W pracy przedstawiono wybrane poglądy na temat bólu jako doświadczenia i jako zjawiska oraz przeanalizowano czynniki, które wpływają na ograniczoną skuteczność jego leczenia u kobiet chorych na nowotwory narządu rodnego. Ból jest nieodłącznym, naturalnym i potrzebnym elementem życia – przede wszystkim pozwala zareagować na zagrożenie, a także towarzyszy wysiłkowi, którego pokonanie przynosi wiele psychospołecznych korzyści. Subiektywne doświadczenie bólu zależy od kilku czynników: siły bodźca, indywidualnej podatności i odporności na ból czy predyspozycji psychicznych jednostki. Poza stosowaniem właściwej farmakoterapii na skuteczność leczenia bólu u pacjentek z nowotworem złośliwym narządu rodnego wpływ mają zaburzenia w obszarze zdrowia psychicznego (zaburzenia poznawcze, lękowe i depresyjne), utrwalone, subiektywne przekonania na temat choroby i jej leczenia (opisany przez Waltera Kennedy’ego efekt nocebo), wielochorobowość, pamięć bólu, kontakt z lekarzem, poszanowanie praw osoby cierpiącej przez personel medyczny, wiek chorej oraz jakość otrzymywanego przez nią wsparcia. W artykule poddano krytyce zastępowanie sprawdzonych metod leczenia przeciwbólowego, opartego na dowodach naukowych i klinicznych, metodami niekonwencjonalnymi, takimi jak joga, reiki lub bioenergoterapia, których założenia i skuteczność nie zostały dotąd zweryfikowane ani udokumentowane.

The paper presents selected views on pain as an experience and as a phenomenon as well as an analysis of factors reducing the efficacy of pain management in women with gynecologic cancer. Pain is an inseparable, natural and necessary element of life, which is primarily involved in responding to threat, but also accompanies effort, overcoming of which brings a number of psychosocial benefits. The subjective experience of pain depends on several factors, such as stimulant intensity, individual susceptibility and resistance to pain, or individual mental predispositions. In addition to appropriate pharmacotherapy, the efficacy of pain management in patients with gynecologic malignancies is further affected by mental disorders (cognitive, anxiety and depressive disorders), fixed, subjective ideas about the disease and its treatment (coined as nocebo by Walter Kennedy), multiple comorbidities, memory of pain, contact with the doctor, respecting patient’s rights by medical staff, age, and the quality of support received by the patient. The paper takes a critical look at replacing proven analgesic treatment strategies based on scientific and clinical evidence with unconventional methods, such as yoga, reiki or bioenergy therapy, which have not been verified or documented.
Osteoarthritis (OA) is the most common joint disorder and the main cause of disability in the elderly. As the average age of the population increases, the prevalence continues to rise. The course of the disease is slowly progressive, and the predominant symptom is pain. Currently, no therapeutic strategies have been proven to inhibit the progression of structural changes. The primary goal of treatment is reduction of pain, co-existing stiffness and disability. Optimal management of osteoarthritis requires the combined application of different therapeutic.

Choroba zwyrodnieniowa - osteoartroza (OA) jest najczęstszą chorobą stawów i najczęstszą przyczyną niesprawności w wieku podeszłym, a wraz ze wzrostem długości życia rośnie częstość jej występowania. Przebieg choroby jest powoli postępujący a dominującym objawem jest ból. Nie istnieje obecnie metoda terapeutyczna skutecznie hamująca progresję zmian strukturalnych. Celem leczenia jest przede wszystkim zmniejszenie bólu, towarzyszącej sztywności stawów, zmniejszenie niesprawności. Optymalne postępowanie w chorobie zwyrodnieniowej stawów wymaga łącznego stosowania różnych metod terapeutycznych.
Therapeutical problems attached to the chronic pain therapy, has stimulated a growing interest in the development of neurodestructive (ablative) methods. Since 1975 the increasing number of the experimental investigators described the results concerning the influence of high frequency current on the nerve tissue. Breakthrough in clinical practice was introduction by Suijter of the new disposable electrode, producing a heat lesion around the active tip. This electrode had a needle form and has a valuable option to measure the tissue temperature during the whole procedure. In clinical practice the above technic received the name of radiofrequency thermocoagulation (RF). Despite of many investigations, the mode of action of RF is still unclear. In this review the results of the most frequently aaplied technics concerning low back pain are presented. The particular attention has also been drown to the trigeminal neuralgia and their therapeutic options. Thermolesions of the nerve system are well recognized therapeutic modality in chronic pain treatment despite the lack of hard evidence, yet. The results of clinical practice have learned, that even short term positive results are a valuable gift for the patient, who are forced to consume several pijnkillers per day otherwise.

Problemy terapeutyczne związane z leczeniem bólu przewleklego spowodowały wzrost zainteresowania neurodestrukcyjnymi metodami leczenia. Od połowy lat siedemdziesiątych ubiegłego stulecia datuje się szybki postęp badań eksperymentalnych dotyczących wpływu prądu o wysokiej częstotliwości na tkanki człowieka, a w szczególności tkankę nerwową. Przełomem w praktyce klinicznej okazało się wprowadzenie w 1981 roku przez Suijtera nowej elektrody. Elektroda ta, w formie igły, pozwala sterować powstającą w trakcie zabiegu podwyższoną temperaturą. Opisane metody leczenia otrzymały nazwę termokoagulacja (Radiofrequency thermocoagulation, RF). Mimo licznych badań, próbujących wyjaśnić mechanizm działania wysokoczęstotliwościowego prądu na tkankę nerwową, jednoznacznej odpowiedzi do dziś nie znamy. Przeglądając wyniki badań, często słabo udokumentowanych, można odnieść wrażenie, że leczymy metodami, które nie wiadomo czy odpowiadają normom stawianym przez EBM. W pracy tej omówione zostały najczęściej stosowane w praktyce klinicznej techniki, a mianowicie te, które dotyczą bólu kręgosłupa lędźwiowego oraz miejsca neuroablacji zwoju Gassera w przypadkach neuralgii nerwu trójdzielnego. Termolezje w zakresie układu nerwowego, opierając się na efektach klinicznych, stanowią wartościową, uzupełniającą metodę leczniczą, która w istotny sposób polepsza komfort życia chorego, pozwalając mu zmniejszyć ilość przyjmowanych leków.
Stressful events can damage neonatal brain through a complexity of events including free radical (FR) generation. We examined whether pain provoked by a routine heel prick can generate an increase in potentially harmful FR in neonatal blood. To this aim, advanced oxidation protein products (AOPP) and total hydroperoxide (TH) concentrations were measured at the beginning (sample A) and at the end (sample B) of each sampling in 64 babies (corrected age: 37.2±2.7 weeks) who underwent heel prick for routine blood tests. We scored pain of every procedure in all newborns. No differences were detected between AOPP and TH blood concentrations at the beginning and at the end of heel prick sampling, considering the whole cohort of babies. Conversely, a significant increase was observed between AOPP and TH blood concentrations considering only those babies who showed the highest pain intensity. When babies’ pain was high (ABC score P4), mean AOPP and TH blood levels increased significantly; in this case, mean AOPP values increased from 53.5 lm/l (SD=41.6) to 63.2 lm/l (SD=44.3) and TH values from 218.3 U Carr (SD=89.2) to 228.7 U Carr (SD=93.3), with a significant p value of 0.02 and 0.036, respectively. A significant correlation was also found between AOPP blood levels ratio (sample B/sample A) in each baby, and the correspondent level of pain. These data show that even common routine procedures can be potentially harmful for the newborn if they provoke a high level of pain.

Stresujące zdarzenia mogą uszkadzać mózg noworodka za pośrednictwem złożonych zdarzeń, w tym tworzenia wolnych rodników. Zbadaliśmy, czy ból wywoływany rutynowym nakłuciem pięty może powodować zwiększenie ilości potencjalnie szkodliwych wolnych rodników we krwi noworodka. tym celu zmierzono stężenie zaawansowanych produktów utleniania białek (advanced oxidation protein products, AOPP) i całkowite stężenie wodoronadtlenków (total hydroperoxide, TH) na początku (próbka A) i na końcu (próbka B) każdego pobierania krwi u 64 noworodków (wiek skorygowany: 37,2±2,7 tygodnia), u których wykonywano nakłucie pięty w celu pobrania krwi do rutynowych badań. Mierzono ból związany z każdą procedurą u wszystkich noworodków. Nie stwierdzono różnic między stężeniami AOPP i TH we krwi na początku i na końcu pobierania krwi drogą nakłucia pięty w całej kohorcie noworodków. Stwierdzono natomiast istotne zwiększenie stężeń AOPP i TH we krwi, jeżeli brano pod uwagę wyłącznie dzieci, które wykazywały największe natężenie bólu. Kiedy ból odczuwany przez dzieci był duży (punktacja ABC≥4), średnie stężenia AOPP i TH istotnie się zwiększyły; w tym przypadku średnie stężenia AOPP zwiększyły się z 53,5 µm/l (SD=41,6) do 63,2 µm/l (SD=44,3), a stężenia TH z 218,3 j. Carr (SD=89,2) do 228,7 j. Carr (SD=93,3), a wartości p dla tych różnic wyniosły odpowiednio 0,02 i 0,036. Stwierdzono również istotną korelację między współczynnikiem stężeń AOPP we krwi (próbka B/próbka A) u każdego dziecka, a nasileniem bólu. Dane te wskazują, że nawet częste rutynowe procedury mogą być potencjalnie szkodliwe dla noworodka, jeżeli powodują ból o dużym nasileniu.
Disturbance dynamics of intracellular signal transmission leads to the development of inflammation, which is the ground of many diseases including cardiovascular diseases, autoimmune diseases, neurodegenerative disorders, infections or cancer and contributes to abnormal propagation of nociceptive stimuli. One of the best known mediators of inflammatory responses are cytokines, which by binding to surface receptors, activate intracellular signaling pathways and lead to changes in gene expression. The important role of cytokines in nociception is observed in development of inflammatory and neuropathic pain. Recent studies have shown that Januse kinase and STAT transcription factors pathway, which is very often activated by stimulation of pro-inflammatory cytokines, is a potential point for construct new drug for inflammatory diseases therapy. Proteins that modulate the activity of intracellular JAK/STAT pathway are regulatory proteins SOCS family. These proteins through interactions with components of the intracellular cascade of signal propagation, regulate cell response to cytokines in many cell types. Better understanding of the mechanisms of action of SOCS proteins in the nervous system and identify pharmacological tools to increase their level in the cell may find therapeutic application in pain therapy.

Zaburzenie dynamiki przekazu sygnałów wewnątrzkomórkowych prowadzi do rozwoju stanu zapalnego, który jest podłożem wielu schorzeń m.in. chorób układu krążenia, autoimmunologicznych, neurodegeneracyjnych, infekcji czy nowotworów a także przyczynia się do nieprawidłowej propagacji bodźców nocyceptywnych. Jednymi z najlepiej poznanych mediatorów odpowiedzi zapalnej są cytokiny, które w wyniku wiązania receptorów powierzchniowych aktywują wewnątrzkomórkowe szlaki sygnałowe i prowadzą do zmiany ekspresji genów. Istotna rola cytokin w nocycepcji jest obserwowana zarówno w rozwoju bólu zapalnego jak i neuropatycznego. Najnowsze badania sugerują, że potencjalnym punktem uchwytu dla nowych leków w terapii chorób o podłożu zapalnym jest szlak kinaz Janusowych i czynników transkrypcyjnych STAT, który bardzo często ulega aktywacji w wyniku stymulacji cytokinami prozapalnymi. Białkami, które modulują aktywność wewnątrzkomórkowego szlaku JAK/STAT są białka regulatorowe z rodziny SOCS. Białka te w wyniku oddziaływania z komponentami wewnątrzkomórkowej kaskady propagacji sygnału, regulują odpowiedź komórki na cytokiny w wielu typach komórek. Lepsze zrozumienie mechanizmów działania białek SOCS w układzie nerwowym oraz zidentyfikowanie farmakologicznych narzędzi pozwalających na zwiększenie ich poziomu w komórce jest istotnym kierunkiem prowadzonych obecnie badań nad strategią terapeutyczną bólu.
Currently, low back pain is a major health social and economic problem. The aim of this study was assessment of low back pain and habits of movement in individuals leading a sedentary lifestyle, due to the nature of their work. The study was conducted from November 2010 to February 2011. Research material consisted of two groups of people: 57 office workers (including 40 women and 17 men) and 43 bus drivers and taxi drivers (men only). In the first group age ranged from 21 to 60 years, and the second between 21 and 63 years. Research tool was the author’s own questionnaire consists of four parts. The first and second part contained questions about personal data and information about the job. In the third part respondents were asked about any pain in the lower spine, while in the fourth part we asked about movement of habits, knowledge of prevention and ways of spending free time. The results indicate frequent occurrence of low back pain among drivers (62,8%). This problem also often applies to people with high abnormal BMI (25 and above). More irregularities in posture appeared in the group of office workers than among drivers.
The aim of this work was to examine the dependence between pain complaints and quality of life or need for nursing care in patients diagnosed with neoplastic disease, and to establish which measures of the studied quality of life were influenced by pain ailments to the greatest degree. Five hundred patients diagnosed with different types of cancer and receiving analgesic treatment were admitted to the study. The data were collected from June 2008 till December 2009. The examination was carried out at patients’ homes. The patients were inhabitants of small cities and villages of the Pomeranian province. The study group comprised 250 women and 250 men in the age from 25 to 90 years (average 61 years). The quality of life was evaluated according to the questionnaire EORTC QLQ-C3O, while pain intensity was estimated based on the Visual Analogue Score. The Cantril’s ladder served to assess general satisfaction from life and the Barthel Scale was used to define the need for nursing care in the opinion of the patients. The general quality of life, and also its individual measures in the studied group were quite low and averaged 35.99 %. Only cognitive functions the patients rated above 50 % and estimated them comparatively well. The patients also complained of coexistence of many different problems worsening their quality of life: 69.9 % suffered from intensification of fatigue, 58.1 % from sleep disorders and 58 % from deranged appetite. The results showed statistically significant relationships between intensity of pain and increased need for nursing care.

Celem pracy była próba oceny zależności pomiędzy dolegliwościami bólowymi a jakością życia i stopniem zapotrzebowania na opiekę pielęgniarską chorych z rozpoznaną chorobą nowotworową oraz ustalenie, na które z obszarów badanej jakości życia największy wpływ mają dolegliwości bólowe. Do badania włączono 500 pacjentów leczonych przeciwbólowo z rozpoznaną chorobą nowotworową, o zróżnicowanej diagnozie. Dane gromadzono od czerwca 2008 roku do grudnia 2009 roku. Badania przeprowadzano w domu pacjentów, mieszkańców małych miast i wsi województwa pomorskiego. W badanej grupie było 250 kobiet i 250 mężczyzn w wieku od 25 do 90 lat (średnia 61 lat). Do oceny jakości życia zastosowano kwestionariusz opracowany przez Grupę Badania Jakości Życia powstałą przy Europejskiej Organizacji Badania i Leczenia Raka EORTC QLQ-C3O (wersja 3.0), Skalę wzrokowo-analogowo VAS (Visual AnalogueScore) służącą do pomiaru stopnia natężenia bólu, Drabinę Cantrila, która posłużyła do oceny ogólnej satysfakcji z życia oraz Skalę Barthel określającą stopień zapotrzebowania na opiekę pielęgniarską w ocenie badanej grupy chorych. Ogólna jakość życia i jej poszczególne obszary w badanej grupie respondentów jest dość niska. Średnia ogólna jakość życia wynosiła 35,99%. Wartość powyżej 50 punktów badani pacjenci przypisują jedynie funkcjonowaniu poznawczemu, które oceniają stosunkowo dobrze. Badani pacjenci wskazywali także na współistnienie wielu innych dolegliwości, zdecydowanie pogarszających ich jakość życia, szczególne znaczenie przypisywali nasileniu zmęczenia 69,9% i zaburzeniom snu 58,1% oraz zaburzeniom łaknienia 58%. Uzyskane wyniki wskazują na istotne statystycznie zależności pomiędzy stopniem natężenia dolegliwości bólowych a zwiększeniem zapotrzebowania na opiekę pielęgniarską.
The aim of this work is to estimate the relations between personality features distinguished in accordance with P.T. Cost and R. McCrae’s The Big Five, a five-factor model of personality, and a way the pain is perceived along with the strategies of dealing with chronic pain chosen by the patient. The subject of the work was investigated basing on the author’s research, during which the following methods were used: Personality Inventory NEO-FFI (P.T. Cost, R. McCrae), Coping Strategies Questionnaire (A. C. Rosentiel, F. J. Keefe), McGill Pain Questionnaire (R. Melzack, translated by K. Sedlak) used as a scale of rating pain quality and quantity. The research involved 100 patients, both males and females, who suffered from chronic pain for more than three months, which was diagnosed by their physicians. It was assumed that there are relations between personality features and perception, and also the ways of dealing with the pain felt were chosen intentionally. Neuroticism is significantly connected with catastrophic thinking and the rate of pain estimation, whereas the extroversion is linked to cognitive distraction, declarations of being able to cope with the problem, neglect of pain experience and behavioural activity increase. The index of current pain intensity is the pain control level pointed by the patients.

Celem pracy jest ocena związków zachodzących pomiędzy cechami osobowości wyróżnionymi w ramach pięcioczynnikowego modelu osobowości tzw. Wielkiej Piątki P.T. Costa i R. McCrae, a postrzeganiem bólu oraz wybieranymi strategiami radzenia sobie z bólem przewlekłym przez osoby chore. Przedmiot pracy był realizowany w oparciu o własne badania, w których wykorzystano następujące metody: Inwentarz osobowości NEO-FFI (P. T. Costa i R. McCrae), Kwestionariusz Strategii Radzenia Sobie Z Bólem (A. C. Rosentiel, F. J. Keefe) i Kwestionariusz Bólu McGill (R. Melzack, tłumaczenie: K. Sedlak), służący do jakościowej i ilościowej oceny bólu. W przeprowadzonych badaniach uczestniczyło 100 osób, zarówno kobiety jak i mężczyźni, z lekarskim rozpoznaniem bólu przewlekłego utrzymującego się dłużej niż 3 miesiące. Zostało stwierdzone, że występują relacje między cechami osobowości a percepcją i wybieranymi sposobami radzenia sobie z doświadczanym bólem. Neurotyczność jest silnie powiązana z myśleniem katastroficznym i wskaźnikiem oceny bólu, natomiast ekstrawersja łączy się z poznawczym odwracaniem uwagi, deklarowaniem radzenia sobie, ignorowaniem doznań bólowych oraz ze zwiększoną aktywnością behawioralną. Predyktorem oceny aktualnej intensywności dolegliwości bólowych jest wskazywany przez pacjentów poziom kontroli bólu.
Celem badania była ocena bezpieczeństwa i skuteczności zastosowania implantu Ventralight ST z systemem pozycjonującym ECHO i dedykowanym urządzeniem mocującym – staplerem SorbaFix w prospektywnym badaniu kohortowym. Marteriał i metodyka. Badanie zaplanowano jako prospektywne jednoośrodkowe badanie kohorty chorych z jednorocznym okresem obserwacyjnym. Do badania włączono 52 chorych poddanych interwencji z powodu przepukliny brzusznej za pomocą laparoskopowego zaopatrzenia z implantem Ventralight ST ECHO PS. Wielkość siatki i sposób mocowania oparto na teoretycznych założeniach matematycznych. Głównym ocenianym parametrem było wystąpienie nawrotu w 12-miesięcznej obserwacji. Wyniki. Zanotowano dwa nawroty przepukliny, jeden u chorego z przepukliną okołostomijną i drugi w dużej przepuklinie pooperacyjnej. Ból obserwowano u 22 chorych (41%) po zabiegu. U większości chorych ból ustępował po okresie 3 miesięcy (7%). Obserwowano niskie natężenie bólu (VAS <2). Jednak silny ból występował u 2 chorych (VAS >6) w całym okresie obserwacji. Wnioski. Użycie implantu Ventralight ST Echo PS mocowanego stapleremSorbafix jest wartościową i bezpieczną opcją w laparoskopowym zaopatrzeniu przepukliny brzusznej. Rozwiązanie może być użyte do zaopatrzenia chorych zakwalifikowanych do laparoskopii z uwzględnieniem wielkości przepukliny. W odniesieniu do modeli matematycznych i przedstawionych danych klinicznych autorzy nie rekomendują używania opisanego implantu do przepuklin o pierścieniu większym niż 10 cm.
Wprowadzenie. Hokej na trawie jest grą zespołową, w której rywalizujące drużyny próbują strzelić bramkę drużynie przeciwnej powiększając swój dorobek punktowy. Zdobywa- nie bramek odbywa się m.in w wyniku szybkich ataków, nagłych zmian kierunków ruchu, w kontakcie z przeciwnikiem co naraża graczy na urazy w układzie mięśniowo-szkieletowym i w efekcie ból. Cel: Badanie miało na celu udokumentowanie występowania bólu wśród zawodniczek drużyny narodowej RPA w hokeju na trawie Metody: Zastosowano kwestionariusz, w którym zebrano informacje biograficzne, epidemiologiczne i dotyczące treningu sportowego w ostatnich 12 miesiącach. Zebrane dane zostały opracowane statystycznie i posłużyły do przeprowadzonych analiz. Wyniki: 28 zawodniczek (93,33%) skarżyło się na bóle mięśniowo-szkieletowo w ostatnich 12 miesiącach (p<0,001). Najczęściej ból występował w stawach skokowych (24,14%), krę- gosłupie i stawie biodrowym (22,41%), podudziu (13,79%), stawach kolanowych (12,07%) i udach (10,34%) (p<0,01). Głównymi czynnikiem wywołującymi ból były gwałtowne ruchy obrotowe (43.75%), kolizja z przeciwnikiem (18.75%) i uderzenie piłką (12.5%) (p<0.01). „Pain exposure” physical therapy (PEPT) is a new treatment for patients with complex regional pain syndrome type 1 (CRPS-1) that consists of a progressive-loading exercise program and management of painavoidance behavior without the use of specific CRPS-1 medication or analgesics. The aim of this study was to investigate primarily whether PEPT could be applied safely in patients with CRPS-1. Twenty patients with CRPS-1 were consecutively enrolled in the study after giving informed consent. The diagnosis of CRPS-1 was defined using the Bruehl and Harden/IASP diagnostic criteria. CRPS-1 was diagnosed between 3 and 18 months after the inciting event (trauma). According to a multiple single-case design (baseline [A1], treatment [B], follow-up [A2]), multiple baseline and follow-up measurements were performed to evaluate changes in CRPS signs and symptoms and to assess functional parameters. When comparing the baseline with the follow-up phase, patients improved significantly with respect to pain on the visual analogue scale (57%), pain intensity (48%), muscle strength (52%), arm/shoulder/hand disability (36%), 10-meter walking speed (29%), pain disability index (60%), kinesiophobia (18%), and the domains of perceived health change in the SF-36 survey (269%). Three patients initially showed increased vegetative signs but improved in all other CRPS parameters and showed good functional recovery at follow-up. We conclude that PEPT is a safe and effective treatment for patients with CRPS-1.

Fizjoterapia polegająca na narażeniu na ból („pain exposure” physical therapy, PEPT) to nowa metoda leczenia chorych na wieloobjawowy miejscowy zespół bólowy typu 1 (CRPS-1), która polega na programie ćwiczeń fizycznych o stopniowego zwiększanym obciążeniu i opanowywaniem zachowania polegającego na unikaniu bólu bez używania leków swoistych dla CRPS-1 i bez leków przeciwbólowych. Celem badania było przede wszystkim sprawdzenie, czy PEPT mogłaby być stosowana w bezpieczny sposób u chorych na CRPS-1. Po uzyskaniu świadomej zgody do badania włączono 20 kolejnych chorych na CRPS-1. Rozpoznanie CRPS-1 było ustalane na podstawie kryteriów diagnostycznych Bruehl i Harden/IASP. CRPS-1 rozpoznawano po 3-18 miesiącach od pierwotnego zdarzenia (urazu). W układzie wielokrotnej obserwacji poszczególnych przypadków (w punkcie początkowym, podczas leczenia i w okresie obserwacji) dokonano wielokrotnych pomiarów w punkcie początkowym i w okresie obserwacji w celu oceny zmian nasilenia objawów i dolegliwości CRPS i parametrów czynnościowych. W okresie obserwacji stwierdzono istotną poprawę w porównaniu z punktem początkowym w nasileniu bólu we wzrokowej skali analogowej (57%), intensywności bólu (48%), sile mięśni (52%), niesprawności ramienia, barku lub ręki (36%), szybkości chodzenia na odcinku 10 metrów (29%), wskaźnika niesprawności wynikającej z bólu (60%), kinezjofobii (18%) oraz w częściach kwestionariusza SF-36 dotyczących odczuwanej zmiany stanu zdrowia (269%). Trzech pacjentów wykazywało początkowo zwiększone nasilenie objawów wegetatywnych, ale jednocześnie poprawę w zakresie wszystkich pozostałych parametrów CRPS; stwierdzono u nich dobrą poprawę czynnościową w okresie obserwacji. Wnioskujemy, że PEPT jest bezpieczną i skuteczną metodą leczenia chorych na CRPS-1.
Introduction. Pain in cancer diseases is a significant factor increasing suffering, anxiety and sufferer's dependence on other people. The stronger the pain, the more dependent the patients are. They require more help in everyday activities. Nurses play the key role in recognition and controlling the pain since they spend a considerable amount of time with patients in hospice. Aim. The aim of this study was to assess the knowledge of nurses on pain treatment delivered in training on methods of pain treatment. Material and method. The study used questionnaire carried out among 50 nurses working in Nursing Home facility in Kraszewo Czubaki. The study was conducted with the use of diagnostic survey. Results. More than 84% of respondents participating in training claim that they are familiar with the methods of pain treatment. However, 83% of respondents, who have not participated in any post-graduate training on methods of pain treatment, do not have sufficient knowledge about methods of pain treatment. Conclusions. The study showed that training has significant influence on the knowledge about methods of pain treatment used by nurses working in hospice.

Wstęp. Ból w chorobie nowotworowej jest znaczącym czynnikiem zwiększającym cierpienie, niepokój i zależność chorego od innych osób. Im dolegliwości bólowe są silniejsze, tym pacjent jest mniej samodzielny i wymaga większego wsparcia w prostych czynnościach dnia codziennego. Zasadniczą rolę w procesie rozpoznawania i kontrolowania bólu odgrywa pielęgniarka, która spędza w hospicjum z pacjentem najwięcej czasu. Cel. Celem pracy była ocena wiedzy pielęgniarek uzyskanej w czasie szkoleń w zakresie metod leczenia bólu. Materiały i metody. Materiał do analizy stanowiły ankiety przeprowadzone wśród 50 pielęgniarek pracujących w Zakładzie Opiekuńczo Leczniczym Kraszewo Czubaki. Badanie zostało przeprowadzone z wykorzystaniem metody sondażu diagnostycznego. Wyniki. Ponad 84% osób uczestniczących w szkoleniach twierdzi, że zna metody leczenia bólu. Natomiast ponad 83% badanych, które nie brały udziału w formach kształcenia podyplomowego przyznaje, że nie posiada wiedzy w zakresie terapii bólu. Wnioski. W wyniku przeprowadzonych badań ustalono istotny wpływ szkoleń na znajomość metod leczenia bólu przez pielęgniarki pracujące w hospicjum.
Introduction: Labor pain is a physiological phenomenon and demonstrates progression of the labor. Feeling pain, also labor pain, is strongly individualized internal experience. Some parturients do not need any pharmacological intervention, for others labor pain is so strong that it requires a medical intervention. People also differ in terms of beliefs about the ability to control pain. These beliefs affect their attitudes towards pain sensations and expectations of medical interventions aimed at pain relief. The aim of this study was to determine whether regular physical activity during pregnancy varies beliefs about control over labor pain and to establish determinants of these beliefs and their correlates. Material and methods: 100 pregnant women aged 19-38 years, including 50 women who regularly practiced physical exercise during pregnancy and 50 pregnant physically inactive took part in this study. Modified “Beliefs About Pain Control Questionnaire” (BPCQ) Skevington’s, containing 11 statements about labor pain control, a questionnaire on obstetric history and personal situation, and Inventory of State and Trait Anxiety by Spielberger et al. were used in this study. Results: Results indicate that physically inactive pregnant women are more strongly convinced than women taking physical activity during pregnancy, that labor pain intensity depends on external factors (medical assistance, random factors). Trait anxiety, financial situation, previous miscarriage and physical activity are predictors of beliefs about labor pain control. Conclusions: Pregnant women physically active during pregnancy reveal stronger beliefs about personal control in labor pain, indicating a more internal location of the control. It is expected that those women will present active attitude during childbirth, cooperating with medical personnel, those women will less expect medical intervention also those targeted on pain relief for to labor acceleration.
Wstęp: Mózgowe porażenie dziecięce (MPDz) jest zespołem, w którym zaburzenia ruchu i postawy mają charakter postępujący, przy stacjonarnym charakterze uszkodzenia ośrodkowego układu nerwowego. Pomimo heterogeniczności obrazu klinicznego i stanu funkcjonalnego pacjenci z MPDz stanowią grupę obarczoną wysokim ryzykiem występowania różnorodnego bólu. Cel: Celem pracy było dokonanie przeglądu piśmiennictwa dotyczącego zjawiska bólu w przebiegu mózgowego porażenia dziecięcego. Podjęto próbę przedstawienia charakterystyki zjawiska bólu, jak i zebrania narzędzi służących do jego oceny. Metodyka: Dokonano przeglądu piśmiennictwa polskiego i anglojęzycznego, w którym zostało omówione zagadnienie bólu w przebiegu mózgowego porażenia dziecięcego (pod kątem epidemiologii, charakterystyki oraz metod oceny). Analizowano prace badawcze i przeglądowe z ostatnich 15 lat zamieszczone w bazach danych (PubMed, Medline i stronach polskich wydawnictw medycznych). Prace wyszukiwano posługując się słowami kluczowymi: mózgowe porażenie dziecięce, upośledzenie umysłowe, ból, skale oceny bólu. Wyniki i wnioski: Osoby z mózgowym porażeniem dziecięcym – ze względu na ich chroniczne zaburzenia zdrowotne i stosowane zabiegi diagnostyczno-lecznicze oraz fizjoterapeutyczne –narażone są na doznawanie ostrego i przewlekłego bólu. Istnieje potrzeba poszerzenia wiedzy na temat występowania zjawiska bólu u pacjentów z MPDz. Dla optymalnego prowadzenia terapii tej grupy pacjentów niezbędna jest kompleksowa i wielopłaszczyznowa współpraca lekarzy, fizjoterapeutów, psychologów oraz pacjentów i ich bezpośrednich opiekunów. Specjaliści pracujący z pacjentami z MPDz powinni znać potencjalne przyczyny występowania bólu, posiadać narzędzia oceny oraz dysponować umiejętnościami, a przede wszystkim chęcią ich zastosowania.

Cerebral palsy (CP) is a syndrome in which movement and posture disorders are progressive, along with the stationary nature of damage to the central nervous system. Despite the heterogeneity of clinical and functional status, patients with CP are a group of patients at high-risk of the occurrence of diverse pain. The aim of this study was to review literature on the phenomenon of pain in the course of cerebral palsy. An attempt was made to present the characteristics of the phenomenon of pain, and gather tools for its evaluation. A review of Polish and English language literature, in which the issue of pain in the course of cerebral palsy was discussed (in terms of epidemiology, characteristics and evaluation methods). Research papers and reports from the past 15 years included in the databases: PubMed, Medline and websites of Polish medical publishing houses, were analyzed. The works were searched using the key words: cerebral palsy, mental retardation, pain, pain assessment scales. Patients with cerebral palsy – due to their chronic health problems and performed diagnostic-treatment and physiotherapeutic treatments – are at risk of experiencing acute and chronic pain. There is a need to improve knowledge regarding the phenomenon of pain in patients with CP. The optimal course of therapy in this group of patients requires comprehensive and multi-faceted co-operation of doctors, physiotherapists, psychologists and patients as well as their immediate caregivers. The combined oral contraceptive pill (COCP) has been implicated in the development of a number of chronic pain conditions. Modern COCP formulations produce a low endogenous estradiol, low progesterone environment similar to the early follicular phase of the natural menstrual cycle, with a variable effect on serum androgen levels. We used behavioural measures and functional magnetic resonance imaging to investigate the response to experimental thermal stimuli in healthy women, in both a natural and COCPinduced low endogenous estradiol state, to investigate whether alterations in central pain processing may underlie these observations in COCP users. Although COCP users overall did not require lower temperatures to obtain a fixed pain intensity, alterations in the brain response to these stimuli were observed. In a subgroup of COCP users with significantly reduced serum testosterone, however, lower temperatures were required. Region-of-interest analysis revealed that within key regions of the descending pain inhibitory system, activity in response to noxious stimulation varied with serum testosterone levels in both groups of women. Of particular interest, in COCP users, activity in the rostral ventromedial medulla increased with increasing testosterone and in those women with low testosterone, was significantly reduced compared to controls. These findings suggest that, in a low endogenous estradiol state, testosterone may be a key factor in modulating pain sensitivity via descending pathways. Specifically, failure to engage descending inhibition at the level of the rostral ventromedial medulla may be responsible for the reduction in temperature required by COCP users with low circulating testosterone.

Stosowanie złożonych doustnych środków antykoncepcyjnych (ZDŚA) wiązano z rozwojem wielu zespołów bólu przewlekłego. Nowoczesne postacie ZDŚA wywołują stan małego stężenia endogennego estradiolu i małego stężenia progesteronu, zbliżony do wczesnej fazy pęcherzykowej naturalnego cyklu miesięcznego; w zróżnicowany sposób wpływają na stężenia androgenów w surowicy. Wykorzystaliśmy miary behawioralne i obrazowanie za pomocą czynnościowego rezonansu magnetycznego do zbadania reakcji na eksperymentalne bodźce cieplne u zdrowych kobiet w stanie małego stężenia endogennego estradiolu (naturalnym i wywołanym ZDŚA) aby zbadać, czy zmiany w zakresie ośrodkowego przetwarzania bólu mogą leżeć u podłoża tych obserwacji u osób stosujących ZDŚA. Chociaż osoby stosujące ZDŚA ogólnie biorąc nie wymagały niższych temperatur do uzyskania ustalonego nasilenia bólu, obserwowano zmiany w zakresie reakcji mózgu na te bodźce. W podgrupie osób stosujących ZDŚA, u których stężenie testosteronu w surowicy było zmniejszone, wymagane temperatury bodźca były niższe. Analiza obszarów zainteresowania wykazała, że w obrębie kluczowych obszarów zstępującego układu hamującego ból aktywność w reakcji na bodziec bólowy różniła się w zależności od stężenia testosteronu w surowicy w obu badanych grupach kobiet. Szczególnie interesujące było stwierdzenie, że u osób stosujących ZDŚA aktywność dogłowowej części brzuszno-przyśrodkowego obszaru rdzenia przedłużonego zwiększała się wraz ze stężeniem testosteronu, a u kobiet z małym stężeniem testosteronu była istotnie zmniejszona w porównaniu z grupą kontrolną. Wyniki te wskazują, że w stanie małego stężenia endogennego estradiolu testosteron może być kluczowym czynnikiem modulującym wrażliwość na ból za pośrednictwem dróg zstępujących. Dokładniej zaś niezdolność do angażowania zstępującego hamowania na poziomie dogłowowej części brzuszno-przyśrodkowego obszaru rdzenia przedłużonego może odpowiadać za niższą temperaturę wymaganą do wywołania odczucia bólu o określonym nasileniu u osób stosujących ZDŚA i z małym stężeniem krążącego testosteronu.
Introduction: The disc disease of the lumbosacral spine is frequently occurs whose the main cause is the intervertebral disc degeneration. Pain in the lower back section of the spine and a considerable impairment of physical fitness is the main ailment. It is a chronic disease with periodic recurrent episodes of intense pain. Coping with the disease means, among other things, reinforcement perivertebral muscles through regular physical exercise. It is assumed that sociodemographic factors will be related to undertaking health-promoting actions in the form of physical activity adjusted to the degenerative disc disease of the lumbosacral spine. The aim of the paper was to answer the question whether demographic variables have any connection with undertaking physical activity adjusted to the disease of the lower back section of the spine. Material and methods: The study was a longitudinal type of research (two measurement points at half yearly interval) and it covered 92 people in whom the disc disease of the lumbosacral spine was found by means of MRI scan. The study used an interview (the sociodemographic data), the eleven-point VAS scale, and a questionnaire concerned undertaken phisical activity. Results: The patients showed a low level of physical activity undertaken due to the disease of the spine (M=2.33 SD=2.21). A dependency between younger age (H(2, N=92)=4.5, p<0.08) and higher education (H(3, N=91)=16.62,p=0.001) and undertaking physical activity adjusted to the disease of the lower back section of the spine was noticed. Conclusions: The people who took part in the study undertook little physical activity due to the spine disease. Age and education are related to the undertaking of physical activity adjusted to the disease of the lower back section of the spine.

The disease impairs not only physical condition of patients but also their social and psychological life. It is a chronic illness, therefore it requires changing of one’s unhealthy lifestyle to healthy living on a permanent and regular basis. One of manifestations of a health-promoting attitude in the disc disease is the performance of regular exercise chosen for the ailment in order to reinforce perivertebral muscles. Physical activity is both treatment and prevention in the disc disease of the lumbosacral spine. As there is an opinion that pain experienced by the ill people is the main motivating factor for treating the disc disease of the lumbosacral spine, the authors decided to investigate whether there is a relation between pain felt and physical activity undertaken in people with the disc disease of the lumbosacral spine and whether the relation is modified by sociodemographic variables. Material and methods: The study was a longitudinal type of research (two measurement points at half yearly interval) and it covered 92 people who suffer from disc disease of the lumbosacral spine. The diagnosis was found by MRI scan. The study used an interview (the sociodemographic data), the VAS scale, and a questionnaire on the indicator of healthpromoting attitude in the disc disease specially prepared for the purpose of this project. Results: For the group of people with the disc disease of the lumbosacral spine, no relation was found between the pain felt and the physical activity undertaken. A positive relation between the pain felt and the physical activity was found only for the group of younger patients. Conclusions: The pain felt may be the motivating factor for undertaking regular physical exercise only for the group of the youngest patients examined.
Choroba zwyrodnieniowa jest najczęstszą chorobą stawów. Liczba chorych cierpiących z powodu bólu stawów i pogłębiającej się niepełnosprawności rośnie, zwłaszcza w krajach wysoko rozwiniętych. Według danych WHO jest obecnie czwartą co do częstości przyczyną niepełnosprawności u kobiet i ósmą u mężczyzn. Jest także główną przyczyną bólu i pogorszenia jakości życia chorych, znajdując się jednocześnie wśród 10 chorób na świecie generujących najwyższe koszty społeczne. Ból oraz niepełnosprawność, które prowadzą często do trwałego kalectwa niosą za sobą wiele negatywnych konsekwencji nie tylko fizycznych, ale także psychologicznych i społecznych. Poczucie osamotnienia, bezradności, nieprzydatności, a także lęk oraz objawy depresji w znaczącym stopniu wpływają na jakość życia chorych, którą należy postrzegać wieloaspektowo w celu osiągnięcia oczekiwanych sukcesów w leczeniu. Podkreślana przez wielu autorów potrzeba holistycznego prowadzenia często wieloletniej terapii choroby somatycznej, która wymaga leczenia współwystępujących zaburzeń psychicznych (lęk, depresja i in.), w praktyce często nie jest stosowana – nawet jeśli zaburzenia te zostaną właściwie rozpoznane. Tymczasem sprawna komunikacja pacjenta z lekarzem jest niespecyficznym czynnikiem leczącym, który wpływa na motywację chorego do leczenia oraz pełne stosowanie się do zaleceń. Niestety, pomimo rozwoju wiedzy na temat współwystępowania zaburzeń psychicznych z chorobami przewlekłymi (w tym w chorobie zwyrodnieniowej stawów) oraz wdrażania jej do standardów postępowania, często w praktyce zagadnienia te traktowane są marginesowo – zarówno przez terapeutów, jak i samych pacjentów.

Osteoarthritis is the commonest condition affecting joints. The number of patients suffering from joint pain and progressive disability has been growing, especially in the most developed countries. It is also the major cause of pain and deterioration of the patient’s quality of life, being among the 10 diseases worldwide, generating the highest social costs. Pain and disability, often leading to permanent disability entail numerous adverse consequences, not only of physical ones but also of psychological and social nature. The feeling of loneliness, helplessness and uselessness as well as anxiety and depression symptoms significantly affect the patient’s quality of life which should be perceived in multiple aspects in order to achieve the expected success in treatment. The need for holistic approach to frequently long-lasting treatment of a somatic condition, requiring treatment of concomitant mental disorders (anxiety, depression, etc.) is rarely implemented in practice, even if the disorders are properly diagnosed. However, good communication between the patient and the physician is a nonspecific factor in the course of treatment, which affects the patient’s motivation to undergo treatment and full adherence to physician’s recommendation. Regretfully. Despite the progress in the knowledge of the incidence of mental disorders and chronic conditions (including osteoarthritis) and implementation of this knowledge to standard medical procedures, in practice these recommendations are marginally followed, both by the therapists and patients themselves.
The Functional Disability Inventory (FDI; Walker LS, Greene JW. The functional disability inventory: measuring a neglected dimension of child health status. J Pediatr Psychol 1991;16:39-58) assesses activity limitations in children and adolescents with a variety of pediatric conditions. This study evaluated the psychometric properties of the FDI in pediatric pain patients. Participants included 596 patients with chronic abdominal pain, ages 8-17, and a subset of their parents (n=151) who completed the FDI and measures of pain, limitations in school activities, and somatic and depressive symptoms at a clinic visit. Test-retest reliability was high at 2 weeks (child report, .74; parentreport,.64) and moderate at 3 months (child report, 0.48; parent report, 0.39). Internal consistency reliability was excellent, ranging from .86 to .91. Validity was supported by significant correlations of child- and parent-report FDI scores with measures of school-related disability, pain, and somatic symptoms. Study results add to a growing body of empirical literature supporting the reliability and validity of the FDI for functional assessment of pediatric patients with chronic pain.

Inwentarz Niepełnosprawności Czynnościowej (ang. Functional Disability Inventory - FDI; [31] Walker LS, Greene JW. Inwentarz niepełnosprawności czynnościowej: pomiar zaniedbywanego aspektu stanu zdrowia dziecka. J Pediatr Psychol 1991;16:39-58) ocenia ograniczenia aktywności dzieci i nastolatków z różnymi schorzeniami pediatrycznymi. Badanie oceniało psychometryczne własności FDI u pediatrycznych pacjentów z bólem. Uczestniczyło w nim 596 pacjentów z przewlekłym bólem brzucha w wieku 8-17 lat, oraz podgrupa ich rodziców (n=151), którzy podczas wizyty w klinice wypełniali FDI oraz udzielali informacji na temat stopnia intensywności bólu, ograniczeń w zajęciach szkolnych oraz objawów somatycznych i depresyjnych. Niezawodność pomiaru powtarzanego była wysoka po upływie 2 tygodni (relacja dziecka, 0,74; relacja rodzica, 0,64) i umiarkowana po upływie 3 miesięcy (relacja dziecka, 0,48; relacja rodzica, 0,39). Spójność wewnętrzna była doskonała i wahała się między 0,86 a 0,91. Trafność potwierdziła znacząca korelacja punktacji FDI w relacjach dzieci i rodziców z pomiarami niepełnosprawności szkolnej, bólu i objawów somatycznych. Wyniki badania powiększają rosnący zbiór empirycznej literatury potwierdzającej niezawodność i trafność FDI w czynnościowej ocenie pediatrycznych pacjentów z przewlekłym bólem.
