Fibromatoza jest rzadką chorobą nowotworową. Charakteryzuje się miejscową agresywnością i dużą tendencją do wznowy miejscowej. Lokalizacja fibromatozy w rejonie głowy i szyi wiąże się z cięższym przebiegiem i gorszym rokowaniem w związku z występowaniem w tej okolicy ważnych życiowo struktur anatomicznych. Przedstawiono opis procesu diagnostycznego i postępowania terapeutycznego u 33-letniej pacjentki z fibromatozą przestrzeni przygardłowej po stronnie lewej. Zastosowana resekcja chirurgiczna zmiany z dostępu szyjnego pozwoliła na radykalne usunięcie nowotworu bez istotnych powikłań pooperacyjnych. Chora pozostaje w obserwacji ambulatoryjnej bez wznowy choroby.

Introduction: Chronic parotid sialadenitis is a disorder of multifactorial etiology. The main cause of this condition is usually a presence of deposits that narrow the parotid duct lumen. Obturative lesions and ongoing inflammation may lead to the development of retention cysts. Case report: In the reported case, a 56-year-old patient experienced a massive polycystic hyperplasia of the parotid gland secondary to chronic calculous sialadenitis, with total parotidectomy as the only effective method of treatment. The methods of treatment and procedural models in the management of chronic parotid sialadenitis are discussed.

Wstęp: Przewlekłe zapalenie ślinianki przyusznej jest chorobą o etiologii wieloczynnikowej. Przyczyną jest najczęściej zwężenie światła przewodów wyprowadzających przez zalegający złóg. Na tle zmian obturacyjnych oraz toczącego się procesu zapalnego może dochodzić do powstawania torbieli retencyjnych. Opis przypadku: W opisanym przypadku u 56-letniego pacjenta doszło do masywnego wielotorbielowatego rozrostu ślinianki na tle kamiczego przewlekłego zapalenia, w którym jedyną skuteczną metodą leczenia była parotidektomia całkowita. Omówiono metody leczenia i model postępowania w przewlekłym zapaleniu przyusznicy.

Desmoid fibromatosis is defined as a rare fibroblastic proliferation. Although histologically benign, desmoids are locally invasive and associated with a high local recurrence rate. When located in the head and neck region, fibromatosis tends to be more aggressive and associated with significant morbidity, which may be attributed to the proximity of the vital structures. We report a case of 33-year-old woman with aggressive fibromatosis of parapharyngeal space. Complete surgical excision of the lesion was performed and the postoperative period was without complications. The patient remains under careful follow-up and there is no recurrence of the disease.

Autorzy opisali przypadek 62-letniego mężczyzny, nałogowego palacza papierosów, przyjętego do szpitala z powodu duszności krtaniowej. Przedstawiamy przypadek mężczyzny u którego zdiagnozowano raka krtani i przełyku jednocześnie. Pacjent cierpi również na raka prostaty. Częstość występowania mnogich pierwotnych nowotworów złośliwych wynosi około 0,5-10%

The authors describe the case of 62 year-old Man, heavy smoker, admitted to the hospital due to dyspnea. We report a case of a man in whom laryngeal carcinoma, and esophageal carcinoma were diagnosed simultaneously. Patient suffers also from prostate adenocarcinoma. The incidence of multiple primary malignant neoplasms is around 0,5 –10%.

January 15, 2021 marks a hundred years from the establishment of the Clinic of Otorhinolaryngology, Head and Neck Surgery at the Medical University of Warsaw. The authors representing the oldest generations of the current Team present the history of the Clinic from the foundation until the present day. The main achievements throughout history and selected activities of the currently working Team are described.

Rak podstawnokomórkowy (BCC) jest najczęstszym nowotworem złośliwym skóry. W niniejszej pracy przedstawiamy przypadek 59-letniej kobiety z guzem skóry twarzy stopniowo powiększającym się od 5 lat. Badanie MR ujawniło nieprawidłowy naciek w lewym oczodole. Pacjentka została zakwalifikowana do częściowej resekcji nosa zewnętrznego, enukleacji oczodołu, maksylektomii przyśrodkowej wraz z rekonstrukcją ubytku tkanek płatem przedniobocznym uda. Autorzy przedstawiają problemy diagnostyczne oraz metody leczenia BCC.

Objectives: Treatment of patients with recurrent pleomorphic adenoma of the parotid gland is a challenge for a surgeon due to frequent problems with complete resection of all tumour foci while preserving continuity and function of the facial nerve. The aim of this study was to evaluate the clinical presentation and treatment results of patients with recurrent pleomorphic adenoma of the parotid gland. Materials and Methods: The medical records of 35 patients (25 women and 10 men) operated on (44 operations) for recurrent pleomorphic adenoma between the years 1988 and 2008 at the Otolaryngology Department, Medical University of Warsaw, were reviewed. These patients accounted for 8.9% of all patients treated for pleomorphic adenoma of the parotid gland (N=395). A retrospective analysis was performed to examine clinical features, surgical technique and facial nerve management. Results: All patients had palpable, nontender mass or masses in a parotid bed after 1 to 5 previous operations. Multifocal recurrences were present in 79.5% of cases. Median interval between initial treatment and commencement of recurrences was 6.1 (0.25-29) years. Patients with more than one recurrence were younger than patients who had only one. Malignant transformation of recurrent pleomorphic adenoma was observed in two patients (5.7%). Postoperative facial nerve paresis occurred in 15 cases. In two additional cases eradication of recurrent tumour required the facial nerve resection and reconstruction. Conclusions: Recurrent pleomorphic adenoma occurs more often in younger patients and women. The risk of the facial nerve injury increases with each successive operation. Surgical treatment should be individualized, taking into consideration the extent of the previous surgery and the type of recurrence.

W niniejszej pracy opisane zostały najważniejsze osiągnięcia na przestrzeni dziejów oraz wybrane aktywności pracującego obecnie Zespołu.

Każdy laryngolog na pewnym etapie swojej pracy spotyka się z koniecznością wykonania zabiegu usunięcia ciała obcego z dróg oddechowych czy pokarmowych. Niejednokrotnie może nastręczać to wielu problemów, takich jak uszkodzenia otaczających tkanek, czy trudności związanych z przemieszczaniem się, w miarę upływu czasu, ciała obcego w tkankach. Najczęściej ciało obce zlokalizowane jest w przestrzeni gardła środkowego i dolnego. Zdarzają się również w mniej typowych miejscach, w których należy wykazać się większą czujnością ze względu na trudniejszą diagnostykę. Tak jest w przypadku ciał obcych w obrębie tkanek miękkich. Autorzy przedstawiają przypadek metalicznego ciała obcego tkwiącego w języku przez ponad 6 tygodni przed ostatecznym jego rozpoznaniem i usunięciem.

Basal cell carcinoma (BCC) is the most frequent malignant neoplasm of the skin. In this report we present a case of a 59-year-old patient, who had noticed for five years gradually growing tumor of the facial skin. MR scans revealed pathological lesion in the left orbit. The patient required partial resection of the nose, orbital exenteration with anterolateral thigh free flap reconstruction. The authors present diagnostic problems and methods of treatment BCC.

Probably every laryngologist in his career will face off an issue of foreign body removal. It can be problematic especially that foreign body cause the trauma of surrounding tissues or may translocate unpredictably. A space of the middle and lower throat is the most common location. Occasionally it can be located in less obviously place, which force the laryngologist to prevent more viligant diagnostic. A foreign body pounded within soft tissue of the oral cavity is the example. A case report of a metallic foreign body inherent in the tongue for more than six weeks before the final treatment is presented.

Introduction: Inverted papilloma is benign epidermal neoplasm of not recognized etiology. The lesion is estimated to represent 0.5-4% of sinonasal tumors. Generally inverted papilloma is unilateral, arises from the lateral nasal wall and characterizes with local destruction and rapid growth. The tumor can possibly undergo malignant transformation. The treatment of choice is surgery, recently endoscopic approach is considered to be superior to the open approach. The aim of study: was evaluation of recurrent inverted papilloma treatment results in the experience of Otolaryngology Department, Medical University of Warsaw, from 1982 to 2009. Material: The group of 124 patient (66 men and 58 women) aged from 26 to 79 with inverted papilloma operated between 1982 and 2009 were enrolled in the study. Follow up was from 6 months to 27 years. Results: Most common primary localization of the tumor was combined lateral nasal wall and maxillary sinus (42%). In 37% the lesion involved unilaterally the lateral nasal wall, maxillary sinus and ethmoid sinuses, rarely it was localized only in the lateral nasal wall (16.1%) and exclusively in the nasal septum (4.8%). All the patients were treated surgically, 46% of patients underwent endoscopic procedure, 37.1% lateral rhinotomy, 15% Denker rhinotomy and 1.6% midfacial degloving. Recurrence was observed in 15.3% including 57.9% after endoscopic surgery, 26% after Denker operation and 15.8% after lateral rhinotomy. Malignant transformation was observed in 5 patient (4%). All the patients with recurrent inverted papilloma were treated surgically, 5.3% of patient underwent midfacial degloving, 5.3% Caldwell- Luc operation, 10.5% endoscopic procedure, 26.3% Denker operation and 52.6% lateral rhinotomy. Conclusions: 1. Endonasal and open approach should be considered in the surgical treatment of recurrent inverted papilloma. 2. Endoscopic approach is preferred in the recent years but qualifi cation for each method should depend on lesion localization, extent and volume. 3. Regular follow-up enables early recurrence diagnosis and treatment.
