Statins might cause and/or aggravate the immune-mediated myositis in patients on long-term, stable treatment. We provide a case of polymyositis with an immunological background and gastrointestinal and urinary manifestations in patient on long-term, stable atorvastatin treatment for the past six years. The diagnose of polymyositis was established based on clinical symptoms and signs, electromyography and laboratory test results (elevated aspartate aminotransferase 279 U/L, reference range 0–40 U/L; alanine aminotransferase 198 U/L, 0–33 U/L; lactate dehydrogenase 2200 U/L, 103-227 U/L; creatine kinase 7820 U/L, 15–84 U/L; and positive antinuclear antibodies test, titer of 1:160, with suspect antisynthetase antibodies). Polymyositis was probably related to atorvastatin treatment (Naranjo score, 5). Other probable causes of the myositis were rejected. Coricosteroid therapy, methotrexate and supplementation with vitamin D did not improve the condition. The patient remained bedridden and died two months after the hospital discharge due to the acute myocardial infarction.

The aim was to evaluate vitamin D levels in young healthy Lithuanian males in winter and to find possible associations of vitamin D concentration to body composition, cognitive functioning, emotional state. Subjects and Methods A total of 130 healthy males (age range, 18–26 years) were divided into the subgroups according to vitamin D concentration. The Profile of Mood States (POMS) questionnaire and Hospital Anxiety and Depression Scale were employed in the assessment of emotional state. Cognitive functioning was assessed by the Trail Making Test and the Digit Symbol Test of the Wechsler Adult Intelligence Scale. Results The mean concentration of vitamin D for the entire sample was 13.0±5.3 ng/ml. Only 2 persons (1.6%) had the recommended vitamin D level. Nearly half (45.4%) of study participants had vitamin D deficiency. Lower concentrations were associated with a significantly higher score on the POMS confusion-bewilderment scale. A tendency toward a lower mean depression-dejection score in the participants with a sufficient vitamin D level was observed. Vitamin D concentration correlated positively with body mass index (BMI) and inversely with the confusion-bewilderment score. In conclusion almost half of the young healthy males participating in the study were detected to have vitamin D deficiency in winter. Low vitamin D concentrations are associated with a worse emotional state.

Advances in studies on vitamin D reveal increasingly wider possibilities for its use in the prevention and even treatment of many diseases. Vitamin D is increasingly more frequently called prohormone or cytokine, and nowadays it is not only associated with the regulation of calcium and phosphate metabolism. It is now known that because of the fact that vitamin D has an influence on the production of cytokine, it reduces the incidence of autoimmune illnesses such as diabetes type 1. Vitamin D is also used in the prevention and treatment of cardiovascular diseases, tumors and depression. Changes in metabolism, insufficient acquisition of this compound during skin synthesis, abnormal biochemical changes to the active form, or inadequate dietary intake of this compound may disrupt many important processes for maintaining health.

Postępy w badaniach nad witaminą D ujawniają coraz szersze możliwości jej wykorzystania w zapobieganiu, a nawet leczeniu wielu jednostek chorobowych. Witamina D jest coraz częściej określana również jako prohormon lub cytokina, obecnie nie jest już tylko kojarzona z regulacją gospodarki wapniowo-fosforanowej, wiadomo, że wpływając na produkcję cytokin zmniejsza częstość występowania chorób autoimmunologicznych, np. cukrzycy typu 1. Witamina D ma również zastosowanie w profilaktyce i leczeniu nowotworów, chorób układu krążenia oraz depresji. Zmiany w metabolizmie, zbyt małe pozyskiwanie tego związku w trakcie syntezy skórnej, zaburzenia przemian biochemicznych do formy aktywnej czy niedostateczna podaż w diecie tego związku mogą zaburzyć wiele ważnych dla zachowania zdrowia procesów.
