In addition to linguistic impairments, the cognitive underpinnings of pragmatic abilities in aphasic individuals can be severely compromised. Impairments include incoherent discourse and other speech organization deficits. At the same time, preserved pragmatic patterns reveal communicative abilities that can go unnoticed if only standardized screening tests are used. Conversational Partners also appear to play a role in mitigating compromised linguistic ability. Although some tools assessing the communicative abilities of neurological patients have been recently employed, the question of whether pragmatic performance declines in consistent patterns remains poorly understood. We applied the Pragmatic Evaluation Protocol – Revised (PREP-R) to video-recorded interviews of Greek individuals with aphasia, presented here as separate case studies. The tool offers a detailed account of pragmatic ability across 29 distinct categories and consists of three distinct subcomponents, namely enunciative pragmatics, textual pragmatics, and interactional pragmatics. Our results showed considerable variation in the performance of each patient and across our three aphasic patients. We also found that the role of the Key Conversational Partner was critical to effective communication, in line with previous research. As the communication repertoire of each of our participants was found to be highly idiosyncratic, we propose that further research should shift away from the mere evaluation of isolated verbal abilities.
The main aim of this study was to evaluate the level of cognitive flexibility in patients with speech disorders after ischaemic cerebral stroke. The study was conducted in a group of 43 patients (18 women and 25 men) who had experienced cerebral ischaemic stroke. The patients under study were divided into groups based on the type of speech disorders, i.e.: aphasia, lack of speech disorders and dysarthria. A Mini-Mental State Examination (MMSE) and a Clock Drawing Test (CDT) were applied for the general evaluation of the efficiency of cognitive functions. Cognitive flexibility – a component of executive functions, was evaluated with the use of a Trail Making Test (TMT). The results obtained prove that patients with aphasia show the lowest level of cognitive flexibility. Disorders of executive functions can be related to the dysfunction of the prefrontal cortex which has been damaged as a result of ischaemic cerebral stroke. Presumably, there are common functional neuroanatomical circuits for both language skills and components of executive functions. In the case of damage to the structures that are of key importance for both skills, language and executive dysfunctions can therefore occur in parallel. The presence of executive dysfunctions in patients with aphasia can additionally impede the functioning of the patient, and also negatively influence the process of rehabilitation the aim of which is to improve the efficiency of communication.

Nadrzędnym celem pracy było określenie poziomu elastyczności poznawczej za pomocą Testu Łączenia Punktów (Trail Making Test, TMT) u chorych z zaburzeniami mowy po udarze mózgu. Badaniem objęto 43 chorych (18 kobiet oraz 25 mężczyzn) z niedokrwiennym udarem mózgu. Badanych przydzielono do trzech grup w zależności od rodzaju zaburzenia mowy: chorych z afazją, brakiem afazji i dyzartrią. Do oceny ogólnej sprawności funkcji poznawczych zastosowano Krótką Skalę Oceny Stanu Psychicznego (Mini-Mental State Examination, MMSE) oraz Test Rysowania Zegara (Clock Drawing Test, CDT). Elastyczność poznawcza była badana przy użyciu Testu Łączenia Punktów. Uzyskane wyniki wykazały, że chorzy z afazją wykazują najniższy poziom elastyczności poznawczej. Zaburzenia elastyczności poznawczej mogą być związane z dysfunkcją kory przedczołowej, która uległa uszkodzeniu w wyniku niedokrwiennego udaru mózgu. Przypuszczalnie dla umiejętności językowych i komponentów funkcji wykonawczych istnieją wspólne neuroanatomiczne obwody funkcjonalne. Stąd w przypadku uszkodzenia kluczowych dla obu zdolności struktur dysfunkcje językowe i wykonawcze mogą występować paralelnie. Współwystępowanie deficytów poznawczych u chorych z afazją może dodatkowo utrudniać funkcjonowanie chorego, a także mieć negatywny wpływ na proces rehabilitacji sprawności porozumiewania się.
Prompt and effective therapeutic intervention in patients with aphasia provides several benefits. First and foremost it reduces the risk of development of pathological patterns of verbal response, increasing the chances for earlier recovery of psychomotor functions and affecting favourably the patient’ comfort. There are several theories explaining the mechanism of development of aphasias and several techniques of assistance offered to people affected. Great variety of speech disorders necessitates implementation of diverse therapies and adjusting them to individual patient’s needs. The dynamic theory of location of cognition and the concept of functional system presented in this paper, establish a rationale for treatment of disorders resulting from brain injury. According to the premises of the dynamic localization theory, execution of all higher mental functions, including speech, involves variable activation several brain areas. Disturbed functions may undergo restorative processes as a result of structural rearrangement within the framework of the dynamic system, utilizing unaffected regions of both cerebral hemispheres. Hence, the dynamic theory of location of function encourages initiation of rehabilitation, also in patients with substantial functional loss, providing hope and the feeling of partnership, both to patients and their families. The authors discuss the impact of disturbed neurophysiological processes on purposeful use of articulated speech and present several practical exercises designed to help patients affected with various types of aphasia.

Podjęcie szybkiej i skutecznej interwencji terapeutycznej u osób dotkniętych afatycznymi zaburzeniami mowy przynosi liczne korzyści. Przede wszystkim zmniejsza ryzyko pojawienia się patologicznych wzorców reagowania werbalnego, ponadto zwiększa szansę na wcześniejszy powrót funkcji psychomotorycznych oraz, co niezwykle istotne, wpływa na poprawę samopoczucia pacjenta. Istnieje wiele teorii tłumaczących mechanizm powstawania zaburzeń afatycznych, a także wiele metod pomocy pacjentom nimi dotkniętych. Różnorodność zaburzeń mowy stwarza konieczność stosowania rozmaitych sposobów oddziaływania terapeutycznego, a także doboru tych metod do indywidualnych potrzeb konkretnego pacjenta. Opisane w niniejszej pracy dynamiczna teoria lokalizacji funkcji psychicznych i koncepcja „układu funkcjonalnego” stwarzają teoretyczne przesłanki dla terapii zaburzeń powstałych w wyniku uszkodzenia ośrodkowego układu nerwowego. Zgodnie z założeniami teorii „dynamicznej lokalizacji” wszelkie czynności wyższe, w tym mowa, są realizowane przy zmiennym zaangażowaniu wielu okolic mózgu. Zaburzone funkcje mogą podlegać procesom odtwórczym, dzięki przebudowie organizacyjnej w ramach dynamicznego układu, wykorzystując nieuszkodzone obszary obydwu półkul. Dynamiczna teoria lokalizacji funkcji zachęca zatem do podjęcia rehabilitacji także u osób z wyraźnymi deficytami, dając nadzieję i poczucie podmiotowości w działaniu pacjentom i ich rodzinom. W pracy omówiono ponadto wpływ zaburzeń procesów neurofizjologicznych na posługiwanie się mową artykułowaną, jak również przedstawiono szereg propozycji praktycznych ćwiczeń usprawniających przeznaczonych dla osób z różnymi rodzajami zaburzeń afatycznych.
