We present 2 cases of acute coronary syndrome in patients with concomitant neoplastic disease during chemotherapy. In both patients, we found unusual angiographic image which showed that despite the presence of very large thrombi, there were no or hardly any atherosclerotic lesions in the vessel occlusion site. Aspiration thrombectomy was successfully performed in both cases to restore coronary flow and in the second patient, two bare metal stents were also implanted. After the patients were discharged from hospital, we decided to add enoxaparin to the routine dual antiplatelet therapy of both of them. So far, no guidelines have been developed on how to manage patients with acute coronary syndrome and concomitant neoplastic diseases while on chemotherapy.
INTRODUCTION: Systematic analysis of risk factors, causes of sudden death and patient survivability allows implementation of increasingly effective methods and procedures for emergency cardiac arrest (SCA). The conditions of the emergency room (ER) allow for initial medical imaging and laboratory diagnostics, which facilitate the assessment of critical parameters that may be a predictor of SCA. The aim of the study is to determine the survival level of patients with SCA that were staying in ER and to indicate the factors that increase the likelihood of SCA. MATERIAL AND METHODS: The study was conducted in 2018 based on medical records of SOR in 73 patients with sudden cardiac arrest in SOR. Descriptive statistics and data analysis were performed using parametric tests (Pearson test). The level of significance was determined for p <0,05. RESULTS: The average age of SOR patients was 72 years (SD ± 16.29). In most cases, the patients were brought to the ER by the EMS. Sinus rhythm dominated in ECG tests,before the onset of SCA,. The ECG mechanisms in which SCA occurred were: asystole (50.7%), PEA (32.9%) and VF / pVT (16.4%), respectively. Among half of the patients (50.7%) of ER spontaneous circulation was restored, while 49.3% of ER patients were fatal. The relationship between mortality and O2 partial pressure, methanol, MPV, D-dimer, pH and HCO3 has been demonstrated. CONCLUSIONS: In the examined group of patients with SCA,non-defibrillatory rhythms (asystole, PEA) dominate. Half of the patients manage to achieve ROSC under ER conditions. There are predictors of SCA in patients in ER.

WSTĘP: Systematyczna analiza czynników ryzyka, przyczyn nagłej śmierci oraz przeżywalności pacjentów pozwala na wdrażanie coraz skuteczniejszych metod i procedur postępowania w nagłym zatrzymaniu krążenia (NZK). Warunki Szpitalnego Oddziału Ratunkowego pozwalają na wstępną diagnostykę obrazową i laboratoryjną, co pozwala na ocenę parametrów krytycznych, mogących stanowić czynnik predykcyjny nagłego zatrzymania krążenia. Celem pracy jest określenie poziomu przeżywalności pacjentów z NZK przebywających w SOR oraz wskazanie czynników wpływających na wzrost prawdopodobieństwa wystąpienia nagłego zgonu sercowego. MATERIAŁ I METODY: Badanie zostało przeprowadzone w 2018 r. na podstawie dokumentacji medycznej SOR-u 73 pacjentów z nagłym zatrzymaniem krążenia na SOR. Dokonano statystyki opisowej oraz analizy danych za pomocą testów parametrycznych (test Pearsona). Poziom istotności ustalono dla p<0,05. WYNIKI: Średnia wieku pacjentów SOR-u wynosiła 72 lata (SD ± 16,29). W większości przypadków chorzy zostali przywiezieni na SOR przez zespół ratownictwa medycznego (ZRM). Przed wystąpieniem NZK w badaniach EKG dominował rytm zatokowy. Mechanizmy EKG, w jakim doszło do NZK to kolejno: asystolia (50,7%), PEA (32,9%) oraz VF/pVT (16,4%). U ponad połowy pacjentów (50,7%) SOR nastąpił powrót spontanicznego krążenia, natomiast u 49,3% pacjentów SOR odnotowano zgon. Wykazano zależność śmiertelności od ciśnienia parcjalnego O2, poziomu metanolu, MPV, D-dimerów, pH oraz HCO3. WNIOSKI: W przebadanej grupie pacjentów z NZK na SOR, zdecydowanie dominują rytmy niedefibrylacyjne (asystolia, PEA). U połowy pacjentów udaje się uzyskać ROSC w warunkach SOR. Istnieją czynniki predykcyjne występowania nagłej śmierci sercowej u pacjentów przebywających w SOR.
5-fluorouracil (5-FU) cardiotoxicity is not a common, but with potentially deleterious effects on the patient condition. In this paper we present the case of an 53-year-old woman undergone local resection for localized anal cancer and then she was treated with NIGRO protocol. 48 hours after the end of 5-FU i.v. administration an acute coronary syndrome was developed. The electrocardiogram showed hyperacute T waves in all leads except for V1 and aVL that showed negative T waves in combination with chest pain and diaphoresis promptly resolved after sublingual isosorbide dinitrate. A subsequent coronary angiography was performed and normal coronary arteries were shown. The potential causes and the treatment are being discussing.

Kardiotoksyczność 5-fluorouracylu (5-FU) nie jest powszechna, lecz może wykazywać niekorzystny wpływ na stan pacjenta. W artykule przedstawiono przypadek 53-letniej kobiety, która przeszła miejscową resekcję z powodu zlokalizowanego raka odbytu, a następnie była leczona protokołem NIGRO. 48 godzin po zakończeniu dożylnej podaży 5-FU wystąpił ostry zespół wieńcowy. Elektrokardiogram pokazał hiperostre załamki T we wszystkich odprowadzeniach, z wyjątkiem odprowadzeń V1 i aVL, wraz z ujemnymi załamkami T. Wystąpił także ból w klatce piersiowej, który ustąpił po podjęzykowym diazotanie izosorbidu. Wykonano kolejną angiografię wieńcową i ukazano prawidłowe tętnice wieńcowe. Dyskutowane są potencjalne przyczyny i leczenie przypadku.
Acute coronary syndrome (ACS) and oncological disease are more frequently observed in the general population as discussed in the part I of this article. Treatment of myocardial infarction in oncological patients becomes a real struggle for clinicians, especially that the data from clinical trials including cancer patients with ACS are very limited. The choice of treatment modality should consider many existing factors considering the type of ACS – non-ST-segment elevation myocardial infarction vs ST-segment elevation myocardial infarction, patient’s condition, type of cancer and oncological treatment applied. Taking into consideration above mentioned factors, clinicians have to face three therapeutic options: invasive, conservative or combination of both in order to choose the best and most beneficial treatment. This article summarizes the current therapeutic approach to the management of ACS in cancer patients.
Kounis syndrome (KS) is an acute coronary syndrome developing as a consequence of an anaphylactic or allergic reaction. Multiple mediators (especially histamine) released by mast cells, platelets, and some immune cells may lead to coronary vasospasm or thrombosis and cause an acute coronary syndrome. A clinical case of the KS in a hospitalized patient being treated for the COVID-19-associated pneumonia is presented. A 62-year-old woman was treated for COVID-19-associated pneumonia. In 15 min after the beginning of the Moxifloxacin infusion, the patient complained of severe headache, crushing chest pain, abdominal pain, severe general weakness, shortness of breath. Hypotension and decrease in SpO2 developed. Immediately performed ECG showed the ST-segment elevation in leads I and aVL. There were reciprocal changes in leads III, aVF, V3-V6. Troponin I was slightly elevated. Coronary angiography showed no hemodynamically significant coronary artery lesions. The KS type 1 was diagnosed. Repeated ECG in 2 hours showed isoelectric ST segments in all leads. It may be difficult to diagnose KS. Clinical signs of an allergic or anaphylactic reaction should raise the suspicion of KS. Moxifloxacin as well as other fluoroquinolones may be associated with the development of KS. We suggest a possible association of COVID-19 with KS. However, this issue requires further observation.

Zespół Kounisa (ZK) jest ostrym zespołem wieńcowym rozwijającym się w następstwie wstrząsu anafilaktycznego lub reakcji alergicznej. Wiele mediatorów (zwłaszcza histaminy) uwalnianych przez komórki tuczne, płytki krwi i niektóre komórki odpornościowe może prowadzić do skurczu naczyń wieńcowych lub zakrzepicy i spowodować ostry zespół wieńcowy. Przypadek kliniczny ZK przedstawiono na przykładzie hospitalizowanej pacjentki leczonej z powodu zapalenia płuc związanego z COVID-19. 62-letnia kobieta była leczona z powodu zapalenia płuc związanego z COVID-19. W ciągu 15 minut po rozpoczęciu wlewu Moxifloxacin pacjentka zaczęła się uskarżać na silny ból głowy, miażdżący ból w klatce piersiowej, ból brzucha, silne ogólne osłabienie, duszność. Powstało niedociśnienie i spadek SpO2. Natychmiastowe wykonanie EKG wykazało uniesienie odcinka ST w odprowadzeniach I i aVL. Wystąpiły wzajemne zmiany w odprowadzeniach III, aVF, V3-V6. Troponina I była lekko podwyższona. Angiografia naczyń wieńcowych nie wykazała istotnych hemodynamicznie zmian w tętnicach. Rozpoznano ZK typu 1. Powtórzone EKG w ciągu 2 godzin wykazało izoelektryczne odcinki ST we wszystkich odprowadzeniach. Diagnoza ZK może być trudna. Objawy kliniczne reakcji alergicznej lub wstrząsu anafilaktycznego powinny budzić podejrzenie ZK. Moksifloksacyna i inne fluorochinolony mogą być związane z rozwojem ZK. Sugerujemy możliwy związek COVID-19 z ZK. Jednak ten problem wymaga dalszej obserwacji.
Coexistence of cardiovascular diseases and cancer is more and more common in everyday practice. This is due to the aging of the population and the fact that most of the classic cardiovascular risk factors also predispose to cancer. During anti-cancer treatment, cardiovascular components may be damaged and eventually lead to acute coronary syndrome. The first part of the review article presents the pathophysiology, clinical presentation and key elements of the diagnostic process of this life-threatening condition in cancer patients.
Objectives: The evaluation of the direct and remote effects of stationary rehabilitation following interventionally treated acute coronary syndrome and its extension by a three-month period of supervised training in ambulatory conditions within the scope of physical capacity psychic state and quality of life.Materials and methods: The research was conducted in a group of 44 patients (32 men and 12 women) aged 56.9±9, 62 years old for a period of 2-3 weeks following interventional treatment of acute coronary syndrome. All the patients had participated in stationary rehabilitation, 14 of whom expressed a willingness to participate in a futher three-month period of ambulatory rehabilitation. All patients prior to the commencement of rehabilitation had threadmill exertion tests. Fear was evaluated by means of the SOPER questionnaire. Basic mood and psychic tension as well as the feeling of health were assessed by means of the VAS scale of the Euro-Qol 5D questionnaire. The set of tests was repeated after the completion of the stationary phase, and after time periods of 3 months and a year.Results: Patients subjected to the extended 3-month ambulatory phase of rehabilitation displayed a greater increase in exercise tolerance when compared with the group that had merely undergone the 3-week stationary rehabilitation. Patients with extended rehabilitation gradually improved their evaluation of their state of health and after a year maintained it at the level it had been after the completion of the stationary rehabilitation. In patients who did not express a willingness to continue an organised form of exercise in ambulatory conditions the noticeable improvement in health following the end of rehabilitation was not long lasting. After a year, regardless of the programme of rehabilitation selected, an improvement in the psychic state was noted, which was expressed in an increased positive mood although the level of fear did not change to a significant degree.Conclusions: 1. Extended rehabilitation, combining stationary and ambulatory forms, after interventional treatment of ACS gives a better long-term effect in the area of improving exercise capacity and patient self-evaluation of state of health than is in the case after three-week stationary rehabilitation. 2. The extending of the period of rehabilitation has no influence on attaining the postrehabilitation changes on the level of mood and fear.
Cel: Spacery i marsze są prostą a jednocześnie efektywną formą aktywności fizycznej powszechnie stosowaną w rehabilitacji kardiologicznej. W ostatnich latach podejmuje się próby urozmaicenia ćwiczeń marszowych poprzez szersze włączenie pracy kończyn górnych, co umożliwia między innymi coraz bardziej popularna forma aktywności ruchowej znana jako Nordic Walking. Celem niniejszej pracy była ocena wpływu marszu nordyckiego na stopień poprawy tolerancji wysiłku oraz sprawność ogólną chorych rehabilitowanych po zawale serca (ZS). Metoda: Badaniom poddano 30 pacjentów, którzy w okresie 14–28 dni po ZS przebywali na Oddziale Rehabilitacji Kardiologicznej. U wszystkich pacjentów włączonych do programu badawczego przed rehabilitacją i po jej zakończeniu wykonano próbę wysiłkową wg zmodyfikowanego protokołu Bruce’a, 6-minutowy test marszowy oraz test sprawności fizycznej „Fullerton”. Chorych podzielono na dwie grupy. 10 pacjentów uczestniczyło w standardowym programie treningowym (grupa K), a u 20 pacjentów zastosowano dodatkowo (5x w tygodniu po 40 min) trening marszowy typu Nordic Walking (grupa NW). Polegał on na przejściu 3 kilometrów z przerwami przeznaczonymi na ćwiczenia oddechowe i rozciągające. W trakcie marszu oraz ćwiczeń z pacjenci wykorzystywali specjalne kije skonstruowane do tej formy aktywności ruchowej Wyniki: W obu grupach trenujących uzyskano poprawę tolerancji wysiłku, jednak odsetek poprawy w grupie NW (30%) był wyższy niż w grupie K (14%). Podobnie zachowywały się wartości iloczynu podwójnego (RPP). Istotne statystycznie zwiększenie RPP po treningu rehabilitacyjnym stwierdzono jedynie w grupie z NW (z 18,2 x 103 ±3,3 x 103 do 20,7 x 103 ±4,4 x 103) (p<0,05). 6-minutowa próba marszowa wykazała wyższy odsetek poprawy w grupie NW (22%) niż w grupie K (17%). Wyniki testu „Fullerton” wykazują istotną statystycznie poprawę analizowanych parametrów w obu grupach. Wnioski: Wstępne badania wskazują na celowość wdrażania marszu nordyckiego do programów rehabilitacji kardiologicznej po zawale serca z uwagi na wysoką efektywność w zakresie wpływu na poprawę tolerancji wysiłku i sprawności fizycznej. Pełne zaadoptowanie Nordic Walking jako jednej z istotnych metod wczesnej rehabilitacji kardiologicznej będzie możliwe po dokonaniu i przeanalizowaniu większej liczby obserwacji poszerzonych o dodatkowe parametry, głównie w zakresie wpływu na czynniki ryzyka oraz parametry jakości życia.

Background: Walks and marches are a simple yet effective forms of physical activity, commonly used in the cardiac rehabilitation. It has been recommended recently to combine march training with exercises involving upper limbs. One of the most popular methods is Nordic Walking, during which special poles are used to increase the work of upper part of the body. The aim of the study was to assess the influence of Nordic Walking on the improvement in exercise tolerance and physical performance in patients rehabilitated after a myocardial infarction.Methods: The study population consisted of 30 patients hospitalized in a cardiac rehabilitation ward 14 to 28 days after myocardial infarction. On admission and after completing the rehabilitation program patients performed an exercise test following the modified Bruce protocol, a six-minute walk test and a test assessing physical performance (Fullerton test). 10 subjects were assigned to a control group, which followed a standard training program, and 20 patients to a group which additionally performed a march training using the Nordic Walking method 5 times a week. Each session of the march training lasted 40 minutes and consisted of a 3-kilometer walk interrupted by breaks during which respiratory and stretching exercises were performed. During the march and exercises patients used special poles for Nordic Walking.Results: Exercise tolerance improved in both study groups. However, the increase in the exercise tolerance was greater in the group performing Nordic Walking than in the control group (30% vs. 14%, respectively; P < 0.05). The rate-pressure product during the exercise test increased significantly only in the group performing Nordic Walking (from 18.2 x 103 ± 3.3 x 103 to 20.7 x 103 ± 4.4 x 103; P < 0.05). The improvement in the results of the six-minute walk test was greater in the group performing Nordic Walking (22% in comparison with 17% in the control group; P < 0.05). The results of the test assessing physical performance (Fullerton test) improved in both study groups.Conclusion: The results of this preliminary study indicate that march training using the Nordic Walking method effectively increases exercise tolerance and physical fitness in patients after myocardial infarction. If these findings are confirmed by the results of larger studies, Nordic Walking could be widely used in the early phases of cardiac rehabilitation. The influence of Nordic Walking on the cardiovascular risk factors and quality of life parameters should be investigated.
Introduction. Based on the literature and scientific articles, the paper accurately depicts the anatomy of the human heart, the characteristics of acute coronary syndrome, epidemiology, and etiology. The symptoms and complications of acute coronary syndrome are discussed. Patient qualification for coronary angiography and the contribution of nursing care to the patient after coronary artery angiography are presented. The problems faced by the patient are characterized and nursing interventions are suggested, considering the patient's history and the physical examination performed. Aim. The purpose of the work is to present the tasks of a nurse and to define the nursing problems of a patient with acute coronary syndrome after a coronary angiography procedure. Case study. A 66-year-old patient admitted to the cardiological intensive care unit due to pain in the chest, located behind the sternum, with a burning character thatra diates to the left arm, shortness of breath, palpitations and sweating. Currently, the patient is on the first day after coronary angiography. The patient doesn’t report heart pain, but reports pain, redness and bruising at the puncture site. The patient feels anxious about his health, feels lonely. The patient enquires the help of the nursing staff in self-service activities. Conclusions. The statements developed in the work based on literature, scientific articles and the implementation of the nursing process for a patient with acute coronary syndrome after coronary angiography correctly and clearly show the commitment of nurses to the patient. Nursing care of the patient after coronary angiography consists primarily in monitoring the patient 's vital signs and documenting them, systematically assessing the puncture site of the artery, observing the patient, the supply of painkillers on a written medical order, assessing the level of anxiety, providing support and reliable information on the history of acute coronary syndrome and the performed coronary angiography. A very important action is also patient education, whose task is to equalize knowledge and provide the patient with the necessary skills for self-care.

Wstęp. Na podstawie literatury oraz artykułów naukowych w pracy precyzyjnie zobrazowano anatomię ludzkiego serca, charakterystykę ostrego zespołu wieńcowego, epidemiologię i etiologię. Omówiono objawy i powikłania ostrego zespołu wieńcowego. Przedstawiono kwalifikacje pacjenta do koronarografii oraz udział opieki pielęgniarskiej nad pacjentem po angiografii tętnic wieńcowych. Scharakteryzowano problemy, z którymi boryka się pacjent oraz zaproponowano interwencje pielęgniarskie, biorąc pod uwagę wywiad z chorym i przeprowadzone badanie fizykalne. Cel. Celem pracy jest przedstawienie zadań pielęgniarki oraz określenie problemów pielęgnacyjnych pacjenta z ostrym zespołem wieńcowym po przeprowadzonym zabiegu koronarografii. Prezentacja przypadku. Pacjent, lat 66, przyjęty w oddział intensywnej opieki kardiologicznej, z powodu bólu w klatce piersiowej, zlokalizowanym za mostkiem, o charakterze pieczenia, który promieniuje do lewej ręki, pojawiającej się duszności, kołataniem serca i potliwością. Obecnie pacjent jest w I dobie po koronarografii. Chory nie zgłasza dolegliwości bólowych ze strony serca, jednak informuje o bólu, zaczerwienieniu oraz zasinieniu w miejscu nakłucia. Pacjent odczuwa niepokój związany ze swoim stanem zdrowia, czuje się osamotniony, jego nastrój jest średni. Chory wymaga pomocy personelu pielęgniarskiego w czynnościach samoobsługowych. Wnioski. Zaczerpnięte twierdzenia opracowane w pracy na podstawie piśmiennictwa, artykułów naukowych oraz zastosowaniu wdrożenia procesu pielęgnowania wobec pacjenta z ostrym zespołem wieńcowym po koronarografii w poprawny i oczywisty sposób ukazują zaangażowanie pielęgniarek wobec pacjenta. Opieka pielęgniarska nad chorym po koronarografii polega przede wszystkim na: monitorowaniu parametrów życiowych chorego i ich dokumentowanie, systematycznym ocenianiu miejsca nakłucia tętnicy, obserwowaniu pacjenta, podaży leków przeciwbólowych na pisemne zlecenie lekarskie, ocenianiu poziomu nasilenia niepokoju, udzieleniu wsparcia oraz rzetelnych informacji na temat przebytego ostrego zespołu wieńcowego i przeprowadzonego zabiegu koronarografii. Bardzo ważnym działaniem jest także edukacja pacjenta, której zadaniem jest wyrównanie wiedzy oraz zapewnienie choremu niezbędnych umiejętności do lepszej samoopieki.
