Between 2010–2011 the first EGFR thyrosine kinase inhibitor (EGFR TKI) – gefitinib, was registered in many countries in I line of treatment in patients with advanced and locally advanced non-small cell lung cancer (NSCLC), among whom EGFR gene mutations have been detected. It was also found that the presence of these mutations is crucial for the effectiveness of EGFR TKI – erlotinib, used so far mainly in monotherapy in II line in NSCLC patients, with unknown EGFR gene status. Studies of these two drugs concerned mostly Asian NSCLC populations, where EGFR gene mutations occurrence is much higher than in Caucasian populations. The EURTAC study, and a study by Rosell et al. previous to it, were the first ones that confirmed the effectiveness of erlotinib in adenocarcinoma patients of Caucasian origin, who carried EGFR gene mutations: exon 19 deletions or L858R substitution in exon 21 of this gene. The innovative nature of this study is to attempt to detect EGFR gene mutations in free circulating DNA in peripheral blood, which could broaden the possibility of advanced NSCLC patients qualification to EGFR TKI treatment. The EURTAC study also has its weaknesses, among which disqualification of patients with other forms of non-squamous NSCLC (e.g. large cell carcinoma patients, who also might be carriers of EGFR gene mutations) and patients with rare EGFR gene mutations, are the most distinct.

W latach 2010–2011 pierwszy inhibitor kinazy tyrozynowej EGFR (IKT EGFR) – gefitynib uzyskał w wielu krajach rejestrację do leczenia w I linii chorych na zaawansowanego i miejscowo zaawansowanego niedrobnokomórkowego raka płuca (NDRP), u których występowały mutacje w genie EGFR. Okazało się także, że obecność tych mutacji jest kluczowa dla skuteczności innego odwracalnego IKT EGFR – erlotynibu, stosowanego dotychczas głównie w monoterapii II linii u chorych na NDRP z nieznanym statusem genu EGFR. Badania obu leków przeprowadzano najczęściej na azjatyckich populacjach chorych na NDRP, w których częstość występowania mutacji w genie EGFR jest dużo wyższa niż w populacjach kaukaskich. Badanie EURTAC oraz poprzedzające je badanie przeprowadzone przez Rosella i wsp. były pierwszymi, które potwierdziły skuteczność erlotynibu u chorych na gruczołowego raka płuca rasy kaukaskiej posiadających najczęstsze mutacje w genie EGFR, tj. delecje w eksonie 19 lub substytucję L858R w eksonie 21 tego genu. Nowatorski charakter w tym badaniu miała próba wykrycia mutacji w genie EGFR w wolnym DNA krążącym we krwi obwodowej, co mogłoby rozszerzyć możliwości kwalifikacji chorych na zaawansowanego NDRP do leczenia IKT EGFR. Badanie EURTAC ma także pewne mankamenty, do których należy przede wszystkim dyskwalifikacja z leczenia erlotynibem chorych z innymi postaciami niepłaskonabłonkowego NDRP (np. pacjentów z rakiem wielkokomórkowym mogących być nosicielami mutacji w genie EGFR) oraz z rzadkimi mutacjami w genie EGFR.
Introduction. In sports success depends on how well the players are prepared for the match in terms of their technical, tactical and motor skills, as well as their mental attitude. The training process is focused on improving the players' capabilities and eliminating their errors. The aim of the study was to describe the errors made in handball matches taking into account their timing and quantity. Material and methods. The material consisted of recordings of 50 matches of men's handball held in the EHF Champions League in 2012-2013. The study included 8 teams that took part in the tournament. The following matches were analysed for each team: 4 matches in the knockout stage (2 played at home and 2 away) and 4 in the group stage (2 at home and 2 away). The errors were determined based on EHF regulations. Results. A difference was found between the errors committed in matches at home and away in the knockout stage. The most errors (35) in this stage of the tournament were made between the 25th and 30th minute, and the fewest (18) between the 30th and 35th minute, in both cases in matches played at home. In the group stage the most errors (36) were made between the 40th and 45th minute away, and the fewest (15) between the 10th and 15th minute at home. In the knockout stage the four teams made a total of 632 errors, including 71 errors made by the winner of the tournament, who committed the smallest number of errors. The smallest number of errors (53) in the group stage were made by HC Metalurg Skopje. Conclusions. The number of errors made did not have a direct influence on a team's position in the tournament, but the winner made the least errors in the knockout stage. The differences between the numbers of errors made in the knockout stage may indicate that the players were more comfortable when playing at home. The support of fans may be one of the factors which contribute to the fact that players commit fewer errors in this stage of the tournament. The rise in the number of errors made towards the end of the match, on the other hand, is most likely due to an increased level of physical and emotional fatigue, particularly considering the risk of being eliminated from the tournament.
Amputacja prowadząca do utraty części ciała wiąże się nie tylko ze znacznymi kosztami ekonomicznymi, ale i poważnymi konsekwencjami natury medycznej i społeczno-psychologicznej. Jest ostatecznym środkiem mającym na celu ochronę życia lub poprawę jego jakości. Do najtrudniejszych wyzwań, z jakimi przychodzi mierzyć się osobom po amputacji należy zaliczyć zaakceptowanie zmian we własnej fizjonomii oraz wynikające z nich, ograniczenia życiowe. Pacjent po amputacji ma przed sobą niezwykle trudny okres adaptacyjny, w trakcie którego powinien dążyć do maksymalnego stopnia samodzielności. Niestety duża ich grupa zmaga się także z różnego rodzaju doznaniami i dolegliwościami bólowymi umiejscowionymi w obrębie utraconej kończyny − tak zwanymi wrażeniami fantomowymi. Jest to szczególna grupa odczuć o zróżnicowanym charakterze „umiejscowionych” w obrębie utraconej kończyny. Występowanie zespołu kończyny fantomowej u pacjentów po amputacjach jest niezwykle częste. Problem ten dotyka od 45% do nawet 98% z nich po amputacji jednej lub obu kończyn górnych i dolnych. Głównym celem artykułu było opisanie wrażeń fantomowych, obserwowanych u chorych po amputacji kończyn w świetle aktualnej literatury. Przedstawiono definicję wrażeń fantomowych i ich rodzaje, rys historyczny, hipotetyczne patomechanizmy, czynniki wpływające na częstość i intensywność wrażeń fantomowych oraz dostępne sposoby leczenia. Praca powstała w oparciu o liczne źródła tekstowe i doświadczenia własne.

Amputation leading to the loss of a body part is associated not only with significant economic costs, but also serious consequences of medical and socio-psychological nature. It is the ultimate means to save a life or improve its quality. The most difficult challenges faced by amputees include accepting changes regarding their own physiognomy and the resulting life restrictions. The patient subjected to amputation is faced with an extremely difficult adaptation process, during which s/he should strive for a maximum degree of independence. Unfortunately, a large group of patients also struggles with various types of sensations and pain located within the lost limb − i.e., so-called phantom phenomena. This is a special group of phenomena of diverse nature, “located” within the lost limb. The occurrence of phantom limb syndrome in amputee patients is extremely common. This problem affects from 45% to even 98% of patients after amputation of one or both upper and lower limbs. The main purpose of this article is to describe phantom phenomena observed in patients after limb amputation in light of current literature. The definition, historical outline, types of phantom phenomena are presented, as well as hypothetical pathomechanisms, factors influencing the frequency and intensity of phantom phenomena and available treatment methods. The work was based on numerous text sources and the author’s own experience.
