Background: Knowledge about diabetes mellitus (DM)-related complications, the direct and indirect costs incur red by patients with DM is still insufficient in Poland. Economedica DM-PL (Economedica) survey was a cross-sectional observational study that aimed to assess these issues. Material and methods: The survey was carried out in 2016 on a representative sample of 2032 diabetic patients recruited by 195 general practioners (GP) and 48 diabetologists (D) randomly selected from the national registry of physicians. Each GP and D recruited 8 to 10 patients with DM according to the specified protocol. The study included 1938 medical questionnaires (comple ted by the GPs and Ds), 1938 questionnaires about costs (completed by the patients) and 1532 treatment adherence questionnaires. Results: The Economedica survey provided new insights into DM epidemiological and cost data. 70% of patients with type-1 DM were diagnosed with the disease before the age of 40. For 76% patients with type-2 DM, it was diagnosed aeftr the age of 50. The average monthly out-of-pocket medical costs for DM patients in 2016 reached 66.2 EUR, while their non-medical costs were 36.8 EUR. Conclusions: The methodology applied in our study proved useful for providing new dimensions of DM epidemiological and cost data in Poland.

The goal of the study was to assess the prevalence of stress urinary incontinence in a group of elite female endurance athletes, as professional sport is one of the risk factors for stress urinary incontinence. SUI rates in the groups of female cross-country skiers and runners were compared to determine whether the training weather conditions like temperature and humidity influenced the prevalence of urinary incontinence. An anonymous questionnaire was distributed among 112 elite female athletes ie., 57 cross-country skiers and 55 runners. We used a short form of the Urogenital Distress Inventory (UDI-6) to assess the presence of SUI symptoms and the level of urogenital distress. Only women who had been practicing sport professionally for at least 3 years, on an international and national level, were included in the research. The study group consisted of 76% nulliparous and 24% parous women. 45.54% of all participants reported leakage of urine associated with sneezing or coughing which indicates stress urinary incontinence. 29.46% were not bothered by the urogenital distress symptoms. 42.86% of the participants were slightly bothered by the symptoms, 18.75% were moderately bothered, 8.04% were significantly bothered and 0.89% were heavily bothered. The absence of statistically significant differences between both groups seems to indicate that training weather conditions did not influence the prevalence of SUI in elite female endurance athletes.

Superficial mycoses in the Nis region, Southeast-Serbia

The aim of the study is to investigate the most frequent cause of superficial mycoses in patients from the territory of city Niš Southeast Serbia in the period from 1998 to 2010. A total of 3223 samples from 2887 patients with suspected dermatomycoses were examined. Superficial mycoses were diagnosed using standard microbiology techniques (conventional microscopy and cultivation). Dermatophytes were determined on the basis of their macroscopic and microscopic morphological and morphometric characteristics. Morphometric characteristics were obtained by Laboratory Universal Computer Image Analysis system (Lucia M, 1996). Species of genus Candida were identified using the test of production of germ tube in sera, by growth on comertial chromatogen medium (Chromotogenic Candida, Liofichem/Bacteriology products, Italy) and by using Auxacolor TMBioRad, France. The results were elaborated with the statistical method of descriptive and quantitative analysis (SPSS 14.0 for Windows 2003). The prevalence of superficial mycoses was 25,1%. Dermatophytes were identified in 67.6% of all positive cultures. Microsporum canis was the most prevalent (50.3%) dermatophyte isolated, followed by Trichophyton metagrophytes var. mentagrophytes (35.4%). Yeast genus Candida has become a more frequent cause of superficial fungal infection since 2001. and C. albicans was the dominant yeast (61.1%).

The study examined the body weight and parasites prevalence of African giant rat (Cricetomys gambianus Waterhouse, 1840) in rural setting of Gboko Local Government of Benue State. A total of 15 rats (10 males and 5 females) were collected from the wild and subjected to parasites isolation and body weight measurement at the Department of Fisheries and Aquaculture laboratory, Joseph Sarwuan Tarka University, Makurdi. Data was analyzed using descriptive statistics and student T-test. Results revealed a significantly higher (1.99kg) mean body weight in males rats compared to females (1.2 kg) (p<0.05). Parasite prevalence revealed 3 groups of Endo parasites (Roundworm, Flagellates and Tapeworm) and an Ecto parasite (flea) in the rats. However, there was no significant difference between the male and female rats in terms of the Endo/Ecto parasites count and load (p> 0.05). More so, body weight, endo and ecto parasites count and load were highly correlated with sex in the animals. For healthier consumption and domestication, adequate attention should be given towards the control of both endo and ecto parasite of captive reared African giant rat.

Not at risk - Nevertheless a pressure ulcer

When conducting prevalence surveys pressure ulcers were found in participants clearly identified not to be at risk. This article determines and analyses persons in German hospitals and nursing homes who suffer from pressure ulcers but are not at risk. In the years 2002, 2003 and 2004 there were 7,097 nursing home residents and 23,966 hospital patients examined in annual pressure ulcer prevalence surveys. A risk assessment according to the Braden Scale was performed for each participant on the day of the survey. “Not at risk” participants were defined by Braden score cut-off > 20 points. There were 440 of 3,012 (14.6%) persons with pressure ulcer who were considered not to be at risk. In hospitals, 16.1% of all patients with pressure ulcers were not at risk, in nursing homes it was 8.2%. A high variance between medical specialties and individual institutions was found in the number of those not at risk but with pressure ulcer. In the group not at risk, persons with and without a pressure ulcer differed regarding activity and friction and shear in nursing homes. In hospitals those persons differed regarding age and all single items of the Braden scale apart from sensory perception. Pressure ulcers that are more severe, located at the hip or lower back or the origin of which is unknown are more likely to be considered to be at risk by the Braden risk assessment tool. The results may indicate insufficient abilities of the Braden scale for certain kind of pressure ulcer wounds.

The reported prevalence of periampullary duodenal diverticula varies between 9 and 32.8%. The aim of the study was to evaluate the prevalence of periampullary diverticula in the studied population and establish whether their presence influence the risk of choledocholithiasis and the risk of Endoscopic Retrograde Cholangio Pancreatography (ERCP) related complications. Material and methods. The study group of 3788 patients who underwent ERCP between 1996 and 2016at the 2nd Department of General Surgery Jagiellonian University Medical College in Kraków were analyzed. The group comprised of 2464 women (mean age 61.7 years) and 1324 men (mean age 61.8 years). The patients were divided into two groups. Group A included patients in whom there were no periampullary diverticula detected. Group B included patients in whom the opening of the bile duct was in the vicinity of a duodenal diverticulum. Results. There were 3332 patients included in group A (2154 women and 1178 men) and 456 patients in group B (310 women and 146 men). The prevalence of periampullary duodenal diverticula in the analyzed group was 12.8%. The presence of stones or biliary sludge was diagnosed in 1542 patients (47.6%) in group A and 290 patients (68.1%) in group B. Recurrence of choledocholithiasis occurred in 4.5% of patients (70/1542) in group A and 10.3% of patients (30/290) in group B. Complications occurred in a total of 76 patients in group A (2.3%) and 22 patients in group B (4.8%). Conclusions. The presence of choledocholithiasis and the risk of ERCP related complications are significantly higher in the group with duodenal diverticula.

Coral reefs have undergone significant degradation as a result of the impact of human disturbance and natural factors on biodiversity and ecosystem function. The impacts ranged from an increase in the appearance of coral bleaching, coral disease, to coral death. Especially for coral disease, it is confirmed that it has contributed to the reduction of live coral cover and the productivity of coral reef ecosystems. The coral disease can be epidemic due to environmental pressures, both anthropogenic and natural, as well as infection from pathogenic microbes. Recently in several places, including Semak Daun Island, Seribu Islands-Jakarta, it was reported that the condition of coral reef cover had increased. However, a high percent of coral cover can increase disease prevalence and abundance. Acropora coral is a type of coral that is commonly found on Semak Daun Island. However, these corals are very susceptible to stress so that the threat of disease is high. So it is necessary to conduct a study that aims to determine the type and prevalence of diseases that disturb Acropora coral in Semak Daun Island, Seribu Islands, Jakarta. The field survey was carried out for coral observation using the line transect method at four stations. Coral disease identification refers to the Coral Disease Handbook and Underwater Cards for Assessing Coral Health. Observation objects, both coral species, and signs of disease were documented using underwater cameras. The study results indicate that 15 types of Acropora corals were dominated by A. humilis. Meanwhile, two groups of diseases were found, namely Tissue loss - Predation caused by the COTS and Drupella, while those from the Compromised Health group included RFA, SD, and Sponges.

Childhood cancer is curable today with advances in treatment, but this can leave physical or psychosocial consequences. Our work consists in evaluating the state of satisfaction in patients aged less than 30 years. The study involved 27 patients (10 males and 17 females). We have chosen the life satisfaction scale (SVS-5) as a sampling tool. The results obtained identified 7 types of cancer, the most frequent of which are leukemia and breast cancer. The satisfaction test showed a prevalence of 14.8% (unsatisfied child) and 85.2% with moderate satisfaction and a large part of which can be converted to unsatisfied if the support and monitoring conditions are not met. by the family or by the health authorities. Faced with this encouraging situation, authorities should step up communication with the families of patients to explain to them how to behave with these children who are in a decisive phase for their future, especially in school.

The aim of the study was to evaluate the nature and frequency of sharps injuries among doctors and nurses from the same surgical/gynecological wards and the prevalence of HBV/HCV/HIV infection.Material and methods. An anonymous cross-sectional sero-survey, with ELISA system used to detect anti-HBc, anti-HCV, anti-HIV, was conducted among 89 doctors and 414 nurses from 16 randomly selected hospitals in West Pomerania, Poland, between January-June 2009.Results. During the preceding 12 months, 82% doctors and 44.4% nurses (p<0.0001) had sustained at least one sharps injury; 12.3% doctors vs 2.2% nurses (p<0.003) sustained more than 10 injuries. The multivariable regression model revealed that being a doctor was associated with a greater odds (OR 4.2) of being injured with sharps. Sixty nine percent of nurses sustained a hollow-bore needle injury vs 8.9% doctors; p<0.001. Anti-HBc were found in 16.4% of doctors and 11.2% of nurses, p>0.28; anti-HCV - in 1.1% of doctors vs 1.4% of nurses, p>0.79; no anti-HIV positive cases were found. The analysis of potential risk factors for contracting a HBV revealed that for both job categories only length of employment was associated with an increased odds of being infected.Conclusions. Although the prevalence of HBV/HCV infection between doctors and nurses does not differ significantly, modifiable risk factors for contracting a BBI such as frequency and nature of sharps injuries may differ, which call for tailoring preventive measures to specific job categories. Long lasting exposure to injury events should be taken into consideration while assessing the risk for accuiring an occupational infection with HBV, HCV or HIV.

Recent epidemiologic data have demonstrated a significant increase in eating disorder morbidity among adolescent girls. The goal of the current research was to evaluate attitudes and behaviour directly relevant to eating disorders. Participants were a sample of 611 second form schoolgirls of junior secondary schools in Lodz. A Polish version of the Eating Disorder Inventory (EDI) was performed. The results of the current study were compared with the findings of the Child and Adolescent Psychiatry Clinic researchers from the Institute of Psychiatry and Neurology in Warsaw and with the mean scores of female patients with anorexia nervosa (AN). The difference between the nonclinical sample of female adolescents and female patients with AN was significant in every dimension of EDI. The mean EDI score on the “Drive for Thinness” scale for 17% of the study group was equal to or higher than that for AN patients. The mean EDI scores in the “Drive for Thinness”, “Bulimia” and “Interoceptive Awareness” were not significantly different in the samples from Lodz and Warsaw. The findings of the current research and incidence of AN in adolescent females suggest advisability of regular screening of female adolescents for risk factors in behaviour and attitudes. Furthermore, intervention programs are recommended for exposed adolescent females especially at reversible stages of AN disorder, when self-esteem and sexual identity are formed.

W związku z niepokojącymi danymi dotyczącymi stale rosnącej liczby zachorowań na zaburzenia odżywiania w 2003 roku został wprowadzony program oceny nasilenia zachowań i postaw związanych z zaburzeniami jedzenia wśród adolescentek – uczennic drugich klas szkół gimnazjalnych w Łodzi. Programem została objęta grupa łącznie 611 dziewcząt. Autorzy artykułu prezentują, omawiają i oszacowują skalę zjawiska oraz dokonują porównania uzyskanych wyników z danymi z Kliniki Psychiatrii Dzieci i Młodzieży IPIN w Warszawie oraz z badaniami własnymi dr Anny Dunajskiej z tutejszej Kliniki. Grupa uczennic gimnazjów różniła się istotnie od pacjentek z jadłowstrętem psychicznym w zakresie wszystkich analizowanych skal. W badanej grupie aż 17% dziewcząt miało wyniki równe bądź wyższe od średniej uzyskanej przez chore z jadłowstrętem psychicznym w skali „Dążenie do szczupłości”. Nie zaobserwowano istotnych różnic między uczennicami szkół łódzkich i warszawskich w zakresie skal: „Dążenie do szczupłości”, „Bulimia”, „Świadomość bodźców interoceptywnych”. W związku z dużym rozpowszechnieniem zachowań i postaw związanych z zaburzeniami jedzenia, co potwierdzają wyniki skali DdS omawianego w pracy badania, można zalecać wykonywanie regularnych badań przesiewowych oraz prowadzenie działań edukacyjnych w młodszych grupach wiekowych – kiedy kształtuje się poczucie własnej wartości i tożsamości związanej z płcią.

Biopsychosocial model of psychotic disorders accounts for the role of extrabiological environmental factors in their development. Early trauma (ET) is often viewed as a potential psychosocial risk factor for future development of psychosis. Prevalence of events considered as ET in general population is estimated at 4-30%, while among persons with mental disorders the phenomenon may concern even up to 50% of examinees. Research of rozcorrelations between ET with psychosis is fraught with several methodological problems. One of them is lack of precise and consistent definition of ET. Furthermore, sources of data and ways of their collection are also inconsistent (data from reports of appropriate agencies, past history and questionnaires obtained from the examinees). Studies dealing with the subject often lack control groups or their selection is inadequate, they do not provide for the so-called distracting factors in final statistical analyses. There are not enough prospective studies. The issue of diagnostic difficulties is also highlighted. Authors frequently do not specify the type, clinical phase or severity of psychotic disorders in examined subjects. Disorders possibly associated with ET experience require an indepth differentiation with post-traumatic stress with dissociation disorders. Problems discussed here greatly delimit reaching of unambiguous conclusions and even may preclude a meta-analysis of hitherto performed studies. In some authors’ opinion, it is necessary to mark off a new category of psychotic disorders, i.e. post-traumatic psychosis. Typical features would be: trauma-associated content of symptoms (particularly auditory hallucinations), fluctuating depressive symptoms with suicidal ideation, abuse of psychoactive substances, poor effects of pharmacological treatment and favourable response to cognitive-behavioural therapy.

Model biopsychospołeczny zaburzeń psychotycznych uwzględnia rolę pozabiologicznych czynników środowiskowych w ich powstawaniu. Jako potencjalny czynnik psychospołeczny rozwoju psychoz postrzegane jest między innymi zjawisko tzw. wczesnej traumy (WT). Rozpowszechnienie wydarzeń ocenianych jako WT szacuje się w granicach od 4 do niemal 30% w populacjach ogólnych. W populacji chorych psychicznie zjawisko to może dotyczyć nawet połowy badanych. Badaniu związku WT z psychozą towarzyszą liczne problemy metodologiczne. Jednym z nich jest brak precyzyjnych i jednolitych definicji WT. Niejednolite są również źródła i sposoby uzyskiwania danych (dane z raportów odpowiednich służb, wywiady lub kwestionariusze uzyskiwane od badanych). Prace dotyczące problemu cechuje także brak lub nieprawidłowy dobór grup kontrolnych oraz brak uwzględniania w analizach statystycznych tzw. czynników zakłócających. Zbyt mała jest liczba badań o charakterze prospektywnym. Podnoszony jest także problem obecności w badaniach trudności diagnostycznych. Autorzy często nie określają rodzaju, fazy przebiegu ani stopnia nasilenia zaburzeń psychotycznych u badanych osób. Zaburzenia potencjalnie związane z przeżyciem WT wymagają wnikliwego różnicowania z zaburzeniem stresowym pourazowym oraz z zaburzeniami dysocjacyjnymi. Opisane problemy w znacznym stopniu ograniczają możliwość uzyskania jednoznacznych wniosków i wręcz uniemożliwiają przeprowadzenie metaanalizy dotychczasowych badań. Część autorów uważa, że konieczne jest wyodrębnienie nowej kategorii zaburzeń psychotycznych - tzw. „psychozy potraumatycznej". Jej cechami charakterystycznymi mają być nawiązanie w treści objawów (zwłaszcza halucynacji słuchowych) do doznanego urazu, podlegające fluktuacjom objawy depresyjne z myślami samobójczymi, nadużywanie środków psychoaktywnych, a także słabe efekty leczenia farmakologicznego oraz dobra odpowiedź na terapię poznawczo-behawioralną.

The authors reviewed available literature on the phenomenon of the elderly people abuse. In connection with ongoing worldwide demographic changes, a rapid growth of the population aged 60 years and more is noted. Such trends will continue, which gives rise to a gradual increase in the number of cases of abused seniors. The article presents definitions of violence against elderly people, with a special emphasis on the symptoms that may suggest abuse. Furthermore, we specified the articles that focus on the assessment of individual risk factors that may increase the occurrence of aggressive behaviour towards seniors. We also focused on analysis of available literature on the prevalence of violence against the elderly, both in Poland and worldwide. Analysis of the data from countries with different structural and cultural conditions clearly confirms the presence of this phenomenon everywhere, although it appears to varying extent. Research results depend on many factors, including the applied research methodology and measurement tools, the accepted definition of violence and sources of information. Worth noting are underestimated statistics which may be due to senior citizens’ reluctance to disclose abuses. In view of the multidimensional nature of the issue, it is important to introduce further system solutions in order to reduce the incidence of elderly people abuse and more effectively detect and resolve the problem, which still constitutes, especially in our country, a taboo subject.

Autorzy dokonali przeglądu dostępnego piśmiennictwa na temat zjawiska przemocy wobec osób starszych. W związku z zachodzącymi na całym świecie zmianami demograficznymi następuje szybki wzrost populacji osób w wieku 60 lat i starszych. Takie tendencje będą się utrzymywać, co niesie ze sobą stopniowy wzrost liczby przypadków znęcania się nad seniorami. W artykule przedstawiono definicje przemocy wobec osób starszych oraz opisano główne rodzaje stosowanej przemocy, ze szczególnym uwzględnieniem symptomów sugerujących nadużycia. Ponadto wyszczególniono prace koncentrujące się na ocenie czynników ryzyka, które mogą zwiększyć występowanie zachowań agresywnych wobec seniorów. Autorzy skupili się również na analizie dostępnej literatury na temat rozpowszechnienia zjawiska przemocy wobec osób starszych, zarówno w Polsce, jak i na świecie. Analiza danych z krajów o zróżnicowanych uwarunkowaniach strukturalnych i kulturowych jednoznacznie potwierdza obecność tego zjawiska wszędzie, jakkolwiek występuje ono z różnym nasileniem. Rezultaty badań zależą od wielu czynników, m.in. Od zastosowanej metodologii badań i narzędzi pomiaru, przyjętej definicji przemocy czy też źródła informacji. Uwagę zwracają zaniżone statystyki, które mogą być spowodowane niechęcią samych seniorów do ujawniania nadużyć. W związku z wielowymiarowym charakterem zagadnienia ważne jest dalsze wprowadzanie rozwiązań systemowych, celem ograniczenia zjawiska przemocy wobec osób starszych oraz skuteczniejszego wykrywania i rozwiązywania problemu, stanowiącego nadal, zwłaszcza w naszym kraju, temat tabu.
