The partial aetiological overlap between overweight/obesity and oral diseases has aroused interest in the medical environment. Studies to investigate the causal relationship between these two diseases are underway. However, most of the available literature reports fail to confirm the direct positive correlation between obesity and dental caries. On the other hand, overweight and obesity are considered risk factors for periodontal diseases. The proinflammatory mediators secreted by adipocytes in obese individuals may impair immune functions, thereby modifying host responses to dental plaque antigens and contributing to blocked defence functions in periodontal diseases. Gingivitis observed in overweight and obese patients is a result of metabolic disorders, inflammatory factors and improper oral hygiene habits. The latter ones are associated with frequent consumption of sweetened, pulpy and highly processed food products and sweetened carbonated beverages, which is a risk factor for overweight, particularly in children and adolescents. Undoubtedly, overweight and obesity affect salivary secretion and properties, including changes in the microbiological composition of saliva. Obese patients are at increased risk of adverse affects of dental treatment, dental surgeries in particular. Overweight and obese individuals require permanent preventive and medical dental care addressing their periodontal preventive and therapeutic needs. Dentists should actively participate in the prevention of overweight and obesity, especially in children and adolescents, taking advantage of frequent contact with this group of patients. Obese individuals may participate in integrated therapeutic management by means of dental treatment limiting food intake.

Ze względu na fakt, że czynniki etiologiczne nadwagi/otyłości i chorób jamy ustnej częściowo są takie same, budzą zainteresowanie w środowisku medycznym. Prowadzone są badania związków przyczynowo-skutkowych pomiędzy tymi chorobami. Większość wyników badań dostępnych w piśmiennictwie dotyczącym zależności pomiędzy otyłością a próchnicą zębów nie potwierdza bezpośredniej dodatniej korelacji pomiędzy tymi schorzeniami. Natomiast nadwaga i otyłość są uznawane za czynniki rozwoju chorób przyzębia. Wydzielane przez komórki tłuszczowe mediatory prozapalne u osób otyłych mogą, poprzez zaburzanie funkcji układu immunologicznego, modyfikować odpowiedź gospodarza na antygeny płytki nazębnej i przyczyniać się do blokowania funkcji obronnych w chorobach przyzębia. Obserwowane u osób z nadwagą i otyłością zapalenie dziąseł to rezultat wpływu zaburzeń metabolicznych, czynników zapalnych i nieprawidłowych nawyków higienicznych dotyczących jamy ustnej. Te ostatnie są związane z częstym spożywaniem słodzonych, papkowatych i wysoko przetworzonych pokarmów oraz słodzonych napojów gazowanych, co stanowi czynnik warunkujący rozwój nadwagi, zwłaszcza u dzieci i młodzieży. Niewątpliwy jest wpływ nadwagi i otyłości na wydzielanie i właściwości śliny, włącznie ze zmianą jej składu mikrobiologicznego. Pacjenci z otyłością są bardziej narażeni na zdarzenia niepożądane podczas leczenia stomatologicznego, zwłaszcza z zakresu chirurgii stomatologicznej. Osoby z nadwagą i otyłością powinny pozostawać pod stałą stomatologiczną opieką profilaktyczno-leczniczą, w ramach której należy regularnie przeprowadzać periodontologiczne zabiegi profilaktyczno-lecznicze. Stomatolodzy powinni aktywnie uczestniczyć w profilaktyce nadwagi i otyłości, w szczególności u dzieci i młodzieży, wykorzystując częsty kontakt z tą grupą pacjentów. Otyłe osoby dorosłe mogą uczestniczyć w zintegrowanym postępowaniu leczniczym poprzez leczenie stomatologiczne ograniczające pobieranie pokarmów.

Dental Erosion

Dental erosion is described as an irreversible loss of dental hard tissue resulting from exposure to non-bacterial acids or chelating substances. It may be caused by exogenous or endogenous factors. In the former case, food, drink, as well as the environment might be sources of acids; in the latter, acids flowing into the oral cavity from the stomach and duodenum. Exogenous dental erosive lesions are localized mainly on the labial surfaces of the anterior teeth of the maxilla, while the endogenous ones can be found on the palatal and masticatory surfaces of the maxilla and the masticatory and buccal surfaces of the mandible. Reduced saliva secretion, which occurs in a number of diseases, also influences dental erosion, while erosion-causing factors and aggressive tooth brushing immediately after consuming acidic food increases the range and depth of erosion cavities. The consequence of dental erosion is teeth hypersensitivity, which results from exposure of dental tubules and of the pulp leading to the loss of tooth vitality and decrease in occlusal height. Treatment of exogenous dental erosion consists in changing nutritional and hygienic habits. In the case of endogenous erosion, however, the therapy should address mainly an underlying disease. Worn teeth surfaces should be restored with conservatory or/and prosthetic methods. In the context of an increasing prevalence of dental erosion in the population, it is necessary to develop and implement prophylactic measures, including broadly understood health education on the risk factors, preventive activities, and possibilities of diagnosis and therapy.

Introduction: Negligence of the healthy behaviour associated with oral health, dental prevention and treatment in primary school children can be a source of serious health problems, both dental and systemic. Aim: The aim of this study was to evaluate the effects of selected demographic factors and reasons for visiting a dental office on the dental health of primary school children. Material and methods: The study included 210 patients aged 7–11 years, who in 2015 visited one of the dental offices in Lublin, where dental services are provided both under the contract with the National Health Fund or after payment. Dental condition was assessed by calculating dmf/DMF (decayed, missing, filled) index for deciduous/permanent teeth. Results: More than three-quarters of children (77.14%) visited a dentist for check-up, 20.0% – due to a toothache and 2.86% – due to the loss of filling. The average DMF number was 4.24 ± 3.47, and the average DMF index value was 1.74 ± 2.06. The highest intensity of caries in the primary teeth was found in children visiting the dentist due to toothache, lower – in those reporting for check-up visits and the lowest – due to the loss of filling. In the case of permanent dentition, the highest number of fillings was observed in children attending a check-up visit. Conclusions: High incidence of caries in primary school children confirms the insufficient health knowledge of parents/guardians on the oral health and insufficient dental prevention and treatment in both preschool and early-school children.

Wstęp: Zaniedbania w zachowaniach prozdrowotnych dotyczących jamy ustnej i w stomatologicznej opiece profilaktyczno- -leczniczej nad dziećmi w wieku wczesnoszkolnym mogą być źródłem poważnych problemów zdrowotnych, zarówno stomatologicznych, jak i ogólnoustrojowych. Cel badań: Celem pracy była ocena wpływu wybranych czynników demograficznych i przyczyn zgłoszenia się do lekarza dentysty na stan uzębienia dzieci w wieku wczesnoszkolnym. Materiał i metody: Badaniem objęto 210 dzieci w wieku wczesnoszkolnym (7–11 lat), które zgłosiły się w ciągu całego roku 2015 do jednego z lubelskich gabinetów stomatologicznych, gdzie świadczenia stomatologiczne udzielane są zarówno w ramach kontraktu z Narodowym Funduszem Zdrowia, jak i odpłatnie. Stan uzębienia oceniono, obliczając liczbę puw/PUW (próchnica, usunięte, wypełnione), odpowiednio dla uzębienia mlecznego i dla uzębienia stałego. Wyniki: Ponad ¾ dzieci (77,14%) zgłosiło się na wizytę kontrolną, 20,0% – z powodu bólu zęba, a 2,86% – ze względu na utratę wypełnienia. Średnia wartość liczby PUW wyniosła 4,24 ± 3,47, a średnia wartość PUW – 1,74 ± 2,06. Najwyższą intensywność próchnicy w zębach mlecznych stwierdzono u dzieci zgłaszających się z powodu bólu zęba, niższą – u zgłaszających się na wizyty kontrolne, najniższą – u zgłaszających się ze względu na utratę wypełnienia. W przypadku uzębienia stałego największą liczbę wypełnień mieli pacjenci zgłaszający się na wizytę kontrolną. Wnioski: Wysoka zachorowalność na chorobę próchnicową wśród dzieci wczesnoszkolnych potwierdza niedostateczną wiedzę prozdrowotną rodziców/opiekunów w zakresie zdrowia jamy ustnej oraz uchybienia w stomatologicznej opiece profilaktyczno-leczniczej nad małymi dziećmi i dziećmi w wieku przedszkolnym.

Fungi, mainly yeast-like fungi of the Candida genus, and especially the species Candida albicans, occupy an important place among microorganisms that colonize the oral cavity. The aim of the paper was to assess the susceptibility of different strains to some antifungal drugs. The study material consisted of swab samples taken from the buccal mucosa of 376 healthy individuals of both genders, from neonate to18 years. C. albicans strains were isolated in 79 persons. To evaluate drug resistance phenotypes of the isolated C. albicans strains FungitestTM (Bio-Rad) was used. The study showed that in different age groups the percentage of the tested C. albicans isolates susceptible to all the tested azole drugs varied from 76% to 95%. Drug resistant strains were found only in the eldest age group (4.8%). Phenotypic diversity (drug resistance types) of the studied isolates indicates a wide range of adaptation possibilities enabling colonization of various ecological niches in the oral cavity ecosystem. The presence of C. albicans strains intermediate or resistant to azole drugs among the tested isolates may be due to overuse of that drug group in general therapy and in the oral cavity treatment.

Introduction. Many factors affect the oral health of the population. These include individual factors, the effectiveness of dental care, life styles and political, economic and environmental factors. Aim. The aim of the study was to evaluate the type of financing dental care and frequency of dental visits among patients treated with implants because of missing teeth. Material and methods. The survey was conducted among 464 patients of both genders aged 20-74 years, treated with dental implants at the Non-Public Healthcare Centre “Dental” in Tomaszów Mazowiecki. The patients answered questions included in anonymous questionnaire. The questions concerned age, education, type of dental care financing and frequency of dental visits. Results. Results of the survey indicate that among patients treated with implants there were over 14-times more people benefiting from dental care in private clinics providing preventive and restorative treatments at full cost, compared with people using dental services funded by the National Health Fund. Just over 16% of the surveyed patients frequently enough, i.e. at least once in 6 months reported to the dentist. The frequency of dental visits increased with the education level - people with vocational and secondary education most often visited a dentist irregularly, when necessary, while those with higher education - once a year. Patients below 60 years reported to the dentist 1-2 times a year, whereas older - irregularly, when necessary. Conclusion. In preparing the patient for the treatment of missing teeth with implants one should be aware of his need for paying special attention to oral health and regular, sufficiently frequent follow-up visits which has a substantial impact on the course and therapeutic success in the implant prosthetic treatment

Introduction. Dental prosthetic replacements allow restoring the masticatory function, thus contributing to proper nutrition and correct speech, and also improving facial aesthetics. Aim. The aim of the study was to assess the selected sociodemographic characteristics of people using removable dentures (patient-paid and funded by the National Health Fund). Material and methods. A questionnaire survey involved 321 individuals, resident in the Lubelskie Voivodship, who used removable dental prostheses. Their selected sociodemographic characteristics were analysed: age, sex, education, place of residence and material status, as well as the form of payment for prosthetic replacements. The obtained results were analysed statistically. Results. Almost 70% of the studied population wearing removable prostheses had their dentures funded by the National Health Fund. In each age group, women used National Health Fund funded prostheses twice more frequently than men. Prostheses funded by the National Health Fund were used significantly more frequently by patients aged 51-70 years, living in cities and with a higher education level, in comparison to the respondents from other age groups, with different place of residence and education level. Conclusions. There is correlation between sex, age, place of residence and education level of patients and the frequency of their wearing removable dentures, both patient-paid and funded by the National Health Fund.

Introduction. One of the indications for dental treatment in general anaesthesia of children and adolescents is the patients’ refusal to cooperate with the dentist; this concerns especially the so-called special care patients, and among them, those suffering from epilepsy. Aim. The aim of the study was to evaluate the structure of treatment procedures in conservative dentistry and dental surgery performed under general anaesthesia in children and adolescents with epilepsy. Material and methods. The analysis covered case histories of 109 patients with epilepsy, aged 3-18 years, who received dental treatment under general anaesthesia. The patients were divided into three age groups, according to the dentition type: patients with deciduous dentition - 3-5 years of age; with mixed dentition - 6-12 years, and with permanent dentition - 13-18 years. The analysis concerned the procedures on both deciduous and permanent teeth. Results. An average number of extractions in a child with full deciduous dentition was 3.9 teeth, while in a child with mixed dentition - 2.7 teeth. The greatest number of deciduous teeth extractions concerned first and second molars. Carious cavities in the occlusal surfaces of permanent teeth, due to deep caries, were the most frequently treated lesions. Conclusions. In patients with epilepsy treated under general anaesthesia, extensive treatment needs, resulting from prophylactic and therapeutic neglect in dental care, were found. Conservative and surgical treatment, combined with prophylactic procedures, performed under general anaesthesia seems one of the necessary elements of a complex dental care in uncooperative children and adolescents

Orthodontic treatment aims at restoring correct occlusion and chewing function and improving the aesthetics of the dentition and facial esthetics of the patient. During treatment complications may occur. Most frequently observed complications have been described in the paper. There role of the orthodontist in the therapeutic process and the principles on which doctor- patient cooperation should be based in order to achieve a therapeutic success have been presented. Particular emphasis has been put to the importance of prevention of dental caries and periodontal diseases in orthodontic patients.

Fungal infections are an important medical problem in patients from different risk groups. The majority of these infections are caused by Candida spp., with over 50 % due to C. albicans. The purpose of the study was to evaluate in vitro chlorhexidine effect on C. albicans colonizing the mouth and throat isolated from 5 population groups. The study material included the reference strains of C. albicans ATCC 2091 and C. albicans ATCC 10231, routinely used for evaluation of antimicrobials, and 120 clinical isolates of C. albicans from: hospitalized cancer patients (30 isolates), chronic HCV patients (31 isolates), immunocompromised patients (28 isolates), healthy school-age children (31 isolates), elderly people – aged 65 years or older (30 isolates). Chlorhexidine inhibited the growth of C. albicans at the concentrations of 0.625-5 µg/ml (in particular, 2.5 µg/ml solution was effective against strains from immunocompromised patients and 5 µg/ml – against the remaining isolates). The yeasts were also killed by 2.5-20 µg/ml chlorhexidine solutions. The concentration of 5 µg/ml was particularly active against the strains isolated from the elderly, immunocompromised and lung cancer patients, while 10 µg/ml inhibited the growth of the strains from the remaining two groups. Moreover, C. albicans isolates from hepatitis C patients and healthy children comparing to strains from the elderly were less sensitive to chlorhexidine fungicidal effect and these differences were statistically significant. According to our studies, the fungicidal effect of chlorhexidine seems to depend on the origin of the tested oral C. albicans strains from various patient groups.

Introduction. Caries of deciduous teeth in the population of Polish children is a significant health, social and organizational problem. Aim. The aim of the study was to evaluate the dentition of 3- and 4-year-old children in the Mazowieckie Province. Material and methods. The study comprised 393 kindergarten children aged 3 and 4 years in the Mazowieckie Province, 202 girls and 191 boys, including 159 children aged 3 years and 243 children aged 4 years. In all subjects the state of dentition was assessed. The prevalence of caries was calculated as the percentage of people affected by this disease, and the intensity of dental caries was determined using the dmft index. The results were compared with those that Polish researchers obtained in the 3 – and 4-year-olds after 2000. Results. In the studied group of 3- and 4-year-olds the prevalence of caries was 62.85%, among girls – 61.88% and among boys – 63.87%. For all subjects the average scores for dmft index were 3.22 and its components dt – 2.72 (decayed teeth), mt – 0.1 (missing teeth), ft – 0.4 (filled teeth). Conclusions. The prevalence and intensity of dental caries in 3- and 4-year-old children attending kindergartens in the Ma-zowieckie Province is high and close to the national average. It is advisable to increase the preventive and curative measures in this age group of children on dental caries.

Gold is a dental material with very good mechanical properties. It is also aesthetic and biocompatible with the tissues of the oral cavity even at 100% purity. Prosthetic restorations made of pure gold or its alloys can be practiced either through a normal casting, as well as through using the galvanoforming technique. The electrolytic method was first introduced into dentistry about 20 years ago and it allows for producing “pure” gold (which means 99.99% Au). The lack of additions of other metals improves the properties of dental prostheses, such as marginal tightness, esthetics, biocompatibility, and it helps in eliminating any allergic reactions. The literature review presented in this paper is a comparison of the traditional casting method with the newer galvanoforming technique.
