A growing number of people with dementia translates into the necessity to use modern imaging methods. Function tests assessing glucose metabolism in the brain, such as 18F-FDG PET/CT, are conducted in the case of clinical doubts and facilitate the differential diagnostics of dementia. In the case of the Alzheimer’s disease, glucose metabolism disorders become visible, above all, in the area of both the parietotemporal areas, posterior parts of the callosal gyrus, the precuneus as well as medial temporal lobe. In the case of dementia with the Lewy bodies, glucose metabolism disorders relate to the occipital lobes and both the parietotemporal areas. In the case of frontotemporal dementia, lesions are observed, above all, in the frontal lobes and poles of the anterior temporal lobes. On the other hand, in the case of vasogenic dementia, there are numerous, spread deficits visible in collecting a marker within the brain – both the cortex and the subcortical nuclei. Characteristic glucose metabolism disorders are present also in the case of other, less common neurodegenerative diseases, such as corticobasal degeneration and the atrophy of the posterior part of the brain. In the case of corticobasal degeneration, one can observe a decreased collection of 18F-FDG within the sensory-motor cortex, in the subcortical nuclei and the thalamus on the side with the disease, while in the case of atrophy of the posterior part of the brain – within both the parieto-occipital areas. Besides glucose metabolism, it is possible to assess the presence of β amyloid in the brain. It is deemed that a negative result of PET/CT for β amyloid enables to exclude the Alzheimer’s disease as being the cause of dementia. The utilisation of non-invasive PET/CT makes it possible to early diagnose dementia and determine the prognosis.

Rosnąca liczba starszych osób z otępieniem przekłada się na konieczność stosowania nowoczesnych metod obrazowania. Badania czynnościowe oceniające metabolizm glukozy w mózgowiu, takie jak 18F-FDG PET/CT, są przeprowadzane w razie wątpliwości klinicznych i ułatwiają diagnostykę różnicową chorób otępiennych. W przypadku choroby Alzheimera zaburzenia metabolizmu glukozy uwidaczniają się przede wszystkim w obrębie obu okolic skroniowo-ciemieniowych, tylnych części zakrętów obręczy, przedklinka, a także części przyśrodkowych płatów skroniowych. W otępieniu z ciałami Lewy’ego zaburzenia metabolizmu glukozowego dotyczą płatów potylicznych i obu okolic skroniowo-ciemieniowych. W zwyrodnieniu czołowo-skroniowym zmiany obserwuje się przede wszystkim w płatach czołowych i biegunach przednich płatów skroniowych. Z kolei w otępieniu naczyniopochodnym widoczne są liczne, rozsiane ubytki gromadzenia znacznika w obrębie mózgowia – zarówno kory, jak i jąder podkorowych. Charakterystyczne zaburzenia metabolizmu glukozy są obecne również w innych, rzadszych chorobach neurodegeneracyjnych, takich jak zwyrodnienie korowo-podstawne i zanik tylnej części mózgu. W zwyrodnieniu korowo-podstawnym obserwuje się zmniejszone gromadzenie 18F-FDG w obrębie kory czucioworuchowej, w jądrach podkorowych oraz we wzgórzu po zajętej stronie, natomiast w zaniku tylnej części mózgu – w obrębie obu okolic ciemieniowo-potylicznych. Oprócz metabolizmu glukozy można oceniać obecność amyloidu β w mózgowiu. Uważa się, że negatywny wynik badania PET/CT pod kątem amyloidu β pozwala na wykluczenie choroby Alzheimera jako przyczyny zespołu otępiennego. Zastosowanie nieinwazyjnych badań PET/CT umożliwia wczesne postawienie diagnozy zespołu otępiennego i ustalenie rokowania.
During routine screening after abdominal aorta aneurysm repair, thrombocytopenia accompanied by chronic activation of coagulation was detected in an 80-year old patient. Coagulation activity was assessed by the increased concentration of coagulation and fibrinolysis activation markers (prothrombin fragment F1+2 and D-dimer). After the most common causes of such activation were excluded using imaging and laboratory studies, Indium-111 labeled autologous blood platelet accumulation on a dacron graft was demonstrated using the scintigraphic technique. Thus, the implanted dacron graft was the cause of chronic thrombocytopenia and coagulation activation during the two years of follow up.
Background: The imaging techniques most commonly used in the diagnosis of hyperparathyroidisms are ultrasound and scintigraphy. The diagnostic algorithms vary, depending mainly on the population, and experience of physicians. Aim: Aim of the present research was to determine the usefulness of parathyroid scintigraphy and ultrasonography in patients diagnosed for hyperparathyroidism in own material. Material and method: In the present research, 96 operated patients with documented primary, secondary and tertiary hyperparathyroidism were retrospectively analyzed. All patients underwent a 99mTc hexakis2-methoxyisobutylisonitrile scintigraphy of the neck with the use of subtraction and twophase examinations. Ultrasonography of the neck was performed in all the patients in B mode 2D presentation. A total number of 172 parathyroid glands were analyzed. Results: The sensitivity and specificity of scintigraphy was 68% and 60%, respectively. The sensitivity of ultrasound was 49% and specificity 85%. Both techniques allowed visualization of 76 parathyroid glands. Ultrasound revealed 19 glands that were not visible in scintigraphy. Scintigraphy showed 76 parathyroid glands that were not visualized on ultrasound. Having combined the results of scintigraphy and ultrasound, the sensitivity of 76% and specificity of 50% were obtained. Considering the ability to locate the parathyroid glands in both techniques as a positive result, the sensitivity decreased to 37% and specificity rose to 95%. Conclusions: Scintigraphy showed greater sensitivity than ultrasound in the localization of enlarged parathyroid glands. Ultrasound, in turn, was characterized by a higher specificity. The combined use of scintigraphy and ultrasonography allowed to obtain the specificity of 95%. In the light of obtained results, scintigraphy and ultrasonography are complementary and should be used together.

Pomimo rozwoju technik obrazowych szereg kontrowersji budzi ustalenie algorytmu postępowania diagnostycznego u chorych na nadczynność przytarczyc. Ze względu na swój niewielki rozmiar oraz zmienne położenie prawidłowe przytarczyce nie są widoczne w żadnej z obecnie stosowanych technik obrazowych, a ich uwidocznienie okazuje się możliwe dopiero wówczas, gdy ulegają powiększeniu lub wykazują cechy nadczynności. Problemy związane z decyzją o wykonaniu badania obrazowego dotyczą niemal każdego etapu diagnostyki: kwalifikacji chorych, wyboru rodzaju i techniki wykonania badania obrazowego, często również interpretacji wyników(1–4). W wielu przypadkach badanie obrazowe przed pierwszym zabiegiem operacyjnym nie jest potrzebne. Dopiero w razie nawrotu nadczynności lub nieskutecznej operacji wizualizacja przytarczyc staje się konieczna. Jednak ponieważ w ostatnich latach metodą operacyjną z wyboru stała się minimalnie inwazyjna paratyroidektomia, większość chirurgów opowiada się za wykonaniem badania lokalizacyjnego przed pierwszą operacją(5). Do rutynowo stosowanych metod diagnostyki obrazowej należą ultrasonografia, scyntygrafia, a także tomografia komputerowa i rezonans magnetyczny. Najczęściej wykorzystywanymi technikami są badanie USG i scyntygrafia(6–8). W publikowanym piśmiennictwie autorzy są zgodni w ocenie, że scyntygrafia pod względem czułości i swoistości przewyższa inne techniki obrazowe, natomiast USG jest techniką najszerzej dostępną. Jednakże algorytm stosowania poszczególnych badań obrazowych, jak również ich czułość i swoistość różnią się, w zależności od badanej populacji chorych, doświadczenia lekarzy oraz użytych protokołów badań. W niniejszej pracy porównano wyniki badania ultrasonograficznego i scyntygraficznego w do- świadczeniach własnych.
