot pronation, and those parameters changed significantly in 14-year-old children where values were reached typical for a neutral foot.Conclusion: FPI-6 it is a simple and reliable tool for comprehensive foot evaluation, which allows one to use it both in clinical practice as in preventive population based studies.
Ultrasonography is a primary imaging technique in patients with suspected thyroid disease. It allows to assess the location, size and echostructures of the thyroid gland as well as detect focal lesions, along with indication of their size, echogenicity, echostructure and vascularity. Based on these features, ultrasound examination allows to predict abnormal focal lesions for biopsy and monitor the biopsy needle track. This paper presents the standards of thyroid ultrasound examination regarding ultrasound apparatus technical requirements, scanning techniques, readings, measurements, and the description of the examination. It discusses the ultrasound features of increased malignancy risk in focal lesions (nodules) found in the thyroid gland. It presents indications for fi ne needle aspiration biopsy of the thyroid gland for the visibility of single nodules (focal lesions) and numerous lesions as well as discusses contraindications for thyroid biopsy. It describes the biopsy technique, possible complications and rules for post-biopsy monitoring of benign lesions. The paper is an update of the Standards of the Polish Ultrasound Society issued in 2011. It has been prepared on the basis of current literature, taking into account the information contained in the following publications: Thyroid ultrasound examination and Recommendations of the Polish Ultrasound Society for the performance of the FNAB of the thyroid.

Badanie ultrasonografi czne jest podstawowym badaniem obrazowym u osób z podejrzeniem chorób tarczycy. Pozwala na ocenę położenia, wielkości oraz echostruktury gruczołu, w tym wykrywanie zmian ogniskowych, wraz z określeniem ich rozmiaru, echogeniczności, echostruktury i unaczynienia. Na podstawie tych cech badanie ultrasonografi czne umożliwia wytypowanie nieprawidłowej zmiany ogniskowej do biopsji oraz monitorowanie toru igły biopsyjnej. W publikacji przedstawiono standardy badania ultrasonografi cznego tarczycy dotyczące wymogów technicznych aparatów ultrasonografi cznych, techniki wykonywania badania, wskazań i pomiarów oraz opisu badania. Omówiono cechy ultrasonografi czne zwiększonego ryzyka złośliwości zmian ogniskowych (guzków) w tarczycy. Przedstawiono wskazania do biopsji aspiracyjnej cienkoigłowej tego gruczołu w przypadku uwidocznienia pojedynczego guzka (zmiany ogniskowej) oraz zmian mnogich, a także przeciwwskazania do biopsji tarczycy. Opisano technikę wykonywania biopsji, możliwe powikłania, jak również zasady monitorowania zmian łagodnych po biopsji. Praca stanowi aktualizację Standardów badania ultrasonografi cznego Polskiego Towarzystwa Ultrasonografi cznego wydanych w 2011 roku. Została przygotowana na podstawie aktualnego piśmiennictwa, z uwzględnieniem wiedzy zawartej w dwóch pozycjach: Badanie usg tarczycy oraz Zalecenia Polskiego Towarzystwa Ultrasonografi cznego dotyczące wykonywania BACC tarczycy.
