New treatment strategies for HIV/AIDS are very successful in reducing viral load. However, reconstitution of the immune system takes about one year and may be insufficient or remain incomplete. During this time the patient remains prone to opportunistic infections as a result of the complex immune dysfunction caused by the virus. Recombinant granulocyte colony-stimulating factor (G-CSF) has diverse immunomodulatory properties which may be beneficial to aid immune reconstitution.

One of the strongest predictors for HIV-1-associated dementia is the presence of monocytic infiltration in perivascular areas of the brain. Therefore, macrophages have been suggested to play a major role in the development of this disease. This review focuses on possible mechanisms through which the macrophage may enhance disease progression by mediating neuronal damage.

Istnieje wiele chorób człowieka, które mogą doprowadzić do niewydolności narządów. Konsekwencją często jest konieczność przeprowadzenia przeszczepu. Liczba wykonywanych operacji jest bardzo niska ze względu na niedobór narządów do transplantacji. W konsekwencji liczba osób oczekujących na przeszczep ciągle rośnie. Wyjściem z tej sytuacji może być ksenotransplantacja. Pojęcie ksenotransplantacja pochodzi od greckiego xenos oznaczającego obcy, inny. Jest to każdy zabieg polegający na transplantacji, implantacji lub infuzji biorcy – człowiekowi – komórek, tkanek lub organów odzwierzęcych, również płynów ustrojowych, komórek, tkanek, narządów człowieka (lub ich fragmentów), które miały kontakt ex vivo z komórkami, tkankami lub organami zwierzęcymi. Jedną z przeszkód w przeszczepach ksenogenicznych jest zagrożenie przeniesienia patogenów zwierzęcych i zainfekowanie organizmu człowieka. Wirusami, które stanowią zagrożenie w przeszczepach, w układzie małpy naczelne-człowiek to między innymi: ludzki wirus upośledzenia odporności HIV (z ang. human immunodeficiency virus) i wirus Marburg, które zostały opisane w niniejszej pracy. Ponadto przedstawiono wirusy stanowiące problem w transplantacjach w układzie świnia-człowiek, czyli: endogennego retrowirusa PERV (z ang. porcine endogenous retrovirus), wirusa cytomegalii świni PCMV (z ang. porcine cytomegalovirus), wirusa limfotroficznego świni PLHV (z ang. porcine lymphotropic herpesvirus) oraz wirusa zapalenia wątroby typu E - HEV (z ang. hepatitis E virus). Niniejszy przegląd literatury stanowi najnowszy stan wiedzy na temat mikrobiologicznego bezpieczeństwa ksenotransplantacji.

There are a number of human diseases, which can lead to organ failure. The consequence is often the need for a transplant. The number of performed operations is very low due to the shortage of organs for transplantation. As a consequence, the number of people waiting for transplant is still growing. The solution to this situation may be xenotransplantation. Xenotransplantation word comes from the Greek xenos meaning stranger, the other. It is defined as any procedure that involves the transplantation, implantation or infusion of tissues or zoonotic organs into a human recipient, but also human body fluids, cells, tissues, organs (or fragments) that have ex vivo contact with zoonotic cells, tissues or organs. One of the obstacles of the xenograft transplantation is the risk of animal pathogens transmission to the humans. Viruses that pose risk in the non-human primates-to-human xenotransplantation includes: the human immunodeficiency virus - HIV and the Marburg virus described in this paper. In addition viruses, which is a problem in pig-to-human xenotransplantation have also been described, including: porcine endogenous retrovirus - PERV, porcine cytomegalovirus - PCMV, porcine lymphotropic herpesvirus - PLHV and hepatitis E virus - E - HEV. This review of literature is the latest knowledge of the microbiological safety of xenotransplantation.

Increasing clinical evidence is emerging that other persistent viral infections can act as important co-factors affecting the progression of human immunodeficiency virus-1 (HIV-1). It appears that hepatitis C (HCV) and cytomegalovirus (CMV) have a deleterious effect on HIV progression, whereas hepatitis G (GBV-C) benefits HIV-1 progression. At the same time, the aggressive nature of HCV infection in HIV is clearly recognized. Here we discuss this clinical evidence and go on to review scientific work pertaining to these interactions in the context of the known and theoretical immunological effects of these viruses. This is discussed at the level of the generation of adaptive immune responses and their effector functions. It is clear that co-infection with persistent viral infections may pose special problems for the human immune system, as pathogenic effects may not be specific to the actual eliciting virus and can therefore multiply the difficulties faced by host defenses. We also highlight the need for further therapies for HIV/HCV co-infected persons, as this is currently a complex and severe syndrome.

Macrophages, centrally involved in both the innate and adaptive arms of the immune system, are not only the chief target of the human immunodeficiency virus (HIV), but also its main reservoir and vehicle of transmission. Macrophage-tropic viruses are responsible for the initial infection, predominate in the asymptomatic phase, and persist throughout infection, even after the emergence of preferential T cell- and/or dual-tropic HIV-1 variants. Functional impairment of HIV-infected macrophages plays a role in the immune dysregulation characteristic of AIDS (acquired immunodeficiency syndrome). Efforts directed ant understanding the cellular and molecular mechanisms underlying HIV-macrophage interactions remain the basis for devising novel and efficacious therapeutic strategies against HIV and the AIDS epidemic

Ocena stanu wiedzy młodych ludzi na temat HIV/AIDS

Introduction. Acquired Immunodeficiency Syndrome (AIDS) caused by human immunodeficiency virus (HIV) is one of the most serious infectious diseases on a global scale. Since the outbreak of the epidemic there was 78 million people infected with HIV, 35 million of whom have died. The AIDS control in Poland began in 1988. Aim. Assessment the knowledge level of young Polish people about AIDS. Materials and methods. The survey was conducted among 346 people utilizing the social media channels. They received 11 questions including the basic definitions, ways of infection and diagnosis of AIDS. Results. The results of our study confirm that people have insufficient knowledge of this serious disease. Less than 40% of all respondents could point the most common paths of infection and the time of first symptoms after exposure. Nearly 50% knew that AIDS and HIV are not equal terms. Conclusions. There is a need to raise awareness about HIV and AIDS. Young people should learn much more about mechanisms, symptoms and particularly prevention of this still untreatable disorder.

Wprowadzenie. Zespół nabytego niedoboru odporności, AIDS (ang. Acquired Immunodeficiency Syndrome), wywoływany przez ludzkiego wirusa niedoboru odporności (HIV, ang. human immunodeficiency virus) to jedna z najpoważniejszych chorób w skali świata. Szacuje się, że od wybuchu epidemii zakażeniu wirusem uległo 78 milionów ludzi, a około 35 milionów chorych zmarło. W Polsce epidemiologia i problematyka AIDS oraz HIV została podjęta w 1988 roku. Cel pracy. Ocena stanu wiedzy młodych ludzi na temat HIV i AIDS. Materiały i metody. Przeprowadzono ankietę internetową wśród 346 osób. Respondenci otrzymali 12 pytań na temat podstawowych pojęć, dróg zakażenia oraz diagnostyki AIDS. Wyniki. Wyniki ankiety potwierdzają, że wiedza młodych ludzi na temat HIV i AIDS jest niewystarczająca. Niecałe 40% ankietowanych potrafiło wskazać poprawnie główne drogi infekcji oraz prawidłowy przedział czasu pomiędzy ekspozycją a pojawieniem się pierwszych niespecyficznych objawów. Blisko połowa badanych miała świadomość, że pojęcia AIDS i HIV nie są tożsame. Wnioski. Istnieje potrzeba szerzenia wiedzy na temat HIV i AIDS, szczególnie wśród młodych ludzi. Tylko świadomość mechanizmu choroby, dróg zakażenia, a przede wszystkim możliwej profilaktyki, pozwoli na zminimalizowanie ryzyka zachorowania na chorobę, na którą obecnie wciąż nie wynaleziono remedium.

Numerous metabolic and genetic diseases are due to mutations in adaptins, dynamin-like GTP-ases or disorders in trafficking machinery mediated by Rab proteins. A great number of pathogenes including viruses (HIV, SIV), bacteria and protozoa use various elements of endocytic/trafficking machinery to get into the host cells and to make their infection successful. Their different strategies are discussed.

Interleukin 10 (IL-10), a cytokine with inhibitory activity on inflammation and cell -mediated immune responses (CMIR), holds enormous potential for the treatment of inflammatory and autoimmune disorders. In addition, IL-10 has also been implicated in the immunopathogenesis of a number of infectious diseases through the use of IL-10 knock-out or IL-10 transgenic mouse models. In this review, we delineate infectious and inflammatory conditions in which IL-10 has shown potential for therapeutic manipulation. Specifically, we review the role of IL-10 in human endotoxemia/sepsis and in HIV infection, conditions for which preliminary phase I trials have recently been undertaken. It is suggested that the therapeutic potential of IL-10 to selectively ameliorate human infectious and inflammatory processes can be realized through a careful selection of the clinical conditions in which patients are undergoing concomitant treatment with anti-microbial regimens.

A number of antiviral lectins, small proteins that bind carbohydrates found on viral envelopes, are currently in pre-clinical trials as potential drugs for prevention of transmission of human immunodeficiency virus (HIV) and other enveloped viruses, such as the Ebola virus and the coronavirus responsible for severe acute respiratory syndrome (SARS). Lectins of algal origin whose antiviral properties make them candidate agents for prevention of viral transmission through topical applications include cyanovirin-N, Microcystis viridis lectin, scytovirin, and griffithsin. Although all these proteins exhibit significant antiviral activity, their structures are unrelated and their mode of binding of carbohydrates differs significantly. This review summarizes the current state of knowledge of the structures of algal lectins, their mode of binding of carbohydrates, and their potential medical applications.

The Human immunodeficiency virus (HIV) is a lentivirus that causes HIV infection, and over time, Acquired Immunodeficiency Syndrome (AIDS). HIV infection (simply called HIV at its early stage and AIDS at its later stage) is a very prevalent global disease condition affecting about 36.7 million people, both young and old. Sadly, HIV/AIDS affects almost all the systems in the body (particularly the neurological and musculoskeletal systems), which may result into physical disability. Physiotherapists are known to play important roles in the management of physical disability. In this paper, the authors provide information on the clinical and epidemiological features of HIV/AIDS, as well as the treatment modalities for the neurological and musculoskeletal problems experienced by people living with HIV/AIDS.

Introduction. Hepatitis C virus (HCV) infection is a global health problem which can lead to liver cirrhosis or hepatocellular carcinoma in one-fifth of chronically infected patients. Materials and methods. The study group consisted of 123 patients: 90 with HCV mono- and 33 with HIV/HCV co-infection, who were treated with pegylated interferon alfa (Peg-IFN-α) and ribavirin. We analyzed selected pretreatment factors: age, sex, HIV/HCV co-infection, grade of inflammation, necrotic changes and fibrosis in histological analysis of liver bioptates, HCV viral load, HCV genotypes, and single nucleotide polymorphisms (SNPs) of IL28B and tried to find out which of them influence sustained virological response (SVR). The IL28B SNP C/T (rs12979860) was analyzed using Custom® SNP Genotyping Assays (Applied Biosystems). Results. Multivariate analysis demonstrated that after adjusting for the other variables three predictors independently influence SVR, namely genotype 3 of HCV, presence of the CC genotype and age >40 years (OR respectively 15.14, 3.62, and 0.36). HCV mono-infected patients were infected with HCV genotype 3 or 4 less frequently (p=0.0001) compared to HIV/HCV co-infected individuals. In patients with HIV/HCV co-infection the CC variant occurred more frequently whereas CT was found less frequently (p=0.001, p=0.0146, respectively). In patients with HIV/HCV co-infection, 3 and 4 genotype of HCV occurred more frequently compared to patients with HCV mono-infection (p=0.0001). Conclusions. These data suggest that age, HCV genotype and IL28B polymorphism are useful for prediction of the response to treatment with Peg-IFN-α and ribavirin. The more frequent occurrence of HCV genotypes 3 or 4 in patients with HIV/HCV co-infection could be associated with the route of transmission.

Anal human papillomavirus (HPV) infection commonly affects men who have sex with men (MSM) and is associated with the development of anal cancer, with the highest prevalence in MSM who are positive for human immunodeficiency virus (HIV). The purpose of this study was to determine the prevalence of anal HPV infection and genotype distribution in Slovenian MSM and its correlation with behavioral patterns in order to predict the possible impact of quadrivalent HPV vaccine in this population. To our knowledge, this is the first study of prevalence and genotype distribution of anal HPV infection in MSM from any of the Eastern European countries. The study included 136 MSM who visited two outpatient offices from January 2007 through December 2008. All participants were clinically examined and tested for HIV, and anal swabs were taken for HPV testing. The results showed a high prevalence of anal HPV infection in both HIV-negative (75%) and HIV-positive (95%) MSM. Promiscuity and use of “poppers” (alkyl nitrites taken for recreational purposes through direct inhalation) were clearly associated with a higher prevalence of anal HPV infection. The four most common HPV genotypes were 6, 11, 16, and 18. The prevalence of anal HPV infection in Slovenian MSM is higher in comparison with other studies. Quadrivalent HPV vaccine could have great potential in this population.

The aim of the study was to evaluate the nature and frequency of sharps injuries among doctors and nurses from the same surgical/gynecological wards and the prevalence of HBV/HCV/HIV infection.Material and methods. An anonymous cross-sectional sero-survey, with ELISA system used to detect anti-HBc, anti-HCV, anti-HIV, was conducted among 89 doctors and 414 nurses from 16 randomly selected hospitals in West Pomerania, Poland, between January-June 2009.Results. During the preceding 12 months, 82% doctors and 44.4% nurses (p<0.0001) had sustained at least one sharps injury; 12.3% doctors vs 2.2% nurses (p<0.003) sustained more than 10 injuries. The multivariable regression model revealed that being a doctor was associated with a greater odds (OR 4.2) of being injured with sharps. Sixty nine percent of nurses sustained a hollow-bore needle injury vs 8.9% doctors; p<0.001. Anti-HBc were found in 16.4% of doctors and 11.2% of nurses, p>0.28; anti-HCV - in 1.1% of doctors vs 1.4% of nurses, p>0.79; no anti-HIV positive cases were found. The analysis of potential risk factors for contracting a HBV revealed that for both job categories only length of employment was associated with an increased odds of being infected.Conclusions. Although the prevalence of HBV/HCV infection between doctors and nurses does not differ significantly, modifiable risk factors for contracting a BBI such as frequency and nature of sharps injuries may differ, which call for tailoring preventive measures to specific job categories. Long lasting exposure to injury events should be taken into consideration while assessing the risk for accuiring an occupational infection with HBV, HCV or HIV.

Introduction. Antimicrobial proteins (antimicrobial peptides - AMP) probably belong to one of the oldest and most primitive defense mechanisms of the body. It is now known about 800 types of AMP present in plants, insects and animals. Two main types of AMP is likely to occur in mammalian defensins and cathelicidin antimicrobial activity showing. Defensins are small cationic proteins that are part of the innate immunity, humoral immunity. Among of mammalian defensins can be distinguished: defensin α, β defensins and θ defensins. Human defensins are located in various tissues, mainly synthesized in neutrophils and other immune cells. Purpose. Provide an overview of the literature on the construction, function and possible use of defensins in the treatment of infections. Materials and methods. Using the key words: antibiotics, Antimicrobial peptides, defensins, treatment of infections, HIV searched bibliographic databases: Medline, Science Direct, Ebsco, Springer Link, Wiley Online Library. Results. Selected articles were used to describe the structure and function of defensins. Discusses the latest news on the possible use of defensins in the treatment of infections and the state of knowledge about their function in the course of infection with human immunodeficiency virus (HIV) and infections involving multidrug-resistant bacteria. Conclusions. The literature review shows the important function of defensins they play in the course of infection. Defensins through a variety of properties are an attractive alternative to traditional methods of treating infections caused by pathogenic microorganisms. It is necessary to better understand the mechanism of action of these substances in vivo models.

Wstęp. Białka antydrobnoustrojowe (antimicrobial peptides – AMP) prawdopodobnie należą do jednych z najstarszych i najbardziej prymitywnych mechanizmów obronnych organizmu. Obecnie jest znanych około 800 rodzajów AMP występujących u roślin, owadów oraz zwierząt. Dwa główne rodzaje AMP występujące u ssaków to katelicydyny oraz defensyny wykazujące działanie przeciwdrobnoustrojowe. Defensyny to małe białka kationowe, które są elementem odporności nieswoistej, humoralnej. Wśród defensyn występujących u ssaków można wyróżnić: defensyny α, defensyny β oraz defensyny θ. Ludzkie defensyny zlokalizowane są w różnorodnych tkankach, głównie syntetyzowane w neutrofilach i innych komórkach układu odpornościowego. Cel. Przedstawienie przeglądu literatury na temat budowy, funkcji oraz możliwości wykorzystania defensyn w leczeniu zakażeń. Materiały i metody. Posługując się kluczowymi słowami: antybiotyki, peptydy antydrobnoustrojowe, defensyny, leczenie zakażeń, HIV przeszukano bazy bibliograficzne: Medline, ScienceDirect, Ebsco, SpringerLink, Wiley Online Library. Wyniki. Wybrane artykuły wykorzystano do opisu budowy oraz funkcji defensyn. Omówiono najnowsze doniesienia na temat możliwości wykorzystania defensyn w leczeniu zakażeń oraz stan wiedzy na temat ich funkcji w przebiegu zakażenia ludzkim wirusem niedoboru odporności (HIV) oraz zakażeń z udziałem wielolekoopornych bakterii. Wnioski. Przegląd literatury ukazuje ważną funkcję defensyn jaką pełnią w przebiegu zakażeń. Defensyny dzięki różnorodnym właściwościom stanowią atrakcyjną alternatywę dla tradycyjnych metod leczenia zakażeń wywołanych patogennymi mikroorganizmami. Konieczne jest lepsze poznanie mechanizmu działania tych substancji w modelach in vivo.

The authors discuss new data on morbidity rates and susceptibility to HIV infection, next to the progress of AIDS. They quote the current epidemiological data, which show that in Central and Western European conutries the epidemic proceeds diff erently than in other regions of the world, so that it does not meet here pessimistic predictions about the expected rise of morbidity and mortality rates. The authors then focused on highlighting the detected diff erences in genetic and immunological state of recently discerned groups of patients, whose symptoms of AIDS do not appear for more than 10 years (LTNP – long term nonprogressors), and patients characterized by rapid disease progression (RP – rapid progressors), and in particular, people resistant to infection despite intensive exposure (EU-exposed uninfected). Discussed and detected diff erences enable to undertaken reasonable eff orts to develop an eff ective vaccine. What is more, the authors also mentioned the fi rst results achieved through new research methods, such as testing all genes (whole genome) of many individuals to detect diff erences occurring from individual to individual (genome-wide association study – GWA) or determination of all the particles within RNA profi les concerning a population of given cells (genome-wide transcriptome

Autorzy omawiają nowe dane dotyczące zróżnicowania podatności na zakażenie HIV i zapadalność na AIDS oraz tempo progresji choroby. Na wstępie przypominają aktualne dane epidemiologiczne, z których wynika, iż w krajach Europy Środkowej i Zachodniej epidemia przebiega odmiennie niż w innych regionach świata, nie spełniły się tutaj pesymistyczne przewidywania dotyczące narastania zachorowalności i umieralności. Autorzy koncentrują się na odmiennościach w zakresie uwarunkowań genetycznych i immunologicznych w grupach chorych, u których objawy choroby AIDS nie występują przez okres dłuższy niż 10 lat (LTNP – long term nonprogressors) i chorych z szybkim postępem choroby (RP – rapid progressors) oraz w szczególności u osób odpornych na zakażenie mimo intensywnej ekspozycji (EU – exposed uninfected). Omówione w pracy różnice umożliwiają racjonalne scharakteryzowanie podejmowanych współcześnie prób opracowania skutecznej szczepionki. Wspomniano także o pierwszych efektach nowych metod badawczych, takich jak badanie wszystkich genów (całości genomów) wielu osobników w celu wykrycia różnic między osobnikami (genome-wide association study – GWA) lub oznaczanie profili wszystkich cząstek RNA pewnej populacji komórek (genome-wide transcriptome profiling study), które mogą zastosować jedynie bardzo dobrze wyposażone placówki badawcze.

Lactoferrin is a secreted protein found in high concentrations in colostrum and milk, less aboundant in saliva, tears, semen and mucus etc. etc., has been attracting the attention of researchers because of the broad spectrum of action. Lactoferrin has an antibacterial, antifungal, antiviral, immunomodulatory, antineoplastic activity and many others. The following article describes the results of recent research on medical applications of lactoferrin. This protein is very interesting because of the variety of functions performed in the absence of toxicity, as evidenced by the certificates have GRAS (Generally Recognized as Safe).

W poniższym artykule opisano wyniki najnowszych badań dotyczących medycznych zastosowań laktoferyny. Białko to jest niezwykle interesujące ze względu na różnorodność pełnionych funkcji przy jednoczesnym braku toksyczności, czego dowodem jest posiadanie certyfikatu GRAS (Generally Recognized as Safe).
