Cognitive problems and a deepening dependence on one’s immediate environment inherently accompany Duchenne Muscular Dystrophy (DMD). The disease is progressive, and the size of the dystrophin gene determines the extraordinary complexity of the causes of this disease at the genetic and molecular level. The aim of the study was to characterize the cognitive problems and the extent of independence of patients with genetically confirmed DMD. An attempt was also made to reconstruct the patient's life history in three periods: before the appearance of the first symptoms, during the search for a diagnosis and after confirming the diagnosis of DMD. The study group consisted of male patients between 10 and 13 years of age (N = 14). The Diagnosis of Cognitive Functions Battery - PU1 and an experimental tool for studying autonomy were used. Information on the condition of the attention, memory and executive functions of patients was obtained. The study of autonomy measurably supplemented knowledge in terms of the degree of the patients’ dependence on the environment with regard to everyday functioning. The best functioning component of attention in the examined patients is orientation (o) (13 patients achieved average results in this aspect). Selectivity (s) turned out to be the weakest component, as only five patients obtained average results (the others obtained low results) in this aspect. Autonomy results (AU) indicate group diversity and inter-subject variability in the disease progression (subjects scored from 6 to 47 points). The clinical picture of the disease is not homogeneous. Patients, despite their similar age, differ in the progression of the disease and the resulting effects. This induces the need for an individual approach to each patient and the preparation of a unique set of therapeutic interactions for each of them.
The study concerns deficits in attention in adults with symptoms of hyperkinetic syndrome. The subject of measure was speed and accuracy of detection the stimuli physically, semantically and emotionally congruent to a given criterion and the accuracy of ignoring the stimuli which did not meet this criterion. Experiment has been conducted with 30 subjects with ADHD syndrome and 30 control subjects without any symptoms of deficits. It has been demonstrated that subjects with hyperkinetic syndrome less accurate and slower detect stimuli congruent with a criterion, and this effect is more apparent in the case of emotional stimuli. Differences has also been emerged in ignoring stimuli which not meet the given criteria. However it has been shown that differences in performance between groups fade in the next trial, what can be interpreted in the terms of efficient mechanism of skill acquisition in subjects with hyperkinetic syndrome.
Stroke is a sudden-onset neurological deficit resulting from focal vascular lesions. This is either due to a clot-induced obstruction of a vessel (ischemic stroke) or a rupture of a vessel causing haemorrhage (hemorrhagic stroke). The appropriate diagnosis of brain stroke aphasic patients is a major public health problem one so important for effective rehabilitation. Here an important role is played by the diagnosis of impaired cognitive processes. The aim of the research was to find the index of impaired cognitive control with the use of ERPs in a patient following an ischemic stroke with aphasia. A male patient, aged 69, after an ischemic brain stroke experienced 4 months previously with resulting naming problem, was admitted to the Reintegrative and Teaching Center of the Polish Neuropsychological Society. In the neuropsychological evaluation three neuropsychological tests were employed: (a) the Boston Naming Test-Polish version, to evaluate the naming and word finding problem; (b) the nonverbal Bell test to ascertain a selective, visuospatial and strategic attentional evaluation; (c) the Digit Memory test to evaluate working memory capabilities; (d) ERPs as a neurophysiological index of impaired cognitive control. Significant changes were observed in testing. All cognitive functions including naming, nonverbal, visuospatial and strategic attention, along with the digit memory deviate substantially from the norm. The patient obtained a lo wer score, compared to the norm (p <0.05). Boston Naming Test (patient = 21< Mean-Norm = 57.29; SD= 0.52). Bell test (patient = 23 < Mean-Norm = 31.29; SD= 2.52). Individuals in the early stages of dementia may demonstrate language difficulties. The aim of the study was an evaluation of the differences in narrative discourse abilities across two types of dementia, i.e., Vascular Dementia (VaD) and Alzheimer’s Disease (AD) in comparison to the young and old elderly. The AD and VaD groups displayed a lower performance than the age-matched YE on tasks involving reasoning. The VaD partici- pants outperformed patients with AD in verbal memory and narrative discourse. Discourse macrostructure analyses showed that the VaD reproduced more propositions than did the AD participants, but that these were comparable to YE and OE. There were more conjunctions in narratives reproduced by the VaD participants as compared to other groups, although this tendency was only present in the story but not in fairy tale reproductions themselves. Individ- uals in the AD group had more difficulties than YE and OE individuals in figuring out the moral of fairy tales. Clinical and control groups reproduced the microstructure and superstructure of texts comparatively well. Discourse recall correlated with performance on verbal memory, attention/working memory, and reasoning. Differences in narrative discourse abilities were found. Alzheimer’s Disease (AD) patients scored lower in verbal memory than did Vascular Dementia (VaD) patients. Both groups however obtained lower results than the young and old elderly.
This paper discussed a case study of a 15-year-old girl who had had a hemorrhagic stroke, caused by arteriovenous malformation. A brain arteriovenous malformation is a defect in the vascular system, a tangle of abnormal blood vessels connecting arteries and veins in the brain. The patient attended regularly neuropsychological rehabilitation and several various therapeutic methods were applied. A neuropsychological diagnosis was performed before and after therapy. The first assessment showed some cognitive disturbances: difficulties in the organization of memorized material, difficulties in maintaining attention and deficits in abstract thinking. Furthermore there were observed emotional problems: depression and high perfectionism. The patient participated in individual therapy lasting one year. Neuropsychological rehabilitation took into consideration the personal psychological resources and cognitive and emotional difficulties. As a result, there was observed an improvement in consequent cognitive functions: organization of memorized material, working memory, abstract thinking and verbal fluency. The emotional state was stable. It is important that the therapy should be holistic and includes all aspects of the patient’s functioning, which may have an influence on the final result of rehabilitation. One of the most important aims of the therapy psychologist was an acceptance of the disease. The cognitive tasks can also include psychological and therapeutic exercises.
Purpose. This study investigated if (1) the beneficial effects of an external focus of attention on learning a motor skill were influenced by an internal focus of attention provided at initial instruction (2) or by an internal focus of attention at the early stage of the acquisition phase and (3) their relation to the automation hypothesis. Methods. Three separate experiments were performed with 168 college students on the acquisition, transfer, and retention of a golf-putting task. Results. In conjunction, the results of the three experiments pointed to the positive effects of an internal attention of focus instructions followed by an external attentional focus on motor learning. Conclusions. These results support the development of an alternative hypothesis on the effects of attentional focus on motor skill acquisition.
Interferon beta is the most widely prescribed therapy for the multiple sclerosis (MS), positively modifying the disease course. The aim of this study was to examine effects of long-term treatment with interferon beta-1b (8 mln. IU every other day, for 24 months) on the nonverbal learning and sustained visual attention in MS patients. Nineteen relapsing-remitting patients were included. The mean age was 30.3 (18-43) years, mean disease duration and mean EDSS scores were 3.2 (0.5-7.0) years and 2.05 (0.5-5.0), respectively. The patients underwent psychological testing three times: before interferon beta-1b treatment, 12 and 24 months later. The Nonverbal Learning Test, the Visual Continuous Attention Test and Beck Depression Inventory were used. Nineteen matched (age, sex, education, EDSS scors, disease duration) control subjects, also with relapsingremitting MS, not reciving interferon beta, underwent the same testing procedure. Before interferon beta-1b treatment there were no significant differences among examined and control groups on any tests. 24 months treatment with interferon beta-1b positively influenced nonverbal learning (p<0.05), and did not change sustained attention and mood. Treatment with interferon beta-1b reduced MS attack frequency (p<0.05) and had beneficial (statistically unsignificant) effect on disability assessed by the EDSS. Serious adverse events were not observed in treated patients. Conclusions: 1. Treatment with interferon beta-1b positively influences nonverbal learning. 2. During interferon beta-1b therapy were not observed significant changes in sustained attention and mood.

Interferon beta jest najczęściej stosowanym lekiem łagodzącym przebieg kliniczny stwardnienia rozsianego (SM). Celem tej pracy była ocena wpływu przewlekłego leczenia interferonem beta-1b (8 mln. j.m. podawane co drugi dzień, przez 24 miesiące) na uczenie się niewerbalne i ciągłość uwagi wzrokowej u chorych na stwardnienie rozsiane. Zbadano 19 pacjentów z postacią rzutowo-remisyjną SM. Średni wiek chorych wynosił 30,3 (18-43) lata, średni czas trwania choroby – 3,2 (0,5-7,0) lata, średnia wartość skali EDSS – 2,05 (0,5-5,0) punktu. Pacjentów poddano 3-krotnie (przed rozpoczęciem leczenia interferonem, po upływie 12 i 24 miesięcy) badaniu psychologicznemu. Wykorzystano: test uczenia się niewerbalnego (NVLT), test ciągłości uwagi (DAUF) i Inwentarz Depresji Becka. Grupę kontrolą stanowiło 19 chorych również z postacią rzutowo-remisyjną SM, odpowiednio dobranych pod względem wieku, płci, wykształcenia, stopnia niesprawności (EDSS) i czasu trwania choroby, nieleczonych interferonem beta, których poddano takiej samej procedurze badawczej. Wyniki osiągnięte przed leczeniem interferonem beta-1b przez pacjentów z grupy badanej i kontrolnej nie różniły się istotnie w żadnym z testów. Trwające 24 miesiące leczenie interferonem beta-1b korzystnie wpłynęło na uczenie się niewerbalne (p<0,05), nie zmieniając istotnie ciągłości uwagi ani nastroju. Terapia zmniejszała częstość rzutów choroby (p<0,05) oraz miała korzystny wpływ na narastanie niesprawności ocenianej w skali EDSS (brak istotności statystycznej). Nie obserwowano poważnych działań niepożądanych w grupie chorych leczonych interferonem beta-1b. Wnioski: 1. Leczenie interferonem beta-1b wpływa korzystnie na niewerbalne uczenie się. 2. W trakcie terapii interferonem beta-1b nie obserwowano istotnych zmian ciągłości uwagi ani nastroju.
Objective: The objective of this study is to elucidate the specific nature of attention and response inhibition deficits in three clinical groups: attention-deficit/hyperactivity disorder, oppositional defiant disorder, and high-functioning autism, as compared to children with a typical development. The analysis approached task performance dynamics as a function of time and the presence of distracting stimuli. Material and method: 108 children aged 7–12 years participated in the study – 21 diagnosed with oppositional defiant disorder, 21 with high-functioning autism, 19 with attention-deficit/hyperactivity disorder; 47 made the control group. The study employed the MOXO-CPT to evaluate attention and inhibition functions. Results: Pairwise comparisons of clinical groups with typically-developing children in their performance on the entire test indicated considerable differences between the control group and children with both oppositional defiant disorders and attention-deficit/hyperactivity disorder, but not between healthy subjects and children with autism. Performance profiles varied depending on the group, i.e. the type of disorder, and the level of the test, i.e. stimulus duration and intensity, but they were different for the particular studied aspects of attention and/or inhibition. High levels of similarity in functioning for all clinical groups were found in the measures of response accuracy, i.e. sustained attention and the speed of accurate response. The tendency to provide unnecessary responses and difficulties in complying with rules were found only in children with oppositional-defiant disorders. Impulsiveness rates increased over time in the attention-deficit/hyperactivity disorder group, fluctuated over time in autism, while in the oppositional defiant disorder group performance was stable over time, but worse than in the control group. Conclusions: The dynamics of attentional and inhibitory control in clinical groups differs considerably in comparison to typically-developing children. The most substantial differences between clinical groups are observed in the inhibition indices.

Cel: Celem podjętych badań było poszukiwanie specyfiki deficytów funkcjonowania uwagi i hamowania w trzech grupach klinicznych: zespole nadpobudliwości psychoruchowej z deficytem uwagi, zaburzeniu opozycyjno-buntowniczym i autyzmie wysokofunkcjonującym w porównaniu z dziećmi typowo rozwijającymi się. Analizowano dynamikę wykonania zadań w kontekście efektu zmiennej czasu oraz obecności bodźców dystrakcyjnych. Materiał i metoda: Zbadano 108 dzieci w wieku 7;00–12;00 lat, w tym 21 z zaburzeniem opozycyjno-buntowniczym, 21 z autyzmem wysokofunkcjonującym, 19 z zespołem nadpobudliwości psychoruchowej z deficytem uwagi i 47 z grupy kontrolnej. Do oceny aspektów uwagi i hamowania zastosowano test MOXO-CPT. Wyniki: We wskaźnikach wykonania całego testu porównania parami grup klinicznych z dziećmi zdrowymi wykazały istotne różnice pomiędzy grupą kontrolną a dziećmi z zaburzeniami opozycyjno- -buntowniczymi i zespołem nadpobudliwości psychoruchowej z deficytem uwagi, natomiast pomiędzy dziećmi zdrowymi a dziećmi z autyzmem nie stwierdzono różnic. Istnieje zróżnicowanie profili wykonań w zależności od grupy, czyli rodzaju zaburzenia oraz od etapu testu, czyli czasu i jakości dystraktora, ale zależy to od badanego aspektu uwagi i/lub hamowania. Wysokie podobieństwo funkcjonowania we wszystkich grupach klinicznych uzyskano w miarach poprawności reakcji, czyli podtrzymywaniu uwagi oraz szybkości poprawnej reakcji. Tendencja do bezcelowego reagowania i trudności w przestrzeganiu reguł pojawiły się tylko u dzieci z zaburzeniami opozycyjno-buntowniczymi. W grupie dzieci z zespołem nadpobudliwości psychoruchowej z deficytem uwagi impulsywność nasilała się z czasem, w autyzmie była zróżnicowana w czasie, a w grupie dzieci z zaburzeniami opozycyjno-buntowniczymi wykonanie było stabilne w czasie, ale gorsze niż w grupie kontrolnej. Wnioski: Grupy kliniczne różnią się istotnie w dynamice funkcjonowania uwagi i hamowania od dzieci typowo rozwijających się. Największe różnice pomiędzy grupami klinicznymi widoczne są we wskaźnikach hamowania.
Nowadays, more and more scientific reports highlight the importance of cognitive skills in motor control. It is believed that movement also engages higher mental processes such as executive functions, attention and working memory. Executive functions include cognitive processes, such as the ability to initiate, plan, modify and control behaviour. They play an integrative role in the processing of information, including both cognitive and behavioural elements, necessary for goal-directed and effective action. Executive functions play a key role in the regulation of gait in the case of taking new steps or modifying previously learned motor programmes. Neuroimaging studies show that there is a common pattern of neural activity for walking, executive functions and attention, involving the frontal cortex and cortico-subcortical neuronal network. Many studies have shown that the impairment of executive functions may contribute to gait disturbances and increased risk of falls. Executive functions allow movement patterns to be modified, and enable the introduction of adaptive compensatory strategies in response to changing internal and external environmental stimuli. Studies with dual-task paradigm also stress the importance of attention in maintaining the control over gait. The addition of a cognitive task slows gait in the elderly. The incidence of falls is higher in subjects with dementia. The identification of cognitive risk factors of falls may allow more effective diagnostic and therapeutic methods to be developed. The aim of this study was to elucidate the relationship between cognitive function, i.e. executive functions and attention, and the risk of falls.

Współcześnie powstaje coraz więcej doniesień naukowych podkreślających znaczenie sprawności funkcji poznawczych w kontroli motorycznej. Uważa się, że chód angażuje także wyższe procesy psychiczne: funkcje wykonawcze, uwagę i pamięć operacyjną. Funkcje wykonawcze obejmują takie procesy poznawcze, jak zdolność do inicjowania, planowania, modyfikowania i kontroli zachowania. Pełnią integracyjną funkcję w przetwarzaniu informacji – obejmują elementy poznawcze i behawioralne niezbędne do skutecznego działania, zorientowanego na cel. Funkcje wykonawcze odgrywają kluczową rolę w regulacji chodu w przypadku podejmowania nowych czynności bądź modyfikowania uprzednio wyuczonych programów ruchowych. Jak dowodzą badania neuroobrazowe, dla chodu, funkcji wykonawczych i uwagi istnieje wspólny wzorzec aktywności neuronalnej, obejmujący korę czołową i jej korowo-podkorową sieć neuronalną. W wielu badaniach wykazano, że osłabienie funkcji wykonawczych może przyczyniać się do powstawania zaburzeń chodu i zwiększać ryzyko upadków. Funkcje wykonawcze umożliwiają modyfikowanie wzorców ruchowych, jak również wprowadzanie adaptacyjnych strategii kompensacyjnych w odpowiedzi na zmieniające się bodźce – pochodzące ze środowiska zarówno wewnętrznego, jak i zewnętrznego. Badania w paradygmacie podwójnego zadania podkreślają też znaczenie uwagi w utrzymaniu kontroli chodu. Dodanie zadania angażującego funkcje poznawcze przekłada się w grupie ludzi starszych na spowolnienie chodu. Częstość upadków jest wyższa u osób z otępieniem. Identyfikacja poznawczych czynników ryzyka upadków może pozwolić na opracowanie skuteczniejszych metod diagnostycznych i terapeutycznych. Celem niniejszej pracy jest próba wyjaśnienia mechanizmów relacji między funkcjami poznawczymi – funkcjami wykonawczymi i uwagą – a ryzykiem upadków.
The study was aimed at finding out whether at the early school age the effectiveness of executive functions distinguishes children with ADHD from those of the control group. Besides, the aim was to check to what extent the use of diagnostic methods evaluating executive functions in children at the early school age is justified. The analysis comprised cognitive flexibility, sustained attention, interference control and planning ability. Those methods of neuropsychological evaluation were used which are mostly applied to characterize executive functions: Wisconsin Card Sorting Test, interference task based on the Stroop Interference Test, and tests of verbal fluency and Tower of London. The examined group consisted of 50 children aged 7-10: 25 children with hyperactivity of combined type and 25 children of the control group. Each group consisted of 23 boys and 2 girls. The average age in the criterial group was 8 years and 10 months (SD=10 months), whereas in the control group – 8 years and 6 months (SD=11 months). According to the obtained results, children with ADHD at early school age do not exhibit a wide spectrum of executive functions deficits, which is probably associated with immaturity of executive processes in all children of that age. The findings comprised only difficulties in inhibition of response, monitoring of activity, and ability of executive attention to intentional guidance of the mental effort depending on the task’s requirements. In investigations of children with ADHD at early school age the use of neuropsychological tests and trials designed for evaluation of executive functions is justified only in limited degree. They do not significantly distinguish between children with ADHD and children without this disorder, therefore the results may be mainly of descriptive, and not explanatory, value.

Celem badań było ustalenie, czy w młodszym wieku szkolnym efektywność pracy funkcji wykonawczych różni dzieci z ADHD od dzieci z grupy kontrolnej. Istotne było także sprawdzenie, w jakim stopniu uzasadnione jest stosowanie metod diagnostycznych oceniających funkcje wykonawcze wobec dzieci w młodszym wieku szkolnym. Analizowano elastyczność poznawczą, zdolność podtrzymywania nastawienia, podatność na interferencję i zdolność planowania. Zastosowano metody oceny neuropsychologicznej, najczęściej wykorzystywane do charakterystyki funkcji wykonawczych: Test Sortowania Kart z Wisconsin, zadanie interferencji na podstawie testu interferencji Stroopa, zadanie fluencji werbalnej i Test Wieży Londyńskiej. W badaniach wzięło udział 50 dzieci w wieku 7-10 lat, w tym 25 dzieci z nadpobudliwością psychoruchową o typie mieszanym i 25 dzieci z grupy kontrolnej. W każdej grupie było 23 chłopców i 2 dziewczynki. Średni wiek w grupie kryterialnej wynosił 8 lat i 10 miesięcy (SD=10 mies.), a w grupie kontrolnej 8 lat i 6 miesięcy (SD=11 mies.). Zgodnie z uzyskanymi wynikami u dzieci z ADHD w młodszym wieku szkolnym nie obserwuje się szerokiego spektrum deficytów funkcji wykonawczych, co prawdopodobnie wiąże się z niedojrzałością procesów wykonawczych u wszystkich dzieci w tym wieku. Stwierdzono jedynie trudności w hamowaniu reakcji, monitorowaniu czynności i w zdolności uwagi wykonawczej do intencjonalnego kierowania wysiłkiem umysłowym w zależności od wymagań zadania. W badaniach dzieci z ADHD w młodszym wieku szkolnym zastosowanie testów i prób neuropsychologicznych przeznaczonych do oceny funkcji wykonawczych jest uzasadnione jedynie w ograniczonym zakresie. Nie różnicują one istotnie dzieci z ADHD i dzieci bez zaburzenia, zatem wyniki mogą mieć wartość głównie deskryptywną, a nie wyjaśniającą.
The authors analyse available literature devoted to potential impact of polyunsaturated fatty acids (PUFAs) on cognitive functions. Background: Cognitive functions are abilities associated with reception of information, memory, learning, thinking and emission of information. Disturbance of these functions may be due to trauma, diseases of the central nervous system (CNS) and vitamin deficiency. Recent publications also highlight the role of exogenous PUFAs, which are indispensable for many key developmental and functional processes in the CNS, including maturation and migration of neurons, synaptogenesis, CNS plasticity, neuronogenesis and neurotransmission. The paper presents experimental and clinical studies assessing the role of w-3 and w-6 in psychomotor development, attention, memory, praxia and reading skills. Conclusions: Studies performed to date confirmed that: a) PUFAs supplementation is safe and adverse effects associated therewith are few and mild, mainly limited to digestive tract disturbances; b) PUFA exert a favourable effect on psychomotor effectiveness, visual-spatial coordination, memory and attention. These findings, in line with reports on inadequate content of PUFAs in Poles’ diet, may indicate the need of using w-3 and w-6 PUFAs as preventive measure and as adjuvant treatment of disorders manifesting by disturbances of cognitive functions.

Cel: Autorzy omawiają dostępne piśmiennictwo na temat możliwego wpływu wielonienasyconych kwasów tłuszczowych (WKT) na funkcje poznawcze. Poglądy: Funkcje poznawcze to zdolności związane z odbiorem informacji, pamięcią i uczeniem się, myśleniem oraz z przekazywaniem informacji. Etiologia zaburzeń tych funkcji wiąże się m.in. z urazami, chorobami ośrodkowego układu nerwowego (OUN) i niedoborami witamin. Ostatnio zwraca się także uwagę na możliwą rolę egzogennych wielonienasyconych kwasów tłuszczowych, które są niezbędne dla kluczowych procesów rozwojowych i czynnościowych OUN, w tym dojrzewania neuronów, ich migracji, synaptogenezy, plastyczności, neuronogenezy i neurotransmisji. Omówiono prace eksperymentalne i kliniczne oceniające wpływ kwasów w-3 i w-6 na rozwój psychomotoryczny, procesy uwagi, pamięci, praksję i zdolność czytania. Wnioski: Dotychczasowe badania wskazują na: a) bezpieczeństwo suplementacji WKT z występowaniem nielicznych i mało nasilonych objawów niepożądanych, głównie ze strony układu pokarmowego; b) korzystny wpływ tych związków m.in. na sprawność psychomotoryczną, zdolności wzrokowo-przestrzenne, procesy pamięci i uwagi. Powyższe obserwacje wraz z doniesieniami o niedostatecznej zawartości WKT w diecie Polaków mogą wskazywać na możliwość zastosowania WKT w-3 i w-6 w profilaktyce i wspomaganiu terapii zaburzeń przebiegających z zakłóceniami funkcji poznawczych.
