The spreading of COVID-19 pandemic caused by the SARS-CoV-2 coronavirus affects the entire healthcare system in the world. As one of the side effects of the pandemic, the need for surgery deferral has emerged. The purpose of this analysis is to review current literature discussing the issue of a limited access to critical care and surgical services. Following the outbreak of a pandemic, many national scientific societies, as well as numerous medical specialty associations, have begun working on developing guidelines to deal with these issues to enable access to medical services for the most seriously ill patients affected by COVID-19. Moreover, limited access to intensive care beds, shortages in personal protective equipment chain supply and the risk of an unintended spread of infection among the staff and the patients led to a severe limitation of all elective operations, mostly excluding oncological, ur-gent/emergent and trauma interventions. This period of uncertainty about the risk of virus transmission can last for many months, if not years. Therefore, it is necessary to develop such behavioral patterns that will allow us to work safely without limiting the number of elective operations and maintaining continuous access to critical and surgical care. As elective operations were cancelled in the world to the extent that history did not witnessed so far, there is a simultaneous unintended harmful effect of this phenomenon. The overall survival time of patients may be shortened, their quality of life might be reduced, the risk of complications and the need for critical care in the most severe cases will increase. Currently, it seems of the utmost importance to develop a plan for a safe return to elective surgery. At the same time, international organizations should warrant the development of alternative plans for dealing with similar events in the future.

Rozprzestrzenianie się pandemii COVID-19 wywołanej przez koronawirusa SARS-CoV-2 wpływa na systemy opieki zdrowotnej na całym świecie. Jako jeden ze skutków ubocznych pandemii pojawiła się potrzeba odroczenia operacji. Cel tej analizy ma na celu przegląd aktualnego piśmiennictwa omawiającego kwestię ograniczonego dostępu do intensywnej terapii i usług chirurgicznych. Po wybuchu pandemii wiele krajowych towarzystw naukowych, a także liczne organizacje medyczne i stowarzyszenia rozpoczęły działania nad opracowaniem wytycznych dotyczących tych problemów, aby umożliwić dostęp do opieki medycznej najciężej chorym pacjentom z COVID-19. Ponadto ograniczony dostęp do stanowisk intensywnej terapii, niedobory łańcucha dostaw środków ochrony osobistej i ryzyko niezamierzonego rozprzestrzeniania się infekcji wśród personelu i pacjentów doprowadziły do poważnego ograniczenia wszystkich operacji planowych, nie licząc onkologicznych, ratujących życie i interwencji urazowych. Ten okres niepewności co do ryzyka przeniesienia wirusa może trwać wiele miesięcy, jeśli nie lat. Dlatego konieczne jest opracowanie takich wzorców zachowań, które pozwolą bezpiecznie pracować bez ograniczania liczby operacji planowych i utrzymać ciągły dostęp do intensywnej terapii oraz opieki chirurgicznej. Zabiegi operacyjne zostały ograniczone na świecie tak, jak jeszcze nigdy w historii, a jednocześnie nie określono jeszcze szkodliwych efektów tego zjawiska. Ogólny czas przeżycia pacjentów może zostać skrócony, ich jakość życia może zostać obniżona, a ryzyko powikłań i potrzeba intensywnej terapii w najcięższych przypadkach wzrośnie. Obecnie wydaje się, że ogromne znaczenie ma opracowanie planu bezpiecznego powrotu do planowej operacji. Jednocześnie organizacje międzynarodowe powinny uzasadnić opracowanie alternatywnych planów radzenia sobie z podobnymi wydarzeniami w przyszłości.
The safety of healthcare workers is a priority for public health protection. This is the basic principle we teach apprentices in paramedics, nursing and medicine. Only a safe specialist can help those in need. The COVID-19 pandemic suddenly appeared and resulted in more attention being paid to the possibility of droplet transmission. Until now, medical staff only paid attention to blood-borne and contact infections, and the air-borne route was underestimated. Current painful experiences raised by SARS-CoV-2 require us to verify the current procedures performance and introduce the principles of safe for the staff to perform endotracheal intubation with appropriate protection measures. Healthcare workers should use barrier surgical caps, goggles or face shield, FFP3 mask, protective gowns, double gloves and utilize proper hand washing technique. Additionally, some protective barriers might be used to minimize aerosol dispersion during endotracheal intubation.

Bezpieczeństwo pracowników służby zdrowia jest priorytetem publicznego systemu ochrony zdrowia. To jest podstawowa zasada, której uczymy praktykantów w zakresie ratownictwa medycznego, pielęgniarstwa i medycyny. Tylko odpowiednio zabezpieczony specjalista może pomóc potrzebującym. Pandemia COVID-19 pojawiła się nagle, w wyniku czego zwrócono większą uwagę na możliwość przenoszenia zakażeń drogą kropelkową. Aż do chwili obecnej personel medyczny zwracał uwagę tylko na infekcje krwiopochodne i kontaktowe, a droga powietrzna była niedoceniana. Obecne bolesne doświadczenia związane z SARS-CoV-2 wymagają od nas weryfikacji aktualnych procedur i wprowadzenia zasad bezpiecznego dla personelu wykonywania intubacji dotchawiczej przy zachowaniu odpowiednich środków ochronnych. Pracownicy służby zdrowia powinni używać ochronnych czepków chirurgicznych, okularów ochronnych lub osłony twarzy, maski FFP3, fartuchów ochronnych, podwójnych rękawic i stosować odpowiednią technikę mycia rąk. Dodatkowo w celu zminimalizowania ryzyka można zastosować dodatkowe bariery ochronne zapobiegające dyspersji aerozolu podczas intubacji dotchawiczej.
Availability of surgical care in Africa is severely limited. This is due to the lack of surgeons and a small number of public hospitals. Only 25 out of 100,000 patients with inguinal hernia undergo a surgical treatment. As many as 65% of inguinal hernia repairs are performed urgently because of incarceration. Among patients with incarceration who do not reach the hospital there is recorded as many as 87 deaths per 100 cases. In order to improve the availability of treatment of inguinal hernia in Africa, humanitarian medical missions involving surgeons from Europe are organized. During regular visits to selected centers in Africa, they also carry out intensified treatment of patients and training of the local staff. The aim of the study was to present the experience of Polish surgeons from the humanitarian medical mission in Tamale in northern Ghana undertaken in fall of 2014. Material and methods. Surgical repair was performed in 87 patients (74 men – 85% and 13 women – 15%) between the ages of 26 to 70 years (mean 52.8 years; SD 10.3), who underwent a total of 98 inguinal hernia repairs under local anesthesia. Results. Lichtenstein procedure was performed in 93 and Desarda technique in 5 patients. Patients reported the long-term presence of hernia symptoms - from one to 7 years (mean 3.4 years, SD 1.4). In most patients, hernia occurred more than 3 years earlier (61 patients; 70%). There were no intraoperative complications. All patients were discharged the next day after surgery. There was one wound infection in postoperative period which required mesh explantation. Conclusions. Inguinal hernia commonly found in Ghana is a major issue for the inefficient health care system. Humanitarian medical missions can help to improve the treatment results, as long as they are carried out periodically and allow for training of local personnel. Scarce equipment of medical facilities in Ghana is not a significant difficulty in performing the Lichtenstein repair under the local anesthesia.
Nearly 1% of population is affected with burn trauma annually. Among patients seeking advanced medical care almost 8% require hospital stay. Most burns are superficial and do not cause full thickness damage to the skin. Standard topical treatment of burn wounds with creams and ointments requires frequent dressings’ changes. The healing process is long and expensive. The technological revolution, as occurred in the last decade in the field of dressing care allows us to introduce the use of advanced material solutions. Modern materials allow to shorten the treatment time, reduce the expenses and diminish the patient discomfort leading to good outcome. The study presented a case of four patients treated due to the partial thickness burn wound. Hydrofiber dressing with silver ions were introduced in the treatment. Meckel diverticulum is the most common congenital abnormality of gastrointestinal track (2-4%), however complications occur rarely (4-16%). We describe a case of 63- years old female presenting simultaneously two serious complications of Meckel diverticulum. Operated patient was diagnosed with perforated tumor of Meckel diverticulum. Segmental resection of small bowel including tumor was performed. Pathology examination revealed gastrointestinal stromal tumor (GIST) in Meckel diverticulum. No significant malignancy risk factors were found (low mitotic count). Consequently, computed tomography periodic surveillance was implemented.We report the possibility of simultaneous presentation of two serious complications of Meckel diverticulum. Tumors of Meckel diverticulum may mimic other abdominal pathologies and thus, they should be considered in differential diagnosis of abdominal tumors.
INTRODUCTION: Bleeding into the gastrointestinal tract in the prehospital setting is often a cause of a life-threatening condition. The vast majority of cases (80%) involve bleeding from the upper gastrointestinal tract and the mortality reaches even 14% of cases. It is then necessary to quickly diagnose and direct the patient for hospitalisation as the most effective treatment in the endoscopic therapy. The study aimed to analyse the profile of the patient referred to a hospital by a primary healthcare physician with suspicion and/or diagnosis of bleeding from the gastrointestinal tract. MATERIAL AND METHODS: The authors attempted to assess the initial laboratory results of critical parameters (RBC, HGB, HCT and PLT) of patients admitted on an emergency basis in 2017-2020 to the municipal hospital in Siedlce, Poland. The total number of patients with bleeding symptoms referred by a primary healthcare physician was 843. The inclusion procedure included patients referred on an emergency basis only (n=56). The statistical analysis was conducted by means of the Shapiro–Wilk test of normal distribution, Spearman's rho correlation and Chi-Square Tests of Independence. All results were regarded as significant at p < 0.05. RESULTS: The study included 32 males and 24 females referred in 60.71% of cases with the ICD-10 diagnosis of K92.2. The mean age of the patients was 65.02 years (SD±14.69). The mean hospitalisation time was 5.11 days (SD±12.14), both in males and females. The average blood test results in males were as follows: RBC: 3.28 106/μL; HGB: 9.26 g/dL; HCT: 27.94%; PLT: 229.13 103/μL, while in females: RBC: 3.26 106/μL; HGB: 9.76 g/dL; HCT: 29.12%; PLT: 240.33 103/μL. Therapy with packed red blood cells was significantly dependent on the level of RBC (p=0.000), HGB (p=0.001) and HCT (p=0.000). However, it was not dependent on PLT (p=0.141). A similar tendency was shown in the correlation of plasma therapy with critical values of selected laboratory tests (RBC: p=0,000; HGB: p=0.021; HCT: p=0.005; PLT: p=0.116). CONCLUSIONS:The hospitalisation of patients from the study group concerned mostly to older males, whose laboratory test results were usually much below the normal limits. Patients referred to hospital on an emergency basis usually warrant therapy related to deep anaemia. Despite a high survival rate, educational programmes aiming at early diagnosis of symptoms and implementation of diagnostics for bleeding from the gastrointestinal tract are to be considered.

WSTĘP: Krwawienia do przewodu pokarmowego w warunkach przedszpitalnych niejednokrotnie są przyczyną wystąpienia stanu zagrożenia życia. Zdecydowana większość przypadków (80%) dotyczy krwawienia z górnego odcinka przewodu pokarmowego, a śmiertelność wynosi wówczas nawet do 14% przypadków. Niezbędne jest wówczas szybkie rozpoznanie i skierowanie pacjenta na hospitalizację, gdyż najskuteczniejszym leczeniem jest terapia endoskopowa. Celem badania była analiza profilu pacjenta skierowanego do szpitala przez lekarza pierwszego kontaktu z podejrzeniem i/lub rozpoznaniem krwawienia z przewodu pokarmowego. MATERIAŁ I METODY: Autorzy podjęli próbę oceny wyjściowych wyników laboratoryjnych parametrów krytycznych (RBC, HGB, HCT, PLT) pacjentów przyjętych w trybie pilnym w latach 2017-2020 do szpitala miejskiego w Siedlcach, Polska. Łączna liczba chorych skierowanych przez lekarza pierwszego kontaktu z objawami krwawienia wynosiła 843. Procedura włączenia uwzględniała pacjentów skierowanych jedynie w trybie pilnym (n=56). Do opracowania statystycznego wykorzystano test normalności rozkładu zmiennych Shapiro-Wilka, korelację rho-Spearmana oraz testy niezależności Chi kwadrat. Wszystkie wyniki uznano za istotne przy p < 0,05. WYNIKI: Badaniem objęto 32 mężczyzn oraz 24 kobiety, kierowanych w 60,71% przypadkach z rozpoznaniem ICD-10: K92.2. Średnia wieku chorych wynosiła 65,02 lat (SD±14,69). Średnia długość hospitalizacji zarówno u kobiet jak i u mężczyzn wyniosła 5,11 dnia (SD±12,14). Wyniki krwi mężczyzn wynosiły średnio dla RBC: 3,28 106/µl; HGB: 9,26 g/dl; HCT: 27,94%; PLT: 229,13 103/µl, zaś u kobiet RBC: 3,26 106/µl; HGB: 9,76 g/dl; HCT: 29,12%; PLT: 240,33 103/µl. Terapia koncentratami krwinek czerwonych była istotnie zależna od wartości RBC (p=0,000), HGB (p=0,001) i HCT (p=0,000). Nie była jednak zależna od PLT (p=0,141). Podobną tendencję wykazała korelacja terapii osoczem z krytycznymi wartościami wybranych badań laboratoryjnych (RBC: p=0,000; HGB: p=0,021; HCT: p=0,005; PLT: p=0,116). WNIOSKI: Hospitalizacja pacjentów z grupy badanej dotyczyła przeważnie osób starszych płci męskiej, u których wyniki badań laboratoryjnych były zazwyczaj znacznie poniżej dolnej granic norm. Pacjenci kierowani do szpitala w trybie pilnym wymagają zazwyczaj terapii związanej z głęboką niedokrwistością. Pomimo wysokiego współczynnika przeżywalności należy rozważyć wdrożenie programów edukacyjnych mających na celu wcześniejsze rozpoznawanie objawów i wdrożenie diagnostyki w kierunku krwawienia z przewodu pokarmowego.
INTRODUCTION: For many months now, the entire world experiences effects of the COVID-19 pandemic, and current data indicate that this condition will continue even longer. About 5% of patients who become infected with the SARSCoV- 2 coronavirus may develop a critical form of severe respiratory failure, due to which they will undergo intubation and mechanical ventilation. Prolonged mechanical ventilation will be an indication for an open tracheostomy, burdened with an increased production of aerosol containing virus particles and an increased risk of transmitting the highly contagious pathogen to medical personnel. In this situation, surgical departments with a limited number of operating theaters may be dedicated only to COVID-19 patients. Their use for other procedures will be significantly reduced or even impossible to prevent transmission of infections and exposure to other people. Taking into account the well-being of patients and staff and limiting the transmission of the virus to other departments, other safe solutions are being sought to perform a tracheostomy. An alternative procedure may be to perform a tracheostomy directly at the bedside (point-of- care) of a patient hospitalized in an intensive care unit (ICU). PURPOSE: The purpose of the study is to present an effective performance of safe, open tracheostomy in the ICU. METHODS: The bedside tracheotomy as an alternative to the procedure performed in the setting of an operating theater has been presented. A case study was based on the analysis of the procedure performed in a 42-year-old patient who was installed an open tracheotomy tube on the twenty-ninth day of intubation. CONCLUSIONS:In order to limit the transmission of the SARS-CoV-2 coronavirus outside the intensive care unit, it is recommended to perform a tracheotomy in ICU, in patients requiring mechanical ventilation, as the method is effective and safe.

WSTĘP: Od wielu miesięcy cały świat doświadcza skutków pandemii COVID-19, a obecne dane wskazują, że ten stan będzie trwał jeszcze długo. Około 5% pacjentów zarażonych koronawirusem SARSCoV-2 może rozwinąć krytyczną postać ciężkiej niewydolności oddechowej, w wyniku której zostaną poddani intubacji i wentylacji mechanicznej. Przedłużona wentylacja mechaniczna będzie wskazaniem do otwartej tracheostomii obciążonej zwiększoną produkcją aerozolu zawierającego cząsteczki wirusów i zwiększonym ryzykiem przenoszenia ich zakaźnego patogenu na personel medyczny. W tej sytuacji oddziały chirurgiczne z ograniczoną liczbą operacji mogą być przeznaczone wyłącznie dla pacjentów z COVID-19. Ich użycie do innych procedur zostanie znacznie ograniczone lub nawet niemożliwe, aby zapobiec przenoszeniu infekcji i narażeniu innych osób. Biorąc pod uwagę stan pacjentów oraz ograniczenie przenoszenia wirusa do innych oddziałów, poszukuje się innych bezpiecznych rozwiązań, aby wykonać tracheostomię. Alternatywną procedurą może być wykonanie tracheostomii bezpośrednio przy łóżku pacjenta hospitalizowanego na oddziale intensywnej terapii (OIT). CEL: Celem pracy jest przedstawienie skutecznego wykonania bezpiecznej, otwartej tracheostomii na OIT. METODY: Wykonano tracheotomię przyłóżkową jako alternatywę dla zabiegu wykonywanego na sali operacyjnej. Studium przypadku oparto na analizie zabiegu u 42-letniego pacjenta, któremu założono otwartą rurkę tracheotomijną dwudziestego dziewiątego dnia intubacji. WNIOSKI: W celu ograniczenia przenoszenia koronawirusa SARS-CoV-2 poza oddział intensywnej terapii zaleca się wykonanie tracheotomii na OIT u chorych wymagających wentylacji mechanicznej ze względu na skuteczność i bezpieczeństwo tej metody.
