External or endonasal dacryocystorhinostomy is the standard therapy for nasolacrimal duct obstruction in managing epiphora. New innovative surgical techniques and adjunctive methods, such as insertion of silicone tubes and the use of laser energy, for the treatment of nasolacrimal obstruction are being introduced continually. Nowadays, it is almost impossible to unequivocally decide which of these methods offers the best results. The aim of this study is to review the surgical methods of nasolacrimal duct obstruction treatment, based on the current literature.

Podstawową metodą leczenia niedrożności dróg łzowych pozostaje zespolenie workowo-nosowe z dostępu zewnętrznego lub przeznosowe. Jednak dotychczasowe metody chirurgiczne ulegają ciągłym modyfikacjom, a dodatkowo wykorzystanie energii laserowej rozszerza chirurgię dróg łzowych o nowe procedury. Dlatego jednoznaczna ocena, która ze współcześnie dostępnych metod jest najbardziej skuteczna w leczeniu niedrożności dróg łzowych, jest w praktyce niemożliwa. Celem pracy jest przegląd najczęściej obecnie stosowanych metod chirurgicznego leczenia niedrożności dróg łzowych w oparciu o aktualne piśmiennictwo.
Obstruction of the nasolacrimal duct, impeding the proper outflow of tears from the conjunctival sac to the nasal cavity, is a common disorder that is clinically manifested as tearing. Dacryocystorhinostomy is the creation of a direct fistula between the tear sack and the nasal cavity. The surgical approach may be external or endoscopic with or without a laser. Laser dacryocystorhinostomy is now a recognized procedure for the treatment of lacrimation resulting from nasal-lacrimal duct obstruction. The method has many advantages over conventional treatment. These advantages include: the absence of external scars, the possibility of simultaneous treatment of nasal cavity pathology, sparing of the medial lining of the eyelids, intact tear pump mechanism. The laser treatment time is also shorter with a comparable success rate to classic surgery. The introduction of a laser for tear-ditch surgery made it possible to treat tear-lacrimal obstruction in a simpler and less invasive way.

Niedrożność przewodu nosowo-łzowego, upośledzająca prawidłowy odpływ łez z worka spojówkowego do jamy nosa, jest częstym zaburzeniem, które klinicznie objawia się jako łzawienie. Operacja udrożnienia dróg łzowych – dakriocystorinostomia – polega na wytworzeniu bezpośredniej przetoki między workiem łzowym a jamą nosa. Podejście operacyjne może być zewnętrzne lub endoskopowe z zastosowaniem lasera lub bez. Laserowa dakriocystorinostomia stanowi obecnie uznaną procedurę w leczeniu łzawienia wynikającego z niedrożności przewodów nosowo-łzowych. Metoda ma wiele zalet w porównaniu z zabiegiem konwencjonalnym, np.: brak blizn zewnętrznych, możliwość jednoczesnego leczenia patologii jamy nosa, oszczędzenie więzadła przyśrodkowego powiek, nienaruszony mechanizm pompy łzowej. Czas zabiegu z użyciem lasera jest ponadto krótszy przy porównywalnym wskaźniku sukcesu z zabiegiem klasycznym. Wprowadzenie lasera do chirurgii dróg łzowych pozwoliło leczyć ich niedrożność w sposób prostszy oraz mniej inwazyjny.
Szacuje się, że niskie ustawienie powieki górnej dotyczy w Polsce ponad 1 mln pacjentów. Ptoza ogranicza pole widzenia, powoduje wyrównawcze ustawienia głowy i odczucie pogorszenia widzenia. Nieestetyczny wygląd oczu wpływa dodatkowo na niską samoocenę osób dotkniętych opadnięciem powiek. Celem artykułu jest przegląd współczesnych algorytmów diagnostycznych stosowanych w przypadku opadniętej powieki. W pracy omówiono również techniki operacyjne, które są stosowane w ptozie, oraz wskazówki dotyczące prawidłowego kwalifikowania chorego do danej metody operacyjnej.

It is estimated that the low position of the upper eyelid affects over 1 million patients in Poland. Ptosis limits the visual field, causes compensatory head positions and the feeling of visual deterioration. The unaesthetic appearance of the eyes additionally contributes to low self-esteem. The aim of this article is to review modern diagnostic algorithms used in the case of a dropped eyelid. The paper also discusses the surgical techniques that are used in the case of ptosis and the guidelines for the correct qualification of the patient to a given surgical method.
Floppy eyelid syndrome is a common ophthalmic condition characterized by a sagging eyelid, which causes its spontaneous wrinkling during sleep. It can be a result of local and systemic diseases. A characteristic group of patients who suffer from floppy eyelid syndrome are middle aged men with an increased body mass index. Obesity in this group of patients is recognized as the strongest risk factor for the occurrence of obstructive sleep apnea syndrome. The aim of the article is to review the diagnostic methods that are used in the case of floppy eyelid syndrome. The paper also discusses therapeutic methods including surgical techniques.
