Praca ma na celu ocenę zmian sprawności funkcjonalnej osób po urazie rdzenia kręgowego podczas obozów Aktywnej Rehabilitacji. Badaniom poddano grupę 52 uczestników obozów w wieku od 18 do 43 lat, znajdujących się w okresie od 6 miesięcy do 2 lat po urazie rdzenia kręgowego. W ocenie skuteczności programu usprawniającego Aktywnej Rehabilitacji wzięto pod uwagę wyniki uzyskane w teście umiejętności użytkownika wózka (Wheelchair Skills Test Version 2.4) oraz teście samoobsługi. W pracy wykazano zwiększenie sprawności funkcjonalnej uczestników obozów AR. W grupie tetraplegików zaobserwowane zwiększenie samodzielności w czynnościach życia codziennego, już po ośmiodniowym okresie usprawniania, związane było z opanowaniem nowych wzorców ruchowych opartych na mechanizmach kompensacyjnych. Natomiast w grupie paraplegików największe zmiany zaobserwowano w technice jazdy na wózku.

The aim of the study is to evaluate functional effficiency in people after spinal cord injury during Active Rehabilitation camps. The examination group consisted of 52 camp participants aged 18 to 43 years, within a period from 6 to 24 months after the spinal cord injury. In order to evaluate the effectiveness of Active Rehabilitation program the Wheelchair Skill Test Version 2.4 and the self-service test were used. Improvement of functional effficiency was observed in the participants of Active Rehabilitation camps. The observed increase of independence while carrying out daily activities after eight days of treatment in the group of tetraplegics was related to mastering of new motor patterns which were based on compensating mechanisms. However, in the group of paraplegics the greatest improvement was observed in the wheelchair driving technique.

Co roku wzrasta liczba młodych ludzi, którzy na skutek urazu rdzenia kręgowego zagrożeni są wykluczeniem społecznym. Osoby te mogą liczyć na pomoc Fundacji Aktywnej Rehabilitacji, której program współtworzony jest przez osoby niepełnosprawne poruszające się na wózku. Jedną z form pomocy jest uczestnictwo w turnusie (obozie) Aktywnej Rehabilitacji. Celem obozu jest nauka czynności samoobsługowych oraz poruszania się na wózku. Uczestnicy poznają indywidualne techniki wykonywania czynności samoobsługowych przy wykorzystaniu mechanizmów kompensacyjnych. Program oparty jest na pięciu dyscyplinach - technice jazdy na wózku, pływaniu, treningu ogólnokondycyjnym, tenisie stołowym oraz łucznictwie. Dobór dyscyplin ma na celu kształtowanie konkretnych umiejętności ruchowych ułatwiających samodzielną egzystencję.

The number of young people, who due to the injury of spinal cord are threatened to be excluded from the society, increases every year. Such people can depend on the help from the Foundation of Active Rehabilitation, whose programme is created together with the handicapped people moving on wheelchairs. One of the forms of help is participation in an Active Rehabilitation camp. The aim of the camp is the study of self-service actions and moving on a wheelchair. Participants know individual techniques of doing self-service actions with the usage of compensation movements. The programme is based on five disciplines i.e. the technique of moving on a wheelchair, swimming, general condition training, table tennis and archery. The selection of disciplines is to mould particular movement techniques which facilitate independent existence.

A 17-year-old paraplegic patient sustained a subtrochanteric femoral fracture due to inadequate trauma. The unusual anatomical conditions associated with his congenital paraplegia did not allow treatment with a standard intramedullary implant for the femur. Because his soft tissues were already compromised, alternative options like plate osteosynthesis were considered unfavourable as a salvage procedure. Therefore, we used an implant designed for the humerus. A satisfactory result of osteosynthesis was achieved despite varus deformity, shortening and rotational error.

Introduction: Every year in Poland spinal cord injury occurs in about 800 people. They have to learn to live in a completely new world suited to the skills and abilities of healthy people. The aim of the research was to find out how the adaptation to self-disability among people with paraplegia proceeds in relation to its duration. Material and methods: 40 people with spinal cord injury took part in the research. They were divided into two groups according to the duration of their disability: up to two years of disability and from five years on. In the research the “Psycho-social adaptation” anamnesis created by the author of this paper was used. Its questions covered four categories: Attitude towards self-disability; Interpersonal relations; Work and recreation; Plans, desires, dreams.Results: Persons with a shorter course of paraplegia more often emphasized the negative sides of their disability: barriers, their own defects and restraints. On the other hand, persons with a longer course of paraplegia were aware of their limitations, but they were also able to point out a lot of positive sides of their disability.Conclusion: The research has confirmed the fact that a longer period of disability leads to better adaptation to it, but further analyses with a larger group of the examined is necessary. It must also be taken into consideration that the course of time is just one of many factors positively affecting the adaptation process.

Neisseria meningitis is a common etiological agent in bacterial meningitis in humans. Common complications associated with meningococcal meningitis include cranial nerve palsies, hydrocephalus, seizures, stroke, cerebritis or brain abscesses. Spinal cord dysfunction is a very rare complication of meningococcal meningitis and its occurrence is limited to case reports only. We report such a case along with a review of available literature and underlying pathogenetic mechanisms. Case report: Our patient diagnosed as a case of meningococcal meningitis developed multiple cranial nerve palsies followed by flaccid paraplegia with bowel bladder involvement during the recovery phase of the disease. Spine MRI showed mild cord edema and diffuse patchy areas of altered intramedullary signal intensity in dorsal cord suggestive of myelitis. The patient was treated with antibiotics and intravenous methyl-prednisolone along with supportive physiotherapy. His neurological deficits showed marked improvement on subsequent follow up.

Objectives: To evaluate changes in Pulmonary Function Test (PFT) parameters in individuals with paraplegia following Partial Body Weight Supported Treadmill Training (PBWSTT). Design: Randomized controlled trial Setting: Inpatient rehabilitation facility Participants: Adults with chronic SCI (n = 42). Intervention: Patients were randomly allocated in CR group (N= 20) receiving Conventional Rehabilitation or in PBWSTT group (N=22) receiving both Conventional Rehabilitation and PBWSTT for 4 weeks. Main outcome measure(s): Changes in % predicted PFT parameter for the subject’s age, sex and BMI. Results: With PBWSTT, significant PFT changes were VC (P =.009), PEF (p = .001) and ERV (p = .032). In complete SCI, PEF (p = .026) improved, while in incomplete SCI VC (p = .005), ERV (p = .029), PEF( p = .001) improved with PBWSTT. In upper neurological level of injury (NLI) (T6-T11), PBWSTT improved PEF (p = .004) alone while in lower NLI (T12-L2), with PBWSTT both ERV (p = .016) and PEF (p = .035) improved. Conclusions: With added PBWSTT most parameters including Vital Capacity, the global measure of PFT, improved significantly, especially in Lower NLI and incomplete SCI. The positive role of this noninvasive exercise based intervention in improving lung functions comes as an added benefit to the usual benefit of locomotion. This may encourage researchers to design future larger studies to validate it aiming the inclusion of PBWSTT in routine SCI rehabilitation protocols.

Background. A spinal cord injury (SCI) leads to patho-physiological changes that can affect physical and psychological performance. The aim of this observational study was to evaluate the relationship between exercise capacity, functioning and quality of life in patients 12 weeks after traumatic paraplegia participating in early rehabilitation. Material and methods. 13 patients participated in this study and performed cardio-pulmonary exercise testing (CPET) on an arm-crank ergometer to determine peak exercise capacity (VO2peak). Data from the spinal cord independence measure (SCIM) were used to assess different areas of functioning. The 12-item short form survey (SF12) questionnaire was applied to measure quality of life. Spearman correlations were used to relate VO2peak with SCIM data and results from the SF12 questionnaire. Results. VO2peak ranged between 12.6 and 28.1 ml/kg/min. A significant relationship was found between VO2peak and the physical component of the SF12 questionnaire, whereas no correlations were found with either SCIM sub or total score, or with the mental component or the total score of the SF12 questionnaire. Conclusions. 1. Patients with traumatic paraplegia showed fair to average exercise capacity after 12 weeks of early rehabilitation. 2. A significant relationship between VO2peak and subjectively rated physical fitness exists at this time point. 3. The implementation of an individual fitness program tailored to the patients' needs based on CPET results is highly recommended in order to improve functioning and quality of life.

Wstęp: Nurkowanie staje się popularną formą aktywności wśród osób niepełnosprawnych, stwarzającą szczególne warunki oddziaływania rehabilitacyjnego. Korzystnie wpływa na zdolności ruchowe i stan psychiczny. W środowisku wodnym fizyczne ograniczenia zmniejszają się, a niepełnosprawni odkrywają nowe możliwości ruchowe własnego ciała. W piśmiennictwie brak jest wystarczających informacji o wpływie nurkowania na różne aspekty fizjologii osób niepełnosprawnych, w tym na autonomiczną regulację czynności układu krąŜenia u osób z uszkodzeniem rdzenia kręgowego. Cel: Zbadanie czy nurkowanie wpływa na równowagę współczulno-przywspółczulną układu autonomicznego u osób z uszkodzeniem rdzenia kręgowego z następową paraplegią. Materiał i metody: Badanie obserwacyjne, seria przypadków. 19 mężczyzn z paraplegią zbadano podczas dwóch odbywających się w wodach otwartych 20-minutowych nurkowań w skafandrach mokrych na głębokość 6 metrów. Badanie obejmowało okres 10 minut przed i 10 minut po 2 rodzajach nurkowania: turystycznym i zadaniowym. Parametry zmienności rytmu zatokowego serca pozyskano za pomocą sport testera Polar Vantage. Wyniki: Nie wykazano istotnego wpływu nurkowania na zmienność rytmu serca. Ujawniono jednak występowanie różnic międzyosobniczych. Wnioski: Nie potwierdzono niekorzystnego wpływu nurkowania na równowagę autonomiczną badanych, jednak występujące różnice międzyosobnicze nakazują ostroŜność w kwalifikowaniu do nurkowania osób z paraplegią.

Background: Diving has become popular among individualss with various kinds of disabilities and may be applied as a rehabilitation method. This form of activity has a beneficial effect on movement abilities and the psychological state. Some physical limitations disappear under water enabling disabled persons to discover new possibilities for their bodies. Yet there is not enough information - about the influence of diving on different aspects of physiology in disabled persons, in particular about sympathetic-parasympathetic balance in cardiovascular system regulation.Aim: To investigate whether diving can influence heart rate variability (HRV) in individuals with paraplegia.Material and methods: In the case series study 19 men with paraplegia were examined during the couse of two 20-minute, 6 meters deep open water dives in wet suits. There were two phases of the investigation: 10 minutes before and 10 minutes after diving. Two kinds of diving were tested: tourist and task diving. HRV parameters measured before and after diving were obtained by means of the sport-tester Polar Vantage .Results: There were no significant changes in heart rate variability after diving but interindividual differences were shown.Conclusions: The unfavourable influence of diving on balance in the autonomic regulation of the cardiovascular system was not confirmed but due to interindividual differences one is unable to foresee heart rate variability changes in persons with paraplegy during diving.

Organophosphate insecticides have been widely used for pest control. They have been readily used as a suicidal agent in developing countries. This paper reports the case of a middle aged male patient with acute organophosphate compound poisoning who in turn had acute cholinergic crisis and was put on ventilator. Within one week, he developed flaccid areflexic paraplegia with preserved sensation. Two weeks later, he had spasticity in both lower limbs with hyperreflexia. The reported case demonstrates the myelopathic presentation of organophosphate-induced delayed polyneuropathy.

Instektycydy fosforoorganiczne znajdują zastosowanie na całym świecie jako środki zwalczania szkodników. Z uwagi na ich łatwą dostępność często sięgają po nie samobójcy, szczególnie w krajach rozwijających się. W niniejszej publikacji przedstawiono przypadek mężczyzny w średnim wieku, u którego wskutek ostrego zatrucia związkami organofosforanowymi doszło do przełomu cholinergicznego – pacjenta trzeba było podłączyć do respiratora. W ciągu tygodnia nastąpiło porażenie poprzeczne wiotkie kończyn dolnych z arefleksją, jednak z zachowaniem czucia. Dwa tygodnie później w obu kończynach wystąpiła spastyczność z nadwrażliwością odruchową. Omawiany w niniejszej pracy przypadek stanowi opis mielopatii w związku z opóźnioną polineuropatią wywołaną organofosforanem.
