Introduction: To date, suction drainage has been routinely used after hip joint replacement. Currently, the validity of this practice is questioned in the literature. Hematoma is a risk factor of periprosthetic infection. Post-operative ultrasonography enables precise assessment of hip joint hematoma. Aim: The aim of the study was to evaluate the usefulness of hip joint ultrasonography with respect to the validity of using suction drainage after primary hip arthroplasty. Material: Inclusion criteria: coxarthrosis. Exclusion criteria: primary and secondary coagulopathy, renal or hepatic failure and history of venous or arterial thrombosis. In total, 90 patients were enrolled. Methods: The study was prospective. The patients were assigned into groups in accordance with simple randomization. On the third day postsurgery, an ultrasound examination was conducted in all patients. Results: Deep infection was found in two patients with suction drainage. Hematoma was almost twice bigger in the drainage group. There were no statistically significant differences in the Harris Hip Score between the groups. No statistically significant differences were found between the groups in: complete blood count parameters and C-reactive protein values in the first and third day after surgery, the amount of transfused packed red blood cells, duration of hospital stay, cost of hospital stay and the relationship between osteophyte removal and hematoma size. Conclusions: Ultrasonography performed after hip replacement surgeries is useful in the assessment of hematoma. The randomized study did not reveal statistically significant differences between the group with and without drainage, thus suggesting that this practice can be abandoned, except for selected cases. Due to a short hospital stay, it is recommended to conduct an ultrasound scan in addition to routine radiography and laboratory tests in order to reduce the risk of complications.

Wstęp: Dotychczas rutynowo po endoprotezoplastyce biodra stosowano drenaż ssący. Obecnie w piśmiennictwie podważana jest zasadność stosowania pooperacyjnego drenażu ssącego. Krwiak pooperacyjny stanowi czynnik ryzyka infekcji okołoprotezowej. Zastosowanie badania ultrasonograficznego w diagnostyce pooperacyjnej pozwala na precyzyjną ocenę krwiaka stawu biodrowego. Cel: Celem pracy jest analiza wartości badania ultrasonograficznego stawu biodrowego w odniesieniu do zasadności stosowania drenażu ssącego po pierwotnej endoprotezoplastyce biodra. Materiał: Kryteria włączenia: koksartroza. Kryteria wyłączenia: pierwotna i wtórna koagulopatia, niewydolność wątroby lub nerek, przebyta zakrzepica tętnicza lub żylna. Do badania włączono 90 chorych. Metody: Badanie zaplanowano jako prospektywne. Chorych przydzielano do dwóch grup zgodnie z zasadami randomizacji prostej. W trzeciej dobie po operacji wykonywano badanie ultrasonograficzne u wszystkich chorych. Wyniki: U dwóch chorych w grupie z drenem stwierdzono infekcję głęboką. W grupie z drenem zaobserwowano średnio prawie dwukrotnie większy krwiak w stawie. W obu grupach w ocenie w skali Harris Hip Score nie odnotowano istotnych statystycznie różnic. Nie stwierdzono istotnej statystycznie różnicy między grupami w ocenie: morfologii i białka ostrej fazy w pierwszej oraz trzeciej dobie po operacji, ilości przetaczanego koncentratu krwinek czerwonych, długości hospitalizacji, pod względem kosztu pobytu, jak również między usunięciem osteofitów a wielkością krwiaka. Wnioski: Badanie ultrasonograficzne wykonywane po endoprotezoplastyce biodra jest przydatne w ocenie krwiaka. W przeprowadzonym randomizowanym badaniu nie stwierdzono istotności statystycznej pomiędzy grupami z drenem i bez drenu, co sugeruje odstąpienie od drenażu ssącego, poza szczególnymi przypadkami. Ze względu na krótki okres pobytu chorych w szpitalu zalecamy, oprócz rutynowo wykonywanych zdjęć rentgenowskich i badań laboratoryjnych, diagnostykę ultrasonograficzną w celu zmniejszenia ryzyka powikłań. Introduction The aim of the study is to present the evaluation of the functional capacity of elderly patients with osteoarthritis of the knee before and after the endoprosthesis procedure. Material and methods The research was carried out after obtaining the consent of the Bioethical Committee at the Nicolaus Copernicus University in Toruń, Collegium Medicum in Bydgoszcz in the period from 27/05/2018 to 27/05/2019 in the 10th Military Clinical Hospital with the Polyclinic in Bydgoszcz of the Department of Traumatology and Orthopedics. The research material consisted of 100 patients aged 65+ (62 women and 38 men) qualified to undergo knee endoprosthesis surgery. Results In general, the highest increase in the quality of life was recorded among rural residents – 21.21 points, in the age of 71 76 – 14.95 points and in the age of 68 70 – 14.81 points. The lowest, however, in the 50-67 age group – 12.28 points, for city residents over 100,000. – 9.38 points and 15-49 thousand among the city’s inhabitants – 9.03 points. Conclusions The conducted studies found a statistically significant improvement in the functional efficiency of patients, as well as a reduction in pain in the knee joint and improvement in the quality of life of patients after surgery for knee replacement – endoprosthesis.
Introduction Coxarthritis, in Poland, affects approximately 8 million people. Early physiotherapy determines the success of treatment. Aim The aim of the study was to analyse the effectiveness of rehabilitation process of patients undergoing hip surgery. Material and methods Medical documentation of 100 patients who have undergone surgery was analysed. Results The most common diagnosis in patients was coxarthritis. The BMI values of the patient’s differed significan¬tly depending on their place of residence. Patients started rehabilitation between day 1 and day 4 after the surgery. The verticalisation in 89% of pa-tients was carried out on day 1. Ninety-three patients decided to learn to walk with crutches. Conclusions 1. Primary coxarthritis is an indication to refer the patient to endoprosthesis. 2. In the course of rehabilitation, there is a progress of the patient’s functional level. 3. The rehabilitation process is not affected by the patient’s gender.

Wstęp Koksartroza w Polsce dotyka ok. 8 mln. osób. Wcześnie wdrożona fizjoterapia decyduje o sukcesie leczenia. Cel Celem pracy była analiza procesu rehabilitacji pacjentów poddanych operacji stawu biodrowego. Materiał i metody Analizowano dokumentację medyczną 100 pacjentów poddanych operacji. Wyniki Najczęstszym rozpoznaniem u pacjentów była koksartroza. Wartości BMI badanych różniły się istotnie w zależności od miejsca zamieszkania. Pacjenci rozpoczynali rehabilitację pomiędzy 1- a 4 dniem. Pioniza¬cję w 89% przypadków przeprowadzono 1 dnia. Naukę chodu o kulach podjęło 93 pacjentów. Wnioski 1. Pierwotna koksartroza jest wskazaniem do skierowania pacjenta na endoprotezoplastykę. 2. Wraz z przebiegiem rehabilitacji występuje progres poziomu funkcjonalności pacjentów. 3. Na przebieg rehabilitacji nie ma wpływu płeć.
Wstęp. Pomimo dostępnych w bazach bibliograficznych artykułów i prowadzonych analizach, w dalszym ciągu nie ma konkretnych wytycznych zarówno dla lekarzy ortopedów, jak i fizjoterapeutów, kiedy pacjent po endoprotezoplastyce stawu kolanowego (TKA) może wrócić do prowadzenia samochodu. Wpływ wielu czynników takich jak: strona operowana, dolegliwości bólowe, brak odpowiedniej siły potrzebnej do wyhamowania w warunkach ekstremalnych, czas reakcji pacjenta czy brak pewności siebie muszą być wzięte pod uwagę przy podejmowaniu tak ważnej decyzji. Zarówno pacjent, jak i lekarz podejmując decyzję o powrocie pacjenta do prowadzenia samochodu musi mieć świadomość, że jest to czynność złożona i odpowiedzialna, gdyż od niej może zależeć bezpieczeństwo pacjenta, jak i innych uczestników drogi. Celem pracy była analiza przeprowadzonych do tej pory badań i pomoc lekarzom oraz fizjoterapeutom w odpowiedzi pacjentowi na to pytanie. Większość dostępnych badań opiera się na symulatorach jazdy i automatycznych skrzyniach biegu, w których bezpieczny element hamowania jest osiągnięty w 3-6 tygodniu, gdzie pacjenci po lewostronnej TKA mogą wrócić do prowadzenia pojazdu po 4 tygodniach. Najbardziej obiecujące wydają się badania w jeździe rzeczywistej, która pozwala na powrót do jazdy już po 3 tygodniach od zabiegu. Materiał i metody. Poprzez medyczne bazy danych tj., PubMed i Medline wyselekcjonowano artykuły dotyczące badanego zagadnienia. Zakres wyszukiwania obejmował całość baz danych do kwietnia 2020 roku. Większość przedstawionych prac opiera się na badaniach i artykule Spaldinga z 1994 roku, który jako pierwszy przeprowadził badania dotyczące powrotu pacjentów po TKA do prowadzenia samochodu. Wyniki i Wnioski. Bezpieczny, możliwy powrót do powadzenia samochodu u pacjentów po TKA jest możliwy po 6 tygodniu od zabiegu.

Background. Despite the availability of numerous articles and analyses carried out in many healthcare centres, there is still no specific guidelines for both orthopaedic surgeons, and physical therapists for patients preparation after total knee arthroplasty to return to driving a car. The influence of many factors should be taken into account, such as: the operated side, the pain associated with the injury and surgery, the lack of sufficient force to slow down in extreme conditions, the patient’s reaction time and the lack of self-confidence when making such an important decision. Both the patient and the doctor who decides to let the patient drive a car must be aware that this is a very complex and responsible action. This may have an impact on the safety of the patient himself as well as other road users. The aim of the study was to analyse the research carried out so far and to help doctors and physiotherapists in taking the decision of returning the patient to driving a car. Most of the available studies are based on driving simulators and automatic gearboxes where a safe braking element is achieved in three to six weeks, where left-sided TKA patients can return to driving after 4 weeks. The most promising research seems to be in real driving, which allows them to return to driving just 3 weeks after the procedure. Material and metods. The medical databases, i.e. PubMed and Medline were searched for articles concerning the studied issue. The search have covered all databases until April 2020. Most of the work presented is based on a 1994 study and article made by Spalding, who was the first who conduct a study concerning the return of TKA patients to driving. Results and Conclusions. Safe, possible return to driving in patients after TKA is possible 6 weeks after the procedure.
