The achieve pathologic complete response is proven to be the most important parameter of prognosis. Thereports evaluating the impact of obesity on the obtained pathologic response to chemotherapy are unequal. The aim of the study was to evaluate in locally advanced breast cancer patients, treated with AT(doxorubicin plus docetaxel) neoadjuvant chemotherapy: 1. The relationship of obesity with obtaining pathological response. 2. The relationship of obesity and free of disease recurrence survival (DFS) and overall survival (OS) associated with the tumour. Material and methods. A retrospective study was carried out in a group of 105 patients with locally advanced breast cancer, treated with AT neoadjuvant chemotherapy and then treated with radical surgery. Two variants of pathological response have been adopted: a pCR (T0N0) and pCR1 (TisN0, TxN1, T1N0, T1N1, T0N1). The relationship of obesity with pathological response and survival was investigated. Results. In univariate analysis the pCR1 was obtained with its arising from the borderline of statistical significance with lower incidence of obesity. In pCR1 multivariate analysis, negative pCR1 relationship with obesity was on the borderline of the statistical significance. The multivariate analysis showed a significant negative association OS with obesity (p=0.047) and positive with the occurrence of menopause (p = 0.029). Conclusions. In patients with locally advanced breast cancer treated with AT neoadjuvant chemotherapy. 1. Obesity seems to be an independent and unfavourable predictor of the lack of obtaining pCR1 pathological response 2. In the multivariate analysis, the obesity was a significant independent factor related to shorter OS.
Classical abdominal surgeries usually require long incisions of the abdominal integuments followed by tight closure with adequate suturing material. Nonabsorbable sutures may cause various reactions, including granuloma reactions, both sterile and infl ammatory. The aim of the study was to analyze prospective ultrasound examinations of the abdominal integuments in order to detect tissue reactions to surgical sutures. Material and methods: For 10 years, ultrasound examinations of the abdominal integuments involved the assessment of surgical scars in all patients who underwent open or closed surgeries for various reasons (in total 2254 patients). Ultrasound examinations were performed only with the use of linear probes with the frequency ranging from 7 to 12 MHz. Each scar in the abdominal integuments was scanned in at least two planes. When a lesion was detected, the image was enlarged and the transducer was rotated by approximately 180° in order to capture the dimensions of the granuloma and the most characteristic image of the suture. Moreover, vascularization of the lesion was also assessed with the use of color Doppler mode set to detect the lowest fl ows. Results: All granulomas (19 lesions, two in one patient) created hypoechoic oval or round nodules, were relatively well-circumscribed and their size ranged from 8 × 4 mm to 40 × 14 mm. In the center of the lesion, it was possible to notice a thread that was coiled to various degrees and presented itself as a double, curved hyperechoic line. In 9 out of 19 granulomas, slight peripheral vascularization was observed. The substantial majority of the lesions (n = 15) were in contact with the fascia. In seven patients, compression with the transducer induced known local pain (n = 4) or intensifi ed pain that had already been present (n = 3); all of these granulomas infi ltrated the fascia and showed slight peripheral vascularization. Cutaneous fi stulae developed in two patients with purulent reactions to the running stitch (in one patient – two fi stulae). Conclusions: Suture granulomas in the abdominal integuments manifest themselves as nodular hypoechoic lesions, usually localized at the edge of the fascia – subcutaneous fat. A pathognomonic sign of this type of granulomas is the presence of a thread in their center that usually manifests itself as a chaotically shaped, double hyperechoic line. In some granulomas, particularly those with clinical presentation, slight peripheral vascularization is observed.

Klasyczne operacje brzuszne zazwyczaj wymagają długich nacięć powłok, a następnie ich szczelnego zamknięcia za pomocą odpowiedniego materiału szewnego. Głównie niewchłanialne szwy mogą powodować różne reakcje, w tym odczyny ziarniniakowe o charakterze jałowym lub zapalnym. Celem pracy jest analiza prospektywnych badań ultrasonografi cznych powłok brzusznych ukierunkowanych na wykrycie odczynów tkankowych na zastosowane nici chirurgiczne. Materiał i metoda: Przez 10 lat w badaniach powłok brzusznych każdorazowo oceniano ultrasonografi cznie stan blizny pooperacyjnej u wszystkich pacjentów leczonych z różnych przyczyn chirurgicznie metodą otwartą albo zamkniętą (łącznie 2254 chorych). Badania ultrasonografi czne przeprowadzano wyłącznie głowicami liniowymi o częstotliwości w zakresie 7–12 MHz. Każdą bliznę powłok brzusznych skanowano co najmniej na dwóch przekrojach. W momencie wykrycia zmiany powiększano obraz i wykonywano obrót głowicą w granicach 180°, aby uchwycić największe wymiary ziarniniaka i najbardziej charakterystyczny obraz zwoju nici, a także oceniano jej unaczynienie, używając skali kolorowego dopplera ustawionego na najniższe przepływy. Wyniki: Wszystkie ziarniniaki (łącznie 19, u jednego pacjenta dwa) tworzyły guzki hipoechogeniczne kształtu owalnego lub okrągłego, dość dobrze odgraniczone, wielkości od 8 × 4 mm do 40 × 14 mm. W centralnej części zmiany udawało się wypatrzyć w różnym stopniu zwiniętą nić w formie podwójnej, zakrzywionej linii hiperechogenicznej. W 9 z 19 ziarniniaków zarejestrowano zazwyczaj niewielkie unaczynienie na obwodzie. Zdecydowana większość zmian (n = 15) miała kontakt z powięzią. U siedmiu pacjentów ucisk guzka głowicą wyzwalał znaną bolesność miejscową (n = 4) albo nasilał istniejący ból (n = 3); wszystkie te ziarniniaki naciekały powięź i wykazywały niewielkie unaczynienie na obwodzie. U dwóch osób z ropnym odczynem na założony szew ciągły wystąpiły przetoki skórne (u jednej osoby dwie). Wnioski: Ziarniniaki wokół szwów chirurgicznych w powłokach brzusznych manifestują się jako guzkowa zmiana hipoechogeniczna, najczęściej położona na granicy powięź – tłuszcz podskórny. Patognomonicznym objawem tego rodzaju ziarniniaka jest wykazanie w jego centralnej części nici, która zazwyczaj objawia się bezładnie ukształtowaną, podwójną linią hiperechogeniczną. W części ziarniniaków, zwłaszcza z kliniczną prezentacją, obserwuje się niewielkie unaczynienie obwodowe.
Optimal management of asymptomatic generalized rectal cancer is still the matter of debate.The aim of the study was to review stage IV rectal cancer patients who were treated in our clinic since 2000 till 2008 in order to evaluate the effectiveness of surgery.Material and methods. Fifty-two generalized rectal cancer patients treated with elective resection of primary tumor were identified. Patients' age, sex, duration of hospital stay, modality of surgery, complications, postoperative mortality rate and survival rate were assessed.Results. Median survival was 16.3 months. Postoperative complications occurred in 29% patients. Postoperative mortality rate was 1.9%.Conclusions. In properly selected group of patients elective resection of primary tumor may cause low mortality rate and acceptable morbidity rate. This surgical modality allows to avoid potential complications of tumor local growth.
Laparoscopic cholecystectomy, which was introduced to the arsenal of surgical procedures in the middle of the 1980s, is a common alternative for conventional cholecystectomy. Its primary advantage is less invasive character which entails shorter hospitalization and faster recovery. Nevertheless, the complications of both procedures are comparable and encompass multiple organs and tissues. The paper presents ultrasound presentation of the surgical bed after laparoscopic cholecystectomy and of complications associated with this procedure. In the first week following the surgery, the presence of up to 60 ml of fluid in the removed gallbladder bed should be considered normal in certain patients. The fluid will gradually absorb. In single cases, slight amounts of fluid are detected in the peritoneal cavity, which also should not be alarming. Carbon dioxide absorbs from the peritoneal cavity within two days. Ultrasound assessment of the surgical bed after cholecystectomy is inhibited by hemostatic material left during the surgery. Its presentation may mimic an abscess. In such cases, the decisive examination is magnetic resonance imaging but not computed tomography. On the other hand, rapidly accumulating fluid around the liver is an alarming symptom, particularly when there is inadequate blood supply or when peritoneum irritation symptoms develop. Depending on the suspected cause of the patient’s deteriorating condition, it is essential to perform urgent computed tomography angiography, celiac angiography or endoscopic retrograde cholangiopancreatography or magnetic resonance cholangiopancreatography. The character of the fluid collection may be determined by its ultrasound-guided puncture. This procedure allows for aspiration of fluid and placement of a drain. Moreover, transabdominal ultrasound examination after laparoscopic cholecystectomy may contribute to the identification of: dropped stones in the right hypochondriac region, residual fragment of the gallbladder with possible concretions, undiagnosed choledocholithiasis, existing cholestasis, pseudoaneurysm of the hepatic artery, portal vein thrombosis and hematoma as well as hernias of the abdominal walls. Moreover, ultrasound examination helps to identify optimal sites in the abdominal integuments, which enables collision-free access to the peritoneal cavity.

Cholecystektomia laparoskopowa, wprowadzona do arsenału leczenia chirurgicznego w połowie lat osiemdziesiątych ubiegłego stulecia, uchodzi obecnie za powszechną alternatywę dla cholecystektomii klasycznej. Jej podstawową zaletą jest mniejsza inwazyjność, a przez to skrócenie hospitalizacji oraz szybsza rekonwalescencja. Niemniej jednak powikłania po obu rodzajach operacji są porównywalne i obejmują wiele narządów i tkanek. W pracy przedstawiono obraz ultrasonograficzny tkanek loży po cholecystektomii laparoskopowej i powikłań związanych z tym zabiegiem. W pierwszym tygodniu po operacji za obraz prawidłowy u niektórych pacjentów należy przyjąć obecność do 60 ml treści płynnej w loży po usuniętym pęcherzyku żółciowym, która stopniowo zostaje zresorbowana. W pojedynczych przypadkach stwierdzana jest niewielka ilość płynu w jamie otrzewnej, co zazwyczaj także nie powinno niepokoić. Dwutlenek węgla wchłania się z jamy otrzewnej w ciągu dwóch dób. Ocenę ultrasonograficzną loży po cholecystektomii komplikuje pozostawiony w czasie zabiegu materiał hemostatyczny, obrazem przypominający ropień. W takich przypadkach badaniem rozstrzygającym o rozpoznaniu powinien być rezonans magnetyczny, a nie tomografia komputerowa. Alarmującym objawem będzie gromadzący się w szybkim tempie płyn wokół wątroby, zwłaszcza przy współistniejącej anemizacji chorego lub przy pojawieniu się objawów podrażnienia otrzewnej. W zależności od podejrzewanej przyczyny pogorszenia stanu chorego należy pilnie wykonać tomografię komputerową w wersji angiograficznej lub celiakografię, ewentualnie endoskopową wsteczną cholangiopankreatografię lub cholangiopankreatografię rezonansu magnetycznego. Charakter zbiornika płynu można określić, wykonując jego nakłucie pod kontrolą obrazu ultrasonograficznego. Procedura ta pozwala na aspirację płynu i założenie drenażu. Ponadto ultrasonografia przezpowłokowa po cholecystektomii laparoskopowej może pomóc w rozpoznaniu: wypadniętych kamieni w nadbrzuszu prawym, pozostawionego fragmentu pęcherzyka żółciowego z ewentualnymi złogami, nierozpoznanej kamicy przewodowej, istniejącej cholestazy, tętniaka rzekomego tętnicy wątrobowej, zakrzepicy żyły wrotnej i krwiaka oraz przepukliny powłok brzusznych. Ultrasonografia pozwala ponadto na identyfikację optymalnych miejsc w powłokach brzusznych, co umożliwia bezkolizyjne wniknięcie laparoskopem do jamy otrzewnej.
Effective treatment is the primary objective of surgeon in the treatment of advanced gastric cancer. Poor prognosis and significant advancement of gastric cancer at the time of diagnosis are decisive factors for the only possible surgical management method being palliative procedures.The aim of the study was the evaluation of the value of palliative resection procedures in patients with advanced gastric cancer.Material and methods. The subject in the study was a group of 105 patients with gastric adenocarcinoma at stage 4 of advancement, in whom curative treatment was not possible. The group constituted 44.5% of patients operated on due to gastric cancer at the Department of General and Oncological Surgery, PUM, in the years 1998-2009. The patients were divided into two groups: the first one comprised 44 patients post palliative resections, the second - 61 patients post non-resection procedures. The subject of analysis were early and late treatment results post palliative resections, and they were compared with the treatment results post non-resection procedures.Results. Palliative resections were performed in 44 patients (19 females and 25 males), while in 61 patients (38 males and 23 females) non-resection procedures were performed. Postoperative complications were observed in 25% of patients in the group post palliative resections and in 11.5% in the group of patients without the resection of primary focus. In-hospital mortality stood at 4.5% in the group post palliative resections and 4.8% in the group post non-resection procedures. The percentage of 1-year and 4-year survival post palliative resections stood at 43% and 8.8%, respectively. In the group without the resection of primary focus, 16% survived 1 year and nobody survived 2 years. Conclusions. Palliative resections improve the survival of patients with incurable gastric cancer and should be considered if only the loco-regional conditions are favourable.
The aim of the study was to evaluate the influence of the surgeons' caseload on the results of therapy in rectal cancer.Material and methods. 286 consecutive patients (155 males and 131 females) were enrolled and operated on for rectal cancer stage T2 (112 patients) and T3 (174 patients) in 8 surgical centers of Szczecin between January 1993 and December 1997. Studied group included about 79% of radically operated patients due were to rectal cancer in analyzed period.Results. The patients were assigned to one of two groups with regard to the surgeon's caseload. The first group was comprised of 116 patients (including 72 stage T3 and 54 stage T2 patients) operated on by surgeons more experienced in rectal surgery and the second group was comprised of 160 patients operated on by 36 surgeons with fewer caseloads. The surgeon's experience in rectal surgery was measured by the surgeon's caseload throughout the entire study period. We considered surgeons with greater than 25 cases over the study period experienced.Analysis of survival with regard to the managing center revealed significant differences for stage T3, with 5-year-survival rates ranging from 14% to 60%. Distinct differences were also noted for survival rates in stage T2 (5-year-survival rates ranged from 38% to 86%); however, these differences were not statistically significant. Analysis of the influence of surgeon's caseload on outcomes in rectal cancer revealed a significant influence in stage T3 and a lack of influence in stage T2.Conclusion. The surgeon's experience is an independent prognostic factor for stage T3 rectal cancer patients.
Parasitic protozoans of the Cryptosporidium genus are intracellular intestinal parasites of mammals, causing cryptosporidiosis. Clinically, cryptosporidiosis manifests as chronic diarrhoea. Individuals with immune disorders, including those with neoplasms, are at risk of symptomatic invasion.The aim of the study was the evaluation of Cryptosporidium sp. prevalence in patients with diagnosed colorectal cancer.Material and methods. The studied group encompassed 87 patients with diagnosed colorectal cancer, undergoing surgery at the Department of General and Oncological Surgery, Pomeranian Medical University, in the years 2009-2010. Immunoenzymatic tests for Cryptosporidium sp. on faeces samples were performed with the use of commercial test kit, ProSpecT®Cryptosporidium Microplate Assay (Remel Inc).Results. The presence of Cryptosporidium sp. was found in 12.6% of studied patients with colorectal cancer. The performed statistical analysis did not reveal any correlation between Cryptosporidium sp. infection and gender, age, neoplasm advancement stage as per Astler-Coller scale, neoplasm differentiation grade, or neoplastic tumour localisation in relation to the splenic flexure.Conclusions. There was found high prevalence of Cryptosporidium sp. in patients with colorectal cancer. It was comparable to the prevalence reported for patients with immune deficiency.
Introduction: Rectal cancer is one of the most common malignancies in developed countries. However, despite the increasingly better preoperative diagnostics, adaptation of surgical techniques to the location and advancement of the tumor, the combination of surgical treatment with neoadjuvant therapy and adjuvant treatment, standardized control tests, Poland still has not obtained satisfactory results regarding long-term survival. In addition, the effects of the therapy often differ significantly from those expected by patients and the doctors treating them. Aim of the study: To evaluate the effects of rectal cancer treatment among patients of the General and Oncological Surgery Clinic of the Pomeranian Medical University in Szczecin. The impact of numerous factors on postoperative quality of life was analyzed. Material and methods: Between 2007 - 2015, 263 radical resection procedures were performed in patients with diagnosed rectal cancer. Retrospectively, based on medical records, a database was created covering a range of clinical data. Information about death dates of some patients was obtained at the Registry Office in Szczecin. A survey supplementing clinical data and standardized quality of life assessment forms (EORTC QLQ - C30 and CR29) were sent to 120 living patients. A telephone conversation was carried out with some patients who did not respond to the surveys. Finally, data from 90 people was collected, which represents 75% of the patients enrolled in the study. Patients quality of life was assessed using EORTC questionnaire evaluation guides. Results: The patients quality of life worsened the most as a result of anorectal dysfunction. Incontinence of gases and stool, urgency and difficulty in defecation were demonstrated primarily in patients undergoing low rectal resection and irradiation. Patients undergoing radiotherapy, as a result of persistent low anterior resection syndrome, were forced to partially or completely withdraw from professional activity and to limit the pursuit of their interests. Their contacts with family, friends and acquaintances have also deteriorated. The presence of the intestinal stoma significantly affected the deterioration of the reception of the body's own image. However, no relation was found between the existence of the fistula and other aspects of the patients everyday life, including functioning in life and social roles. Conclusions: Due to the acceptable postoperative quality of life of patients with fistula and numerous imperfections of sphincter preserving techniques, operations resulting in terminal ostomy should not be considered as an extremity, and in the case of tumors of the lower rectum with unaffected sphincters, they should be considered as alternative methods for low anterior resection.
Wprowadzenie: Rak odbytnicy jest jednym z najczęściej występujących nowotworów złośliwych w krajach wysoko rozwiniętych. Jednak, pomimo coraz lepszej diagnostyki przedoperacyjnej, dostosowywania technik chirurgicznych do położenia i zaawansowania nowotworu, skojarzenia leczenia operacyjnego z terapią neoadjuwantową oraz leczeniem uzupełniającym, wystandaryzowanych badań kontrolnych, nadal nie uzyskano w Polsce satysfakcjonujących wyników dotyczących odległych przeżyć. Ponadto, efekty zastosowanej terapii niejednokrotnie znacznie odbiegają od tych oczekiwanych przez pacjentów oraz prowadzących ich lekarzy. Cel: Celem niniejszej pracy jest ocena efektów leczenia nowotworów odbytnicy wśród pacjentów Kliniki Chirurgii Ogólnej i Onkologicznej Pomorskiego Uniwersytetu Medycznego w Szczecinie. Analizie poddany został wpływ licznych czynników na pooperacyjną jakość życia. Materiał i metody: W latach 2007–2015 w ramach Kliniki Chirurgii Ogólnej i Onkologicznej Pomorskiego Uniwersytetu Medycznego w Szczecinie wykonano 263 radykalne zabiegi resekcyjne u pacjentów z rozpoznanym nowotworem złośliwym odbytnicy. Retrospektywnie, na podstawie historii chorób, utworzono bazę danych obejmującą szereg danych klinicznych. W Urzędzie Stanu Cywilnego uzyskano informację o datach zgonów części chorych. Do 120 żyjących osób wysłano ankietę uzupełniającą dane kliniczne oraz standaryzowane formularze oceny jakości życia (EORTC QLQ – C30 oraz CR29). Z częścią pacjentów, którzy nie odpowiedzieli na ankiety, przeprowadzono rozmowę telefoniczną. Ostatecznie zebrano dane od 90 osób, co stanowi 75% pacjentów włączonych do badania. Jakość życia pacjentów oszacowano, wykorzystując przewodniki oceny kwestionariuszy EORTC. Wyniki: Jakość życia chorych w największym stopniu uległa pogorszeniu w wyniku dysfunkcji anorektalnej. Nietrzymanie gazów i stolca, parcia naglące oraz trudności z wypróżnieniami wykazano przede wszystkim u pacjentów poddanych niskiej resekcji odbytnicy oraz naświetlaniu. Osoby poddane radioterapii, na skutek przetrwałego zespołu niskiej resekcji odbytnicy, zmuszone były do częściowego lub całkowitego wycofania się z aktywności zawodowej oraz ograniczenia realizacji swoich zainteresowań. Pogorszeniu uległy również ich kontakty z rodziną, przyjaciółmi i znajomymi. Obecność stomii jelitowej w istotny sposób wpłynęła na pogorszenie odbioru obrazu własnego ciała. Nie stwierdzono jednak związku pomiędzy istnieniem przetoki a innymi aspektami codziennego życia pacjentów, łącznie z funkcjonowaniem w rolach życiowych i społecznych. Wnioski: Z uwagi na akceptowalną pooperacyjną jakość życia pacjentów z przetoką oraz liczne niedoskonałości technik prowadzących do zaoszczędzenia aparatu zwieraczowego, operacje skutkujące wyłonieniem stomii nie powinny być traktowane jako ostateczność, a w przypadku guzów dolnej części odbytnicy z niezajętymi zwieraczami, należałoby rozważać je jako metody alternatywne do przedniej niskiej resekcji.
The management of peritoneal surface malignancy is a significant clinical problem in oncology. It was demonstrated that the combination of complete cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (HIPEC) may lead to long-term control of the disease or improved survival in selected patients. The aim of this paper was to present the optimal indications and technical guidelines for performing HIPEC in Poland. The application of this method requires experience of a multidisciplinary team of physicians (gynecologic oncologist, surgeon and clinical oncologist), availability of diagnostic and therapeutic resources (intensive care unit) as well as a dedicated perfusion system. A crucial aspect for obtaining optimal treatment outcomes is the selection of patients. Such a selection takes place both at the beginning of treatment and intraoperatively. The initial selection of patients qualified for HIPEC includes ruling out extraperitoneal spread of cancer and metastases to the liver (single resectable liver metastases in patients with colorectal carcinoma are not contraindications) and lungs. According to current international guidelines, the HIPEC procedure is a standard treatment in patients with ovarian carcinoma that metastasizes to the peritoneum, in colorectal cancer, when PCI <20 and in patients with peritoneal mesothelioma or pseudomyxoma peritonei as well as in patients with gastric cancer.

Leczenie nowotworów złośliwych powierzchni otrzewnej stanowi istotny problem w onkologii. Wykazano, że skojarzenie maksymalnej cytoredukcji chirurgicznej z dootrzewnową chemioterapią w hipertermii (hyperthermic intraperitoneal chemotherapy, HIPEC) może prowadzić do długotrwałej kontroli choroby lub wydłużenia przeżyć u odpowiednio wybranych chorych. Celem niniejszego opracowania są zalecenia dla optymalnego zakresu wskazań i sposobu prowadzenia HIPEC w Polsce. Zastosowanie tej metody wymaga doświadczenia wielospecjalistycznego zespołu lekarskiego (ginekolog onkolog, chirurg, onkolog kliniczny), a także zaplecza diagnostycznego i leczniczego (oddziału intensywnej opieki medycznej) oraz dedykowanego systemu do perfuzji. Kluczową kwestią dla uzyskania optymalnego wyniku leczenia jest odpowiedni dobór chorych. Dobór ten następuje zarówno na etapie planowania leczenia, jak i śródoperacyjnie. Wstępna selekcja pacjentów kwalifikowanych do leczenia z wykorzystaniem HIPEC obejmuje wykluczenie pozaotrzewnowych przerzutów nowotworu oraz przerzutów do wątroby (pojedyncze resekcyjne przerzuty do wątroby u chorych na raka jelita grubego nie są przeciwwskazaniem) i płuc. Zgodnie z istniejącymi zaleceniami międzynarodowymi procedura HIPEC jest standardowo stosowana u chorych z przerzutami do otrzewnej raka jajnika, raka jelita grubego, gdy PCI <20 oraz u chorych na międzybłoniaka lub śluzaka rzekomego otrzewnej i wybranych chorych na raka żołądka.
