Co roku wzrasta liczba młodych ludzi, którzy na skutek urazu rdzenia kręgowego zagrożeni są wykluczeniem społecznym. Osoby te mogą liczyć na pomoc Fundacji Aktywnej Rehabilitacji, której program współtworzony jest przez osoby niepełnosprawne poruszające się na wózku. Jedną z form pomocy jest uczestnictwo w turnusie (obozie) Aktywnej Rehabilitacji. Celem obozu jest nauka czynności samoobsługowych oraz poruszania się na wózku. Uczestnicy poznają indywidualne techniki wykonywania czynności samoobsługowych przy wykorzystaniu mechanizmów kompensacyjnych. Program oparty jest na pięciu dyscyplinach - technice jazdy na wózku, pływaniu, treningu ogólnokondycyjnym, tenisie stołowym oraz łucznictwie. Dobór dyscyplin ma na celu kształtowanie konkretnych umiejętności ruchowych ułatwiających samodzielną egzystencję.

The number of young people, who due to the injury of spinal cord are threatened to be excluded from the society, increases every year. Such people can depend on the help from the Foundation of Active Rehabilitation, whose programme is created together with the handicapped people moving on wheelchairs. One of the forms of help is participation in an Active Rehabilitation camp. The aim of the camp is the study of self-service actions and moving on a wheelchair. Participants know individual techniques of doing self-service actions with the usage of compensation movements. The programme is based on five disciplines i.e. the technique of moving on a wheelchair, swimming, general condition training, table tennis and archery. The selection of disciplines is to mould particular movement techniques which facilitate independent existence.
Praca ma na celu ocenę zmian sprawności funkcjonalnej osób po urazie rdzenia kręgowego podczas obozów Aktywnej Rehabilitacji. Badaniom poddano grupę 52 uczestników obozów w wieku od 18 do 43 lat, znajdujących się w okresie od 6 miesięcy do 2 lat po urazie rdzenia kręgowego. W ocenie skuteczności programu usprawniającego Aktywnej Rehabilitacji wzięto pod uwagę wyniki uzyskane w teście umiejętności użytkownika wózka (Wheelchair Skills Test Version 2.4) oraz teście samoobsługi. W pracy wykazano zwiększenie sprawności funkcjonalnej uczestników obozów AR. W grupie tetraplegików zaobserwowane zwiększenie samodzielności w czynnościach życia codziennego, już po ośmiodniowym okresie usprawniania, związane było z opanowaniem nowych wzorców ruchowych opartych na mechanizmach kompensacyjnych. Natomiast w grupie paraplegików największe zmiany zaobserwowano w technice jazdy na wózku.

The aim of the study is to evaluate functional effficiency in people after spinal cord injury during Active Rehabilitation camps. The examination group consisted of 52 camp participants aged 18 to 43 years, within a period from 6 to 24 months after the spinal cord injury. In order to evaluate the effectiveness of Active Rehabilitation program the Wheelchair Skill Test Version 2.4 and the self-service test were used. Improvement of functional effficiency was observed in the participants of Active Rehabilitation camps. The observed increase of independence while carrying out daily activities after eight days of treatment in the group of tetraplegics was related to mastering of new motor patterns which were based on compensating mechanisms. However, in the group of paraplegics the greatest improvement was observed in the wheelchair driving technique.
Introduction: Every year, about 400 new patients with tetraplegia arise in Poland. The solution ensuring the continuation of the rehabilitation process of such patients is the activity of non-governmental organizations associating disabled people. Among them Foundation of Active Rehabilitation (FAR) has greatest experience in rehabilitation of people after SCI. Annually, about 70 persons with tetraplegia participate in Active Rehabilitation (AR) camps. Aim of this study was to estimate the influence of AR camps on the functional abilities in persons with tetraplegia. Material and methods: 40 men with tetraplegia were examined. Subjects were divided into two groups: participants of AR camps (group A, n=20) and control (group B, n=20). TheASIA motor score was used to estimate the degree of upper limb impairment. Functional Independence Measure (FIM)was used to assess functional abilities. All subjects were assessed in the beginning of the study and reassessed after 8 days. Results: Both groups during preliminary investigation were homogeneous under regard the ASIA motor score as well as the results of FIM. During final investigations significant improvement of FIM was noted only in group A. The mean value of FIM significantly increased about 10.8 points. In group A significant correlation between FIM scores and ASIA motor score were found (p<0.001). Conclusion: Improvement of functional skills among patients with tetraplegia which participated in AR camps was observed. The highest improvement in FIM was observed among persons with a C6 tetraplegia. Obtained results (ceiling effect) through of examined persons from C7 tetraplegia were suggested the need of applying more sensitive tests.
The paper presents changes in the motor functions of a patient with spinal muscular atrophy (SMA) IIIb as a result of physiotherapy supported by Nusinersen (Spinraza) therapy. The extent of the changes was monitored by analyzing the scores using the Hammersmith Functional Motor Scale Expanded (HFMSE). When testing with the HFMSE scale, the subject was assessed on a three-point scale consisting of thirty-three activities aimed at functional classification of people with SMA III. In the initial study the HFMSE score was 24 out of 66 possible. The increase from 24 to 28 points was noted after six months of treatment and physiotherapy, the changes occurred in the tests of the function of changing the position from supine to prone and vice versa. The next increase from 28 to 31 points occurred after three months, the changes occurred in the tests of the body support function and the change of position from supine to sitting. The last increase from 31 to 34 points was observed after the next three months, the changes took place in the tests of the function of entering the quadruple position and moving in it. After analyzing the conclusions and comparing them with the results of clinical trials published in scientific articles, an increase in motor functions in the HFMSE scale was observed in a patient with SMA IIIb as a result of physiotherapy supported by Nusinersen therapy.

Praca przedstawia zmiany w funkcjach motorycznych pacjenta z rdzeniowym zanikiem mięśni (SMA) IIIb w wyniku prowadzonej fizjoterapii i farmakoterapii lekiem Nusinersen (Spinraza). Zakres zmian monitorowano na podstawie analizy wyników punktacji przy użyciu skali Hammersmith Functional Motor Scale Expanded (HFMSE). Przy wykonywaniu testów z użyciem skali HFMSE badany był oceniany w trzystopniowej skali, składającej się z trzydziestu trzech czynności, mających na celu klasyfikację funkcjonalną osób z SMA III. W początkowym badaniu zaobserwowano wynik w skali HFMSE na poziomie 24 punktów z 66 możliwych do zdobycia. Wzrost zanotowano po sześciu miesiącach leczenia oraz prowadzonej fizjoterapii z 24 na 28 punktów, zmiany nastąpiły w testach dotyczących funkcji zmiany pozycji z leżenia tyłem do leżenia przodem i odwrotnie. Następny wzrost wystąpił po trzech miesiącach z 28 na 31 punktów, zmiany nastąpiły w testach dotyczących funkcji podporu ciała oraz zmiany pozycji z leżenia tyłem do siadu. Ostatni wzrost zaobserwowano po kolejnych trzech miesiącach z 31 na 34 punkty, zmiany nastąpiły w testach dotyczących funkcji wejścia do pozycji czworaczej oraz przemieszczania się w niej. Po przeanalizowaniu wniosków oraz porównaniu ich z opublikowanymi w artykułach naukowych wyników badań klinicznych, zaobserwowano wzrost funkcji motorycznych w skali HFMSE u pacjenta z SMA IIIb w wyniku prowadzonej fizjoterapii wspomaganej terapią lekiem Nusinersen.
