One of the quite frequent long-term effects of radiation therapy of malignant tumors, such as breast cancer and Hodgkin’s lymphoma primarily, when the irradiation of the mediastinum is performed, is the development of radiation-induced valvular disease. It is noted that the clinical manifestation of valve dysfunction can typically arise approximately 5 years after a course of anticancer radiotherapy. Thereafter in these patients increases the risk of infective endocarditis. However, in therapeutic and oncological clinical practice the difficulty of identifying and making the differential diagnosis of endocarditis in patients with “cancer” history is still quite common. At the moment cases of infective endocarditis after polychemotherapy and radiation therapy are not widely covered for cardiologists and internists in the medical literature. In addition before starting specific treatment due to the possible threat of infectious complications, including endocarditis, in oncology guidelines it is recommended to patients to make a thorough oral cavity sanitation, while the internists are little aware of this need, both at the stage of antitumor therapy and at late period. Today, with the increasing number of patients who are healed or achieved long-term remission after the anticancer treatment, including elderly people, it is necessary to raise the awareness among internists about occurrence options of comorbid disease in these group.

Implantable cardiac electronic device (ICED) infections include- lead infection (ICED-LI), device pocket infection (PI) and infective endocarditis (ICED-IE). The aim of this study is to analyze the records of patients with ICED, who developed implantable device-related infections. We analyzed retrospectively the records of the University Clinical Centre (Gdańsk) patients who in 2012-2018 underwent transvenous lead extraction (TLE) due to infections. In order to identify potential ICED infection risk factors we included patients who underwent any electrotherapy procedure within 2 years prior to the TLE. ICED infections that led to septic shock were defined as severe. The analyzed sample included 59 patients with infectious complications (37 male and 22 female) with median age of 74. The in-hospital mortality was 8.5%. All patients with severe ICED infection were diagnosed with ICED-LI, whereas the rest of the sample was diagnosed mostly with PI (p<0.001). The most commonly cultured pathogens were S. aureus and S. epidermidis. In the analyzed sample, the most common infectious complication related to the ICED was PI and the most common etiological agents were S. aureus and S. epidermidis. Severe ICED infections that present with septic shock are associated with a 50% in-hospital mortality rate.

Infective endocarditis (IE) is a life-threatening condition whose diagnosis often remains a major challenge in everyday clinical practice. Accurate and fast diagnosis of IE improves the patient’s prognosis. Over the past decades there have been significant changes in the aetiology, demographic characteristics of patients as well as in the clinical presentation. IE diagnosis requires evaluation of the clinical presentation, microbiological tests and cardiac imaging. Modified Duke University criteria enable the diagnosis of most IE cases but frequently they are insufficient, particularly in difficult-todiagnose patients. The 2015 European Society of Cardiology guidelines on IE have defined when new imaging modalities should be used to aid in the diagnosis of IE. The aim of this paper is to show the advantages and disadvantages of current imaging modalities in the diagnosis of IE.

Infekcyjne zapalenie wsierdzia (IZW) jest zagrażającym życiu schorzeniem, którego rozpoznanie w codziennej praktyce klinicznej często pozostaje dużym wyzwaniem. Od szybkiego postawienia właściwego rozpoznania zależy rokowanie chorego. W ciągu ostatnich dekad doszło do istotnych zmian w zakresie etiologii, charakterystyki demograficznej pacjentów oraz w obrazie klinicznym IZW. Rozpoznanie IZW wymaga połączenia danych z obrazu klinicznego, badań mikrobiologicznych oraz wyników badań obrazowych. Stosowane do tej pory zmodyfikowane kryteria diagnostyczne z Uniwersytetu Duke pozwalają na postawienie rozpoznania w większości przypadków, jednak coraz częściej okazują się one niewystarczające. Opublikowana w 2015 roku przez Europejskie Towarzystwo Kardiologiczne aktualizacja wytycznych dotyczących postępowania w IZW pozycjonuje rolę nowych technik obrazowych, których użycie w wątpliwych diagnostycznie przypadkach pozwala na ostateczne postawienie rozpoznania u większości chorych. Celem niniejszego artykułu jest omówienie roli i znaczenia poszczególnych metod diagnostyki obrazowej w diagnostyce IZW z uwzględnieniem ich możliwości oraz ograniczeń.

The clinical manifestation of infective endocarditis is usually non-specific. The signs and symptoms may be due to various infections, auto-aggressive or rheumatologic diseases. In some cases symptoms of endocarditis can mimic neoplastic disease. It is essential to diagnose infective endocarditis correctly and start the treatment as soon as possible because the outcome of the disease may be severe or even fatal. The disease is caused by the vegetations, characteristic changes. Vegetation is a mass of different cells: platelets, inflammatory cells as well as fibrin and microorganisms. Lesions are usually found on the valves of the left heart and lead to local destruction of the inner layer of the heart muscle. Parts of vegetation may drift with the blood flow and become the cause of cerebral or peripheral thrombus. However neurological disorders such as temporary ischemic attack or stroke can by the first symptom of endocarditis, usually the disease starts with fever. Physician can also find a new murmur of valve regurgitation. It is important to emphasize that there is an growing number of endocarditis due to cardiosurgical and cardiological procedures, especially concerning prosthetic valve or permanent electrode implantation. Intravenous drug abuse by addicts also may lead to endocarditis, in those cases lesions are often found on the tricuspid and pulmonary valves. Generally the most common cause of infective endocarditis is Staphylococcus spp. Diagnosis of endocarditis is based on clinical investigation, echocardiography and blood cultures. Primary treatment consists of multidrug antibiotic therapy and has to be started as soon as possible. In some cases surgical intervention is necessary. As far as endocarditis is concerned it is also essential to know when patients should be given antibiotics as prevention treatment. The rules of prevention of endocarditis have changed many times for the last few years. In summary: although endocarditis is rare disease, it is often difficult to diagnose and may lead to death. Therefore it is important for physicians to have at least a basic knowledge of infective endocarditis.

Infekcyjne zapalenie wsierdzia (IZW) cechuje niejednorodny obraz kliniczny, z pogranicza przewlekłych, niesprecyzowanych chorób infekcyjnych, autoimmunologicznych, reumatologicznych oraz nowotworowych. Szybka diagnostyka i włączenie leczenia celowanego IZW są konieczne z uwagi na poważne rokowanie i bezpośrednie zagrożenie życia pacjenta. W procesie IZW w obrębie zastawek serca tworzą się konglomeraty bakterii, komórek zapalnych, płytek krwi i włóknika, zwane wegetacjami. Dotyczą najczęściej zastawek lewej strony serca. Wegetacje doprowadzają do miejscowej destrukcji wsierdzia oraz stanowią źródło zatorowości obwodowej. Najczęstszym objawem IZW jest gorączka, natomiast w badaniu przedmiotowym zwraca uwagę nowy szmer wysłuchiwany nad sercem, świadczący o uszkodzeniu zastawek lub nici ścięgnistych. Zdarza się, że pierwszym symptomem choroby są zaburzenia neurologiczne spowodowane udarem mózgu powstałym w mechanizmie zatorowym. W ostatnim czasie wzrasta liczba zachorowań na IZW związanych ze stosowanymi procedurami inwazyjnymi wewnątrzsercowymi z implantacją sztucznych zastawek oraz elektrod. Szczególną grupę chorych stanowią również narkomani, u których najczęściej dochodzi do zajęcia zastawek prawej strony serca. Głównym czynnikiem etiologicznym zapalenia wsierdzia są bakterie, wśród nich obecnie dominują zakażenia gronkowcowe. Rozpoznanie choroby opiera się na danych klinicznych, obrazie echokardiograficznym oraz badaniach bakteriologicznych krwi. Leczeniem przyczynowym IZW jest antybiotykoterapia wielolekowa prowadzona w warunkach szpitalnych. Konieczne jest ścisłe monitorowanie choroby i rozważenie wskazań do leczenia kardiochirurgicznego. Istotnym zagadnieniem są również metody zapobiegania zakażeniom wsierdzia. Zasady profilaktyki IZW na przestrzeni ostatnich lat uległy znacznym modyfikacjom. Infekcyjne zapalenie wsierdzia wciąż pozostaje chorobą o poważnym rokowaniu, niejednokrotnie kończącą się zgonem pacjenta, a podstawowa wiedza o tym schorzeniu wydaje się niezbędna dla każdego lekarza.
